PU.1 Is Identified as a Novel Metastasis Suppressor in Hepatocellular Carcinoma Regulating the miR-615-5p/IGF2 Axis

  • Invasion and metastasis is the major cause of tumor recurrence, difficulty for cure and low survival rate. Excavating key transcription factors, which can regulate tumor invasion and metastasis, are crucial to the development of therapeutic strategies for cancers. PU.1 is a master hematopoietic transcription factor and a vital regulator in life. Here, we report that, compared to adjacent non-cancerous tissues, expression of PU.1 mRNA in metastatic hepatocellular carcinoma (HCC), but not primary HCC, was significantly down-regulated. In addition, levels of PU.1 mRNA in metastatic hepatoma cell lines MHCC97L and MHCC97H were much lower than in non-metastatic Hep3B cells. Transwell invasion assays after PU.1 siRNA transfection showed that the invasion of hepatoma cell lines was increased markedly by PU.1 knockdown. Oppositely, overexpression of PU.1 suppressed the invasion of these cells. However, knockdown and overexpression of PU.1 did not influence proliferation. Finally, we tried to explore the potential mechanism of PU.1 suppressing hepatoma cell invasion. ChIP-qPCR analysis showed that PU.1 exhibited a high binding capacity with miR-615-5p promoter sequence. Overexpression of PU.1 caused a dramatic increase of pri-, pre- and mature miR-615-5p, as well as a marked decrease of miR-615-5p target gene IGF2. These data indicate that PU.1 inhibits invasion of human HCC through promoting miR-615-5p and suppressing IGF2. These findings improve our understanding of PU.1 regulatory roles and provided a potential target for metastatic HCC diagnosis and therapy.

Effects of Definitive Chemoradiotherapy on Respiratory Function Tests and Quality of Life Scores During Treatment of Lung Cancer

  • Background: Chemoradiotherapy is an important treatment modality for lung cancers. The aim of this study was to investigate alterations in, as well as the interrelationship between, lung function and quality of life of patients receiving chemoradiotherapy due to locally advanced non-small cell lung cancer (NSCLC) and small cell lung cancer (SCLC) limited to the thorax. Materials and Methods: The study included patients receiving definitive chemoradiotherapy for lung carcinoma. The respiratory function of the patients was assessed by measuring forced expiratory volume in 1 s per unit (FEV1) and forced expiratory volume in 1s per unit of vital capacity (FEV1/VC) before, in the middle of and after treatment. During the study, EORTC QLQ C30 and LC13 questionnaires developed by the Committee of the European Organization for Research and Treatment of Cancer (EORTC) were employed to evaluate the quality of life on the same day as respiratory function tests (RFT). Findings: The study included 23 patients in total: 19 (82.6%) diagnosed with NSCLC and 4 (17.4%) with SCLC. The average percentage FEV1 was $55.6{\pm}21.8%$ in the pre-treatment period, $56.2{\pm}19.2%$ in the middle of treatment and $60.4{\pm}22%$ at the end of treatment. The improvement in functional scores, symptom scores and general health scores during treatment was not statistically significant (P= 0.568, P= 0.734, P= 0.680, P=0.757 respectively). Conclusions: Although this study showed an improvement in respiratory function and quality of life of patients during treatment with thoracic chemoradiotherapy, no statistically significant results were obtained. While evaluating the effectiveness of treatments for lung carcinoma, the effects of treatment on respiratory function and quality of life should be considered.

한국인 흡연자들의 담배 물질 대사 효소의 유전자 다형성에 따른 폐기능 차이 (Difference in Lung Functions according to Genetic Polymorphism of Tobacco Substance Metabolizing Enzymes of Korean Smokers)

  • 흡연자들의 흡연 물질 대사효소의 유전적 다형성에 따른 폐기능의 차이를 보기 위하여 질병력과 정신과적 병력이 없는 신체적·정신적으로 건강한 만 20~27세 이하의 흡연자 31명( 남 29, 여 3)을 대상으로 연구를 진행하였다. 폐활량 측정기(Wright Respirometer, Ferraris Development and Engineering Co, Ltd, UK)를 이용하여, 노력성 폐활량(Forced vital capacity, FVC), 1초간 노력성 호기량(Forced expiratory volume at one second, FEV 1), 1초간 노력성 호기량의 노력성 폐활량에 대한 비(FEV1 % FVC)을 측정하였으며, 유전자 검사는 DNA로 PCR하여 CYP1A1과 TP53의 유전자 발현검사를 하였다. 실험결과 유전자 돌연변이형이 없는 TT와 Arg/Arg의 폐기능 평균값이 가장 높았으며, CYP1A1와 lung functions의 ANOVA 분석에서 FVC의 P-값이 0.049로 그룹 간의 차이가 있는 것으로 나타났다. 즉 담배성분의 대사 활성화와 연관이 많은 Cytochrome P-450 1A1 (CYP1A1) 유전자의 돌연변이형이 없을때 FVC의 값이 높게 나타난 것이다.

기관지확장증의 환기역학 (Ventilatory Dynamics in Bronchiectasis)

  • 연구배경 : 기관지 확장증은 비가역적 병변으로 만성적이고 반복적인 폐기관지의 감염으로 인하여 많은 예에서 만성기관지염, 폐기종 및 기관지천식등이 동반되는 경우가 많다. 따라서 이들에서의 폐기능도 동반되는 질환이나 침범범위등에 따라서 다양하리라 생각이 되어 기관지 확장증의 병리해부학적 형태의 차이, 그리고 호흡곤란의 정도에 따른 폐기능의 차이를 조사하였다. 방법 : 1985년 1월부터 1991년 12월까지 경북대학병원에서 기관지 조영술사진에 의해 기관지확장증으로 확진된 93예를 대상으로 폐활량, 최대환기량, 노력성호기곡선 및 기류-용량 곡선에서 계측할 수 있는 각종지표와 closing volume을 구하여 기관지조영술상의 낭종형, 원통형 및 혼합형 등의 기관지확장증의 형태 및 Hugh-Jones의 호흡곤란의 정도에 따른 성적을 비교검토 하였다. 결과: 1) 이환된 폐구역수의 평균은 원통형(tubular), 낭종형(saccular) 및 혼합형(mixed) 에서 각각 4.7, 6.9, 7.8개였고 폐활량 및 노력성 호기곡선의 모든 계측치와 PEF를 제외한 호기류속도를 반영하는 계측치는 낭종형 및 혼합형에서 저명하게 감소하였다. 2) MVV는 낭종형 및 혼합형에서 감소하는 경향이었으며 CV/VC는 낭종형 및 혼합형에서 증가하는 경향이었다. 3) 호흡곤란의 정도가 심할수록 이환된 폐구역수는 유의하게 증가되었으며 폐환기능은 이와 비례하여 저하하였다. 결론 : 이상의 결과로 기관지확장증에서의 환기기능은 제한성환기 장애와 폐쇄성환기 장애가 동반되어 있으며 이는 호흡곤란의 정도가 심할수록, 낭종형 및 혼합형일수록 뚜렷하였다.

Azithromycin 투여로 폐기능이 호전된 폐쇄성 세기관지염 1예 (Improvement of Pulmonary Function after Administration of Azithromycin in a Patient with Bronchiolitis Obliterans: a Case Report)

  • 폐쇄성 세기관지염은 골수 이식 후 폐에 발생하는 합병증이다. 현재 스테로이드와 면역 억제제를 투여하여 적극적인 치료를 하더라도 폐기능의 호전을 보이는 경우는 일부에 불과하다. 저자들은 기존의 치료에도 불구하고 호전을 보이지 않은 골수 이식 후 발생한 폐쇄성 세기관지염 환자에서 macrolide계 항생제인 azithromycin을 1년간 경구 투여하여 폐기능이 호전됨을 경험하였기에 문헌고찰과 함께 보고하는 바이다.

강우 이벤트가 태화산 잣나무 식재림의 각 발생원별 $CO_2$ 발생량에 미치는 영향 (The Effect of Rain Fall Event on $CO_2$ Emission in Pinus koraiensis Plantation in Mt. Taehwa)

  • 본 연구는 몬순기후에 의해 장마라는 특징적인 강우패턴을 가지는 환경조건에서 강우 패턴 및 강우 강도의 변화가 삼림 내 지표 유출수와 토양수분함량을 변화시켜 토양호흡의 배출 양상을 어떻게 변화시키는지 알아보기 위하여 수행되었다. 대상지는 경기도 광주시에 위치한 서울대학교 학술림 내 잣나무식재림으로 2012년 1년 동안 자동 토양호흡 쳄버를 이용하여 연변화를 측정하였고 단근처리 기법을 이용하여 토양호흡 중 약 24%가 뿌리호흡에서 기인한 것으로 추정되었다. 이와 더불어 장마와 태풍으로 인한 다양한 강도의 강우가 발생한 7월 말부터 9월 말까지 휴대용 토양호흡 측정기를 이용하여 관측지 내 단근처리구와 단근 및 강우차단처리구에서 종속영양생물호흡을 측정하였다. 토양수분함량의 경우 고강도의 강우가 며칠씩 이어지더라도 20%를 초과하지 않았다. 이는 강수가 상대적으로 적은 시기에는 수목활성이나 토양미생물의 활성에 영향을 미칠 것으로 추정된다. 강우 강도가 10 mm 이하의 저강도 강우에서는 종속영양생물호흡이 약 14.4% 상승하였으나 10 mm 이상의 고강도 강우조건에서는 종속영양생물호흡이 25.5% 감소하였다. 이는 강우 강도에 따라 종속영양생물호흡의 배출 양상이 달라짐을 보여주었다.

호흡운동을 병행한 몸통 근력운동과 심부 안정화 운동이 배근육 두께와 호흡에 미치는 영향 (Effect of Trunk Strength Exercise and Deep Stabilization Exercise Combined with Breathing Exercise on Abdominal Muscle Thickness and Respiration)

  • Purpose : The purpose of this study is to compare the effects on abdominal muscle thickness and breathing by applying trunk strength exercise and deep stabilization exercise along with breathing exercise, which is the main respiratory muscle during breathing, to present an efficient exercise method with diaphragm breathing. Methods : This study was performed on normal 6 females and 14 males subjects. They were divided into 2 groups which trunk strength exercise and deep stabilization exercise group. The trunk strength exercise group (TSE) attended prone press-up, crunch and pelvic tiling. The deep stabilization exercise group (DSE) attended abdominal drawing, horizontal side-support and bridging exercise. Breathing exercise was performed for each set break time for 1 minute. Results : First, in the comparison of the change in the thickness of the abdominal muscle between the trunk strength training group and the deep stabilization group before and after exercise, there was a statistically significant difference in the comparison of transverse abdominis (TrA), rectus femoris (RF), external oblique (EO), internal oblique (IO) (p<.05). However, there was no significant difference in any comparison between groups (p>.05). Second, in the comparison of changes in respiratory function between the trunk strength exercise group and the deep stabilization exercise group before and after exercise, there were statistically significant differences in the exerted forced vital capacity (FVC), forced expiratory volume at one second (FEV1), peak expiratory flow (PEF) in the comparison before and after the experiment (p<.05). However, there was no significant difference in any comparison between groups (p>.05). Conclusion : As a result of this study, it can be said that both trunk strength exercises and deep stabilization exercises along with diaphragm breathing are exercises that strengthen deep and superficial muscles, and have a positive effect on breathing function as well as muscle strength. However, it is not known which exercise was more effective, and because it was combined with breathing exercise, the interference effect appeared.

The Effect of Body Composition on Pulmonary Function

  • Background: The pulmonary function test is the most basic test method to diagnosis lung disease. The purpose of this study was to research the correlation of the body mass index (BMI), the fat percentage of the body mass (fat%), the muscle mass, the fat-free mass (FFM) and the fat-free mass index (FFMI), waist-hip ratio (WHR), on the forced expiratory volume curve. Methods: Between March and April 2009, a total of 291 subjects were enrolled. There were 152 men and 139 female (mean age, $46.3{\pm}9.92$ years), and they were measured for the following: forced vital capacity (FVC), forced expiratory volume at 1 second ($FEV_1$), and forced expiratory flow during the middle half of the FVC ($FEF_{25-75}$) from the forced expiratory volume curve by the spirometry, and the body composition by the bioelectrical impedance method. Correlation and a multiple linear regression, between the body composition and pulmonary function, were used. Results: BMI and fat% had no correlation with FVC, $FEV_1$ in male, but FFMI showed a positive correlation. In contrast, BMI and fat% had correlation with FVC, $FEV_1$ in female, but FFMI showed no correlation. Both male and female, FVC and $FEV_1$ had a negative correlation with WHR (male, FVC r=-0.327, $FEV_1$ r=-0.36; p<0.05; female, FVC r=-0.175, $FEV_1$ r=-0.213; p<0.05). In a multiple linear regression of considering the body composition of the total group, FVC explained FFM, BMI, and FFMI in order ($r^2$=0.579, 0.657, 0.663). $FEV_1$ was explained only fat% ($r^2$=0.011), and $FEF_{25-75}$ was explained muscle mass, FFMI, FFM ($r^2$=0.126, 0.138, 0.148). Conclusion: The BMI, fat%, muscle mass, FFM, FFMI, WHR have significant association with pulmonary function but $r^2$ (adjusted coefficient of determination) were not high enough for explaining lung function.

결핵성 파괴폐의 흉부 전산화단층촬영 소견 및 폐기능과의 상관관계 (CT Radiologic Findings in Patients with Tuberculous Destroyed Lung and Correlation with Lung Function)

  • Background: A tuberculous destroyed lung is sequelae of pulmonary tuberculosis and causes various respiratory symptoms and pulmonary dysfunction. The patients with a tuberculous destroyed lung account for a significant portion of those with chronic lung disease in Korea. However, few reports can be found in the literature. We investigated the computed tomography (CT) findings in a tuberculous destroyed lung and the correlation with lung function. Methods: A retrospective analysis was carried out for 44 patients who were diagnosed with a tuberculous destroyed lung at the Keimyung University Dongsan Hospital between January 2004 and December 2009. Results: A chest CT scan showed various thoracic sequelae of tuberculosis. In lung parenchymal lesions, there were cicatrization atelectasis in 37 cases (84.1%) and emphysema in 13 cases. Bronchiectasis (n=39, 88.6%) was most commonly found in airway lesions. The mean number of destroyed bronchopulmonary segments was 7.7 (range, 4~14). The most common injured segment was the apicoposterior segment of the left upper lobe (n=36, 81.8%). In the pulmonary function test, obstructive ventilatory defects were observed in 31 cases (70.5%), followed by a mixed (n=7) and restrictive ventilatory defect (n=5). The number of destroyed bronchopulmonary segments showed a significant negative correlation with forced vital capacity (FVC), % predicted (r=-0.379, p=0.001) and forced expiratory volume in one second ($FEV_1$), % predicted (r=-0.349, p=0.020). After adjustment for age and smoking status (pack-years), the number of destroyed segments also showed a significant negative correlation with FVC, % predicted (B=-0.070, p=0.014) and $FEV_1$, % predicted (B=-0.050, p=0.022). Conclusion: Tuberculous destroyed lungs commonly showed obstructive ventilatory defects, possibly due to bronchiectasis and emphysema. There was negative correlation between the extent of destruction and lung function.

체력진단 평가를 통한 뇌성마비 아동과 정상아동의 운동능력 비교연구 (Comparison of Motor Fitness of Cerebral Palsy Chidren with normal throug Phyisical Fitness Diagnosis Evaluation)

  • The aim of this study is to compared the cerebral palsy children with normal children in the exercise and cardio vascular ability after this study as the fundamental data fer mating programs for the cerebral palsy children. The test of this study is about twenty girls, the control group of the normal children(n=10) and the experiment group of cerebral palsy children(n=10). They were studied in four aspects which were the anthropometry, the medical check in the rest, the physical fitness or exercise roads test and the change of the target heart rate during exercise. The result were as follows : 1. The characteristic of the physical type : The control group is higher than the experiment group in the standing height and the body weight but their's little difference between them (p<0.05). The control group is lower than the experiment group in the body fat. 2. The characteristic of the medical check in the rest : The control group is higher than the experiment group in the vital capacity and flood expiratoryvolume one second. The control group is higher that the experiment group in the blood pressure of systolic and Diastolic. There's little difference between them(p<0.01). The control is lower than the experiment group in the heart pulse rate. There's little difference between them(p<0.005). 3. The characteristic of basic physical strength evaluation : The experiment group is the lowest dynamic muscular endurance, balance, agility and endurance which need to move the body with weight. The control group is much higher than the experiment group in the flexibility and muscle strength(Back strength). There's no difference between them(p<0.05). 4. The characteristics of the exercise stress last : The control group is higher than the experiment group in the endurance, the maximum of oxygen intake, endurance level and the out take of calory. There's little difference between them(p<0.01). 5. The characteristic of the change of the target heart rate during exercise : The control group is lower than the experiment group exchange of target heart rate, There's no difference them.
