The status of breast cancer screening of women at a breast clinic in a small city in Korea-Using medical records Purpose: This study was conducted to investigate breast cancer incidence by age, including those under 40, and to emphasize the importance of early breast cancer screening for young breast cancer patients. Method: The study was designed as a retrospective survey method. We investigated the medical charts of 483 patients diagnosed with breast cancer among 23,200 visitors to local breast clinic from May 1, 2010 to April 30, 2020. Results: The average age of 483 patients was 47.9, with 36% in their 40s and 28.6% under 40. Among the patients, 5.4% had a family history of breast cancer, and 70.8% were pre-menopausal. The most common reason for visiting the clinic was the mass(54.2%). The size of tumors was less than 2cm (56%), followed by 2-5cm (43.4%) and more than 5cm (5.6%). The size of tumors was smaller in the asymptomatic case than in the symptomatic case. In the pathologic results, invasive ductal cancer was the most common at 80.3%. Conclusions: Breast cancer screening program should be activated even for young women under the age of 40. We suggest educating the importance of breast cancer screening, and lowering the age of national breast cancer screening program.
Elderly people want to live in the community even if they are in poor health. However, there is no integrated care support system suitable for the health and functional status of the elderly. So the elderly are choosing living facilities. The purpose of this study is to explore exploring whether the elderly are applying for a long-term care certification. First, mild geriatric patients were mainly using home care services such as visitation care. However, some of the milder elderly were enrolled in nursing homes. And the service that does not fit the functional status of the elderly is used. Second, it is concentrated on the use of visiting care services. Third, elderly people outside the class did not receive sufficient help for daily life, and the use of community welfare services such as the elderly welfare center was low. As a result, long-term care admitters are not able to continue to live in the community even though their health and functioning status is mild, and elderly people out of grade are unable to properly use the necessary community care services. The condition is likely to deteriorate.
This study aimed to investigate factors affecting frailty by urinary incontinence groups among the vulnerable elderly woman in Korea. In this secondary analysis, data were collected from records for 3,251 elders registered in the Visiting Health Management program of Public Health Centers in 2012. body mass index, waist circumference, timed up & go, depression, self rated helath, walking exercise, flexibility exercise, strengh exercise and frailty were assessed. Data were analyzed using $x^2$-test, t-test, ANOVA, Pearson's correlation and stepwise regression to determine the associated factors of frailty by urinary incontinence. Depression, walking exercise, timed up & go and age were found to be factors significantly associated with frailty among the elders with incontinence(F=38.321, p<.001). Age, depression, walking exercise, self rated health and tined up & go were found to be factors associated with frailty in the elders without incontinence(F=265.666 p<.001). The findings show that frailty of elders and associated factors were different by urinary incontinence, and common factors affecting frailty were depression, walking exercise, timed up & go, age. Thus, these factors should be considered in the development of intervention program for care and prevention of frailty and program should be modified according to urinary incontinence.

Doctors are consulted only by 23.9% when they are sick whereas 59.7% resorted to the drug stores. The lower the educational attainment of the parents, the lower the rate of visiting clinics. ⑦ 36.7% of their parents pointed out the problems of personality guidance as the most difficult thing at home 71.3% of their parents worried about and unsatisfied with their children's personality traits. Purpose: The purpose of this study is to investigate the current state of the home-based cancer patient management project of public health centers throughout the country. The results of the investigation is employed to identify obstacles to the execution of the program and, finally, to develop an efficient management program of home-based cancer patients. Methods: Data on the home-based cancer patient management project were collected and analyzed through visiting interviews or telephone interviews with 225 public health centers throughout the country for six months from July to December, 2006. Results: Obstacles to the present execution of the home-based cancer patient management project were identified. Some of them are : (1) patients' low trust in cancer patient management by local health centers, (2) absence of programs customized to local communities, (3) lack of personnel and vehicles for home-based cancer patient management, (4) lack of education program for personnel in charge of home-based cancer patient management, (5) problems in public health doctors, weak connection to private medical institutions, (6) absence of medical institutions and hospice facilities for cancer patients, and (7) non-standardized volunteer workers, so on. Considering all these problems, some effective management methods are proposed. The basic concept is to keep the autonomy and variety of the local helath centers. And based on this concept, three models of (1) public health center controlled model, (2) medical institutions and hospice facilities-entrusted model and (3) medical institutions and hospice facilities-cooperative model are developed. Conclusion: By adopting an adequate model among proposed three models, the public health centers are expected to achieve an efficient utilization of material resources and manpower. In addition, by inventing their own programs that are proper for the local societies, they can improve the home-based cancer patient management.
