The vertical changes of the lip and perioral soft tissue resulting from incisor retraction (전치의 후방견인에 따른 입술과 주위 연조직의 수직적 변화)

  • Patients who want to reduce their lip protrusion usually estimate the severity of the lip protrusion on the frontal aspect. Most orthodontists have a perplexed experience of a reduced thin line of vermilion border on the frontal aspect as incisors we retracted, even though the lip protrusion is thought to be reduced favorably on the sagittal aspect. Some patients also look older after orthodontic treatment because of severe lip thinning. This unaesthetic reduction of vermilion border urges us to study the vertical lip change during orthodontic procedure. The purpose of this study was to evaluate the vertical lip and perioral soft tissue changes in respect to incisor retraction in an effort to analyze which factors might be responsible for their vertical changes, using the multiple regression analysis. The results were as follows. 1. Upper and lower lip philtrum length(SnLs, LiB') were increased after retraction of anterior teeth, where as upper and lower vermilion height(LsSuls, StmiLi), and vermilion length(LsLi) were decreased. 2. Upper and lower lip length(SnStms, StmiB'), and soft tissue lower anterior facial height(SnMe') did not show any significant difference after treatment. 3. The increase of the upper lip philtrum length was mainly influenced by the extrusion of upper anterior teeth(${\Delta}U1V$), and the increase of the lower lip philtrum length was mainly influenced by the initial overjet before treatment. 4. The decrease of the upper and lower lip vermilion height was mainly influenced by the decrease of upper lip thickness.

A COMPARATIVE STUDY OF THE AMOUNT OF DISPLACEMENT AND OCCLUSION FORMS IN THE CHANGE FROM CR TO CO (중심위 교합에서 중심 교합으로 전위될 때의 변위량과 교합형태에 관한 비교 연구)

  • Statement of problems: The concept of CR has also changed continuously.?In order to find out the factors that affect the centric slide, studies were carried out to compare the forms of wisdom teeth eruption, lateral movement, premature contact in CR, and anterior movement. Research and statistical methods were based on the report by the 1980 Korean dental association. Material and method: In our study, 403 dentists in their twenties and dentistry students who could understand CR and CO (and who did not receive occlusal, orthodontic treatment, without extreme caries and large prosthodontic care) were compared with the 25 year old results. A segment of line parallel to the upper incisor was marked on the lower incisor. When seen laterally, a line perpendicular to the occlusal plane was drawn on the foremost area of the upper incisor. This line was extended to the lower incisor and the two points (points at the lower and upper incisors) were used as reference points for the CO. After guiding the occlusion to the CR, two lines were marked by using the same method that was used for the CO. The point in which these lines meet became the reference point of CR occlusion Results and conclusions: Results of the experiment completed in 1980 show that all 307 research members had anterior-posterior and upper-lower displacement. Displacement measurements were $0.7{\pm}0.4mm$ for the anterior-posterior displacement, $0.99{\pm}0.50mm$ for the upper-lower displacement,0.18{\pm}0.31 mm for the lateral displacement, and $1.32{\pm}0.67mm$ for the total displacement. Results of the 2006 experiment show that all 409 research members had anterior-posterior and upper-lower displacement. The anterior-posterior displacement was $1.12{\pm}0.86mm$, the upper-lower displacement was $1.02{\pm}0.71mm$, the lateral displacement was $0.61{\pm}0.56mm$, and the total displacement was $1.80{\pm}0.99mm$. No specific differences were found between each group when comparing displacement according to the forms of wisdom teeth eruption. Since 1980, the percentage of unerupted teeth has increased from 35.16% (111/307 people) to 57.5% (236/409 people). Westernization of the Korean cranial form and intraoral structure has brought about these results. In our experiment, 26.7% (109/409 people) of the subjects were cuspid guided, while 7.3% (30/409 people) were mutually guided. No specific differences were found in the amount of displacement between the two groups. Only the subjects with anteriors coming in contact made up the largest percentage group (42.3%, 173/409 people) in our study. No specific differences were found between each group.

THE STUDY ON THE PERIODONTAL VASCULAR CHANGES OF RAT INCISORS FOLLOWING EXPERIMENTAL TOOTH MOVEMENT (실험적 치아이동시 백서 상악절치 치근막의 혈관 변화에 대한 연구)

  • Vascular changes in the periodontal ligament of the rat incisors following application of experimental orthodontic forces were examined by the India ink perfusion method. 57 rats were used for this experiment. The rats were divided into experimental group (54 rats) and control group (3 rats). 54 experimental rats were divided into group I (27 rats) and group II (27 rats). The right and left upper incisors of group. I and group II rats were separated distally with forces of 20gm, 70gm respectively. The vascular changes of periodontal ligament were observed histologically by means of light microscope after 1, 2 and 3 days of tooth movement and 1,3,5,8,14, and 21 days after removal of orthodontic force. The results were as follows; 1. After one day of tooth movement, occlusion of blood vessels, hyalinization of periodontal ligament and resorption of alveolar bone adjacent to the alveolar crest on pressure side were observed. Above the tissue changes on the pressure side of group II were more severe than those of group I. Especially, septal bone of group II was separated after 2 days of tooth movement. 2. In tension zones, periodontal space was widened and periodontal fibers were orientated in the direction of puil. The blood vessels of periodontal ligament were distended. New bone deposition was seen along the inner surface of the alveolus after 2 days of tooth movement. 3. After 3 days of tooth movement, deposition of new bone was seen along the periosteal surface of alveolar bone on pressure side, progressing with increasing after removal of orthodontic force. Remodelling of the new bone was occurred 5 days after removal of orthodontic force. 4. 3 days after removal of orthodontic force, invasion of blood vessels into the marginal periodontal ligament on pressure side was observed clearly and the vessels below the epithelial attachment were increased. 5. After removal of orthodontic force, hyalinized structures disappeared concomittantly with an invasion of blood vessels from the neighboring periodontal ligament. 14 days after removal of orthodontic force, the vessels in the periodontal ligament of group I were finished the vascular rearrangement. 21 days after removal of orthodontic force, the vessels in the periodontal ligament of group II were finished the vascular rearrangement.

An evaluation of the adequacy of pont's index (Pont 지수의 임상적 적합성에 대한 평가)

  • Dental arch expansion is one of the method used to solve the dental crowding problem by non-extraction. Many formulae using tooth size have been suggested to predict ideal inter-premolar and inter-molar width. The purpose of this study was to evaluate the adequacy of some upper dental arch width prediction methods, namely Pont's method, Schmuth's method and Cha's method. The sample consisted of the casts of 119 Korean young adults who had no muscular abnormality, no skeletal discrepancy, and Angle's Class I molar relationships. Measurements were obtained directly from plaster casts; they Included mesiodistal crown diameters of the four maxillary incisors, as well as maxillary inter-first-premolar and inter-first-molar arch widths as specified by Pont. The correlation coefficients between the sum of incisors(SI) and upper dental arch width were calculated. The differences between predicted width and actual width were classified as overestimated, properestimated, and underestimated. The data obtained from each group were analyzed for statistical differences. The results were as follows : 1. Upper dental arch width indices were calculated from SI in normal occlusion (81.96 : premolar index, 62.55 : molar index). 2. Low correlations between SI and arch width were noted in normal occlusion (0.50 in the inter-premolar width, 0.39 in the inter-molar width). 3. Pont's formula and Schmuth's formula tended to overestimate the inter-premolar width. A more even distribution of estimates was noted in Cha's fomula. 4. Cases within $\pm$1 mm range of observed inter-premolar width were $45\%$ in the Cha's formula, $40\%$ in the Pont's formula, and $39\%$ in the Schmuth's formula. 5. All formulae had a tendency to underestimate the inter-molar width, but Cha's formula had better predictability than others. 6. Cases within $\pm$1 mm range of observed inter-molar width were $40\%$ in the Cha's formula, $29\%$ in the Pont's formula, and $13\%$ of Schmuth's formula. The data presented in this study does not support the clinical usefulness of ideal arch width prediction methods using the mesiodistal width of maxillary incisors.

Histologic changes of tooth and periodontal tissues applying to contraction & intrusion force for the maxillary four incisors of dogs (성견 상악 4절치의 Contraction과 압하시 치아 및 주위 조직의 조직학적 변화에 관한 실험 연구)

  • The Purposes of this study were to investigate the initial tissue changes on the teeth and surrounding tissues under contraction and intrusive force by contraction UTA. A control and experimental dogs, 10-months in age, were studied. Contraction and intrusive force(60gm) were applied at upper four incisors by contraction UTA. Experimental dogs were sacrificed at 2 weeks and 4 weeks after force application, respectively. In this study, 2 experimental groups were designed by the duration of force applied(E1, E2). The specimens were taken around the upper first and second incisor in each groups and were prepared for the H-E and MT stain for light microscopic observation. From the results of the study, the following conclusions may be drown. : 1. In control group, the periodontal ligament width was constant from apical third to cervical third of the root and periodontal fiber arrangement was horizontal or oblique in cervical third, oblique in middle third and apical third. In alveolar bond, smooth appearance was shown with osteoblast. 2. In experimental group 1, in proportion to force was concentrated at labial middle third and apical third of root of the upper first and second incisors, root of these tooth tipped labially and intruded at a time. 3. In experimental group 2, periodontal ligament width and arrangement was similar to control and observed strong calcified response at the labial middle third of root. But, alveolar bond resorption and cementum resorption were as before seen at labial middle third and apical third of root that force was concentrated.

A COMPARATIVE STUDY OF THE ANGLES BETWEEN CROWN AXIS AND ROOT AXIS IN MESIODISTAL DIRECTION BY USING ORTHOPANTOMOGRAM

  • Orthopantomogram is commonly used to evaluate root parallelism. "Good parallelism" between roots is widely accepted as one of the guidelines of a successful orthodontic treatment. In case there was a large angle between crown axis and root axis, and if we valued only the position of crown in establishing occlusal relationship without considering of the situation of root, the problem of root arrangement between adjacent teeth would be occurred. The estimate of root parallelism in mesiodistal direction before and after orthodontic treatment must be emphasized. The intent of this study was to determine the clinical importance and correlation of the angle between crown axis and root axis. Orthopantomograms of 105 orthodontic patients being treated in Yonsei university were used in this study. Twenty-eight teeth in both maxilla and mandible were selected and analyzed quantitively to evaluate the angle between crown axis and root axis, and obtain the correlationship among the individual teeth. The results are as follows: 1. Among the teeth presenting normal distribution, the maxillary right canine showed the largest mean value( $5.73{\pm}4.42^{\circ}$), which was composed of the crown-root angles, and the mandibular left lateral incisor showed the smallest mean value( $0.60{\pm}3.76^{\circ}$). 2. The crown-root angles of the maxillary incisors and the first molars, and the mandibular central incisors and the first molars didn't show normal distribution and the ranges of these angles were dispersed. 3. Significant differences were present between the crown axis and the root axis except for lower first premolars. (p<0.05) 4. No significant difference was present for the crown-root angle between right and left side, (p<0.05) 5. No significant difference was present for the crown-root angle between male and female except for lower left first premolar. (p<0.05) 6. In the upper right quadrant, significant correlations were present between crown-root angles of the central incisor and lateral incisor, lateral incisor and canine. In the upper left quadrant, significant correlations were present between crown-root angles of the central incisor and lateral incisor. In the lower right quadrant, significant correlations were present between crown-root angles of the central incisor and lateral incisor, first molar and second molar. In the lower left quadrant, significant correlations were present between crown-root angles of the central incisor and lateral incisor, lateral incisor and canine, first molar and second molar. Mode of tooth movement according to the timing of orthodontic force application after extraction (발치 후 교정력 적용 시기에 따른 치아 이동 양상)

  • This study was performed to compare and analyze the mode of tooth movement according to the timing of orthodontic force application alter extraction. The upper right and left third incisors were carefully extracted at three-week interval in lout adult dogs. Both canines were used as an anchorage for the bodily movement of the upper second incisors. Orthodontic forte of 100 gm was simultaneously applied at one week after extraction on one side and four weeks after extraction on the other side using NiTi closed coil spring. While orthodontic force was applied for twelve weeks, the amount of tooth movement was measured at every second week with digital calipers. The animals were sacrificed at twelve weeks and histologic examination was executed to reveal any difference between both sides. The results were obtained as follows. 1. The tooth movement was likely to be faster in lout-week side 4han one-week side for the first two weeks while faster in one-week side during next two weeks 2. The rate of tooth movement was fastest during four to six weeks period, then decreased gradually. 3. The total amount of tooth movement was likely to be larger in one-week side compared to four-week side. 4. Any damage to tooth and periodontal tissue could not be seen in the histologic section of one-week side. These results suggest that earlier application of orthodontic force is better than later after extraction In terms of the rate of tooth movement.

A Retrospective Study on Profile Having Favorable Response to Face Mask (상악 전방 견인 치료에 양호하게 반응하는 안모형태의 후향적 연구)

  • Skeletal Cl III malocclusion is an orthopedic appliance mainly used for growing children with maxillary undergrowth, which largely entails skeletal Cl III malocclusion. It improves anterior crossbite and maxillary position and thus, enables patients to attain favorable Profile but often involves unfavorable profile with protrusive upper and lower lips. Therefore, if orthodontists have knowledge of which condition helps obtain favorable occlusion and profile, they are able to predict the prognosis and limitation of the treatment. This study was done in order to help obtain favorable Profile after treating growing skeletal Cl III children. In the study, we classified childern into two groups, the one with favorable profile(Group 1, n=12) and the other with unfavorable profile(Group 2, n=14) and, with retrospective study using pre- and post-treatment lateral cephalogram, drew the following conclusions. 1. As patients had more serious labioversion of upper incisors, they were more unlikely to have favorable profiles after the treatment. Protrusion of prosthion, which was related with maxillary incisors, also affected profiles. 2. As the NL-ML angle before the treatment was small, it was more likely to get favorable profile. 3. As the degree of lower lip protrusion was high, it was likely to have bialveolar protrusion after the treatment. 4. As the degree of downward and backward rotation of mandible was high, it was likely to get unfavorable profile.

THE STUDY OF COMPARISON BEFORE AND AFTER ORTHODONTIC TREATMENT IN CLASS I BIALVEOLAR PROTRUSION CASES WITH FOUR BICUSPID EXTRACTION (제 1 소구치 발치를 동반한 제 I 급 치조성 양악 전돌 환자의 치료 전${\cdot}$후 비교에 관한 연구)

  • The purposes of present study were to evaluate changes in models and lateral cephalometric head films during orthodontic treatment and to compare the amount of incisal retraction and anterior movement of molars with the two approaches of the retraction method of canine(sectional canine retractions vs sliding canine retractions) and the anchorage management(head gears vs no head gears, transpalatal arches vs no transpalatal arches and lingual arches vs no lingual arches) and to evaluate changes during orthodontic treatment in models with relation to lateral cephalometric head films. 67 Korean women with Angle's Class I bialveolar protrusion were selected, whose initial chronologic age was above 16 yrs. Models and lateral cephalometric head films were taken before and after orthodontic treatment with four bicuspid extraction. the results were obtained as follows. 1. Significant decreases were observed in intermolar width, arch length and arch perimeter of maxilla and mandible but significant difference was not observed in intercanine width of maxilla and mandible during treatment period. 2. The linear change of the upper incisor to upper lip was 2.84:1 and the linear change of the lower incisor to lower lip was 1.45:1 3. There were no significant differences between the two groups(sectional canine retractions vs sliding canine retractions), the !we groups(transpalatal arches vs no transpalatal arches) and the two groups(lingual arches vs no lingual arches) in the amount of incisal retraction and anterior movement of molars. There were a greater amount of maxillary incisal retraction and a lesser amount of anterior movement of maxillary molars with the use of head gears than no use of head gears. 4. Changes during orthodontic treatment in models with relation to lateral cephalometric head films were obtained as follows : 1) Maxilla Central incisors were moved 3.79mm backward, canines were moved 0.22mm laterally and 3.70mm backward, and molars were moved 0.535mm medially and 2.29mm forward. 2) Mandible Central incisors were moved 3.04mm backward, canines were moved 0.145mm laterally and 3.92mm backward, and molars were moved 0.755mm medially and 1.77mm forward.

The Effects of Tooth Bleaching Agents on Microhardness of Enamel in situ (수종 치아미백제가 구강내에서 법랑질의 미세경도에 미치는 영향)

  • The objective of this in situ study was to evaluate the effects of whitening strip (Claren, LG Household & Health Care Ltd, 2.6% hydrogen peroxide) and gel (Opalescence, Ultradent, 10% carbamide peroxide) on microhardness of enamel in comparison with untreated control. Extracted twenty human upper incisors were disinfected, cleaned, and labial side of each incisor sectioned into 3 fragments by 2 ${\times}$ 2 mm size. After sectioning, labial sides of fragments were flattened and fixed to orthodontic bracket using flowable composite resin. Specimens prepared from each tooth were attached to the labial side of upper incisors of twenty volunteers one by one and treated by three different methods: (1) untreated control (2) treated with whitening strip for 14 days (3) treated with whitening gel for 14 days. Microhardness (Microhardness tester, Zwick) of each specimen was measured at the baseline of pre-treatment, immediate after bleaching treatment, 14 days after bleaching treatment and Knoop Hardness Number was determined. Microhardness changes of experimental groups were compared. The results show that tooth whitening strip and gel used in this study does not effect the microhardness of enamel during bleaching procedure.
