Coxsackie Virus에 의한 Pancarditis 1례 (One case of Coxsackievirus induced Pancarditis)

  • Myocarditis refers to inflammation, necrosis, or myocytolysis that may be due to many infectious, connective tissue and many other causes affecting the myocardium or involvement of the endocardium or pericardium. The most common manifestation is congestive heart failure, although arrhythmias and sudden death may be the first sign of myocarditis. Viral myocarditis is typically a sporadic but occasionally epidemic illness, noted as an acute potentially fulminant disease of 1-to 4-wk-old infants, as an acute but more benign myopericarditis of toddlers and young children. The most common casuative agent in viral myocarditis is Coxsackievirus and the outcome of the biopsy-proven chronic dilated cardiomyopathy associated with Coxsackievirus is poor without therapy. Myocarditis may be confirmed by percutaneous endomyocardial biopsy and the viral myocarditis may be diagnosed by the serological viral study with the clinical manifestations. He was admitted for the management of tachyarrhythmias occurred suddenly without prodromal symptoms and signs and diagnosed as viral pancarditis by serological Coxsackievirus study, echocardiogram, chest x-ray, EKG and other clinical manifestations.

포유자돈(哺乳仔豚)에 발생(發生)한 Viral Encephalomyelitis의 병리조직학적관찰(病理組織學的觀察) (Histopathological Observations on Viral Encephalomyelitis in Suckling Piglets Raised in Korea)

  • The outbreak of viral encephalomyelitis (Talfan disease) in suckling piglets was diagnosed pathologically for the first time in Korea. The clinical signs of affected sucklings were very high morbidity without death, and fever ($39^{\circ}C$ or higher) with incoordination of the rear limbs and paresis, while no specific gross lesions were observed at autopsy. Histopathological changes were confined to the nervous system, and were of the nature of a nonsuppurative and nondemyelinating encephalomyelitis which is usual for the viral encephalitis. It was characterized by perivascular cuffing, meningitis, neuronal degeneration, neuronophagia and glial nodules in the brain and spinal cord.

Infection Status of Hospitalized Diarrheal Patients with Gastrointestinal Protozoa, Bacteria, and Viruses in the Republic of Korea

  • To understand protozoan, viral, and bacterial infections in diarrheal patients, we analyzed positivity and mixedinfection status with 3 protozoans, 4 viruses, and 10 bacteria in hospitalized diarrheal patients during 2004-2006 in the Republic of Korea. A total of 76,652 stool samples were collected from 96 hospitals across the nation. The positivity for protozoa, viruses, and bacteria was 129, 1,759, and 1,797 per 10,000 persons, respectively. Especially, Cryptosporidium parvum was highly mixed-infected with rotavirus among pediatric diarrheal patients (29.5 per 100 C. parvum positive cases), and Entamoeba histolytica was mixed-infected with Clostridium perfringens (10.3 per 100 E. histolytica positive cases) in protozoan-diarrheal patients. Those infected with rotavirus and C. perfringens constituted relatively high proportions among mixed infection cases from January to April. The positivity for rotavirus among viral infection for those aged $\leq$ 5 years was significantly higher, while C. perfringens among bacterial infection was higher for $\geq$ 50 years. The information for association of viral and bacterial infections with enteropathogenic protozoa in diarrheal patients may contribute to improvement of care for diarrhea as well as development of control strategies for diarrheal diseases in Korea.

아시아에서 분리된 viral haemorrhagic septicaemia virus (VHSV) isolates의 계통분석학적 비교 (Phylogenetic Analysis of Viral Haemorrhagic Septicaemia Virus (VHSV) Isolates from Asia)

  • 바이러스성 출혈성 패혈증을 일으키는 VHSV는 국내의 넙치양식에 심각한 피해를 주고 있다. 본 연구에서는 우리나라에서 분리한 8종의 VHSV 분리주들의 glycoprotein (G)의 염기서열을 분석하여 Korean VHSV 분리주들과 기보고된 일본 및 중국의 분리주들을 계통발생학적으로 비교하여 G-protein의 8 부위(G34, C528, A755, T834, T951, T1147, T1221, T1336)에서 국내 VHSV 분리주들의 특이적인 서열을 확인하였고, 전 세계적으로 분리된 VHSV의 분리주들과의 비교결과 8 부위 중 3 부위 (A755, T834, T1221)의 국내VHSV 분리주 특이적 염기서열들을 확인하였다. 또한 중국 유래 VHSV 분리주의 특이적 염기서열 11 부위 (C498, C545, C548, C642, G648, T678, A738, C804, C834, G851, C1139)와 일본 유래 VHSV 분리주의 특이적 염기서열 1부위 (C179)를 확인하였다. 이상의 연구결과는 국내 특이적인 염기서열을 확보하여, 구역화된 수산생물 질병의 예찰, 모니터링 및 질병 관리에 유용하게 사용될 것으로 사료된다.

A Bivalent Inactivated Vaccine Prevents Enterovirus 71 and Coxsackievirus A16 Infections in the Mongolian Gerbil

  • Hand-foot-and-mouth disease (HFMD) is a viral infectious disease that occurs in children under 5 years of age. Its main causes are coxsackievirus (CV) and enterovirus (EV). Since there are no efficient therapeutics for HFMD, vaccines are effective in preventing the disease. To develop broad coverage against CV and EV, the development of a bivalent vaccine form is needed. The Mongolian gerbil is an efficient and suitable animal model of EV71 C4a and CVA16 infection used to investigate vaccine efficacy following direct immunization. In this study, Mongolian gerbils were immunized with a bivalent inactivated EV71 C4a and inactivated CVA16 vaccine to test their effectiveness against viral infection. Bivalent vaccine immunization resulted in increased Ag-specific IgG antibody production; specifically, EV71 C4a-specific IgG was increased with medium and high doses and CVA16-specific IgG was increased with all doses of immunization. When gene expression of T cell-biased cytokines was analysed, Th1, Th2, and Th17 responses were found to be highly activated in the high-dose immunization group. Moreover, bivalent vaccine immunization mitigated paralytic signs and increased the survival rate following lethal viral challenges. When the viral RNA content was determined from various organs, all three doses of bivalent vaccine immunization were found to significantly decrease viral amplification. Upon histologic examination, EV71 C4a and CVA16 induced tissue damage to the heart and muscle. However, bivalent vaccine immunization alleviated this in a dose-dependent manner. These results suggest that the bivalent inactivated EV71 C4a/CVA16 vaccine could be a safe and effective candidate HFMD vaccine.

An Antiviral Mechanism Investigated with Ribavirin as an RNA Virus Mutagen for Foot-and-mouth Disease Virus

  • To prove whether error catastrophe /lethal mutagenesis is the primary antiviral mechanism of action of ribavirin against foot-and-mouth disease virus (FMDV). Ribavirin passage experiments were performed and supernatants of $Rp_1$ to $Rp_5$ were harvested. Morphological alterations as well as the levels of viral RNAs, proteins, and infectious particles in the BHK-21 cells infected using the supernatants of $Rp_1$ to $Rp_5$ and control were measured by microscope, real-time RT-PCR, western-blotting and plaque assays, respectively. The mutation frequency was measured by sequencing the complete P1- and 3D-encoding region of FMDV after a single round of virus infection from ribavirin-treated or untreated FMDV-infected cells. Ribavirin treatment for FMDV caused dramatically inhibition of multiplication in cell cultures. The levels of viral RNAs, proteins, and infectious particles in the BHK-21 cells infected were more greatly reduced along with the passage from $Rp_1$ to $Rp_5$, moreover, nucleocapsid protein could not be detected and no recovery of infectious virus in the supernatant or detection of intracellular viral RNA was observed at the $Rp_5$-infected cells. A high mutation rate, giving rise to an 8-and 11-fold increase in mutagenesis and resulting in some amino acid substitutions, was found in viral RNA synthesized at a single round of virus infection in the presence of ribavirin of $1000\;{\mu}M$ and caused a 99.7% loss in viral infectivity in contrast with parallel untreated control virus. These results suggest that the antiviral molecular mechanism of ribavirin is based on the lethal mutagenesis/error catastrophe, that is, the ribavirin is not merely an antiviral reagent but also an effective mutagen.

양식 넙치, Paralichthys olivaceus에서 분리한 viral hemorrhagic septicemia virus (VHSV)의 항원성 분석 (Analysis of antigenicity of viral hemorrhagic septicemia virus (VHSV) glycoprotein from cultured olive flounder Paralichthys olivaceus)

  • 우리나라 양식 넙치에서 분리한 VHSV (KR'01-1)의 glycoprotein 아미노산 배열을 기초로 단백질 전환 구조상 유연성, 친수성 및 항원성이 높고, 표면에 노출되어 있을 가능성(surface probability)이 높다고 분석되는 3개의 peptide (Gp1, Gp2, Gp3)를 선정하였다. 정제한 바이러스 입자 및 3개의 합성 peptide에 대한 polyclonal 항체를 제작하여 항원성 분석을 실시한 결과, 합성 peptide로 제작한 모든 항혈청은 Western blotting 결과 VHSV의 구조단백질과 특이적으로 결합하는 나타났으며, 이 가운데 Gp1 및 Gp2 합성 peptide에 대한 항혈청은 양식 넙치에서 분리한 VHSV를 중화시키는 것으로 나타나, peptide-based antigen의 이용 가능성을 제시하였다.

Is Koi Herpesvirus (KHV) Related to the Mass Mortality Occurring among Cultured Carp, Cyprinus carpio, in Korea?

  • Since 1998, a new viral disease with high mortality has been consistently recorded in Korea in cultured carp, Cyprinus carpio. In this study, we investigated an epizootic of the disease that caused high mortality rates in carp obtained from 11 farms in Korea between 1999 and 2007. Assessment of koi herpesvirus (KHV) levels in diseased carp was carried out to determine if this virus was the etiologic agent of disease in this instance. High mortality rates in carp were recorded mainly in the spring and autumn at water temperatures between $19^{\circ}C$ and $24^{\circ}C$. Diseased fish typically showed surface discoloration, with a thick opaque mucus covering the body and gills. Protozoan parasites and bacteria were recovered from 7/29 (24%) and 2/26 (8%) of fish, respectively. Evidence of viral infection was marked; cytopathic effects (CPEs), characterized by cell rounding and an extended cytoplasm in fathead minnow (FHM) cells, were detected in 40/41 fish (98%). A high mortality rate (80%) resulted when supernatants of cell cultures showing CPEs were applied to previously healthy fish. KHV was detected by polymerase chain reaction in 6/41 fish (15%), but was not detected in supernatants obtained from cell cultures showing CPEs. These results suggest that KHV may not be the etiologic agent of the high mortality occurring among cultured carp in Korea; therefore, some other-as yet unidentified-infective agent must be responsible.

Immuno-protective effect of commercial IBD vaccines against emerging novel variant infectious bursal disease virus in specific-pathogen-free chickens

  • Importance: Infectious bursal disease (IBD) is an important viral poultry disease that vaccination can control. Objective: This study examined the immune protection of immune-complex (Vaccine A) and attenuated live (Vaccine B) IBD vaccines in specific-pathogen-free (SPF) chickens against a novel Malaysian variant IBD virus (vaIBDV) challenge. Methods: One-day-old (n =75) SPF chickens were divided randomly into the following three groups of 25 chicks each: Control, Vaccine A, and Vaccine B groups. The vaIBDV strain, UPM1432/2019, was used for the challenge at 21 and 28days post-vaccination (dpv). Five birds from unchallenged and challenged groups were sacrificed seven days post-challenge, and blood, bursa, spleen, and cloacal swabs were collected. The IBD antibodies (Abs), lymphoid lesions, and viral load were determined. Results: The UPM1432/2019 virus induced bursal damage in vaccinated SPF chickens despite Ab titers. The mean Ab titers of the Vaccine A challenged group were significantly lower (p < 0.002) than in the unchallenged group at 28 dpv. The bursal indices of the vaccinated unchallenged groups did not differ significantly from those of the vaccinated challenged groups (p = 0.94). Microscopically, the bursae of the challenged groups showed significant atrophy. The bursal lesion score was higher (p < 0.05) in the control and Vaccine B challenged groups than the Vaccine A challenged group. The challenged group had a higher viral load than the vaccinated groups (p < 0.001). Conclusions and Relevance: Neither vaccine fully protected against a vaIBDV challenge, highlighting the limitations of current vaccines and the need for further research.
