Treatment of Thoracolumbar and Lumbar Unstable Burst Fractures by Using Combined and Posterior Surgery

  • Purpose: The purpose of this research was to analyze the results of the combined and posterior approaches for treating thoracolumbar and lumbar burst fractures and to find an adequate method of treatment. Methods: We retrospectively analyzed the cases of 46 patients with unstable thoracolumbar and lumbar burst fractures who had been surgically treated. All cases were divided into two groups based on the operation method used. Eleven patients had undergone the combined approach, while 35 patients had undergone the posterior approach. Radiological and clinical evaluations were performed before surgery, after surgery, and at the final follow-up. Results: The stenotic ratios of the area occupied by the retropulsed bony fragments to the estimated area of the original spinal canal were 68.2% and 45.6% for the combined and the posterior approaches, respectively. No significant differences in the neurological improvement or the corrected state of the sagittal index were noted, but the patients who had been treated with the combined approach group had better results than those who had been treated with the posterior approach group in terms of correction and maintenance of the sagittal index. The average kyphosis corrections at the final follow-up were 15.3 degrees for the patients in the combined approach group and 10.0 degrees for those in the posterior approach group. Surgical time and estimated blood loss were all significantly higher for patients in the combined approach group. Conclusion: The combined and the posterior approaches showed similar results in the improvements of the neurologic state and the corrected state of the sagittal index. However, use of the combined approach is recommended for patients with severe kyphosis and with severe canal encroachment.

Study on terahertz (THz) photoconversion technology based on hyperfine energy-level splitting of Positronium (Ps) generated from relativistic electron beams

  • In the state of Positronium (Ps), which is an unstable material created by the temporary combination of electrons and positrons, the imaging technology through photo-conversion methodology is emerging as a new research theme under resonance conditions through terahertz electromagnetic waves. Normally, Positronium can be observed in the positron emission computed tomography (PET) process when an unstable, separate state that remains after the pair annihilation of an electron and a positron remains. In this study, terahertz (THz) waves and Cherenkov radiation (CR) are generated using the principle of ponderomotive force in the plasma wake-field acceleration, and electrons and positrons are simultaneously generated by using a relativistic electron beam without using a PET device. We confirm the possibility of Positronium photoconversion technology in terahertz electromagnetic resonance conditions through experimental studies that generate an unstable state. Here, a relativistic electron beam (REB) energy of 0.5 MeV (γ=2) was used, and the terahertz wave frequencies is G-band. Meanwhile, a THz wave mode converting three-stepped axicon lens was used to apply the photoconversion technology. Through this, light emission in the form of a luminescence-converted Bessel beam can be verified. In the future, it can be used complementarily with PET in nuclear medicine in the field of medical imaging.

돼지 Toxoplasmosis 의 간접 적혈구응집반응과 피내반응에 관한 연구 (Studies on Passive Hemagglutination Test and Skin Test for Toxoplasmosis in Swine)

  • Hemagglutinating antigen of Toxoplasma gondii was prepared and purified by the method of a slight modification of Tsunematsu, and the preparation of the skin test antigen (toxoplasmin) was made by means of acetone-ether treatment described by Nobute et al. With these antigens the passive hemagglutionation and skin tests were performed for the diagnosis of swine toxoplasmosis by using artificially infected pigs. The results obtained were summarized as follows: 1. The hemagglutinating antibody and the skin test antibody were demonstrated one and three weeks after infection, respectively. And these antibodies were maintained over nine weeks after infection. 2. The antigenicity of hemagglutinating antigen was stable when it was kept in frozen state, while was unstable in a liquid state. 3. Freeze-dried skin test antigen (toxoplasmin) was stable for two months or more if it was kept at $5^{\circ}C$ and room temperature, but in the liquid or reconstituted state it was unstable. 4. Freeze-dried skin test antigen could be preserved without loss of antigenicity for more than two months. 5. Passive hemagglutination test could be applied effectively at the early phase of the disease process and skin test at later phase, mainly for epidemiological survey. However, by combiniation of these methods, the more accurate results could be obtained.

Characterization of the Effect of the Inlet Operating Conditions on the Performance of Lean Premixed Gas Turbine Combustors

  • An experimental study of the effect of operating conditions on the behavior of a lean premixed laboratory combustor operating on natural gas has been conducted. Measurements were made characterizing the pressure fluctuations in the combustor and the flame structure over a range of inlet temperatures, inlet velocities and equivalence ratios. In addition the fuel distribution at the inlet to the combustor was varied such that it was an independent parameter in the experiment. Inlet temperature, inlet velocity and equivalence ratio were all found to have an effect on the stability characteristics of the combustor. The nature of this effect, however, depended on the fuel distribution. For example, with one fuel distribution the combustor would become unstable when the temperature was increased, whereas with a different fuel distribution the combustor would become unstable when the temperature was decreased. Similarly, the operating conditions had an effect on the flame structure. For example the intensity-weighted center of mass of the flame was found to move closer to the center body as either the temperature or equivalence ratio increased. It was interesting and somewhat surprising to note, however, that as the location of the center of mass changed with operating conditions it did so by moving along a line of constant flame angle.

  • The sliding-mode control technique could make a system unstable which external disturbance and uncertainty exists in. This paper suggests a robust sliding-mode control algorithm which can be applied to a linear system with parameter uncertainties. To reduce the chattering effect, the whole system is comprised of using a state variable in which the state's estimated value is added. The condition of estimated state results from state observer. The proposed control algorithm uses the optimal feedback controller following the dynamic system equation which consists of a state variable resulting from its own state variable, controller input, estimated state variable. Through comparison with the time optimal control algorithm using simulation, the suggested algorithm shows the improved stability and robustness while it manifests the fast tracking characteristics.

Normal Mode Approach to the Stability Analysis of Rossby-Haurwitz Wave

  • The stability of the steady Rossby-Haurwitz wave (R-H wave) in the nondivergent barotropic model (NBM) on the sphere was investigated with the normal mode method. The linearized NBM equation with respect to the R-H wave was formulated into the eigenvalue-eigenvector problem consisting of the huge sparse matrix by expanding the variables with the spherical harmonic functions. It was shown that the definite threshold R-H wave amplitude for instability could be obtained by the normal mode method. It was revealed that some unstable modes were stationary, which tend to amplify without the time change of the spatial structure. The maximum growth rate of the most unstable mode turned out to be in almost linear proportion to the R-H wave amplitude. As a whole, the growth rate of the unstable mode was found to increase with the zonal- and total-wavenumber. The most unstable mode turned out to consist of more-than-one zonal wavenumber, and in some cases, the mode exhibited a discontinuity over the local domain of weak or vanishing flow. The normal mode method developed here could be readily extended to the basic state comprised of multiple zonalwavenumber components as far as the same total wavenumber is given.

On-line System Identification using State Observer

  • This paper deals one of the methods of system identification, especially on-line system identification in time-domain. The algorithm in this study needs all states of the system as well input to it for system identification. In this reason, Kalman filter is used for state estimation. But in order to implement a state estimator, the fact that a system model must be known is logical contradiction. To overcome this, state estimation and system parameter estimation are performed simultaneously in one sample. And the result of the system parameter estimation is used as basis to state estimation in next sample. On-line system identification comes, in every sample by performing both processes of state estimation and parameter estimation that are related mutually and recursively. This paper demonstrates the validity of proposed algorithm through an example of an unstable inverted pendulum system. This algorithm can be useful for on-line system identification of a system that has fewer number of measurable output than system order or number of states.

Effects of Zinc on the Activity and Conformational Changes of Arginine Kinase and Its Intermediate

  • The effects of zinc on arginine kinase and its collapsed-state intermediate were studied. Both arginine kinase and the collapsed-state intermediate were inactivated in the presence of zinc, following a biphasic kinetic course. The corresponding apparent rate constants of inactivation at different zinc concentrations and conformational changes in the presence of 0.5 mM zinc were obtained. The conformational changes of arginine kinase and the collapsed-state intermediate were followed by fluorescence spectra and circular dichroism spectra. Comparison of the results for arginine kinase and the collapsed-state intermediate showed that the collapsed-state intermediate was more susceptible to zinc, which indicated that the collapsed-state intermediate was more flexible and unstable than arginine kinase. The special structure of arginine kinase might explain these diverse phenomena.

STABILITY OF POSITIVE STEADY-STATE SOLUTIONS IN A DELAYED LOTKA-VOLTERRA DIFFUSION SYSTEM

  • This paper considers the stability of positive steady-state solutions bifurcating from the trivial solution in a delayed Lotka-Volterra two-species predator-prey diffusion system with a discrete delay and subject to the homogeneous Dirichlet boundary conditions on a general bounded open spatial domain with smooth boundary. The existence, uniqueness and asymptotic expressions of small positive steady-sate solutions bifurcating from the trivial solution are given by using the implicit function theorem. By regarding the time delay as the bifurcation parameter and analyzing in detail the eigenvalue problems of system at the positive steady-state solutions, the asymptotic stability of bifurcating steady-state solutions is studied. It is demonstrated that the bifurcating steady-state solutions are asymptotically stable when the delay is less than a certain critical value and is unstable when the delay is greater than this critical value and the system under consideration can undergo a Hopf bifurcation at the bifurcating steady-state solutions when the delay crosses through a sequence of critical values.

  • Objectives : Since vertebroplasty has been introduced, we performed short segment pedicle screw fixation with augmented intra-operative vertebroplasty in patients with unstable thoraco-lumbar fracture. Our intentions are to demonstrate the efficacy and indication of this new technique compare to conventional methods. Material and Methods : The surgery comprised of pedicle screw fixations on one level above and below the fracture site, and the fractured level itself, if pedicle is intact, and intra-operative vertebroplasty under the fluoroscopic guide with in-situ postero-lateral bone graft. Also, in cases of bone apposition, we removed those with small impactor through a transfascetal route. During the last 2 years, we performed in seven(7) unstable thoraco-lumbar fracture patients who consisted of two different characteristics, those four(4) with primary or secondary osteoporosis and three(3) of young and very healthy. All patients were followed clinically by A.S.I.A. score and radiography. Results : Mean follow up period was 14 months. We observed well decompressed state via transfascetal route in cases of bone fragments apposition and no hardware pullout in osteoporotic cases, no poly-methyl-methacrylate (PMMA) leakage through the fracture sites into the spinal canal, and no kyphotic deformities in both cases during follow-up periods. All patients demonstrated solid bony fusion except one following osteoporotic compression fracture on other sites. Conclusions : In the management of unstable thoraco-lumbar fracture, we believe that this short segment pedicle screw fixation with augmented intra-operative vertebroplasty reduce the total length or levels of pedicle screw fixation without post-operative kyphotic deformity.
