개에서 외골격 고정을 이용한 성장 기형의 교정 (Application of External Skeletal Fixations to Correct the Growth Deformities in a Dog)

  • A male, 11-month-old, Shih-tzu dog weighing 5.2kg showing knuckling secondary to severe cranial bowing of left antebrachium with valgus deformity of the carpus was presented to the Veterinary Medicine Teaching Hospital of Seoul National University. Radiographic findings were a premature closure of the distal ulnar physis, a cranial bowing of the radius, subluxation of the elbow joint, and degenerative changes in the carpal joint. The patient was treated by performing an oblique osteotomy of the radius and ulna, followed by application of External Skeletal Fixations. At 18 weeks postoperatively, the radius was healed. An incongruency of the elbow joint and radial angular deformity was corrected.

  • We report a case of 44 years old male patient with neuroma-in-continuity of ulna nerve. In the patient's past history, he had received operative treatment for the open supracondylar fracture of right distal humerus and ulnar nerve injury at 10 years ago, and neurolysis was tried 2 times due to severe neuropathic pain. Despite of these operations, the symptom was not improved. In operative field, we noticed neuroma-in-continuity and decided to resect the neuroma until normal nerve fascicle was noted. The nerve cable graft was done with auto sural nerve on the defect site and the nerve was wrapped with small saphenous vein. At post operative 7 months, pain was markedly decreased and sensory recovery was slightly improved and patient was satisfied with the result.

Cubital Tunnel Syndrome, Associated With Synovial Chondromatosis

  • A 62-year-old female patient suffered from numbness and resting pain in the right ring and little fingers for 3 years. We confirmed cubital tunnel syndrome with electrodiagnostic study and performed the operation. We found seven firm consistent nodules, compressing the overlying the ulnar nerve, proximal to the medial epicondyle in the operation field. Histological finding showed synovial chondromatosis. We report a rare case of a patient with cubital tunnel syndrome caused by synovial chondromatosis.

  • We report a case of difficult endotracheal intubation in a patient with tracheobronchopathia osteochondroplastica. A 65-year-old man was scheduled to undergo ulnar nerve decompression and ganglion excisional biopsy under general anesthesia. During induction of general anesthesia, an endotracheal tube could not be advanced through the vocal cords due to resistance. A large number of nodules were identified below the vocal cords using a $Glidescope^{(R)}$ video-laryngoscopy, and fiberoptic bronchoscopy revealed irregular nodules on the surface of the entire trachea and the main bronchus below the vocal cords. Use of a small endotracheal tube was attempted and failed. a laryngeal mask airway (LMA $Supreme^{TM}$) rather than further intubation was successfully used to maintain the airway.

Single oblique osteotomy for correction of congenital radial head luxation with concurrent complex angular limb deformity in a dog: a case report

  • A 5-month old Shih Tzu was diagnosed with congenital elbow luxation and uniapical complex angular deformity of the radius. Single radial oblique and dynamic ulnar osteotomies were performed, using patient-specific 3D-printed osteotomy guide. External skeletal fixation was maintained for three weeks to prevent re-luxation of elbow joint. Three months after the surgery, objective gait analysis indicated markedly improved limb function. In addition, radiograph showed improved congruity of elbow joint and appropriate bone healing. In dogs with congenital radial head luxation and concurrent complex angular deformity, a single oblique osteotomy might be a viable option to preserve bone length and correct the luxation of elbow joint.

  • Upper extremity vascular injuries occurring with acute compartment syndrome are very challenging to manage in an emergency context in resource-poor settings. The need to always recognize the likelihood of coexisting compartment syndrome guides surgeons to perform concomitant fasciotomy to ensure a better outcome. We managed three vascular injuries in the upper extremities in two patients with concomitant imminent compartment syndrome observed intraoperatively. The first injury was complete brachial artery disruption following blunt trauma, while the second and third injuries were radial and ulnar artery transection caused by sharp glass cuts. Both patients were treated with vascular repair and fasciotomy. Secondary wound coverage was applied with split-thickness skin grafting, and the outcomes were satisfactory. Concomitant fasciotomy potentially improves the outcomes of vascular repair in emergency vascular surgery and should be considered for all injuries with the potential for acute compartment syndrome.

Delayed union of a pediatric lunate fracture in the United Kingdom: a case report and a review of current concepts of non-scaphoid pediatric carpal fractures

  • Pediatric carpal fractures are rare and often difficult to detect. This paper reviews the current literature on pediatric non-scaphoid carpal fractures, with a case report of a lunate fracture associated with a distal radius and ulnar styloid fracture, managed nonoperatively in a 12-year-old boy. There is lack of consensus regarding the management of these fractures due to the low number of reported cases. A frequent lack of long-term follow-up limits our understanding of the outcomes, but good outcomes have been reported for both nonoperative and operative management. This case report brings attention to the current time period for the definition of delayed union in pediatric carpal fractures, and emphasizes the need for prolonged follow-up for the detection of delayed complications leading to functional impairment.

Minimally invasive distal biceps tendon repair: a case series

  • Background: Distal biceps tendon repairs are commonly performed using open techniques. A minimally invasive distal biceps tendon repair technique using a speculum and hooded endoscope was developed to improve visualization, reduce soft-tissue dissection, and minimize complications. This paper describes the technique and reports the outcomes of 75 minimally invasive distal biceps tendon repairs. Methods: The operation reports and outcomes of 75 patients who underwent distal biceps tendon repair using this technique between 2011 and 2021 were retrospectively reviewed. Results: Median time to follow-up was 12 months (interquartile range [IQR], 6-56 months). Primary outcomes were function as measured by the Disabilities of Arm, Shoulder and Hand Score (DASH) questionnaire, and rate of complications. Median DASH score was 1.7 of 100 (IQR, 0-6.8). There were 2 of 75 (2.7%) re-ruptures of the distal tendon. There were no cases of vascular injury, proximal radius fracture, or posterior interosseous nerve, median, or ulnar nerve palsy. Conclusions: In this series, minimally invasive distal biceps repair was safe and effective with a low rate of major complications. Recovery of function, as indicated by low DASH scores, was satisfactory, and inconvenience during recovery was minimized. Level of evidence: IV.

부정교합자의 수완부 골성숙도에 관한 누년적 연구(I) (A Longitudinal Study on the Skeletal Maturity of the Hand and Wrist among Various Malocclusion Groups(I))

  • 성장기 환자의 교정치료에서 신체의 성장 발육상태를 파악하는 것은 중요한 의미를 갖는다. 특히 부정교합에 따라 성장 발육시기가 차이를 보인다면 교정치료 시기의 선택에 있어서 이에 대한 고려가 있어야 하므로 부정교합의 양상에 따른 성장 발육상태를 평가하는 것이 필요할 것이다. 이에 본 연구에서는 부정교합 분류에 따라 골성숙도에 차이가 있는지를 조사하기 위해, 8-10세의 여자 I급 부정교합자 38명, II급 부정교합자 36명, III급 부정교합자 33명을 대상으로 약 6개월 간격으로 12-13세까지 누년적으로 촬영한 수완부골 방사선 사진을 이용하여 골성숙도를 평가한 결과 다음과 같은 결론을 얻었다. 1. 부정교합에 따른 골성숙도의 차이는 없었다. 2. 유구골의 구상돌기 (hamular process)는 $9.16{\pm}0.72$세, 두상골(pisiform bone)은 $9.13{\pm}0.71$세, 모지척측 종자골(ulnar sesamoid)은 $10.34{\pm}0.84$세에 관찰되었다. 3. 골단이 골간을 둘러싸는 시기는 셋째 수지의 원심부 지절과 중간 지절에서 각각 $10.96{\pm}0.80$세, $11.27{\pm}0.87$세였으며, 첫째 수지의 근심부 지절에서는 $11.12{\pm}0.86$세, 요골에서는 $11.21{\pm}0.82$세, 다섯째 수지의 중간 지절에서는 $11.62{\pm}0.85$세였다. 4. 두상골의 출현은 유구골 구상돌기의 출현과(r=0.91), 모지척측 종자골의 출현은 구상돌기의 진행성 화골시기와 높은 상관관계를 보였으며(r=0.86), 골단이 골간을 둘러싸는 시기는 여러 부위가 상호간에 높은 상관관계를 나타내었다(r=0.80-0.90). 5. 다섯째 수지의 중간 지절의 형태가 가장 변이가 심하였다($20.6\%$).

불안정성 원위 요골 골절의 치료에 있어 한국형 이중 연골하지지고정 전방 금속판의 임상적 및 방사선학적 결과 (Clinical and Radiological Results of Treating Unstable Distal Radial Fractures with a Domestically Developed Volar Locking Plate That Has the Characteristic of Double-Tiered Subchondral Support)

  • 목적: 본 연구는 요골 원위부 골절에 대해 국내에서 이중 연골하 지지고정 개념으로 개발된 전방 잠김 금속판의 수술 후 정복 유지 효과 및 이에 영향을 줄 수 있는 요인들에 대해 평가해 보고자 하였다. 대상 및 방법: 2017년 7월부터 2018년 12월까지 요골 원위부 골절 환자 중 관혈적 정복 및 금속판 고정술을 시행한 54예를 대상으로 하였다. 수술 직후와 최종 추시 시 촬영한 단순방사선 사진을 이용해 요골 길이, 요골경사, 수장측 경사, 척골 변위, 원위 배측 피질 거리(distal dorsal cortical distance)를 측정하여 골절 정복 유지 효과에 대해 평가하였다. 환자 나이, 골절 분류, 금속판 위치를 기준으로 전체 환자군을 세부 그룹으로 나누어 비교함으로써 각 요인이 골절 정복 유지에 미치는 영향을 분석하였다. 결과: 수술 직후 원위 배측 피질 거리는 평균 5.91 mm (표준 편차, ±1.95 mm)로 측정되었으며 요골 길이(p=0.038), 척골 변위(p=0.001)는 수술 직후와 최종 추시 시 촬영한 단순 방사선 사진에서 유의한 차이가 확인되었다. 전체 환자군을 각 요인에 따라 세부 그룹으로 나누어 평가했을 시 척골 변위는 나이가 65세 이상이거나 AO/OTA 분류 C3형에 해당하는 경우 혹은 금속판 위치가 Soong classification grade 0에 해당하는 경우 수술 직후에 비해 최종 추시 시 유의하게 증가하였다(p=0.007, p=0.012, p=0.046). 결론: 국내에서 이중 연골하 지지고정 개념으로 생산된 전방 잠김 금속판을 이용하여 요골 원위부 골절 정복술을 시행하는 경우 충분한 정복을 확보할 수 있으며 골절 정복 유지에 대해 전반적으로 긍정적인 결과를 보이나, 환자가 고령인 경우, 관절 내 골절의 분쇄 정도가 심한 경우, 금속판이 분수령선의 근위부에 위치한 경우에 척골 변위의 증가를 보여 요골 월상골와의 정복 유지를 위한 고려가 필요하다.
