Estimation of Gestational Age by Measurement of Fetal and Extra-fetal Structures in Miniature Schnauzer Bitches

  • Serial ultrasonographic examinations were performed on 9 Miniature Schnauzer bitches from day 15 of gestation until parturition to determine the ultrasonographic measurement of gestational structures. Ovulation was designated the day that plasma progesterone concentration exceeded 4.0 ng/ml (day 0). Extra-fetal structures were measured from day 17 or 18 to 60. Outer uterine diameter (OUD) increased from $6.50{\pm}1.06mm$ ($Mean{\pm}SD$) to $50.89{\pm}5.62mm$, inner chorionic cavity diameter (ICCD) increased from $2.10{\pm}0.15mm$ to $37.15{\pm}4.36mm$, and length of placenta (PL) increased from $7.50 {\pm}1.41mm$ to $40.62{\pm}3.27mm$. OUD and ICCD were significantly and linearly relative to gestational age especially through day 37, whereas PL was not significantly relative to gestational age. Of the extra-fetal structures, ICCD was the most accurate for estimation of gestational age before day 38 of gestation. Fetal structures increased at a linear rate to parturition. Fetal crown-rump length (CRL) increased from $2.55{\pm}0.07mm$ on day 22 to $85.25{\pm}9.89mm$ on day 48. Fetal head diameter (HD) increased from $3.43{\pm}0.64mm$ on day 27 to $25.06{\pm}0.41mm$ on day 63. Fetal body diameter (BD) increased from $ 5.96{\pm}0.84mm$ on day 30 to $43.76{\pm}3.36mm$ on day 63. Of the fetal structures, HD was the most accurate for estimation of gestational age after day 38 to parturition.

Ultrasonographic Indeterminate Lymph Nodes in Preoperative Thyroid Cancer Patients: Malignancy Risk and Ultrasonographic Findings Predictive of Malignancy

  • Objective: Proper management of lymph nodes (LNs) with ultrasonographic (US) indeterminate features in thyroid cancer patients remains elusive. We aimed to evaluate the malignancy risk and US findings predictive of malignancy for US indeterminate LNs in preoperative thyroid cancer patients through node-by-node correlation. Materials and Methods: A total of 348 LNs in 284 thyroid cancer patients, who underwent fine-needle aspiration or core-needle biopsy between December 2006 and June 2015, were included. We determined the malignancy risks for US probably benign, indeterminate, and suspicious categories. For US indeterminate LNs, which had neither echogenic hilum nor hilar vascularity in the absence of any suspicious finding, US findings were compared between benign and metastatic LNs using Mann-Whitney U test and Fisher's exact test. Results: US imaging diagnoses were probably benign in 20.7% (n = 72) cases, indeterminate in 23.6% (n = 82), and suspicious in 55.7% (n = 194). Malignancy risk of US indeterminate LNs (19.5% [16/82]) differed from those of the US probably benign (2.8% [2/72]) (p = 0.002) and US suspicious LNs (78.4% [152/194]) (p < 0.001). Among US indeterminate LNs, there were no significant differences in short, long, and long-to-short diameter (L/S) ratios between benign and metastatic LNs (3.9 vs. 3.8 mm, p = 0.619; 7.3 vs. 7.3 mm, p = 0.590; 1.9 vs. 1.9, p = 0.652). Conclusion: US indeterminate LNs were frequently encountered during preoperative evaluation and had intermediate malignancy risk. Given the lack of discriminative power of size criteria and L/S ratio, clinical factors such as surgical strategy and node size should be considered for proper triage of US indeterminate LNs in thyroid cancer.

진도개에서 분만후 자궁수복의 연속적 초음파상 (Serial ultrasonographic appearance of postpartum uterine involution in Korea Jin-do dogs)

  • This study was undertaken to determine the normal appearance of the postpartum uterine involution. Postpartum changes in uterine shape, architecture, echogenicity and diameter were monitored with ultrasonography in 11 Korea Jin-do dogs. Serial ultrasonographic examination was done daily during the first week, 3 days interval from 8 to 30 days, and weekly from 31 to 100 days postpartum, respectively. Of 11 postpartum bitches, 10 bitches (90.9%) had normal involution and 1 bitch (9.1%) had subinvolution of the placental sites (SIPS) by gross findings, vaginal discharges, and by ultrasographic findings, uterine shape and echogenicity. The excretory period of vaginal discharges in 10 normal bitches of uterine involution was finished completely at $20.2{\pm}4.6$ days (Mean${\pm}$SD) postpartum, but in 1 SIPS at 50 days postpartum. The short axis shape of the uterus was initially often flaccid-appearing. It varied from circular to crescent shaped to polygonal. This lasted until $16.5{\pm}3.7$ days postpartum, during which time the short axis uterine shape gradually changed to circular. Also, the long axis shape of the uterus was created a beaded appearance of the horns until $29.9{\pm}3.2$ days postpartum. After 30 days, it was appeared as tubular shape without distinguished between placental and interplacental sites. The ultrasonographic image of the postpartum uterus consisted of four echogenicity distinct layers. Uterine wall was represented the very hyperechoic serosa, hypoechoic myometrium, hyper echoic endometrium and anechoic structures of fluid in the uterine cavity until 7 days postpartum. The individual uterine layers were most prominent during the first week postpartum, and they became progressively less distinct throughout the course of uterine involution. Anechoic structures of fluid in the uterine cavity was detected until $25.0{\pm}6.4$ days postpartum, after which time it was not reliably detected. The uterine diameter was decreased not only in the placental sites frorm $24.1{\pm}2.5mm$ at 1 day to $15.4{\pm}1.4mm$ at 7 days postpartum, but also in the interplacental sites $14.9{\pm}1.5mm$ at 1 day, $10.6{\pm}0.8mm$ at 7 days postpartum. There was a general trend of decreasing uterine diameter, which occurred more rapidly at the placental sites. At 31 days pastpartum, these diameter reached almost same size, after that time, they could be not distinguished between placental and interplacental sites. At 87 days postpartum, the uterine diameter was $5.6{\pm}0.6mm$ both placental and interplacental sites, and the uterine horns were uniform hypoechoic, tubular structures without enlargement. Therefore, complete involution of the uterus occurred at that time. It was concluded that normal postpartum uterine involution in Korea Jin-do bitches appeared to be completed around 87 days postpartum by gross findings such as vaginal discharges, and by ultrasonographic findings, uterine shape and echogenicity. Also ultrasonographic characteristics of the postpartum uterine involution were described. Therefore, these result suggest that ultrasonographic assessment is a reliable method for diagnosing the SIPS and uterine dysfunction, such as pyometra in bitch.

수근관 증후군에서 임상양상척도 및 신경전기진단 결과와 정중신경 초음파 소견의 상관관계 (The Correlation between Ultrasonographic Findings of Median Nerve and Clinical Scale and Electrodiagnotic Data in Carpal Tunnel Syndrome)

  • Background: The aim of this study is to identify the correlation between ultrasonographic findings of median nerve and clinical scale and electrophysiologic data in carpal tunnel syndrome. Methods: Forty three patients (79 hands) with electrophysiologically confirmed carpal tunnel syndrome were evaluated. Clinical symptoms were examined by Historical-Objective (Hi-Ob) scale. Electrophysiologic data and Padua scale were used for severity of electrophysiology. In ultrasonographic study, cross sectional area and flattening ratio of median nerve were measured at distal wrist crease level (DWC), 1cm proximal to distal wrist crease level, and 1cm distal to distal wrist crease level. The correlation between Hi-Ob scale, electrophysiologic data and ultrasonography was measured with Spearman rank test. Results: The mean Hi-Ob scale was 2.4. Mean Padua scale was 4.0. In ultrasnonographic study, cross sectional area and flattening ratio were $0.112\;cm^2{\pm}0.025$ and $3.0{\pm}0.6$ at 1cm proximal to DWC level, $0.118{\pm}0.026\;cm^2$ and $2.9{\pm}0.4$ at DWC level, and $0.107{\pm}0.032\;cm^2$ and $3.0{\pm}0.4$ at 1 cm distal to DWC level. Hi-Ob scale was not correlated with cross sectional area and flattening ratio of median nerve. Hi-Ob scale was correlated with Padua scale positively (r=0.44) and correlated with amplitudes of CMAP and SNAP, negatively (r=-0.33; r=-0.30). Cross sectional area of median nerve was significantly correlated with Padua scale, amplitudes and latencies of CMAP and amplitudes of SNAP. Conclusions: Ultrasonographic findings of median nerve and electrodiagnostic data had statistically significant correlation. Consequently, ultrasonography could be an adjunctive method in diagnosis of carpal tunnel syndrome.

  • Background: The incidence rate of thyroid cancer has been increasing worldwide in recent years, and it is also the most prevalent cancer when it comes to the number of patients among Korean women. With it, ultrasonographic screening test has also become very common. However, there is still controversy over the performance of this screening test. Therefore, the National Evidence-based Healthcare Collaborating Agency (NECA) organized a Round-table Conference on the issues regarding ultrasonographic screening for thyroid cancer in Korea. The objective of the conference was mainly about delivering worthwhile information reflecting social value for the current situation, which was based on evidence surrounding thyroid cancer screening that relevant experts investigated and agreed on. The significance of this Round-table Conference lies in the fact that we reviewed the current evidence, and we were able to discuss the social value and future direction for ultrasonographic screening in Korea.

회전근 개 파열의 수술 전 3차원 초음파 검사에서 관절내 생리식염수 주사 후 검사의 정확도 (Three Dimensional Ultrasonographic Evaluation with Intra-articular Saline Injection in Rotator Cuff Tear)

  • 목적: 회전근 개 파열이 있는 환자들에서 3차원 초음파 검사를 시행할 때 생리식염수 20 ml를 견관절강 내에 주사 하기 전과 후의 파열 길이 측정과 수술시 측정과의 차이를 비교 분석하여 초음파 검사시 생리식염수의 관절내 주사가 파열의 길이 측정에 더 유용한 방법인지 확인하고자 하였다. 대상 및 방법: 2007년 8월부터 2008년 9월까지 회전근 개 파열을 진단받고 수술 받은 환자 14명에 대하여 수술 전 3차원 초음파를 이용하여 측정한 회전근 개 파열의 크기와 실제 수술 중에 측정한 파열의 크기를 비교 분석하였다. 3차원 초음파를 이용한 회전근 개 파열의 측정은 관절강 내에 20 ml의 생리식염수를 주사 하기 전, 후로 파열된 회전근 개의 너비와 내측으로의 이동 정도를 측정하였으며, 관절경 수술시에는 Kirschner 강선을 척수 바늘에 통과시키는 방법으로 파열의 길이를 측정하였고 개방적 수술시에는 자를 가지고 직접 측정하였다. 결과: 3차원 초음파 검사시 생리식염수를 주사하기 전 측정한 크기와 실제 파열된 크기를 비교하였을 때 파열의 너비의 차이는 실제 크기보다 평균 8 mm 작게 측정되었고 내측으로의 이동 정도는 평균 1.9 mm 작게 측정되었다. 생리식염수 주사 후에 측정한 결과는 초음파 검사에서 실제 크기보다 평균 4.1 mm 작게 측정되고, 내측으로의 이동의 정도는 평균 1.6 mm 크게 측정되었다. 결론: 3차원 초음파를 이용한 회전근 개 파열의 진단에서 보다 정확한 크기를 측정하기 위하여 관절강 내에 생리 식염수를 주사한 후 검사하면 보다 정확하고 실제 크기와 유사한 측정이 가능할 것으로 사료된다.

견부손상환자의 팔의 자세에 따른 초음파 영상을 이용한 견봉하공간 거리의 비교 (Comparison of Distance of Subacromial Space Using Ultrasonographic Measurement on Arm Positions of Shoulder Injured Patients)

  • Purpose : Recently ultrasound images has been used to measure the subacromial space with a linear measurement of the acromiohumeral distance. The purpose of this study was to comparison of distance of subacromial space using ultrasonographic measurement on arm positions of shoulder injured patients. Methods : The subjects were 30 shoulder injured patients (19 males and 11 females). Ultrasonography was performed to measure subacromial space during each shoulder resting position, $45^{\circ}$ abduction, internal rotation with $45^{\circ}$ abduction and external rotation with $45^{\circ}$ abduction. Results : Subacromial space was a significantly difference between injured arm and normal arm at shoulder resting position. At $45^{\circ}$ abduction, subacromial space was narrowing significantly between injured arm and normal arm. At external rotation with $45^{\circ}$ abduction, subacromial space were wider in normal arm than in injured arm but it was not significantly. Intra-observer reliability for ultrasonography measurement of subacromial distance was excellent (.96~.99). Conclusion : These results identified that positions of injured shoulder was related to subacromion space.

성인 남녀의 초음파 조영술로 측정한 간지방밀도와 혈청 간기능효소와의 상관성 연구 (A Study on the Ultrasonographic Liver Fat Density and Serum Enzymes for Testing Liver Function in Korean Adults)

  • This study was done to investigate the relationship between ultrasonograph fat density (ULFD) using objective measurement and serum enzymes for testing liver function in 101 healthy adults(43 males and 58 females). Average serum enzyme activities in males and females were GOT27.111U/L and 22.46IU/L, GPT 34.06IU/L and 18.501U/L, and ${\gamma}$-GTP 37.67IU/L and 17.201U/L, respectively. Males showed significantly higher activities of GPT and ${\gamma}$-GTP than females. ULFD of the obese group (BMI$\geq$25) was significantly higher than that of the nonobese group. GOT, GPT, and ${\gamma}$-GTP tended to be high in the obese group. GPT and ${\gamma}$-GTP of the high TG group (TG$\geq$170) tended to be markedly high for males, but not for females. GPT was positively correlated with ULFD, body weight , and weight-to-height, ratio, and ${\gamma}$-GTP was positively correlated with body weight, weight-to-height ratio. BNI, and KI. ULFD and ${\gamma}$-GTP were positively correlated with serum TG. These results suggests that , among serum enzymes for testing liver function, GPT has a close relationship with ULFD using objective measurement, while GOT does not. Also , ${\gamma}$-GTP has a close relationship with parameters for obesity.
