입원 중증 손상 환자의 사망과 전원에 영향을 미치는 요인에 관한 다수준분석 (Multilevel Analysis on Factors Influencing Death and Transfer in Inpatient with Severe Injury)

  • Background: This study was conducted to evaluate the individual and community level factors which were influencing the severe injury patients' death and transfer at discharge. Methods: Analysis data is based on Korean National Hospital Discharge In-depth Survey Data released by the Korea Center for Disease Control and Prevention from 2006 to 2008. Study subjects was 11,026 inpatients with of severe injury. For multi-level analysis, socio-demographic characteristics, injury related characteristics, hospitalization related characteristics were used as individual level factors, and socio-environmental characteristics and health care resource characteristics were used as community level factors. Results: As to community level factors affecting mortality of severe injury, the possibility of death was also high in cases of less numbers of surgeons per a population of 100,000 and more number of operation beds. As to community level factors affecting transfer of severe injury, vulnerable areas with higher social deprivation index and low population density had higher possibility of transfer. Conclusion: Both individual level factors and community level factors affected clinical outcomes of treatment for severe injury. In particular, since there happened higher death and transfer of severe injury in socioeconomic and medical vulnerable areas, special efforts for establishing preventive policy and care system for injury in national and area level should be directed toward such areas.

Relationship between threatened vascular plants and the human population in Japan

  • Using data sets for Japan as a whole, as arranged with approximately $10{\times}10$ km squares (a secondary grid), we investigated the relationship between population density and the habitats of threatened vascular plants listed in the Japanese Red Data Book; depopulated areas in the present and future, areas where under-use may be serious, and those with a predominance of elderly people; and the present state of the habitats in terms of a characteristic land use pattern. Regarding the habitats of threatened vascular plants, the progress of deterioration [$(N_{CR}+N_{EN})/(N_{CR}+N_{EN}+N_{VU})$] in depopulated areas has been confirmed, where $N_{CR}$, $N_{EN}$, and $N_{VU}$ are the numbers of species classified as critically endangered, endangered, and vulnerable, respectively. Moreover, in grid squares used by a human such as farmland, the progress of the deterioration simply increases when population density becomes low. However, for many vascular plants, they are particularly endangered in populous areas. Local populations will decrease throughout Japan with the rate of depopulation in and around large cities being relatively slow. We also propose some issues that need further study. The deterioration by human activity may be reduced. On the other hand, some vascular plants may be adversely influenced by depopulation. Additionally, we should keep a close watch on grasslands and water areas in large cities to preserve vascular plants.

The Trajectory of Depressive Symptoms Across Years of Community Care Utilization Among Older Adults: A 14-Year Follow-up Study Using the 'Korean Welfare Panel Survey'

  • Objectives: While older adults using community care services are known to be vulnerable for depression, community care utilization (CCU) may help to improve the mental health of these elderly. To date, however, it is much less clear how CCU affects depressive symptoms in the elderly population. This study focuses on the trajectory of depressive symptoms across years of CCU among older adults in Korea. Methods: Using the 2006-2019 Korean Welfare Panel Survey, this study is focused on elderly born in 1940 or earlier and selected 3281 persons for baseline interviews in 2006. This consisted of 35 800 person-year observations during a period of 14 years. Panel data analysis were employed to construct years of CCU. Results: After controlling for covariates, linear term of years using community care was negatively associated with depressive symptoms, but a quadratic term was positively significant. The trajectory of depressive symptoms across the years of CCU follows a U-shaped curve. Older adults in the first year of using community care reported the highest level of depressive symptoms. However, a significant and steady decrease in depressive symptoms was observed during the following 9 years of CCU, which then gradually increased. The level of depressive symptoms at the 14th year of using community care remains significantly lower than the level at the outset of its utilization. Conclusions: This finding implies that CCU could be beneficial for improving mental health among older adults.

Transitional Care for Older Adults with Chronic Illnesses as a Vulnerable Population: Theoretical Framework and Future Directions in Nursing

  • Purpose: Effective transitional care is needed to improve the quality of life in older adult patients with chronic illness and avoid discontinuity of care and adverse events. The aim of this article is to provide an overview of the key features, broader implications, and the utility of Meleis' transition theory intended for the transitional care of older adults with chronic illnesses. We present the role of nurse in the context of transitional care and propose future directions to increase the quality of nursing care. Methods: The online databases Cumulative Index of Nursing and Allied Health Literature, MEDLINE, and Science Direct were searched for relevant literature published since 1970 along with textbooks regarding nursing theory. Results: An evaluation of the usefulness of transition theory based on transitional care in older adult patients with chronic illnesses is provided. Healthy transition should be the expected standard of nursing care for older adults across all healthcare settings. Conclusion: Nurses need to contribute to the development of transitional care for vulnerable populations; however, transition theory needs to be enhanced through additional theoretical work and repeated evaluations of the applicability in areas of transitional care.

  • Purpose: The purpose of this study was to estimate the demand and supply of visiting nursing services provided by health centers in urban area, aiming at strengthening infrastructure, which may improved the quality of life and health status of vulnerable population in the community. Methods: This study was conducted through nominal group discussion, focus group study. The demand and supply of visiting nursing were estimated by health economists based on the secondary analysis data from 25 health centers in Seoul. Result: Primary targets for the visiting nursing must be people who are homebound in the community. They can be classified into: a group of Level I: chronic patients who need visiting nursing care at least once a week: and a group of Level II: vulnerable families that need management periodically e. g. twice a month. Based on the estimation of demand for visiting nursing services in the community, the estimated supply required was $651{\sim}770$ visiting nurses including home health nurses in visiting nursing programs based on health centers in Seoul. Conclusions: The estimated demand and supply of visiting nursing are expected to provide basic data for establishing alternative policies on visiting nursing infrastructure that might be accomplished through demand-based visiting nursing programs by districts.

취약계층 노인의 연령별 허약정도와 관련 요인 (Frailty and its related Factors in Vulnerable Elderly Population by Age Groups)

  • Purpose: This study aimed to investigate factors affecting frailty by age groups among vulnerable elders in Korea. Methods: In this secondary analysis, data were collected from records for 22,868 elders registered in the Visiting Health Management program of Public Health Centers in 2012. Health behaviors, clinically diagnosed disease, frailty, depression and cognitive condition were assessed. Data were analyzed using stepwise regression to determine the associated factors of frailty by age group. Results: Alcohol consumption, physical activity, number of diseases, DM, CVA, arthritis, urinary incontinence, depression and cognitive condition were found to be factors significantly associated with frailty among the elders aged 65~74 (F=135.66, p <.001). Alcohol consumption, physical activity, CVA, arthritis, urinary incontinence, depression and cognitive condition were found to be factors associated with frailty in the elders aged 75~84 (F=245.40, p <.001). Physical activity, CVA, arthritis, depression and cognitive condition were factors associated with frailty in the elders over 85 years of age (F=96.48, p <.001). Conclusion: The findings show that frailty of elders and associated factors were different by age group, and common factors affecting frailty were physical activity, CVA, arthritis, depression and cognitive condition. Thus, these factors should be considered in the development of intervention program for care and prevention of frailty and program should be modified according to age group.

The Role and Necessity of Public Health Services in a Remote Area

  • Purpose: This study aims to investigate the national obligation of public health support for residents in medically vulnerable areas in Korea, and to propose a suitable model for public health institutions in this region. Research design, data, and methodology: A survey targeting residents was conducted from August 10 to August 17, 2021, with a sample size of 177 general citizens. The survey utilized a structured questionnaire administered online through Google, employing convenience random sampling. After an editing process to ensure data accuracy, the final dataset of 174 valid samples underwent encoding, coding, and cleaning using the IBM SPSS Statistics 22.0 program for analysis. Results: Health status revealed a moderate level, and 63.8% reported having chronic diseases, particularly prevalent among the elderly. External healthcare institutions were commonly utilized, with proximity and competence of doctors being primary reasons. Respondents expressed a need for improving the public health and medical system, emphasizing the establishment of a County Health Centre and expanding medical departments. Conclusions: In this region, the region's unique challenges, including education, employment, population decline, aging, and transportation, require multidimensional efforts and urgent intervention by public entities. Long-term strategies involve considering the establishment of a health and medical institute, adjusting health centre resources to local realities, and fostering a cooperative system for collaboration among residents and institutions.

Genetic Variability and Geographical Distribution of Mycotoxigenic Fusarium verticillioides Strains Isolated from Maize Fields in Texas

  • Maize is the dominant cereal crop produced in the US. One of the main fungal pathogens of maize is Fusarium verticillioides, the causative agent of ear and stalk rots. Significantly, the fungus produces a group of mycotoxins - fumonisins - on infested kernels, which have been linked to various illnesses in humans and animals. Nonetheless, durable resistance against F. verticillioides in maize is not currently available. In Texas, over 2.1 million acres of maize are vulnerable to fumonisin contamination, but understanding of the distribution of toxigenic F. verticillioides in maize-producing areas is currently lacking. Our goal was to investigate the genetic variability of F. verticillioides in Texas with an emphasis on fumonisin trait and geographical distribution. A total of 164 F. verticillioides cultures were isolated from 65 maize-producing counties. DNA from each isolate was extracted and analyzed by PCR for the presence of FUM1- a key fumonisin biosynthesis gene - and mating type genes. Results showed that all isolates are in fact F. verticillioides capable of producing fumonisins with a 1:1 mating-type gene ratio in the population. To further study the genetic diversity of the population, isolates were analyzed using RAPD fingerprinting. Polymorphic markers were identified and the analysis showed no clear correlation between the RAPD profile of the isolates and their corresponding geographical origin. Our data suggest the toxigenic F. verticillioides population in Texas is widely distributed wherever maize is grown. We also hypothesize that the population is fluid, with active movement and genetic recombination occurring in the field.

Future Domestic Water Demand, Surface Water Availability and Vulnerability Across Rapidly Growing Asian Megacities

  • The rapid urbanization in many Asian countries has taken millions of people from the rural countryside to concentrated megacities, which eventually putting pressure on the existing water resources. The over-growing population and increasing living standard of people in the urban region of developed as well as developing countries such as Korea, China, Japan and India have witnessed a drastic change in terms of domestic water demand for the past few decades. In this study, we used the concept of potential surface water availability in the form of surface runoff for future vulnerability assessment. We focused on 42 megacities having population more than 5 million as per the United Nations (UN) census data 2020. The study shows that 30 out of 42 cities having more than 180L/p/d demand for domestic use based on various references. We have predicted the domestic water demand for all the cities on the basis of current per capita demand up to 2035 using UN projected population data. We found that the projected water demand in megacities such as Seoul, Busan, Shanghai, Ghuanzou are increasing because of high population as well as GDP growth rate. On the contrary, megacities of Japan considered in our stud shows less water demand in future due to decreasing trend of population. As per the past records provided by the local municipalities/authorities, we projected different scenarios based on the future supply for various megacities such as Chennai, Delhi, Karachi, Mumbai, Shanghai, Wuhan, etc. We found that the supply to demand ratio of these cities would be below 75% for future period and if such trend continues then the inhabitants will face serious water stress conditions. Outcomes of this study would help the local policy makers to adopt sustainable initiatives on urban water governance to avoid the severe water stress conditions in the vulnerable megacities.

환대관광산업 소규모기업 사회적 책임활동(CSR): 회사 홈페이지 커뮤니케이션 분석을 중심으로 (Corporate Social Responsibility (CSR) of Small Enterprises in Hospitality and Tourism Industry)

  • Purpose - The purpose of this paper is to explore the CSR activities of small enterprises in hospitality and tourism industry in South Korea. Since previous research on CSR activities has considerably focused on large enterprises whereas small enterprises have relatively less attention, this study aims to explore the characteristics of small enterprises in hospitality and tourism industry and their CSR activities. Research design, data, and methodology - The population of interest for this study was social enterprises registered in Korea Social Enterprise Promotion Agency (2016), and it was used to verify the social enterprises which has a certification for social enterprises. From 1672 companies in total, the sampling frame was a database with 117 companies in hospitality and tourism industry. This study investigates social enterprises' CSR activities on the company's official websites (e.g., company reports, magazines, the news articles, and interviews). The websites of the selected enterprises in hospitality and tourism industry were analyzed for examining CSR activities by the quantitative content analysis. All of the CSR activities in small social enterprises were classified into six dimensions based on the stakeholder theory. Results - The findings of this study provide the characteristics of the 117 small social enterprises and their specific CSR initiatives. A total of eight main business lines were identified: 1) fair travel, 2) leisure/sports, 3) accommodation/camping, 4) medical tourism, 5) exhibitions/art events/cultural events, 6) leisure activities for vulnerable social groups, 7) Korean traditional culture, and 8) ecotourism/agricultural tourism. The CSR initiatives were classified into six dimensions: 1) environment, 2) employment, 3) multicultural families and vulnerable social groups, 4) local community, 5) economic prosperity, and 6) product. Conclusions - This study revealed the special CSR initiative examples of small enterprises in hospitality and tourism industry. Small social enterprises participate in CSR activities mainly related to their own business lines. Moreover, these enterprises are more closely embedded in their local community development, job creation and education for local residents and vulnerable social groups, and traditional heritage preservation. The findings of this study provide theoretical and practical implications and they can contribute to enrich CSR with literature for small enterprises in hospitality and tourism industry.
