Studies on the Effect of AntiConvulsion and Analgesic by Ukgansan and Ukgansangami (억간산(抑肝散)과 억간산가미방(抑肝散加味方)의 진경(鎭痙) 및 진통(鎭痛) 효과(效果)에 관(關)한 실험적(實驗的) 연구(硏究))

  • In order to prove the effectiveness for anticonvulsion and analgesic of Ukgansan, Ukgansan added Jinphi Banha and Ukgansan added Chunma by experiment, experimental animals(mouse) are injected with strychnine, picrotoxin and caffeine to cause convulsion and ovserved the consumed time from convulsion to death. Comparing data with control group and observation data for frequency of writhing syndrome caused by acetic acid and phenylquinone show the results as follows : 1. Anticonvulsion effect on the convulsion induced by strychnine it was significantly effective in all sample groups. 2. Anticonvulsion effect on the convulsion induced by picrotoxin it was significantly effective in sample A and B. 3. Anticonvulsion effect on the convulsion induced by caffeine it was not recognized in all sample groups. 4. Analgesic effect on the pain induced by acetic acid it was significantly effect in sample A and C. 5. Analgesic effect on the pain induced by phenylquinone it was significantly effective in sample A and C. The results show that Ukgansan can be an effectual cure on anticonvulsion and analgrsic, Ukgansan added Jinphi·Banha on anticonvulsion, and Ukgansan added Chunma on analgesic.

Two cases of Ukgansan-gami on Sleep-related Behavior Disorder after Stroke (뇌졸중 이후 수면관련 행동장애를 보이는 환자에 대한 억간산가미 치험 2례)

  • ■ Objectives This case study is to report the two cases of Ukgansan-gami on sleep-related behavior disorder after stroke. ■ Methods Two patients who had sleep-related behavior disorder after stroke were treated by Ukgansan-gami about a month. We evaluated these patients with the frequency and severity of symptoms. ■ Results During treatment period, decreases in both frequency and severity of symptoms were observed in two cases. ■ Conclusion Two patients who were suffered from sleep-related behavior disorder due to sequelae of stroke showed significant improvement of their symptom after taking Ukgansan-gami.

  • ■ Objectives This case report is reported to show the effect of Ukgansan and Hwangnyeonhaedok-tang on a patient who complaining myoclonic seizure due to psychogenic non epileptic seizure, occurred after traumatic stress event. ■ Methods A female patients with myolconic seizure was treated with Ukgansan and Hwangnyeonhaedok-tang during 5 days. Then we assessed dyskinesia severity by AIMS(Abnormal Involuntary Movement Scale) every two days til discharge. ■ Results Seizure decreased two days after hospitalization, then disappeared third day. There was no recurrence till discharge. When she revisited outpatients clinic, there was no symptom. ■ Conclusion This case showed the effect of Ukgansan and Hwangnyeonhaedok-tang on psychogenic epileptic seizure.

Effects of Ukgansan Pharmacopuncture at GB20 on Cognitive Impariment Induced by Focal Brain Injury in Rats (풍지(風池)(GB20) 억간산(抑肝散) 약침이 국소 뇌손상으로 유발된 흰쥐의 인지장애에 미치는 영향)

  • Objectives : This research was performed to investigate the effects of Ukgansan pharmacopuncture(U-PA) of focal brain ischemia induced by middle cerebral artery occlusion(MCAO) in rats. Methods : The subjects were divided into 5 groups : A control group, acupuncture group, pharmacopuncture group U-PA1($2.571mg/250g/40{\mu}{\ell}$), pharmacopuncture group U-PA2($6.428mg/250g/40{\mu}{\ell}$), and pharmacopuncture group U-PA3($12.855mg/250g/40{\mu}{\ell}$). The focal brain ischemia was induced by intraluminal filament insertion into the middle cerebral artery. After 3 days of MCAO, Ukgansan(UGS) pharmacopuncture treatment was performed on the GB20, and the day after being treated with pharmacopuncture, the Morris water maze test was carried out by the assigned group. The series of processes were treated 6 times. Thereafter Bax, Bcl-2, Bax/Bcl-2 ratio, mGluR5, density of neuronal cell, and ChAT were measured. Results : The results were as follows. 1. The intensity of Bax significantly decreased in the U-PA1, U-PA2, U-PA3 groups. 2. The Bax/Bcl-2 ratio significantly decreased in the U-PA3 group compared with the control group. 3. The neuroprotective effect on the hippocampal CA1 significantly increased in the U-PA1, U-PA2, U-PA3 groups compared with the control group. 4. The density of ChAT in the hippocampal CA1 significantly increased in the U-PA1, U-PA2, U-PA3 groups compared with the control group. Conclusion : These results suggest that UGS pharmacopuncture may have anti-apoptotic and neuroprotective effects on focal cerebral ischemia caused by intraluminal filament insertion into the middle cerebral artery in rats.

Effects of Ukgansan (Yokukansan in Japanese, Yigansan in chinese) on the Locomotor Velocity and Glutamate-Induce Paroxysm in Planarian (Planarian 모델을 이용한 억간산의 항발작 효과)

  • Planaria were recently reported to be a simple and sensitive model to investigate the mechanistic aspect of seizure and to screen potential anticonvulsants. Using planarian model, we analyzed the pharmacological effect of ukgansan (UGS), an oriental herbal medicine containing seven medicinal herbs, on the planarian locomotor velocity (pLMV) and glutamate-induced seizure-like activity (pSLA). To test whether D. japonica is suitable for studying anti-seizure agents, we investigated the effect of glutamate on pLMV and pSLA in D. japonica. In the present study we first confirmed that pSLA in D. japonica was induced by L-glutamate. Glutamate significantly produced pSLA in a dose dependent manner, but did not affect pLMV. These glutamate-induced paroxysms were decreased by antiepileptic drug, topiramate. A similar inhibitory effect on glutamate-induced pSLA was observed after the treatment of UGS. The present results suggest that UGS and its active constituents possess useful substance inhibiting seizure in planarian and that D. japonica provides a convenient model to search active herbs containing anti-seizure activity.

A Case of Cerebral Infarction Patient Complaining Post-Stroke Aggression Accompanying Anxiety Treated with Combined Korean Medical Treatment (뇌졸중 후 불안을 동반한 공격성을 호소하는 뇌경색 환자에 대한 복합 한의치험 1례)

  • Post-stroke aggression refers to aggressive behaviors and languages which are occurred after stroke. In stroke patients, over 15% suffer from the anger and the symptom lowers the quality of life of the patients, family, and caregivers. After diagnosing post-stroke aggression, selective serotonin re-uptake inhibitor(SSRI) and beta-blocker, beta adrenergic agonist treatment usually underwent to treat the anger and anxiety of the patient. In the present case, a 81-year-old female patient with post-stroke aggression underwent Korean medical treatment by Jodeungsan and Ukgansan for 49 days. The effect of the treatment was assessed with State-Trait Anger Expression Inventory(STAXI) per a week, roaming time per a week and roaming frequency per a week. After the treatment, STAXI score, roaming time, and roaming frequecy decreased, and the patient could successfully cease to take beta-blocker, which she started to take for treating her aggression, without any side-effects. This case report suggests that Ukgansan might be an effective option with post-stroke aggression patients taking medication including beta-blocker.

  • This study has been carried out to investigate the effects of Ukgansan(UGS) extract on anti-convulsive, antipyretic, analgesic, sedative and GABAergic system of experimental animals. The results of this study were as follows : 1. UGS extract prolonged significantly the beginning time to convulsion and death induced by strychnine. 2. UGS extract prolonged significantly the time to death induced by electrical shock of ECT unit.(3 sec, 200 F, 25 mA) 3. On the experiment of hypothermic effects of UGS extract on the rectal temperature of mouse, UGS extract decreased significantly the rectal temperature of mouse 4. On the experiment of antipyretic effects of UGS extract on the febrile induced by the subcutaneous injection of $150\;{\mu}g/kg$ endotoxin in mouse, UGS extract decreased significantly the rectal temperature of mouse. 5. On the experiment of analgesic effects of UGS extract on the writhing syndrome induced by intraperitoneal injection 0.7% acetic acid 1 ml/100g in mouse, the writhing syndrome induced by acetic acid was reduced significantly by administration of UGS extract. 6. On the experiment of effects of UGS extract on spontaneous motor activity measured by wheel cage method in mice, the spontaneous motor activity was reduced significantly by administration of UGS extract. 7. On the experiment of effects of UGS extract on the activity of GABA-transaminase(GABA-T) in mouse brains after 21 days of oral administration of UGS extract. the activity of GABA-T was reduced significantly by administration of UGS extract. 8. On the experiment of effects of UGS extract on the activity concentration of GABA in mouse brain after 21 days of oral administration of UGS extract, the activity concentration of GABA was reduced significantly by administration of UGS extract. 9 On the experiment of effect of UGS water extract on the activity of GAD in mouse brain after 21 days of oral administration of UGS extract, the activity of GAD was reduced significantly by administration of UGS extract. According to the these results, Ukgansan extracts reveal the effects on the anti-convulsive, antipyretic, analgesic, sedative and GABAergic system.

Two cases of the treatement of hemifacial spasm improved by Ukgansangamibang(抑肝散加味方) and moxa-treatment (억간산가미방(抑肝散加味方) 투여(投與)와 구치료(灸治療)를 위주로 한 포륜진도(胞輪振跳) 치험2례(治驗2例))

  • Objectives : This study was performed to report two cases of improved hemifacial spasm by herbal remedy and moxa-treatment. Methods : This study calendered the improvements of symptoms of two patients with hemifacial spasm who visited the author's clinic. Ukgansangamibang(抑肝散加味方) and moxa-treatment on chung-wan(CV12) were prescribed to the patients concerned due to their antipathy against acupuncture treatment. Results : Significant improvements were observered from both patients not only for hemifacial spasm but neuropsychosis-related symptoms Conclusion : The study shows remarkable results of the efficacy of herb remedy and moxa-treatment, for the treatment of hemifacial spasm , however the cases investigated in this study were only two(2). Therefore, more studies should be followed to support the efficacy of Oriental Medicine for the treatment of hemifacial spasm.

Evaluation on the Quality of Research Field with Traditional Herbal Prescriptions for Dementia Therapy (치매 치료용 한약 처방의 연구성과에 대한 정성평가)

  • Objective : This study aimed to review the performance of traditional herbal prescriptions for treating dementia and present a strategy for research on dementia therapy utilizing herbal medicine. Methods : A definition was made to clarify the technology regarding the development of herbal prescriptions for treating dementia. The queries were compounded based on the initial keywords provided by experts in the field, then applied to the Web of Science database search engines from January 1986 to September 2011 to search related scientific articles. Before performing the analysis, papers were extracted from the initial search reviewed by experts and 80 articles were selected. Then, the selected papers were analyzed in terms of publish year, country, and type of herbal prescriptions. Furthermore, the research performance evaluation for treating dementia by herbal prescriptions was also created in terms of country and organization based on forward citation analysis. In addition to, for the evaluation regarding research quality, we classified and reviewed papers into two types: clinical studies and experimental studies. Results : According to the quantitative information analysis of 80 articles, the number of papers has increased by 21.9% per the yearly mean from 1995, and Japan had the largest portion within this research field. There were 34 kinds of traditional herbal prescriptions, among them Ukgansan had the highest number of studies followed by Jodeungsan, Dangkisoosan and so on. In addition, quality index as calculated by cites per paper is higher than average in Switzerland, Turkey and Japan. In the view of the evaluation on quality there were 12 clinical studies, 8 RCT reported that herbal prescriptions had efficacy at cognition, behavioral & psychological symptoms (BPSD) and activity of daily life (ADL) in various type of dementia. In experimental studies most of the studies were performed using animal models. The studies using Ukgansan were aimed at improving BPSD. The papers studied with Jodeungsan and Dangkisoosan targeted vascular dementia. Conclusions : In this study, research to develop traditional herbal prescriptions for treating dementia has the potential to improve symptoms since herbal medicines work as both multi-function and multi-target in dementia with multiple pathological or neurotoxic pathways. Therefore, the results of the research should be used in order to establish strategies to develop technology for treating dementia with traditional herbal prescriptions in the future.

Current Review of the Studies Reported in the 2012 Annual Meeting of Kampo Medicine Association of the Japan Neurosurgical Society (일본뇌신경외과한방의학회 연구 동향 - 2012년 학술대회 논문을 중심으로)

  • ■ Objectives The purpose is to review the application of the Kampo medicine(traditional Japanese medicine) in the neurosurgical field, through the analysis of the studies are presented at the Annual Meeting of Kampo Medicine Association of the Japan Neurosurgical Society. ■ Methods We have reviewed the studies that were reported at the 21th annual meeting of the Kampo medicine association of the Japan neurosurgical society was held on November 3 2012 in Tokyo, Japan. Seventeen included clinical reports were analyzed among all 25 studies at the meeting. Five studies were related with headache and eight were focused on the cerebrovascular disease including chronic subdural hemorrhage(CSDH) treated with Orungsan(Goreisan: 五苓散) or Siryungtang(Saireto: 柴苓湯), and other four were for pain managements. In the matters of headache, a unique feature was that Osuyutang(Gosyuyuto: 吳茱萸湯) or Oryungsan was considered as the first choice of the treatment for the headache, unlike Korea. It seems to consider to application the treatment of CSDH using Oryungsan in neurosurgery field of Korea. In addition, herbal prescription using Jackyakgamchotang(Syakuya kukanzoto: 芍藥甘草湯), Ukgansan(Yokkansan: 抑肝散), Chitabakilbang(Jidabokuippo: 治打撲一方) for acute/chronic pain is as well one of the peculiar points of Kampo medicine in Japan. ■ Conclusion It is expected that the recent studies performed in Japan neurosurgery fields to inspire domestic research and clinical applications, and needed to additional research and utilize in practice in Korea.
