『상한론(傷寒論)』 변병진단체계(辨病診斷體系)에 근거하여 대함흉탕(大陷胸湯) 투여로 호전된 공황 장애를 동반한 이명 1례 (A Case Report of Tinnitus with Panic Disorder treated by Daehamhyung-tang from the Disease Pattern Identification Diagnostic System based on Shanghanlun provisions)

  • Objective : The purpose of this paper is to report the effect of Daehamhyung-tang(DHHT) on tinnitus with panic disorder from the disease pattern identification diagnostic system based on Shanghanlun provisions. Methods : According to a disease pattern identification diagnostic system based on Shanghanlun provisions, the patient was diagnosed with 'Taeyang-byung Gyeolhyung'. He took DHHT for 82 days. This paper evaluated the results of treatment on tinnitus by Tinnitus Handicap Inventory(THI) and Verbal Numercial Rating Scale(VNRS). To assess the mental state of anxiety, this paper used Hamilton Anxiety Rating Scale(HAM-A) Results : The THI score was declined from 58 to 6 And the VNRS score was declined from 6 to 1. HAM-A score was declined from 28 to 3. Conclusions : This case study suggests that DHHT has effectiveness on a patient suffers from tinnitus with panic disorder and diagnosed with 'Taeyang-byung Gyeolhyung'.

Method : We used Acupuncture, Beevenom, CHUNA manual therapy, Herbal medication, for this patient. And we measured of VNRS and ODI score to evaluate the conservative treatment effects. Result : Patient's LBP & Rt.leg pain VNRS and ODI score were decreased.

  • Objectives This case reports the effect of Modified Hyeongbangjiwhang-tang for essential tremor after total vaginal hysterectomy. Methods The patient was treated with Modified Hyeongbangjiwhang-tang three times a day. To differentiate other diseases, we took her medical history. And, we used the Verbal Numerical Rating Scale (VNRS) and Gastrointestinal Symptom Rating Scale (GSRS) to assess tremor and gastrointestinal symptoms. Results Treatment with modified Hyeongbangjiwhang-tang resulted in a decrease in the patient's complaints of symptoms. Also, VNRS and GSRS scale were improved. Adverse effects were not observed. Conclusions Modified Hyeongbangjiwhang-tang may be effective for essential tremor after total vaginal hysterectomy.

초음파 유도 하 일회성 넙다리신경블록이 슬관절 전치환술 후 통증에 미치는 영향: 0.25% Levobupivacaine과 0.5% Levobupivacaine 주입의 비교 (Effect of Single-injection Femoral Nerve Block Using Real-time Ultrasound on the Postoperative Pain after Bilateral Total Knee Arthroplasty: 0.25% vs 0.5% Levobupivacaine)

  • Background: The purpose of this study was to compare the analgesic effect of 0.25% and 0.5% levobupivacaine for real time ultrasound guided single-injection femoral nerve block for the patients who are undergoing bilateral total knee arthroplasty (TKA). Methods: Femoral nerve block was done to all patients with 20 ml of 0.9% normal saline on one leg and 20 ml of 0.25% levobupivacaine on the other leg for group I (n = 16) and 0.5% levobupivacaine for group II (n = 15) with 1:200,000 epinephrine and using real-time ultrasound and a nerve stimulator. The data concerning the verbal numerical rating scale (VNRS) for each leg, the consumption of the intravenous patient-controlled analgesia (IV PCA) and the demands for the additional analgesics was collected at 0, 1, 6, 12, 24 and 48 hours after the operation. Results: The legs on which femoral nerve block was done with levobupivacaine showed a lower VNRS score than the legs with normal saline in either group I or group II. The VNRS scores between the two legs, the consumption of the IV PCA and the demand for additional analgesics showed no significant differences between the groups. Conclusions: Our results demonstrate that single-injection femoral nerve block using real-time ultrasound with either 0.25% levobupivacaine or 0.5% levobupivacaine 20 ml provides a good effect for the postoperative pain control after TKA.

  • Objectives : The purpose of this study was to evaluate the effect of Oriental medical treatment in patients with acute lumbar Herniated Intervertebral Disc(HIVD). Methods : This clinical study was carried out on 44 patients with acute lumbar HIVD, who had been admitted from Oct. 2011 to Nov. 2011. All of 44 patients were treated with acupuncture, Chuna treatment and herbal medicine during the whole admission period. Verbal numerical rating scale(VNRS) and Oswestry disability index(ODI) were used to evaluated the effectiveness of the Oriental medical treatment. Result : Siginificant improvement of the symptoms was seen when evaluated with VNRS and ODI. Conclusions : These results suggest that in the case of low back pain and lower limb numbness caused by acute HIVD, conservative treatments can be considered as one of the options of treating the symptoms beside surgical way.

Comparison of ultrasound guided pulsed radiofrequency of genicular nerve with local anesthetic and steroid block for management of osteoarthritis knee pain

  • Background: To compare ultrasound-guided pulsed radiofrequency (PRF) of the genicular nerve with the genicular nerve block using local anesthetic and steroid for management of osteoarthritis (OA) knee pain. Methods: Thirty patients with OA knee were randomly allocated to receive either ultrasound-guided PRF of the genicular nerve (PRF group) or nerve block with bupivacaine and methylprednisolone acetate (local anesthetic steroid [LAS] group). Verbal numeric rating scale (VNRS) and Western Ontario McMaster Universities Osteoarthritis Index (WOMAC) scores were measured at pre-procedure and 1-, 4-, and 12-weeks post-procedure. Results: VNRS scores decreased significantly (P < 0.001) in both the groups at 12 weeks and other follow up times compared to baseline. Seventy-three percent of patients in the PRF group and 66% in the LAS group achieved effective pain relief (≥ 50% pain reduction) at 12 weeks (P > 0.999). There was also a statistically significant (P < 0.001) improvement in WOMAC scores in both groups at all follow up times. However, there was no intergroup difference in VNRS (P = 0.893) and WOMAC scores (P = 0.983). No complications were reported. Conclusions: Both ultrasound-guided PRF of the genicular nerve and blocks of genicular nerve with local anesthetic and a steroid provided comparable pain relief without any complications. However, PRF of the genicular nerve is a procedure that takes much more time and equipment than the genicular nerve block.

교통사고 이후 MRI상 경추 추간판 탈출증 진단을 받은 환자에 대한 신바로약침과 봉약침의 치료효과 비교 연구: 후향적 환자군 관찰 연구 (The Comparitive Study on the Effect of Shinbaro Pharmacopuncture and Bee Venom Pharmacopuncture Treatment of Patient Diagnosed as Cervical Disc Hermiation Based on the MRI after Traffic Accident: A Retrospective, Case Series Observational Study)

  • Objectives : The purpose of this study is to compare the effect of Shinbaro pharmacopuncture treatment and bee venom pharmacopuncture treatment on cervical disc herniation and whiplash injury induced by traffic accident. Methods : This study was carried out on the 59 patients with whiplash injury by traffic accident and diagnosed as cervical disc herniation. The patients were divided into 2 groups ; Shinbaro pharmacopuncture group was treated with Shinbaro pharmacopuncture along with herbal medicine and general acupuncture. Bee venom pharmacopuncture group was treated with bee venom pharmacopuncture along with herbal medicine and general acupuncture. We measured verbal numerical rating scale(VNRS) and neck disability index(NDI) before the treatment and 1, 2, 3 weeks afterwards. Results : Shinbaro pharmacopuncture group and Bee venom pharmacopuncture both showed significant decrease in VNRS and NDI. Shinbaro pharmacopuncture group showed significant decrease over Bee venom pharmacopuncture group in VNRS decrement in the first week, while Bee venom pharmacopuncture group showed significant decrease over Shinbaro pharmacopuncture group in the secone week of treatment. Conclusion : According to the result of this study, it is suggested that Shinbaro pharmacopuncture group is more effective in reducing pain in the earlier period of treatment. Also suggests that further study on the effect of Shinbaro pharmacopuncture on patients diagnosed as cervical disc herniation after traffic accident for longer period of treatment.

측두하악관절장애 환자 194례에 대한 임상적 고찰 (The Clinical Study on 194 Patients of Temporomandibular Disorder)

  • Objectives : These studies are designed to make a survey of the effectiveness of oriental medicine treatment on temporomandibular disorder. Methods : The clinical study was done on 194 cases of patients with temporomandibular disorder who was treated in Ja-Seng Oriental Medicine Hospital from January 2008 to June 2008. After treatment we checked VNRS(verbal numerical rating scale) score and five-point linkert scale. Also we observed the change of symptom of click sound and trismus. Results : After the last treatment, The mean VNRS(verbal numerical rating scale) score changed 4.63 to 2.25. The click sound was disappeared of 53% patients, and the trismus was disappeared of 57% patients. And The five-point linkert scale were checked 3.78. Conclusions : It is concluded that oriental medicine treatment has on useful effect on temporomandibular disorder.

  • Objectives : the purpose of this study is to report the improvement of stenosis patients, who treated by M.S.T.(Motion Style Treatment) Methods : the clinical study was done on 2 cases of patients with lumbar stenosis who admission in Jaseng Oriental Medicine hospital. We checked VNRS score after the treatment of M.S.T.(Motion Style Treatment) each day, and checked ODI score at admission and discharge.   • Patients with lumbar disc herniation are faced with lowered quality of life. For this reason, we investigated the effects of Oriental Intensive Therapy (OIT) including acupuncture, pharmacopuncture, manual therapy and herbal medicine on patients with lumbar disc herniation. Ten patients, diagnosed with lumbar disc herniation by MRI in hospital, were treated with OIT for 12 weeks. Symptoms of the patients were assessed using Verbal Numerical Rating Scale (VNRS) and Oswestry Disability Index (ODI). All patients were investigated every a week. In our results, intensity of pain was reduced after 4 weeks treatment significantly by VNRS, Quality of life has also improved after 4 weeks treatment by ODI. In conclusion, this study suggests that OIT may be effective to treat patients with lumbar disc herniation.
