A Study on Dietary Therapy for Patient with Peptic Ulcer (식이요법에 의한 위궤양의 치료 연구)

  • A serious case of the stomach ulcer was cured completely through a dietery treatment. The patient was listed as one of worse cases by the doctors. The patient refused to accept doctors' advises to have al surgical operation and sticked to his idea of dietetics. The doctors openly expressed their scepticism to a successful out come of a dietery treatment. However, it has been a success. A special ulcer diet menu prepared for the patient was based on those principles as follow: 1. Ulcer diet contained high protein (70g/day) and high energy (2,000cal/day). 2. To maintain a minimum degree of pressure on digestive process of the stomach, several small meals more than three times a day were served. Fat was avoided except that contained in milk and small amount of butter on toast. 3. To help to neutralize acid of the stomach, ample amount of milk was served. 4. Irritating food and beverage were prohibited. It was concluded that milk was one of the best diet for a peptic ulcer patient, for it could neutralize acid of the stomach as well as could provide ample amount of high protein which expedite healing craters formed by the stomach or duodenal ulcer. It was reported that most of unsucessful peptic ulcer cases of the local patients could be attributed to their failure in observance of an ulcer diet during their dietery treatment period, particularly their unfriendly attitude toward a milk diet.

  • Objectives : The prevalence of peptic ulcer disease is estimated about 10% in Korea, and has declined due to Helicobacter pylori eradication therapy. Symptoms of peptic ulcer are postprandial epigastric pain, hunger pain, anorexia, nauea, vomiting, soreness, postprandial fullness, postparandial discomfort. The purpose of the study was to evaluate the effectiveness of the Anjungsan in the improvement of the symptoms in patients with peptic ulcer. Methods : After taking Anjungsan to patient with peptic ulcer, Examined to assess the improvemnet by Ordinal Scale VI grade and VAS. Results : Patients with symptoms of the appeal was taking Anjungsan gets better after 3 dyas appeared to be more than half. And 11 days after treatment all symptoms were improved under the VAS 1-2 except for epigastric lump sensation. Conclusions : After treatment with Anjungsan, patient showed improvement in all symptoms associated with peptic ulcer.

Improvement of the Medical Bed for Preventing Decubitus Ulcer through Motion Analysis (운동 분석을 통한 욕창 방지용 침대 기구의 개선)

  • Decubitus ulcer is a pressure-induced ulceration of the skin occurring in persons confined to bed for long periods of time. In order to prevent decubitus ulcer, the medical bed mechanism to minimize pressure, friction, and shear forces acting between bed and patient had been developed. Air mattress also can be effective for reducing pressure over bony prominence. In this study, motion analysis was performed to examine whether this bed mechanism functioned properly in case of using air mattress, which was much thicker than common hospital mattress. We found that the patient on the air mattress above the medical bed for preventing decubitus ulcer slipped upward and downward excessively as the general motored-bed. New bed mechanisms were synthesized kinematically using simple 4-bar and 6-bar linkages so as to reduce sliding between the bed and the patient on the air mattress for preventing decubitus ulcer.

Evaluation of NSAID Usage and Appropriateness for Prevention of NSAID-Related Ulcer Complications (비스테로이드소염제로 인한 궤양 합병증 예방을 위한 비스테로이드소염제 사용 현황 및 적절성 평가)

  • Nonsteroidal antiinflammatory drugs (NSAIDs) are used in the treatment of extensive diseases related to various symptoms; inflammation, pain and fever. NSAIDs work by blocking prostaglandin synthesis, but adverse drug events (ADEs) have been increasing dramatically such as gastrointestinal bleeding, perforation and stenosis, a kind of serious ADEs. Therefore, NSAID-related ulcer complication guidelines have been announced containing various risk factors and symptoms. Thus, this study aims to evaluate of NSAID usage and appropriateness for prevention of NSAID-related ulcer complication based on American journal of gastroenterology (AJG) guideline 2009. Further, the study suggests Korean guideline for prevention of NSAID-related ulcer compared to AJG guideline. For this study, data was collected through electronic medical record (EMR) at Seoul national university of Bundang hospital. The primary end point was a composite of NSAID-related ulcer risk factor, types of NSAIDs, co-prescribed NSAID ulcer prevention drugs and NSAID-related ulcer after taking NSAID. The risk factors include over 65 years, high dose NSAID, previous ulcer history and taking drugs (e.g. aspirin, anticoagulant and steroid) causing ulcer. If a patient has 3 or 4 factors, that patient was classified high risk group. And if 1 or 2 factors that patient was classified moderate risk group. The patient who has no risk factor was in low risk group. I studied 8,120 patients who received NSAID from 1 January 2009 to 31 December 2009. High risk group was 16(0.2%), moderate risk group was 4,364(53.7%), and low risk group was 3,740(46.1%). The results show that high risk group should be prescribed COX-2 inhibitors with ulcer prevention drugs, and moderate or low risk group need traditional NSAIDs with ulcer prevention drugs. This may be different with 2009 AJG guideline because AJG guideline suggested taking COX-2 inhibitor alone in moderate group or taking traditional NSAID alone in low risk group could get higher ulcer complication. The results indicated that choosing preventive drug is important in case that how many risk factors the patients have. The proper drugs would be helpful for safe and effective NSAID usage in each patient group.

  • Objectives : This study is a clinical report about 19-year-old male patient with Duodenal ulcer. Methods : The patient was treated by acupuncture, herb medication. The improvement of the symtoms was evaluated by Visual Analog Scale(VAS), Endoscopy. Results : Brash was gradually reduced and dyspepsia got improved. Conclusions : Oriental medical treatment was effective to improve duodenal ulcer. There should be more observation about patients with duodenal ulcer.

A Case Report of Gastrointestinal Symptoms in Gastric Ulcer Treated with Gamchosasim-tang (감초사심탕(甘草瀉心湯)을 이용한 위궤양 환자에서 나타난 위장관 증상 치험 1례)

  • Objective: The purpose of this case report was to report the effect of Gamchosasim-tang in patients with gastrointestinal symptoms due to gastric ulcer. Methods: We treated a patient suffering from epigastric pain, sucking sensations in the epigastrium, and nausea using Gamchosasim-tang (甘草瀉心湯) for 35 days. The patient's symptoms were charted and evaluated using a visual analogue scale (VAS) and a gastrointestinal symptom rating scale (GSRS). Results: After treatment, the symptoms related to gastric ulcer were decreased and evaluation using the VAS and GSRS showed improvement in the patient. Conclusion: This study shows that Korean medicine treatment with Gamchosasim-tang may be valuable for relieving gastric ulcer symptoms.

A Study on the Relationship between Recurrent Aphthous Ulcer and Oral Mucosal Keratinization (재발성 아프타성 궤양과 구강점막 각화도의 관계에 대한 연구)

  • To investigate the relationship between recurrent aphthous ulcer and oral mucosal keratinization, exfoliative cytology in buccal mucosa, lip mucosa, tongue mucosa were performed on 25 recurrent aphthous ulcer patients and 25 controls whose age ranged from 10 to 65. Keratinization cell ratio was then measured. The results were as follows : 1. Yellow cell ratio in the control group was more than that in the patient group in buccal mucosa, lip mucosa, tongue mucosa. Red cell ratio in the control group was more than that in the patient group in lip mucosa. Blue cell ratio in the patient group was more than that in control group in all regions( p(0.01) 2. In the comparison by sex, the patient group showed no significant difference in all site but, the control group showed different results according to the site; males were more than females in yellow cell, but less than females in red cell Females were more than males in yellow cell, but less than males in red cell. 3. In the comparison by age, patient group showed no significant difference in all site, but the control group showed significantly high yellow cell ratio in buccal and tongue mucosa over the age of 50. In conclusion, there was close relationship between recurrent aphthous ulcer and decreased oral mucosal keratinization. In other words, reduced oral mucosal keratinization must be recommended for prevention of recurrent aphthous ulcer.

Free Tissue Transfer in the Treatment of Infected Diabetic Foot Ulcers (유리 조직 이식술을 이용한 당뇨병성 족부 궤양의 치료)

  • Diabetic foot ulcer is a serious complication which result from long-standing diabetes. Especially, severe infected diabetic foot ulcer results in unwanted lower extremity amputation. The diabetic patient is considered the relative contraindication for microsurgery because of the severe peripheral vascular disease. Recently, microvascular free tissue transfer technique applied to diabetic foot ulcer. It is well known that free tissue transfer provides immediate soft tissue coverage and control of infection. So it is possible that preservation of the lower extremity through free tissue transfer. A retrospective study of diabetic patients who had infected foot ulcer from 1999 to 2000 with foot defects reconstructed with free tissue transfer were reviewed. Thirteen patients were studied with mean follow-up of 12.7 months. There were two deaths during follow-up period. There were two failures after free flap surgery. All eleven survived patients were ambulatory. There was no recurrence of ulcer. No patient need amputation above the ankle joint. We have found that free tissue transfer for infected diabetic foot ulcer is very effective surgical technique. Careful patient selection and regular follow-up is important.

A study on Pain Express Pattern of Peptic Ulcer Patient (소화성궤양환자의 통증표현양상에 관한 연구)

  • Pain is a subjective and multidemensional concept. Therefore the patient's expression of pain have been referedl to the best believable indicator of pain condition but the support data obtained from the patient considered cultural difference is a deficient condition in determinded on the precise nursing diagnosis. The purpose of this reasearch was to understand multiple pain reponses in cultural difference and sensitivity, to encourage communication between medical teams, and to provide the foundation data of on data of precise nursing assessment for the patient in pain. The research problem was to grasp pain express pattern of Korean peptic ulcer patients. The subjects were 20 peptic ulcer patients in medical unit or OPD of twp university hospitals in Seoul. Data were collected from September 7th to 22nd, 1990 by intensive interviews. Interviews were done by the researcher and all were tape - recorded. The Data analysis was done by Phenomenological method from Van Kaam. Validity assured by confirmation of the internal consistency of the statements and catigory by nursing collegue in educational and clinicions in medical care. From the emic data, 96 descriptive statements were organized in 18 theme cluster. The results of study were summerized as follows. 1. Pain Express Pattern cluster of Peptic Ulcer Patients were “Pain as clogging”, “shallow pain”, “pain as pressing”, “nauseating pain”, “pain as smarting”, “pain as pulling”, “pain as pricking”, “pain as bursting”, “wrenching pain”, “excising pain”, “uncontrollable pain for mind and body”, “awakening pain”, “pain as hollowing” and the other cluster. As above mentioned, Pain Express Pattern of Peptic Ulcer Patient appeared diversely in verbal and they were propered to Korean culture. Therefore they will provide for the foundation data of precise nursing assessment.

A Case Report of Stage 4 Pressure Ulcer Patient Treated with Acupuncture and the Herbal Medicine Gamisipjeon-tang (침구 치료와 가미십전탕을 병행하여 호전된 4도 욕창 환자 치험 1례)

  • Objectives: The purpose of this case study is to report the effect of oriental medicine on pressure ulcers. Methods: We treated a patient with a pressure ulcer with acupuncture, Jaungo, and Gamisipjeon-tang. We started with herbal medicine only, and then added the acupuncture treatment after one month. We treated the sore for a total of 4 months. We measured the size and depth of the ulcer and graded it with a National Pressure Ulcer Advisory Panel (NPUAP) score once every two weeks. Results & Conclusions: Even though the wound was not cured completely, the total size of the pressure ulcer was reduced and the NPUAP stage was improved from 4 to 3. Therefore, Korean medicine could be an effective treatment for improving pressure ulcers.
