A Case of Functional Upper Airway Obstruction Due to Vocal Cord Dysfunction in Obstructive Pulmonary Disease (폐쇄성 폐질환에 동반된 성대 운동이상에 의한 기능성 상기도폐색 1예)

  • A functional upper airway obstruction due to a vocal cord dysfunction(VCD) is characterized by a paradoxical adduction of the vocal cords throughout the respiratory cycle with no obvious organic cause for the obstruction. It commonly occurs paroxysmally and imitates acute asthmatic attacks, often in patients with coexisting asthma. They present with episodes of dyspnea associated with inspiratory wheezing that persists despite conventional asthma treatment and a flattening of the inspiratory limb of the flow-volume curve ; an adduction of the vocal cord during inspiration. Failure to recognize concurrent vocal cord dysfunction and asthma has led not only to the excessive use of bronchodilators and corticosteroids, but also to intubation and tracheostomy. Here, we report a case of coexistent obstructive pulmonary disease and functional upper airway obstruction due to a vocal cord dysfunction where a bronchoscopy showed a paradoxical vocal cord motion and typical features of a variable extrathoracic obstruction and a lower airway obstruction on the Flow-volume loop during a symptomatic period.

Nifedipine Enhances Vasodepressor and Natriuretic Responses to Atrial Natriuretic Peptide in Anesthetized Rats (Nifedipine이 Atrial Natriuretic Peptide의 혈압내림효과에 미치는 영향)

  • The interaction between a calcium channel blocker nifedipine and atrial natriuretic peptide (ANP) was examined in normotensive and renal hypertensive rats. The infusion of either ANP or nifedipine produced a significant decrease in mean arterial pressure (MAP). The combined infusion of ANP with nifedipine resulted in a greater fall of MAP than did the infusion of each drug alone. ANP significantly increased urinary volume and excretion of sodium, while nifedipine was without effects. The diuretic/natriuretic effects of ANP were potentiated by the combined infusion with nifedipine. The vasodepressor and renal effects of ANP or nifedipine were qualitatively similar between the normotensive and hypertensive rats. Nifedipine caused an upward and leftward shift of the ANP dose-relaxation curve of the phenylephrine-precontracted thoracic aortic rings isolated from the normotensive rats , suggesting that the vasodilation sensitivity to ANP is increased in the presence of nifedipine. These results indicate that nifedipine enhances the vasodepressor effect of ANP, the likely mechanisms being attributable to a contraction of effective intravascular volume as a consequence of potentiated renal excretion and a greater peripheral vasodilation.

  • Photoplethysmogram(PPG) is the method to obtain the biomedical signal using the linear relationships between the blood volume for changing the cardiac contraction and relaxation and the amount of light for absorbing the hemoglobin in the blood. In this paper, we proposed the analyzed results which show the heart rate variability and the distribution of heart rate for before and after using PPG. Moreover, this paper designed and implemented the system based on personal computer to predict cardiovascular disease in advance using the analyzed results for the autonomic balance from taking the spectral analysis of heart rate and the state of the blood vessel for analyzing APG(acceleration plethysmogram).

  • The rapid motion of granular material is microscopically observed, and investigated by continuum theory. From the binary collision phenomenon two different times are introduced : flying time and contact time. The former says the non -stationary motion and at a same time the variation of bulk volume. The latter is operative by a delayed time during the contact and describes the elastic properties of granular material. With both times a dynamic constitutive equation is postulated for four state variables : dispersive pressure, viscosity, thermal diffusivity and energy annihilation rate. The balance laws of mass, momentum and energy which are represented through above four variabls, are applied to the model, in which due to the elastic property the relaxation and energy absorption are explained.

Effects of a Short-term Home-based Pulmonary Rehabilitation Program in Patients with Chronic Lung Disease (만성폐질환자를 위한 가정 호흡재활 단기 프로그램의 효과)

  • Purpose: The purpose of this study is to exam the effects of a short-term pulmonary program on lung function, exercise tolerance, and quality of life in chronic lung patients. Method: Randomized controlled pre-post test design was used. The outcome measures were forced expiratory volume in one second (FEV1, % predicted), 6 min walking distance (6MWD), Borg score after 6MWD, and Chronic Respiratory Disease Questionnaire (CRDQ). Experimental group performed the 4-week home-based pulmonary rehabilitation program composed of inspiratory muscle training, upper and lower extremity exercise, relaxation, and telephone visit. Patients in control group were only given education about self-management strategies. Thirty four patients with moderate-to-severe respiratory impairment were recruited, and 28 patients (19 in experiments, 15 in control) completed the study. Result: Significant improvements in lung function, exercise tolerance, and health related quality of life were found only in the experiment group. Conclusion: This study yielded evidence for the potential and beneficial effects of home-based pulmonary rehabilitation program in patients with moderate to severe chronic lung disease. The program could be adequately utilized for improvement of health related quality of life in chronic lung patients.

  • A hierarchical multi-scale modeling strategy devoted to the study of a Bitumen-Bound Gravel (BBG) is presented in this paper. More precisely, the paper investigates the temperature-dependent linear viscoelastic of the material when submitted to low deformations levels and moderate number of cycles. In such a hierarchical approach, 3D digital Representative Elementary Volumes are built and the outcomes at a scale (here, the sub-mesoscale) are used as input data at the next higher scale (here, the mesoscale). The viscoelastic behavior of the bituminous phases at each scale is taken into account by means of a generalized Maxwell model: the bulk part of the behavior is separated from the deviatoric one and bulk and shear moduli are expanded into Prony series. Furthermore, the viscoelastic phases are considered to be thermorheologically simple: time and temperature are not independent. This behavior is reproduced by the Williams-Landel-Ferry law. By means of the FE simulations of stress relaxation tests, the parameters of the various features of this temperature-dependent viscoelastic behavior are identified.

Photoluminescence Tuning of Porous Silicon by Electrochemical Etching in Mixed Electrolytes

  • We have systematically studied the evolution of the photoluminescence(PL) tuning of porous silicon(PS) by electrochemical etching in various mixed electrolytes. The electrolytes employed as an etchants were mixtures of HF:CH$_3$COOH:HNO$_3$:C$_2$H$\_$5/OH solutions where the composition ratios (%) were varied from 10:1.98:0:88.02 to 10: 1.98:8.4:79.62 under constant concentration of HF and CH$_3$COOH with a total volume of 100 ml. Changes in the surface morphology of the samples caused by variations in the etching process were investigated by scanning electron microscopy (SEM) and atomic force microscopy (AFM). After samples are etched in various mixed electrolytes, FTIR analyses show that there is the non-photoluminescent state and the photoluminescent state simultaneously. The PL spectra show the PL tuning in the ranging from 560 to 700 nm with the increase of HNO$_3$ concentration. An analysis of the subsequent PL relaxation mechanism was carried out by time-correlated single photon counting (TCSPC) method. Based on experimental results, it is assumed that a red shift of the main PL peak position is related to the HNO$_3$ activated formation of silicon oxygen compounds. Therefore, the use of electrolyte mixtures with composition ratios can be obtained adequate and reproducible results for PL tuning.

Study of Water Diffusion in PE-SiO2 Nanocomposites by Dielectric Spectroscopy

  • In recent years, researchers have extensively investigated polymers filled with inorganic nanoparticles because these materials present improved physical properties relative to those of conventional unfilled polymers. Oxides, silica in particular, are the most commonly used inorganic particles because they possess good properties and can be fabricated at a low cost. However, oxides are hydrophilic in nature, and this leads to the presence of water at the interface between the nanoparticles and the polymer matrix. Due to the predominance of particle-matrix interfaces in nanocomposites, the presence of water at the interlayer region can be problematic. Moreover, the hydrophobic nature of most polymers, particularly for polyolefins such as polyethylene, may make it difficult to remove this interfacial water. In this paper, as-received and moistened samples of agglomerated nanosilica/polyethylene were dried using an isothermal treatment at $60^{\circ}C$, and the efficacy of this treatment was studied using dielectric spectroscopy. The Maxwell-Wagner-Sillars relaxation peaks were observed to shift to lower frequencies by three decades, and this was linked to a modification of the water content, due to drying, at the interfaces between silica and polyethylene and at the interfaces within the nanosilica agglomerates. The evolution of the extracted retardation time is explained by the nanosilica hydrophily and the free volume introduced by the nanoparticles.

  • A two-dimensional unsteady, inviscid flow solver has been developed for the simulation of airfoil-vortex interactions on unstructured dynamically adapted meshes. The Euler solver is based on a second-order accurate implicit time integration using a point Gauss-Seidel relaxation scheme and a dual time-step subiteration. A vertex-centered, finite-volume discretization is used in conjunction with the Roe's flux-difference splitting. An unsteady solution-adaptive dynamic mesh scheme is used by adding and deleting mesh points to take account of both spatial and temporal variations of the flow field. The effect of vortex interaction on the aeroelastic displacement of an airfoil attached to the idealized two degree-of-freedom spring system is investigated.
