Studies on the effect of Kamikuibitang on the Gastric Ulcer in Rats (가미귀비탕(加味歸脾湯)이 흰쥐의 위궤양(胃潰瘍)에 미치는 영향(影響))

  • This study was aimed to evaluate the anti-pain effect of Kamikuibitang in acetic acid method and the anti-ulceration effect of Kamikuibitang in indomethacin, aspirin and immobilization stress method in rats. The results were follows; 1. The anti-pain effects of Kuibitang and Kamikuibitang were decreased compared with those of control group. 2. In indomethacin and aspirin method, the anti-ulcerative effects of experimental groups were shown compared with those of control group. 3. In immobilization stress method, the anti-ulcerative effect of experimental groups was significantly shown compared with that of control group. 4. The serum gastrin levels of Kuibitang groups showed very significant decrease in indomethacin-induced and immobilization stress-induced ulcers. The serum gastrin levels of Kamikuibitang groups showed very significant decrease in indomethacin-induced, aspirin- induced and immobilization stress-induced ulcers. 5. The serum $V_{B12}$ levels of Kuibitang groups showed very significant increase in both indomethacin-induced and immobilization stress-induced ulcers. The serum $V_{B12}$ levels of Kamikuibitang groups showed significant increase in aspirin-induced and immobilization stress-induced ulcers whereas very significant increase in indomethacin-induced ulcer. According to the above results, it was concluded that Kamikuibitang had very significant anti-ulceration effect as well as anti-pain effect on gastric ulcer in rats.

  • In patients with diabetic foot, ulceration and amputation are the most serious consequences and can lead to morbidity and disability. Peripheral arterial sclerosis, peripheral neuropathy, and foot deformities are major causes of foot problems. Foot deformities, following autonomic and motor neuropathy, lead to development of over-pressured focal lesions causing the diabetic foot to be easily injured within the shoe while walking. Wound healing in these patients can be difficult due to impaired phagocytic activity, malnutrition, and ischemia. Correction of deformity or shoe modification to relieve the pressure of over-pressured points is necessary for ulcer management. Application of selective dressings that allow a moist environment following complete debridement of the necrotic tissue is mandatory. In the case of a large soft tissue defect, performance of a wound coverage procedure by either a distant flap operation or a skin graft is necessary. Patients with a Charcot joint should be stabilized and consolidated into a plantigrade foot. The bony prominence of a Charcot foot can be corrected by a bumpectomy in order to prevent ulceration. The most effective management of the diabetic foot is ulcer prevention: controlling blood sugar levels and neuropathic pain, smoking cessation, stretching exercises, frequent examination of the foot, and appropriate education regarding footwear.

  • Although the 'Insamyangwee-tang' has been widely used in clinical purposes in the oriental medicine, its clinical efficacy is only documentated for the cases of gastritis, gastric ulcer and enteritis, but the experimental study on these has not been undertaken. So, to investigate the clinical efficacy of 'Insamyangwee-tang' comparing with animal experiments, This study was carried out. The results showed that relaxing action was shown on the isolated ileum in mice and that strong antagonistic actions were seen on $BaCl_2$, acetylcholine and histamine induced contraction of the ileum in mice, rats, rabbits and guinea-pigs that the relaxing effect of the intestinal smooth muscle was recognized. Inhibitory effects on transport rate in the small intestine of mice. Strong antagonistic actions were seen on acetylcholine induced contraction of duodenum in rats and remarkably inhibiting actions were seen of duodenum in rats. Inhibitory action on the secretion of gastric juice and pepsin, anti-ulceration effect was recognized.

Effects of the Seokchangpo-Wonji-Tang on Recovery from Disorder of Stomach, Liver and Mental-faculty in Alcoholism (석창포원지탕(石菖蒲遠志湯)의 Alcoholism에서의 간위(肝胃) 치유 및 학습능력향상 효과)

  • Effects of the Seokchangpo-Wonji-Tang on recovery from disorder of stomach, liver and mental-faculty in alcoholism were studied using male Sprague-Dawley rats. The rats were assigned into 4 groups; normal, control and Seokchangpo-Wonji-Tang(SWT) group. Control group administered ethanol(25 v/v %) at a dose 3g/kg, while SWT group administered 50mg/kg of SWT 30min before treating same dose of ethanol as control group for 10 days, orally. The gastric ulceration and also GOT and GPT activities in rats were checked, and all groups were subjected to trials of straight channel on the 1st day and to those of multiple T-maze during the following 3 days. The gastric ulceration, GOT and GPT activities were increased in control group, but decreased in SWT group significantly. The time required in normal group for the straight channel of the 2nd and 3rd trials was significantly shorter than that of the 1 st, while the control group showed no significance. In the time required for the multiple T-maze trials, the control group showed no significance. But in the straight or T-maze trials, the SWT group showed significant decrease in the time required against the control group.

Gastroprotective Effect of Korean Rice-Wine (Yakju) (한국 전통약주의 위보호 효과)

  • Gastroprotective effects of Korean rice-wine (Yakju) in two types of acute experimental gastric ulcer induced in rats and in mice were evaluated. Korean rice-wine were administered to 24-hr fasted rats 30 min before administration of 60% EtOH in 150 mM HCl or absolute ethanol. Korean rice-wine prevented formation of gastric ulcers induced by 60% EtOH in 150 mM HCl at oral doses of 250-1,000mg/kg and reduced gastric ulcers induced by absolute ethanol at oral doses of 62.5-1,000mg/kg, and inhibitory effect against 30% alcohol treatment for 7 days (twice/day). These results suggest Korean rice-wine have inhibitory effects on gastric lesion and ulceration.

Relationship of Foot Type to Callus Location in Healthy Subjects

  • The purpose of this study was to determine whether a relationship existed between foot type and the location of plantar callus in healthy subjects. Twenty-five healthy subjects with plantar callus were recruited for this study. Foot deformities were classified according to the operational definitions as 1) a compensated forefoot varus, 2) an uncompensated forefoot varus or forefoot valgus, or 3) a compensated rearfoot varus. The location of plantar callus was divided into two regions. Fourteen of the 19 feet with compensated forefoot varus and six of the 9 feet showed plantar callus at the second, third or fourth metatarsal head. Five of the 6 feet with uncompensated forefoot varus and twenty of the 16 feet with forefoot valgus showed plantar callus at the first or fifth metatarsal head. A significant relationship was found between foot type and location of callus (p<.01). The results support the hypothesis that certain foot types are associated with characteristic patterns of pressure distribution and callus formation. We believe diabetic patients with insensitive feet and with the types of foot deformity should be fit with foot orthoses and footwears that accommodate their respective deformity in a position as near to the subtalar joint as possible with the goal of preventing plantar ulceration.

Treatment Using a Single-Lobed Rotation Flap in Diabetic Forefoot Ulceration: Five Case Reports (단일엽 회전 피판술을 이용한 당뇨병성 전족부 궤양의 치료: 5예 보고)

  • Diabetic foot ulcers can progress to the point where amputation is needed, and so these ulcers require active treatment. Skin grafts or flaps can be performed for coverage of this type of ulcer. Local flap surgery is relatively easy to perform and good results have been previously reported. We performed single-lobed rotation flap on 5 cases of forefoot ulcer around the site of weight bearing. The location of the foot ulcers was the medial part of the first metatarsophalangeal joint in all the patients. The mean size of the defect was 4.70 ㎠. Managing of ulcers, controlling of diabetes and infection, and improving of peripheral blood flow were performed before surgery. In two cases, infection progressed to the articular cartilage and so metatarsophalangeal joint fusions were performed simultaneously. All the cases were completely transplanted. There was no recurrence of the ulcers, and all the patients were able to walk.

Antigastritic and Antiulcerative Action of the Extract of Zingiberis Rhizoma (생강추출물의 항위염 . 항궤양 작용)

  • The rhizoma of Zingiber officinale has been used as antiemetic, expectorants, stomachache relieving drugs and digestive accelerators. From the observation of antigastritic action of the methanol extract of the rhizoma, it was fractionated with hexane, chloroform, ethyl acetate and butanol, followed by bioassay on antigastritic and antiulcerative activity. The hexane and the chloroform fraction reduced significantly HCl ethanol induced gastric lesion at the dose of 370 and 210 mg/kg, p.o., respectively. On the gastric ulceration and gastric secretion in pylorus-ligated rats, the hexane fraction decreased the volume of gastric secretion and acid output, and also increased pH at the dose of 370 mg/kg, i.d.. It showed considerable curative ratio of acetic acid induced ulcer without inhibition of indomethacin induced gastric lesion. The methanol extract showed low acute toxicity with minimum lethal dose of more than 3000 mg/kg, p.o. in mice. In conclusion, Zingiberis rhizoma exhibited antigastric and antiulcerative activity which might be attributable to inhibition of gastric secretion. It is revealed that the active component may be present in the hexane fraction.

  • During the period from April 1991 to July 1992 clinical, hematological, and pathohistological findings of Holstein-friesian calves 47 with bovine leukocyte adhesion deficiency(BLAD, immunologically ascertained), which were referzed to the clinic for diseases of cattle, veterinary school, Hannover, were described. Most cases show poor body condition, rough and dry in haircoat, salivation, gingivitis, reduction of gingiva and alveolar bone, exposing the incisors' necks, loss of teeth, phlegmonous subcutaneous swellings, ulcerated tongue, recurang fever, coughing, dyspnea, pharyngeal and laryngeal stertor, periodical diarrhea, impaired swallowing, placid and less painsensitive. Relevant laboratory findings are persistent leucocytosis(with more than 30,000 up to 150,000 cells per $mm^3$ of blood), marked neutrophilia(without "shift to the left"), hyperproteinemia, and hypergammaglobulinemia. At post-mortem the carcass of BLAD-affected calves is usually emaciated. All lymphnodes of the respiratory and gastrointestinal tract appear markedly activated(swollen). Lesions in the mouth(gingivitis, defective dentition, pulpitis/alveolar paraodontitis, ulcerated tongue), throat and larynx(inflammation/ulceration), and lungs(pneumonic foci) correspond to the clinical symptoms seen on the living animal. There may be ulcers on the prestomachal mucosa, hyperemia of the intestinal mucosa with hyperplasia of Peyer's patches, ulceration and/or intramural abscesses. The spleen shows follicular hyperplasia. Microscopically, both myeloand erythropoesis are markedly activated in the bone marrow ; capillaries in many organs show leucocytostasis.

Treatment of primary cutaneous anaplastic large cell lymphoma

  • Primary cutaneous anaplastic large cell lymphoma (C-ALCL) is a rare subtype of primary cutaneous lymphoma with a favorable prognosis. Primary cutaneous CD30+ lymphoproliferative disorders, which include C-ALCL and lymphomatoid papulosis, are the second most common group of cutaneous T-cell lymphomas. C-ALCL is comprised of large cells with anaplastic, pleomorphic, or immunoblastic cytomorphology, and indeed, more than 75% of the tumor cells express the CD30 antigen. C-ALCL clinically presents with solitary or localized reddish-brown nodules or tumors, and sometimes indurated papules, and they may be with ulceration covering with dark eschar. Multifocal lesions are seen in 20% of the patients. Extracutaneous dissemination, which mainly involves the regional lymph nodes, occurs in 10% of patients. A 69-year-old man noticed a mild elevated cutaneous lesion containing central ulceration covering with brownish black necrotic tissue on the right lower lip, and the lesion was surgically removed. After the first operation, another skin lesion was developed and the histological examination confirmed the diagnosis, C-ALCL. Eight specimens were excised during the 7-month follow-up period. The patient started the treatment with low-dose oral methotrexate (15 mg/wk) and there was no recurrence for 11 months.
