대학생의 A형 간염에 대한 지식, 건강신념 및 예방접종 행태 (Knowledge, Health Belief, and Vaccination Behavior on Hepatitis A among University Students)

  • 이 연구는 대학생의 A형 간염 지식, 건강신념, A형 간염 예방접종 행태를 파악하기 위하여 실시하였다. 대구 경북지역의 보건계열을 제외한 4년제 대학생을 대상으로 2014년 3월 3일부터 3월 25일까지 구조화된 설문지를 이용한 자기기입식 설문조사를 하였으며, 총 197부를 최종분석대상으로 하였다. 연구대상자들의 A형 간염 지식점수는 15점 만점에 $4.59{\pm}3.06$점이었으며, 건강신념 점수는 4점 만점에 $2.39{\pm}0.28$점이었고, A형 간염 예방접종률은 12.7%였다. A형 간염 지식은 A형 간염 검사 경험, 가족 중 A형 간염 예방접종 경험에 따라 유의한 차이가 있었고, 건강신념은 성별에 따라 유의한 차이가 있었다. A형 간염 예방접종은 A형 간염 검사 경험, 가족 중 간질환, 가족 중 A형 간염 예방접종 경험, A형 간염 교육 경험 등에 따라 유의한 차이가 있었다. 로지스틱 회귀분석 결과, A형 간염 예방접종 행태에 유의한 영향을 미치는 변수는 A형 간염 검사 경험과 가족 중 A형 간염 예방접종 경험이었다. 결과적으로 연구대상자들의 A형 간염 지식, 건강신념 수준이 낮았으며 A형 간염 예방접종률 또한 낮았다. 따라서 적절한 보건교육을 통해 지식을 습득할 수 있게 하고, 질병 예방행위 실천 및 A형 간염 예방접종률을 높일 수 있는 방안들이 모색될 필요가 있을 것이다.

Influencing Factors on Intention to Vaccinate Against COVID-19 in College Students

  • Purpose: This cross-sectional study aimed to identify factors affecting coronavirus disease 2019 (COVID-19) vaccination intention. Methods: For an anonymous online survey, recruitment notices were posted on an anonymous community by each university, and an online survey was conducted through online form from June to July 2021. COVID-19 knowledge and health-protective behavior were measured using a questionnaire based on previous literature and reflecting the Korea Centers for Disease Control and Prevention's COVID-19 Response Guidelines. The psychological antecedents of vaccination were measured by 5C scale. Results: Two-hundred and ninety-four college students (women 67.3%) answered the survey; 179 (60.9%) reported that they would accept a COVID-19 vaccine. The mean scores for COVID-19 knowledge and health-protective behavior were 22.97±5.33 (out of 35) and 9.92±2.22 (out of 12), respectively. For the psychological antecedents of vaccination, the mean scores for confidence, collective responsibility, calculation, complacency, and constraints were 4.45 (± 1.24), 5.61 (± 1.09), 5.09 (± 1.18), 2.42 (± 1.11), and 2.37 (± 1.19) out of 5 points, respectively. The confidence, calculation, and collective responsibility were associated with vaccination intention. Additionally, the top reason for those who were less prone to accept vaccination against COVID-19 was concern about vaccine safety. Conclusion: The higher the confidence in the vaccine and the higher the collective responsibility, the higher the vaccination intention. As it is a factor related to an individual's perception of COVID-19 information, it is necessary to increase confidence in the vaccines through obtaining accurate information on the safety, effectiveness, and side effects of the COVID-19 vaccines and vaccination.

Use of Cell-Penetrating Peptides in Dendritic Cell-Based Vaccination

  • Cell-penetrating peptides (CPPs) are short amino acids that have been widely used to deliver macromolecules such as proteins, peptides, DNA, or RNA, to control cellular behavior for therapeutic purposes. CPPs have been used to treat immunological diseases through the delivery of immune modulatory molecules in vivo. Their intracellular delivery efficiency is highly synergistic with the cellular characteristics of the dendritic cells (DCs), which actively uptake foreign antigens. DC-based vaccines are primarily generated by pulsing DCs ex vivo with various immunomodulatory antigens. CPP conjugation to antigens would increase DC uptake as well as antigen processing and presentation on both MHC class II and MHC class I molecules, leading to antigen specific CD4+ and CD8+ T cell responses. CPP-antigen based DC vaccination is considered a promising tool for cancer immunotherapy due to the enhanced CTL response. In this review, we discuss the various applications of CPPs in immune modulation and DC vaccination, and highlight the advantages and limitations of the current CPP-based DC vaccination.

자녀의 코로나19 백신접종에 대한 부모의 행위의도 예측요인: 계획된 행위이론의 수정모형 적용 (Predictors of parental behavioral intentions for vaccinating their children against COVID-19 as determined by the modified theory of planned behavior)

  • Purpose: This study attempted to present a strategy to increase behavioral vaccination intention by identifying factors affecting parents' intention to have their children vaccinated against coronavirus disease 2019 (COVID-19) by applying the modified theory of planned behavior. Methods: An online survey was conducted with 146 parents of children aged 5 to 17 from August 1 to 30, 2022, and, as a result, 146 data were analyzed. The moderating effect of perceived behavioral control in the relationship between parents' attitude, subjective norms, and behavioral intention for children's COVID-19 vaccination was analyzed using the Process Macro (10,000 bootstrapping, 95% confidence interval), and a simple slope analysis was performed to identify differences by group. Results: The moderating effect of perceived behavioral control was statistically significant in the relationship between parent's attitude and subjective norms for COVID-19 vaccination and behavioral intention. In the simple slope analysis, the reinforcement effect was greatest when the level of perceived behavioral control was high. Conclusion: Since vaccination is most likely when recommended by healthcare providers, it is necessary to ensure that health education through community health experts or mass media can be effectively conducted. In particular, objective information on the effectiveness and safety of COVID-19 vaccinations for children, including monitoring of potential side-effects, should be actively reported.

우리나라 성인 당뇨병 환자에서 인플루엔자 예방접종 관련요인 (Factors Influencing Influenza Vaccination Among Korean Adults Diabetic Patients)

  • 본 연구의 목적은 지역사회건강조사 자료를 이용하여 우리나라 성인 당뇨병 환자에서 인플루엔자 예방접종률과 예방접종 관련요인을 알아보고자 하는 것이다. 본 연구는 2015년 지역사회건강조사 원시자료를 이용하였으며, 만 19세 이상 성인 총 228,261명을 최종 연구대상자로 선정하였다. 인플루엔자 백신 접종 관련요인을 파악하기 위하여 대상자의 인구사회학적 요인(연령, 성별, 교육수준, 가구소득, 거주 지역, 결혼상태 및 직업유무), 건강행태(흡연, 음주 및 건강검진), 질병 이환(비만, 고혈압 및 이상지질혈증) 및 주관적 건강상태를 교차분석 및 로지스틱 회귀분석으로 분석하였다. 본 연구 결과 우리나라 성인 당뇨병 환자의 인플루엔자 백신 접종률은 62.4%로 낮은 수준이다. 인플루엔자 예방접종 여부를 종속변수로 모든 변수가 보정된 상태에서 당뇨군에서는 연령, 성별, 교육수준, 거주 지역, 결혼상태, 직업유무, 흡연, 건강검진 및 동반질환이 유의한 변수로 나타났다(p<0.05). 따라서 당뇨병 환자에서 인플루엔자 백신 접종률을 향상시키기 위해서는 예방접종에 대한 인식증대를 위한 체계적인 교육과 홍보가 필요하며, 무료접종 확대 실시와 같은 국가적 정책도 필요하겠다.

만 40세 이상 성인 만성폐쇄성폐질환 환자의 인플루엔자 예방접종에 영향을 미치는 요인 (Factors Influencing on Influenza Vaccination Coverage among Chronic Obstructive Pulmonary Disease patient Over 40 years)

  • 본 연구는 성인 만성폐쇄성폐질환(Chronic Obstructive Pulmonary Disease, COPD) 환자의 인플루엔자 접종률과 접종에 영향을 미치는 요인을 파악하기 시행하였다. 2014년부터 2018년까지 5년간의 국민건강영양조사에 참여한 대상자 중 만 40세 이상이며 폐기능 검사 판정결과 폐쇄성환기장애(FEV1/FVC<0.7)로 나타난 2,370명을 대상으로 인플루엔자 예방접종률과 접종에 미치는 영향요인을 분석하였다. 민간강영양조사 데이터를 분석한 결과 만 40세 이상 성인 COPD 환자의 인플루엔자 예방접종률은 59.2%이었으며, 예방접종에 영향을 미치는 요인은 65세 이상, 낮은 경제수준, 과거흡연자이거나 비흡연자, 최근 2년 이내 건강검진을 받은 경우이었다. COPD가 있는 성인의 예방접종률을 높이기 위하여 인플루엔자 예방접종에 영향을 주는 요인들을 고려하여 예방접종률을 높이기 위한 방안을 모색해야 할 것이다.

여중생 딸을 둔 어머니의 인유두종 바이러스 백신 접종 의도 및 관련 요인 (Factors associated with the Intention of Human Papillomavirus Vaccination among Mothers of Junior High School Daughters)

  • 본 연구는 계획된 행위이론을 적용하여 여중생 딸을 둔 어머니의 HPV(human papillomavirus) 백신 접종 의도를 파악하고, 관련 요인을 알아보기 위함이며, G광역시에 소재한 여중학교 4개교의 어머니 198명을 대상으로 하였다. 연구 도구로 HPV 백신 접종에 대한 태도, 주관적 규범, 지각된 행위 통제는 Askelson 등이 개발한 도구를 사용하였으며 자료 수집 기간은 2012년 9월 17일부터 24일까지였고, 수집된 자료는 SPSS 18.0 Program을 사용하였다. 본 연구 결과 여중생 어머니들의 HPV 백신 접종 의도는 비교적 높았지만, 6개월 이내에 접종하겠다고 한 어머니는 29.8% 뿐이었고, 어머니들이 생각하는 HPV 백신 최적 접종 연령이 평균 17.74세였다. 대상자의 HPV 백신 접종 의도는 태도, 지각된 행위 통제, 주관적 규범과 통계적으로 유의한 상관관계가 있었고, HPV 백신 접종에 대한 태도, 주관적 규범, 지각된 행위 통제가 접종의도의 중요한 영향 요인임이 확인되었으며, 대상자의 HPV 백신 접종 의도를 84.3%를 설명하였다. 그러므로 어머니들이 HPV 백신 접종에 대해 긍정적으로 생각하고, 백신 접종을 쉽고, 용이하게 지각하도록 격려하는 홍보 및 사회적 분위기 조성이 필요하리라 사료된다.

초등학생 아들에 대한 부모의 인유두종바이러스 백신접종 의도와 예측요인: 개정된 계획된 행위이론의 적용 (Updated Theory of Planned Behavior in Predicting Parents' Intentions to Vaccinate Their Sons in Elementary School against Human Papillomavirus)

  • Purpose: This study was to determine how attitude, subjective norm, and perceived behavioral control (PBC) were related to parents' intentions to vaccinate their sons in elementary school against human papillomavirus (HPV), applying the updated theory of planned behavior. Two hypotheses were examined: PBC would moderate the relationship of attitude to intention and subjective norm to intention, respectively. Methods: This was a cross-sectional study with 231 participants recruited in November, 2018. Inclusion criteria for the participants were parents with 5th-6th grade boys in elementary schools. The moderation effects of the hypotheses were analyzed using the PROCESS macro for SPSS. Results: PBC significantly moderated the relationship of attitude to intention on HPV vaccination. The simple effect of attitude to intention was significant under three different levels of the PBC (low, moderate, and high), but the magnitudes of the relationships were not homogeneous. The magnitude of the relationship between attitude and intention was the lowest for the parents with the low level of the PBC. In the relationship of subjective norm to intention on HPV vaccination, the moderating effect of the PBC was not supported. Subjective norm and PBC directly predicted the intention on HPV vaccination. Conclusion: In order to promote the parents' intentions to vaccinate their sons in elementary school against HPV, we need a program that can improve parents' attitude, subjective norm, and PBC, requiring special attention to the parents with the low PBC.

How to Improve Influenza Vaccination Rates in the U.S.

  • Annual epidemics of seasonal influenza occur during autumn and winter in temperate regions and have imposed substantial public health and economic burdens. At the global level, these epidemics cause about 3-5 million severe cases of illness and about 0.25-0.5 million deaths each year. Although annual vaccination is the most effective way to prevent the disease and its severe outcomes, influenza vaccination coverage rates have been at suboptimal levels in many countries. For instance, the coverage rates among the elderly in 20 developed nations in 2008 ranged from 21% to 78% (median 65%). In the U.S., influenza vaccination levels among elderly population appeared to reach a "plateau" of about 70% after the late 1990s, and levels among child populations have remained at less than 50%. In addition, disparities in the coverage rates across subpopulations within a country present another important public health issue. New approaches are needed for countries striving both to improve their overall coverage rates and to eliminate disparities. This review article aims to describe a broad conceptual framework of vaccination, and to illustrate four potential determinants of influenza vaccination based on empirical analyses of U.S. nationally representative populations. These determinants include the ongoing influenza epidemic level, mass media reporting on influenza-related topics, reimbursement rate for providers to administer influenza vaccination, and vaccine supply. It additionally proposes specific policy implications, derived from these empirical analyses, to improve the influenza vaccination coverage rate and associated disparities in the U.S., which could be generalizable to other countries.

간호대학생의 임상 실습 전 예방접종 의도에 영향을 미치는 요인 (Factors related to the intention of pre-clinical vaccination among nursing students)

  • Purpose: The aim of this study was to identify factors related to the vaccination intentions of nursing students before clinical practice. Methods: A descriptive correlational study design was used. The participants were 129 nursing students who were enrolled in one college of nursing in Seoul and were scheduled for clinical training. Data collection was conducted from February 28 to March 7, 2022. The data were analyzed using descriptive statistics, a t-test, ANOVA, Pearson's correlation coefficient, and a latitudinal regression analysis using the IBM SPSS statistics version 26.0 program. Results: The intention to vaccinate showed a significant correlation with vaccination attitude (r=.72, p<.001), subjective norms (r=.68, p<.001), and perceived behavioral control (r=.63, p<.001). The influencing factors on three intent to vaccinate were vaccination attitude (β=.36 p<.001), subjective norms (β=.31, p<.001), and perceived behavioral control (β=.36, p<.001). The explanatory power of these variables was 69.6%. Conclusion: The vaccination rate before the clinical training of nursing students was related to economic burden, a lack of time, and transportation problems that nursing college students experienced during the vaccination process. Therefore, administrative and financial support is needed for improving the vaccination rates of nursing students.
