The Effectiveness of Ultrasound-Guided Soyeom Pharmacopuncture Therapy at Acromioclavicular Joint of Shoulder in Patients with Anterior Shoulder Pain: A Retrospective Study (견관절 전방 통증 환자에서 초음파 유도하 견쇄관절 소염약침요법 병행 치료의 효과: 후향적 연구)

  • Objectives The purpose of this study was to observe effectiveness of ultrasound-guided Soyeom pharmacopuncture therapy at acromioclavicular joint of shoulder in patients with anterior shoulder pain. Methods We analyzed medical records of 9 patients hospitalized with anterior shoulder pain who had admitted to Department of Korean Rehabilitation Medicine, College of Korean Medicine, Daejeon University from March 1, 2021 to June 13, 2021. 9 patients were treated the treatment of Soyeom pharmacopuncture therapy on acromioclavicular joint under ultrasonic guidance with Korean medicine treatment (such as herbal medicine, acupuncture, moxabustion, etc). The study was conducted as a retrospective observation study which analyze the patient's medical records. We used numeric rating scale (NRS) to evaluate pain reduction and the shoulder range of motion (ROM) to evaluate function improvement twice (before pharmacopuncture treatment and 5 days later treatment). Statistical analysis was performed using the IBM SPSS Statistics version 25 program (IBM Co., Armonk, NY, USA). Results The average of numeric rating scale reduced statistically significantly from 5.56±1.13 to 3.11±1.36 (p=0.004). The average of shoulder flexion ROM increased statistically significantly from 166.67±8.17° to 175.00±8.37° (p<0.05). And the average of shoulder abduction ROM increased statistically significantly from 158.57±20.35° to 172.86±12.53° (p<0.05). Both NRS and shouler ROM showed statistically significantly improve after treatment. Conclusions This study shows ultrasound-guided Soyeom pharmacopuncture therapy at acromioclavicular joint of shoulder has a meaningful clinical effect on the improving shoulder pain and ROM especially on flexion and abduction.

The Study on the Perceptions of Radiological Technologist in Medical Imaging Equipment Used by the Oriental Doctor (한의사의 의료영상장비 사용에 대한 방사선사의 인식도)

  • In order to examine how Radiological Technologists perceive the oriental doctor's use of Medical Imaging Equipment, surveys were conducted for the members of the Korean Radiological Technologists Association. The total number of respondents were 515 and 481, with 34 insincere responses removed caused of nonvalidated answer. The results of the analysis are as follows. Although there were no statistical significance in the difference in perception by location of residence, work place, and educational background, respondents with higher education showed a tendency to agree on the use of comprehensive medical imaging equipment, but tended to oppose the use of special medical imaging equipment. Differences in perception by gender showed a greater negative perception toward the oriental doctor's use of medical imaging equipment by women than men. In particular, women showed more negative tendency for oriental doctor's use of special medical imaging equipment such as MRI, CT, and ultrasound equipment compared to men, and this was statistically significant. The difference in perception by age showed that the oriental doctor's use of medical imaging equipment was negative in the 20~30s, neutral in the 40~50s, and positive in the 60s, which were statistically significant. The difference in perception by work experience showed that the longer the work experience was, the more positive it was toward oriental doctor's use of medical imaging equipment. Specifically, the most favorable tendency was found with work experience of more than 30 years, which was statistically significant. The results of this study revealed the Radiological Technologists' perceptions on the oriental doctor's use of Medical Imaging Equipment and this can contribute to the direction of public health promotion in the future.

Endoscopic Bio-Imaging Using Optical Coherence Tomography (마이크로 내시경 및 첨단 광 단층촬영기법을 이용한 생체 이미징)

  • Optical coherence tomography(OCT) is an emerging medical diagnostic tool that draws great attention in medical and biological fields. It has a 10-100 times higher spatial resolution than that of the clinical ultrasound but lower imaging depth such as 1-2 mm. In order to image internal organs, OCT needs an endoscopic probe. In this paper, the principle of Fourier-domain optical coherence tomography with high-speed imaging capability was introduced. An OCT endoscope based on MEMS technology was developed. It was attached to the Fourier-domain OCT system to acquire three-dimensional tomographic images of gastrointestinal tract of New Zealand white rabbit. The endoscope had a two-axis scanning mirror that was driven by electrostatic force. The mirror stirred an incident light to sweep two-dimensional plane by scanning. The outer diameter of the endoscope was 6 mm and the mirror diameter was 1.2 mm. A three-dimensional image rendered by 200 two-dimensional tomographs with $200{\times}500$ pixels was displayed within 3.5 seconds. The spatial resolution of the OCT system was 8 ${\mu}m$ in air.

  • In this parer, a new harmonic imaging technique is Proposed and evaluated experimentally. In the Proposed method. a weighted chirp signal with a hanning window is transmitted. The RF samples obtained on each array element are individually compressed by correlating with the reference signal defined as the 2nd harmonic component($2f_0$) of a transmitted chirp signal generated in a square-law system. The correlator output will then consist of the compressed version of the $2f_0$ component generated in tissue and the crosscorrelation sequence of the fundamental($f_0$) and 2f$_{0}$components. The Proposed method uses the compressed $2f_0$ component to form an image. for which the crosscorrelation term should be suppressed below at least -50dB. The Proposed method has two process, 2f$_{0}$-correlation and $2f_0$-correlation(PI) . $2f_0$-correlation can successfully eliminate the $f_0$ component with a single transmit-receive events and therefore is more efficient than the conventional Pulse inversion method in terms of the frame rate. 2i)-correlation(Pl) Performs pulse compression after applying pulse inversion method for the 2nd harmonic image with high resolution and SNR. Another advantage of the proposed method is that the SNR of 2nd harmonic imaging can be improved without limitation by increasing the duration of the chirp signal. The proposed method was verified through both the computer simulations and actual experiments .ts .

Association of Ultrasonography Features of Follicular Thyroid Carcinoma With Tumor Invasiveness and Prognosis Based on WHO Classification and TERT Promoter Mutation

  • Objective: To investigate the association of ultrasound (US) features of follicular thyroid carcinoma (FTC) with tumor invasiveness and prognosis based on the World Health Organization (WHO) classification and telomerase reverse transcriptase (TERT) promoter mutations. Materials and Methods: This retrospective study included 54 surgically confirmed FTC patients with US images and TERT promoter mutations (41 females and 13 males; median age [interquartile range], 40 years [30-51 years]). The WHO classification consisted of minimally invasive (MI), encapsulated angioinvasive (EA), and widely invasive (WI) FTCs. Alternative classifications included Group 1 (MI-FTC and EA-FTC with wild type TERT), Group 2 (WI-FTC with wild type TERT), and Group 3 (EA-FTC and WI-FTC with mutant TERT). Each nodule was categorized according to the US patterns of the Korean Thyroid Imaging Reporting and Data System (K-TIRADS) and American College of Radiology-TIRADS (ACR-TIRADS). The Jonckheere-Terpstra and Cochran-Armitage tests were used for statistical analysis. Results: Among 54 patients, 29 (53.7%) had MI-FTC, 16 (29.6%) had EA-FTC, and nine (16.7%) had WI-FTC. In both the classifications, lobulation, irregular margins, and final assessment categories showed significant differences (all Ps ≤ 0.04). Furthermore, the incidences of lobulation, irregular margin, and high suspicion category tended to increase with increasing tumor invasiveness and worse prognosis (all Ps for trend ≤ 0.006). In the WHO groups, hypoechogenicity differed significantly among the groups (P = 0.01) and tended to increase in proportion as tumor invasiveness increased (P for trend = 0.02). In the alternative group, punctate echogenic foci were associated with prognosis (P = 0.03, P for trend = 0.03). Conclusion: Increasing tumor invasiveness and worsening prognosis in FTC based on the WHO classification and TERT promoter mutation results were positively correlated with US features that indicate malignant probability according to both K-TIRADS and ACR-TIRADS.

Altered Motor Control in Patients With Neck Pain and Prospective Research Work (경부통증 환자의 변형된 운동조절 특성 분석과 향후 활용 방안)

  • Background: It is necessary to find and develop the effective way of intervention for patients with neck pain, since the neck pain is becoming increasingly common throughout the world. To identify the altered motor control in patient with neck pain would be informative to find and develop the effective way of intervention. Objects: The aim of this study was to review literature regarding the altered motor control in patients with neck pain, measured by using surface electromyography (sEMG), ultrasonography, and functional magnetic resonance imaging (fMRI), and to suggest prospective research work on neck pain. Methods: Case-control (neck pain/healthy) studies published between 2004 and 2015 that investigated neck muscle activation, thickness, cross-sectional area, and fat infiltrate were searched in Scopus, PubMed, and ScienceDirect. Twenty-eight articles were included in this study. Results: sEMG, ultrasonography, and fMRI were used complementarily to investigate the altered superficial and deep neck muscle activation, thickness, cross-sectional area, and fat infiltrate in patients with neck pain. They showed the following altered motor control when compared retrospectively with healthy subjects or during specific functional tasks: (1) increased superficial muscle activation, (2) lesser deep muscle thickness, (3) smaller cross-sectional area of the deep muscle, and (4) greater fat infiltrate in deep muscles. In particular, among the women, the office workers showed higher muscle activation of superficial neck muscles during functional tasks, although they did not have neck pain, than those who were not office workers. Conclusion: Studies revealed that patients with neck pain showed an altered motor control when compared with healthy subjects by using various assessment modalities. Understanding this phenomenon would help researchers design an effective intervention for alleviating neck pain or to evaluate the effectiveness of the intervention. In addition, we recommend that female office workers take measures to care for their necks before developing neck pain.

Preoperative Evaluation of the Facial Artery Using Facial Angio Computed Tomography (전산화단층촬영 혈관조영술을 이용한 얼굴동맥의 수술 전 평가)

  • Purpose: Previous studies of the facial artery have shown significant anatomical variability in this region. The vascular anatomy of the region is considered unreliable in predicting the ideal pedicle. Preoperative imaging has been suggested as a means of improving preoperative awareness, with Doppler ultrasound as useful tools. Multi-detector row angiographic computed tomography (angio CT) is a significant improvement, providing noninvasive operator-independent details of the vascular anatomy. This tool was used to perform an $in$ $vivo$ anatomical study of the facial artery, demonstrating the usefulness of facial angio CT in planning the facial reconstruction. Methods: Eleven consecutive patients underwent facial angio CT of the facial vasculature with the anatomical details of the facial artery assessed. Results: Facial angio CT could demonstrate the size and course of the facial vasculature, particularly the facial artery. Conclusion: The vascular anatomy of the facial artery is highly variable, and thus there is a role for preoperative imaging. Facial angio CT can demonstrate cases where there is an aberrant or non-preferred anatomy, or select the method of a facial reconstruction.

Assessing the Potential of Thermal Imaging in Recognition of Breast Cancer

  • Background: Breast cancer is a common disorder in women, constituting one of the main causes of death all over the world. The purpose of this study was to determine the diagnostic value of the breast tissue diseases by the help of thermography. Materials and Methods: In this paper, we applied non-contact infrared camera, INFREC R500 for evaluating the capabilities of thermography. The study was conducted on 60 patients suspected of breast disease, who were referred to Imam Khomeini Imaging Center. Information obtained from the questionnaires and clinical examinations along with the obtained diagnostic results from ultrasound images, biopsies and thermography, were analyzed. The results indicated that the use of thermography as well as the asymmetry technique is useful in identifying hypoechoic as well as cystic masses. It should be noted that the patient should not suffer from breast discharge. Results: The accuracy of asymmetry technique identification is respectively 91/89% and 92/30%. Also the accuracy of the exact location of identification is on the 61/53% and 75%. The approach also proved effective in identifying heterogeneous lesions, fibroadenomas, and intraductal masses, but not ISO-echoes and calcified masses. Conclusions: According to the results of the investigation, thermography may be useful in the initial screening and supplementation of diagnostic procedures due to its safety (its non-radiation properties), low cost and the good recognition of breast tissue disease.

Development of a Piezoelectric Micro-machined Ultrasonic Transducer for Photoacoustic Imaging that Accounts for the Added Mass Effect of the Acoustic Medium (음향 매질의 추가질량 효과를 고려한 광음향 영상용 초소형 압전 기반 초음파 트랜스듀서의 개발)

  • Typically, photoacoustic images are obtained in water or gelatin because the impedance of these mediums is similar to that of the human body. However, these acoustic mediums can have an additional mass effect that changes the resonance frequency of the transducer. The acoustic radiation impedance in air is negligible because it is very small compared to that of the transducer. However, the high acoustic impedance of mediums such as the human body and water is quite large compared to that of air, making it difficult to ignore. Specifically, in a case where the equivalent mass is very small, such as with a micro-machined ultrasound transducer, the additional mass effects of the acoustic medium should be considered for an accurate resonance frequency design. In this study, a piezoelectric micro-machined ultrasonic transducer (pMUT) was designed to have a resonance frequency of 10 MHz in the acoustic medium of water, which has similar impedance as the human body. At that time, the resonance frequency of the pMUT in air was calculated at 15.2 MHz. When measuring the center displacement of the manufactured pMUT using a laser vibrometer, the resonance frequencies were measured as 14.3-15.1 MHz, which is consistent with the finite element method (FEM) simulation results. Finally, photoacoustic images of human hair samples were successfully obtained using the fabricated pMUT.

A Parametric Image Enhancement Technique for Contrast-Enhanced Ultrasonography (조영증강 의료 초음파 진단에서 파라미터 영상의 개선 기법)

  • The transit time of contrast agents and the parameters of time-intensity curves in ultrasonography are important factors to diagnose various diseases of a digestive organ. We have implemented an automatic parametric imaging method to overcome the difficulty of the diagnosis by naked eyes. However, the micro-bubble noise and the respiratory motions may degrade the reliability of the parameter images. In this paper, we introduce an optimization technique based on MRF(Markov Random Field) model to enhance the quality of the parameter images, and present an image tracking algorithm to compensate the image distortion by respiratory motions. A method to extract the respiration periods from the ultrasound image sequence has been developed. We have implemented the ROI(Region of Interest) tracking algorithm using the dynamic weights and a momentum factor based on these periods. An energy function is defined for the Gibbs sampler of the image enhancement method. Through the experiments using the data to diagnose liver lesions, we have shown that the proposed method improves the quality of the parametric images.
