Purpose : Recently, different results about factors affecting accurate quantitation of 24-hr urinary protein(24UP) amount using spot urine protein/creatinine ratio(PCR) have been reported. The current study was designed to evaluate correlation between 24UP amounts and PCR in children, and the effect of 24UP amounts, age, sex, and glomerular filtration rate(GFR) on this correlation. Methods : Among 94 patients who visited the department of pediatrics in Busan Paik Hospital from March 2002 to August 2002, 68 patients whose urinary creatinine excretion was ${\geq}15mg/kg/day$ were included in this study. All the patients were divided into I, II/A, B group(I : 24UP<500 mg/day, II : $24UP{\geq}500mg/day$, A : <10 years of age, B : ${\geq}10years$ of age). Pearson correlation analysis was performed between 24UP and PCR to evaluate the relationship. We defined fractional difference between 24UP and PCR, and then performed multiple regression analysis with 24UP amount, age, GFR and fractional difference. Results : There was a strong positive linear correlation between 24UP and PCR(R=0.936, P<0.0001) in all patients, and the correlation was also good in each group. Using PCR cutoff values of 0.5, the PCR provided high sensitivity, specificity, positive and negative predictive value in predicting 24UP amount ${\geq}500mg$. The factors affecting accurate quantitation of proteinuria using spot urine PCR was age, not 24UP amount, GFR or sex. Conclusion : Spot urine PCR is a useful test but has limitations in predicting 24UP amount. Therefore, it should be used only as screening method. Age-adjusted PCR cutoff values may be necessary to predict 24UP amount in children with proteinuria.

Purpose: This study was done to evaluate the effects of a fluid intake intervention on increasing fluid intake and ameliorating dehydration status in elders admitted to long-term care hospitals. Methods: A nonequivalent control group, pretest and posttest design was used. The experimental group of 39 participants received the 4-week intervention while the control group of 38 participants received routine care. Outcome variables were daily fluid intake and physiological indexes such as blood urea nitrogen and creatinine ratio (BUN/Cr), urine specific gravity (USG), and urine color. Results: After the intervention to increase fluid intake, there were statistically significant increases in daily fluid intake, normal BUN/Cr, and USG in the experimental group. However, a statistically significant improvement in normal urine color was not found for either group. Conclusion: The findings of this study demonstrated that the fluid intake intervention improved hydration status of the experimental group participants. Consequently, it was confirmed that the intervention is considered to be effective in preventing dehydration which occurs frequently in older adults in long-term care facilities and, thus this intervention may contribute to preventing various health issues resulting from dehydration.
Ethylene glycol (EG) intoxication in dogs can pose a diagnostic challenge. We investigated the effect of a lethal dose of EG to the Shepherd on blood, urine test, and histopathologic observation. Shepherds were treated orally with EG at $10\;m{\ell}/kg$ body weight. Clinically, EG caused the vomition, depression of central brain. Anion gap was increased, which was decreased pH of blood, The blood osmorality was increased at 3 hours, and BUN and creatinine was increased at 24 hour significantly. The pH of urine decreased. The crystal of calcium oxalate appeared at 6 hours. The amount of crystals increased at 12 and 24 hours. Microscopically, the degeneration of proximal tubules were shown at 1 hour and finally progressed as acute tubular nephrosis at 24 hours. These results suggest that the blood and urine test accompanied with histopathological examination be helpful to investigate the EG intoxication.
Pancreatitis is a known complication of cardiac surgery with cardiopulmonary bypass. Although ischemia is believed to be a factor, the exact cause of pancreatitis after cardiopulmonary bypass remains unknown.We prospectively studied 67 consecutive patients undergoing cardiac surgery with cardiopulmonary bypass for evaluation of the pancreatic injury after cardiopulmonary bypas. Serial measurement of amylase level in serum and urine was done postoperatively. Hyperamylasemia was detected in 15 patients[22.4% , of whom no patient had pancreatitis. There was no significant difference between serum amylase level and parameters such as cardiopulmonay bypass time, aortic cross clamp time, mean blood pressure, rectal temperature, flow rate, and use of circulatory arrest during cardiopulmonary bypass. Hyperamylasuria was detected in 8 patients[11.9% , and urine amylase level was elevated significantly in the groups with prolonged cardiopulmonary bypass, mean blood pressure more than 40mmHg, and rectal temperature more than 20 $^{\circ}$C. We recommend that serum amylase level and/or amylase-creatinine clearance ratio is measured for ealy detection and management of pancreatitis after cardiopulmonary bypass.
Objectives: To determine urinary mercury levels in e-waste workers in Southern Thailand and the airborne mercury levels in the e-waste shops where they worked, to describe the associations between urinary and airborne mercury levels, and to evaluate the prevalence of mercury exposure-related health effects among e-waste workers. Methods: A cross-sectional study was conducted by interviewing 79 workers in 25 e-waste shops who lived in Nakhon Si Thammarat Province, Thailand. Information on general and occupational characteristics, personal protective equipment use, and personal hygiene was collected by questionnaire. Urine samples were collected to determine mercury levels using a cold-vapor atomic absorption spectrometer mercury analyzer. Results: The e-waste workers' urinary mercury levels were $11.60{\mu}5.23{\mu}g/g$ creatinine (range, 2.00 to $26.00{\mu}g/g$ creatinine) and the mean airborne mercury levels were $17.00{\mu}0.50{\mu}g/m^3$ (range, 3.00 to $29.00{\mu}g/m^3$). The urinary and airborne mercury levels were significantly correlated (r=0.552, p<0.001). The prevalence of self-reported symptoms was 46.8% for insomnia, 36.7% for muscle atrophy, 24.1% for weakness, and 20.3% for headaches. Conclusions: Personal hygiene was found to be an important protective factor, and should therefore be stressed in educational programs. Employers should implement engineering measures to reduce urinary mercury levels and the prevalence of associated health symptoms among e-waste workers.
Objectives : This study aimed to evaluate the effects of Stephaniae tetrandrae Radix Herbal-acupuncture(SR-HA) at KI10(Umgok) on nephritis induced by lipopolysaccharide(LPS) in rats. Methods : Male SD rats (n=32) were allocated to four groups; normal, LPS, saline, and SR-HA. The rats in the LPS, saline, and SR-HA groups were injected intra-peritoneally with LPS (2 mg/kg) to induce nephritis. The rats in the saline and SR-HA groups were treated with saline injection and SR-HA respectively at KI10 three times a week. To evaluate the effects of SR-HA at KI10 on nephritis in rats, the authors evaluated the numbers of WBC and neutrophils in blood, and the levels of creatinine, TNF-${\alpha}$, CINC-1 in serum, creatinine, total protein in urine and renal MPO. Results : SR-HA at KI10 significantly inhibited the increase of WBC and neutrophils in blood, TNF-${\alpha}$, CINC-1 in serum, creatine, total protein in urine and MPO in kidney of LPS-stimulated rats. Conclusions : SR-HA at KI10 may have therapeutic effects on nephritis in LPS-stimulated rats.
The present study was aimed to determine whether there is an altered regulation of tubular transporters in gentamicin-induced nephropathy. Sprague-Dawley male rats ($200{\sim}250\;g$) were subcutaneously injected with gentamicin (100 mg/kg per day) for 7 days, and the expression of tubular transporters was determined by immunoblotting and immunohistochemistry. The mRNA and protein expression of OAT was also determined. Gentamicin-treated rats exhibited significantly decreased creatinine clearance along with increased plasma creatinine levels. Accordingly, the fractional excretion of sodium increased. Urine volume was increased, while urine osmolality and free water reabsorption were decreased. Immunoblotting and immunohistochemistry revealed decreased expression of $Na^+/K^+$-ATPase, NHE3, NBC1, and AQP1 in the kidney of gentamicin-treated rats. The expression of OAT1 and OAT3 was also decreased. Gentamicin-induced nephropathy may at least in part be causally related with a decreased expression of $Na^+/K^+$-ATPase, NHE3, NBC1, AQP1 and OAT.
The diagnostic method was, evaluated in experimentally induced acute renal failure in doges. Ten male dogs weighing from 5 to 10 kg were assigned to two groups(gentamicin & ethylene glycol treated group) al random. Gentamicin sulfate at 10 mg/kg of body weight, t.i.d., for 7 days and ethylene glycol at 3 ml/kg of body weight once used in a randomized complete block design with tee treatments in block. The samples(blood, urine) were collected before and 1,3,5,7,9 and l1th day after administration. The serum creatinine concentration and BUN(blood urea nitrogen) were sig- nificantly increased at the 7th day than before administration in gentamicin treated group (p<0.05), bolt there was significant increase at the 1st day than before administration in ethylene glycol treated group(p<0.05). The urine GGT(gramma glutamyl transpeptidase) and GGT/creatinine were significantly increased at the 1st (lay after administration in gentamicin treated group (p<0.05). But in ethylene glycol treated group, there was no significant changes. The value of FENa (fractional excretion of sodium) was significantly increased at the 3rd day after administration in gentamicin treated group and the 1 st day after administration in ethylene glycol treated group (p<0.05). These results suggest that FENa was a good parameter of renal function in dogs with acute renal failure.
Thirteen healthy control, 13 pre-eclamptic, 7 diabetic(DM) and 12 gestational diabetic(GDM) pregnant women participated in a study ofthe interrelationships between the levels of protein, calcium, magnesium, phosphorus, zinc and copper in urine. Urinary protein, magnesium and copper levels were significantly higher (p<0.0005, p<0.0003, p<0.005 respectively) in pre-eclamptic women than those of control, DM and GDM women. Urinary zinc excretion in pre-eclamptic women (1.61 mg/g creatinine) was higher than that of DM women (0.81mg/g creatinine); urinary zinc losses of control and GDM women were wre between the other two rups. The GDM women excreted significantly ore phosphorus in their urine in comparison to control and preeclamptic women (p<0.02), but this was not seen in DM women. Among the DM women, urinary protein excretion was positively correlated with glycosylated hemoglobin(r=0.940) and fasting blood glucose concentration (r=0.889). Urinary zinc excretion also was correlated with glycosylated hemoglobin (r=0.853) and fasting blood glucose (r=0.956). In the GDM and pre-eclamptic women there were also significant correlations between urinar calcium and magnesium (r=0.857, r=0.749 respectively) and between urinary protein and copper(r=0.638, r=0.778 respectively).
Correlation between occupational exposure levels to toluene and urinary hippuric acid (HA) concentrations were studied for 124 workers at 54 work places. The highest indoor toluene concentration measured at printing process was 81.116 ppm, and their geometric average concentration was as high as 12.304 ppm. The geometric average concentration of hippuric acid in urine samples from workers who are exposed to toluene was 0.714 g/g creatinine. This is almost five times higher than the average HA concentration from non-exposure workers. Printing workers showed the average HA concentration of 1.145 g/g creatinine from their urine samples. It is the highest concentration among the workers exposed to toluene. The correlation coefficient between HA concentrations in urine and indoor toluene concentration at work places was relatively high as r=0.624 (P<0.01). But the correlations of HA with sex, smoking, drinking, age and employment history was relatively low. We can express the regression equation for the urinary HA concentration which is Y = 0.037X + 0.562 as exposure toluene concentration is X. The urinary HA concentrations showed significantly increase depend on indoor toluene concentration at work place.
