Endoscopic Spinal Surgery for Herniated Lumbar Discs

  • Objective : So called "minimally invasive procedures" have evolved from chemonucleolysis, automated percutaneous discectomy, arthroscopic microdiscectomy that are mainly working within the confines of intradiscal space to transforaminal endoscopic technique to remove herniated epidural disc materials directly. The purpose of this study is to assess the result of endoscopic spinal surgery and favorable indication in the thoracolumbar spine. Methods : The records of 71 patients, 73 endoscopic procedures, were retrospectively analysed. Yeung Endoscopic Spine Surgery system with 7 mm working sleeve and $25^{\circ}$ viewing angle was used. The mean follow up period was 6 months [range, 3-9]. Results : Operated levels were from T12-L1 disc down to L5-L6 of S1 disc. Of 71 cases, 2 patients underwent transforaminal endoscopic surgery twice due to recurrence after initial operation. MacNab's criteria was used to assess the outcome. Favorable outcome, excellent of good, was seen in 78% [57 procedures] of the patients. Among 11 fair outcomes, only 1 procedure was followed by secondary open procedure, laminectomy with discectomy. Two of 5 poor outcomes were operated again by same procedure which resulted in fair outcomes. One patient with aggravated cauda equina syndrome remained poor and a lumbar fusion procedure was performed in other patient with poor outcome. There were 2 postoperative discitis that were treated with conservative care in one and anterior lumbar interbody fusion in the other. Conclusion : Evolving technology of mechanical, visual instrument enables minimal invasive procedure possible and effective. The transforaminal endoscopic spinal surgery can reach as high as T12-L1 disc level. The rate of favorable outcome is mid-range among reported endoscopic lumbar surgery series. Authors believe that the outcome will be better as cases accumulate and will be able to reach the fate of standard open microsurgery.

A Proposal of Output Method of Round Window Stimulation Type Middle Ear Implants using Acoustic Transmission (공기 전도형 출력을 갖는 정원창 자극형 인공중이의 출력방식 제안)

  • In order to broaden the indication of middle ear implant, research has been actively conducted on the reverse output method that stimulates the round window. However, it is very difficult to transmit the vibration output effectively because the indivisual anatomical difference of the round window niche is very large and also the visual field is not secured even by a skilled otolaryngologic surgeon. In this paper, we propose a new reverse stimulation method of middle ear implants that transmits energy to the inner ear by using air as a medium. This can compensate for the disadvantages of the conventional method of transmitting vibration energy and minimizes the energy transfer efficiency interference due to the combination of the excitation point and the output device. It was shown that forward and backward transfer characteristics were obtained by cadaveric experiments, and it was shown that it can overcome the acoustical impedance of high round window and transmit energy to inner ear. The receiver, which is the output device of the conventional hearing aids, can generate a constant volume velocity, so it can have a high output at a limited volume, such as a round window niche. So, suggested method can overcome the high acoustical impedance of the round window and deliver acoustic energy to the inner ear.

  • Until now, there have been various kinds of science information databsae which databased the science technology information, but they do not satisfy the aspiration of the users. Therefore, in the position of the users, it suggests the technology information space as a now paradigm, which supplement the function of science information DB. ICPIS which inputs described papers with keywords, offers the itemized summary of these contents, the visual indication and comparison of similar thesis, and it also supplises the abundant summary information, survey information, more than ten volumes of info communication thesis with starting the casual relation extraction for the users, playing a significant role in ICPIS is called KP, and it is package of domain knowledge that unifies the extraction and structure narration of the technology information. ICPIS extracts the technology information among the thesis that are deserved by the natural language treatment in the itemized KP keywords described, and form the prescribed summary structure in KP.

The Two Cases Report of Bee Venom Injection on Patient with Low Back Pain Maintaining after Heating-Conduction Acupuncture Therapy (전열침 시술에 호전되지 않는 요통환자에 대한 봉독약침 치료 증례보고 2례)

  • Objectives : According to previous reports, heating-conduction acupuncture treatment is very effective for sprain and strain of ligament. But there was no report about pain aggravation by heating-conduction acupuncture and relief from bee venom injection. In this article, we report two cases of pain aggravation by heating-conduction acupuncture treatment and relief from bee venom injection. Methods : We used heating-conduction acupuncture treatment on possible sprain of sacroiliac ligament diagnosed by physical examination. Outcomes were measured by Visual Analogue Scale(VAS), Range of Motion(ROM) and Belt test, Goldthwait test. Results : The patient has shown an aggravation on pain and ROM. And after we use subcutaneous bee venom injection, pain has relieved and ROM improved. Belt test, Goldthwait test also changed from positive to negative. Conclusions : In this report, we assured the effect of Bee venom injection. Further well-designed, controlled studies and more cases are needed to differentiate the indication of heating-conduction acupuncture and bee venom injection.

Reduced wavelet component energy-based approach for damage detection of jacket type offshore platform

  • Identification of damage has become an evolving area of research over the last few decades with increasing the need of online health monitoring of the large structures. The visual damage detection can be impractical, expensive and ineffective in case of large structures, e.g., offshore platforms, offshore pipelines, multi-storied buildings and bridges. Damage in a system causes a change in the dynamic properties of the system. The structural damage is typically a local phenomenon, which tends to be captured by higher frequency signals. Most of vibration-based damage detection methods require modal properties that are obtained from measured signals through the system identification techniques. However, the modal properties such as natural frequencies and mode shapes are not such good sensitive indication of structural damage. Identification of damaged jacket type offshore platform members, based on wavelet packet transform is presented in this paper. The jacket platform is excited by simple wave load. Response of actual jacket needs to be measured. Dynamic signals are measured by finite element analysis result. It is assumed that this is actual response of the platform measured in the field. The dynamic signals first decomposed into wavelet packet components. Then eliminating some of the component signals (eliminate approximation component of wavelet packet decomposition), component energies of remained signal (detail components) are calculated and used for damage assessment. This method is called Detail Signal Energy Rate Index (DSERI). The results show that reduced wavelet packet component energies are good candidate indices which are sensitive to structural damage. These component energies can be used for damage assessment including identifying damage occurrence and are applicable for finding damages' location.

A Study on the Contact Allergic Skin Sensitization of 0.3% DA-5018 Cream, a Non-narcotic Analgesic Agent, with Guinea Pig Maximization Test (Guinea Pig Maximization Test를 이용한 0.3% DA-5018 cream의 접촉성 앨러지에 대한 연구)

  • The aim of this study was to assess the allergenic potential of 0.3% DA-5018 cream, a non-narcotic analgesic agent, using a guinea pig maximization test. Five male and female guinea pigs in the experimental group were sensitized in two steps. First, ,0.3% DA-5018 cream was injected intradermally, and 7 days later, the material was applied topically. After another 2 weeks test material was applied, the skin response was evaluated by visual observation. Five male and female guinea pigs served as cream base group, negative(ultreated) group or positive (2,4-dinitrochlorobenzene, DNCB) group, respectively. 0.3% DA-5018 cream provoked slight erythema in 1 out of 5 cases in male and female guinea pigs sensitized with 0.3% DA-5018 cream or cream base. The animals challenged with cream base also showed slight erythema in 1/5 female guinea pig sensitized with 0.3% 3A-5018 cream or 2/5 male guinea pjgs sensitized with cream base, respectively. Histologically, however, no indication of skin sensitization was observed in all of these cases. The positive control group was sensitized with 0.1% DNCB suspended in olive oil and challenged with 0.01% and 0.1% DNCB ointment, all the animal showed remarkable skin reactions and obvious skin sensitization reactions in a dose dependent manner. From the challenge test it was evident that 0.3% DA-5018 cream did not elicit positive skin reaction interpreted as delayed hypersensitivity reactions, compared with cream base or untreated control group. These findings indicate that allergenic side effects by 0.3% DA-5018 cream is not likely in the clinical use.

  • By analyzing abstract book cover design, this study aims to examine the applicability of the semiotic methodology of Roland Barthes and to conduct a preference survey of how people would accept it. The selected books for the study are and published in Openbooks publishing company to mark the 30th anniversary of the opening. The research results are as follows. First, it was confirmed that Roland Barthes' semiotic approach is a suitable tool for identifying the symbolic phenomena of visual elements that constitute abstract book cover design. Second, people were well informed of what the abstract form of design conveys, but they showed personal differences in preference, making it difficult to achieve clear results. This study, together with the importance of the internal meaning that appears in the book cover design, gives graphic designers an indication that interaction between the communicator and the acceptor should be sought.

  • Objectives : The purpose of this study was to report effectiveness of oriental medical treatments to symptoms and quality of life in the patients with herniated lumbar disc being suggested operation. Methods : Eighteen patients diagnosed with herniated lumbar disc by MRI in hospital were treated by oriental medical treatments. Symptoms of the patients were assessed by visual analogue scale(VAS) and SF-36. Results : After the treatments, the VAS of symptoms of herniated lumbar disc and the score of role-physical scale in SF-36 were improved but the other scores of scales were not significantly different. Conclusions : It could be suggested that oriental medical treatments were effective for the patients with herniated lumbar disc being suggested operation. Further study of the effect of oriental medical treatments on herniated lumbar disc is needed.

Comparison of clinical efficacy of ropivacaine and lignocaine with adrenaline for implant surgery anesthesia: a split-mouth randomized controlled clinical trial

  • Background: The primary indication for using long-acting anesthetics in dentistry is extensive dental procedures that require pulpal anesthesia beyond 90 min and management of postoperative pain. Ropivacaine is an amide local anesthetic that is available at various concentrations with inherent vasoconstrictive properties at low concentrations. Ropivacaine has a 75% greater margin of safety than bupivacaine. Ropivacaine can be a good alternative to bupivacaine as a local anesthetic in dental implant surgery as it provides a longer duration of both pulpal and soft tissue anesthesia after mandibular nerve block and lowers CNS and cardiovascular toxicity. This study aimed to evaluate and compare the clinical efficacy of ropivacaine and lignocaine for implant surgery anesthesia. Methods: Fifteen patients with bilateral edentulous sites indicated for implant placement were recruited for this study. Patients aged 20-60 years of both sexes were randomly recruited. Thirty implant placements were performed in the test and control groups using ropivacaine and lignocaine with adrenaline as local anesthetics, respectively. Results: The results were analyzed statistically. The duration of anesthesia was significantly higher in the test group than in the control group. Ropivacaine was found to be superior to lignocaine in terms of the quality of anesthesia. The comparison of mean visual analog scale scores showed ropivacaine to have better anesthetic and analgesic effects than the control group. Conclusion: Ropivacaine 0.75% provides a significantly longer duration of anesthesia than lignocaine 2% with adrenaline. Ropivacaine 0.75% decreased intraoperative and postoperative analgesia compared to lignocaine 2% with adrenaline. Hence, ropivacaine 0.75% can be used as an alternative to lignocaine in implant surgeries and other intraoral surgical procedures that require a longer duration of anesthesia and analgesia.

Utilization of Immersion-Drying Method for Measuring Damage Depth of Fire-Damaged High-Strength Concrete (화재로 손상된 고강도 콘크리트의 손상 깊이 측정을 위한 수중침지 건조방법의 활용)

  • This study presents a novel approach for evaluating fire-induced damage depth in concrete. The methodology leverages the principle that exposure to high temperatures causes internal expansion within concrete, leading to increased voids and microcracks in the damaged zones. This heightened porosity results in greater absorption rates compared to undamaged areas. By immersing fire-damaged concrete samples in water and subsequently monitoring the drying process, the depth of damage can be assessed. Differences in drying rates and color variations between damaged and undamaged areas serve as visual indicators for determining the extent of the damage. Experimental results from this water immersion method revealed damage depths of 38.7mm and 37.5mm for two different concrete mixtures. These measurements notably surpass the damage depths estimated using traditional phenolphthalein-based methods. This discrepancy suggests that utilizing the absorption rate principle, which is directly linked to the physical changes caused by thermal expansion, offers a more accurate and sensitive assessment of fire damage depth compared to methods relying solely on the presence of Portlandite for colorimetric indication.
