THE EFFECT OF AUTOCLAVE STERILIZATION AND REUSE OF $SMARTPEG^{TM}$ ON THE IMPLANT STABILITY QUOTIENT (ISQ) MEASUREMENT ($Smartpeg^{TM}$의 고압멸균소독 및 재사용이 임플랜트 안정성 지수(ISQ) 측정에 미치는 영향)

  • Statement of problem: Resonance frequency analysis is evaluated as the non-invasive and objective method for the evaluation of implant stability and has been increasingly used. It is necessary to evaluate the factors affect the ISQ measurement stability. Purpose: This study was performed to evaluate the effect of the autoclave sterilization and reuse of $Smartpeg^{TM}$ on ISQ measurement. Material and methods: $SmartPeg^{TM}$ (Integration Diagnostics Ltd., $G\ddot{o}teborg$, Sweden) of autoclave group (A) was autoclave sterilized 9 times and $Smartpeg^{TM}$ of reuse group (R) was reused 9 times. Ten $SmartPeg^{TM}s$ were allocated to each group and after each autoclave sterilization and reuse, implant stability quotient (ISQ) values were measured 3 times from the two directions a and b at a right angle. $Osstell^{TM}$ mentor (Integration Diagnostics Ltd. $G\ddot{o}teborg$, Sweden) was used and type 1 (article no. 100353) $Smartpeg^{TM}$ was selected according to $Smartpeg^{TM}$ reference list. Osstem Implant US II future (Osstem Co., Seoul, Korea) in $4.0mm{\times}11.5mm$ was embedded in the self-curing acrylic resin ($Orthojet^{(R)}$, Lang Dental, U.S.A.). Data was statistically analyzed by one-way ANOVA $({\alpha}=.05)$ and scheffe test was done where a significant difference exist. Correlation test was also done between ISQ value and the number of autoclave sterilization or reuse. Results: 1. In autoclave group, the means and sd. of ISQ value before autoclave sterilization were $84.97{\pm}0.41,\;84.93{\pm}0.74$ at direction a and b. There was significant differences between autoclave groups at direction a and b (P=.000). 2. In reuse group, the means and sd. of ISQ value before reuse were $85.40{\pm}0.62,\;85.50{\pm}0.57$ at direction a and b. There was no significant difference between reuse groups at direction a and b (P>.05). 3. There was a weak positive correlation between the number of reuse and ISQ value at direction a and b (${\gamma}=.207$ and .246, P<.01). Conclusion: Within the limitations of this study, the following conclusions were drawn. Till ninth reuse of $Smartpeg^{TM}$, the ISQ measurement stability did not be affected. After twice autoclave sterilization of $Smartpeg^{TM}$ the ISQ measurement stability was affected.

Comparative study on the estimated blood loss follwing to orthognathic surgeries (다양한 악교정 술식 조합에 따른 실혈양의 비교 연구)

  • Purpose: The aim of this study was to compare the estimated blood loss and determine the change in hemoglobin depending on the combination of each orthognathic surgery. Subjects and Methods: The subjects of this study were patients who underwent orthognathic surgery among those diagnosed with a dentofaical deformity in Mok-Dong hospital, Ewha Womans University from 2002 to 2009. One hundred patients (men - 36, women - 64, mean age of $24.5{\pm}4.6$) participated in the study and were divided into four groups (group 1 - bilateral sagittal ramus osteotomy [BSSRO], group 2 - BSSRO+Genioplasty, group 3 - Lefort 1+BSSRO+genioplasty, group 4 - anterior segmental osteotomy on maxilla and mandible). A comparative study on the estimated blood loss (EBL), operation time, peri-operative changes in hemoglobin was performed using anesthesia records. The results were analyzed statistically using a Mann-Whitney U-test and Spearman's Rho test - SPSS 12.0 (SPSS Inc. Chicago, IL, USA). Results: In group 1 (BSSRO), the mean EBL, operation time and change in hemoglobin was $394.43{\pm}52.69$ ml, $184{\pm}42.33$ minutes, and 1.43, respectively, In group 2 (BSSRO+genioplasty), it was $556.32{\pm}63.42$ ml, $231{\pm}37.45$ minutes, and 1.80, respectively. In group 3 (Lefort 1+BSSRO+Genioplasty), it was $820.55{\pm}105.54$ ml, $320{\pm}15.41$ minutes, and 2.73, respectively. In group 4 (segmental osteotomy), it was $1025.39{\pm}160.21$ ml, $355{\pm}20.10$ minutes, and 3.33, respectively. In particular, in group 3, significant differences were observed depending on the method of the orthognathic surgery. The mean EBL in a Lefort 1 osteotomy with advancement was only 687 ml, whereas Lefort 1 osteotomy with canting correction (992 ml), even impaction (764 ml), and posterior nasal spine impaction (100 ml) showed a much higher EBL. Conclusion: From these results, the EBL and peri-operation hemoglobin increased as treatment plans became more complicated and increasing operation time. Safe orthognathic surgery should be performed by applying proper autologous transfusion plans based on the average EBL of each orthognathic surgery type.

  • 1. In order to investigate the effect of variation of assembly time on glue bond strength, and to determine the optimum range of assembly time with given glue, this experiment was made at the suggestion of the Wood Technology Laboratory, School of Forestry, Yale University. 2. For this investigation, three-ply-plywoods with 1/22 inch, birch veneer, phenolic resin, and soybean glue were made at the following variation of assembly time, that is, 1, 5, 10, 25, 35, 50, and 70 minutes, under both open and closed assembly manners, and the shear strength test at dry and wet were adoptted. 3. The shear strength and wood failure of each plywood panel constructed at the given assembly time have been illustrated in Tables 1, 2, 3 and 4. It has shown that there is a remarkable tendency, for increasing assembly time to give lower shear strength and wood failure throughout almost all cases. The effective range of assembly time of tested glues in this investigation for both open and closed assembly are summarized in the Table 7. Thus, allowable assembly time for Phenolic resin may be up to 10 minutes under open assembly and up to 50 minutes under closed assembly. For soybean glue, the permissible assembly time may be up to 5 minutes under open assembly and up to 15 minutes under closed assembly. The allowable assembly time for open assembly with the same glue is reduced by approximately one third or more than one third as compared with closed assembly time. This might mean that the closed asembly time for these glues is more practical than the open assembly.

Clinical Characteristics of Mycoplasma pneumoniae Pneumonia in Children during Recent 10 Years (최근 10년간 Mycoplasma pneumoniae 폐염의 임상양상에 대한 고찰)

  • Purpose : The purpose of this study is to know the epidemiology and clinical characteristics in patients with M. pneumoniae pneumonia during study period retrospectively. Methods : We had reviewed 500 patients with Mycoplasma pneumoniae pneumonia who were hospitalized at the pediatric ward of Seoul Adventist Hospital from July 1986 through June 1996. we had analysed clinical and laboratory problems of pulmonary and extrapulmonary manifestations including demographics and epidemiological characteristics. Results : Peak occurrence of the disease was 3 years interval and seasonal occurrence of the disease had no difference. 43.4% of patients was below 4 years old and male to female ratio was nearly 1:1. The most common site of infiltration on chest PA was right lower lobe and the most common type of pneumonia was interstitial pneumonia(43%). Pulmonary complications were atelectasis(11cases), emphysema(5cases), pleural effusion(27cases) and sinusitis(13cases) and extrapulmonary manifestations were hepatitis(109cases), skin rash(29cases), proteinuria(20cases) and hematuria(16cases). cold agglutinin test was carried out in 500cases and mycoplasma antibody test was carried out in 448cases. sensitivity of mycoplasma antibody test was 53.3%. Conclusion : Mycoplasma pneumoniae pneumonia showed epidemics at intervals of 3 year in its major occurrence. it exhibits a tendency that occurs at younger age group not in school age group. diagnosis of the disease need more accurate method due to low sensitivity of mycoplasma antibody test. also we need to carried out other laboratory test(for example, LFT, U/A).

Implant stability evaluation according to the bone condition, fixture diameter and shape in the osseointegration simulated resin model (골유착 재현 레진 모델에서 골 상태 및 임플란트 형태에 따른 임플란트 안정성에 관한 연구)

  • Purpose: Resonance frequency analysis, Periotest, and removal torque (RT) test were known as the methods to assess implant stability. The results of these methods are affected by the bone condition, implant diameter and shape. The purpose of this study is to access the meaning and the correlationship of the resonance frequency analysis, Periotest and RT test in osseointegration simulated acrylic resin when the engaged bone thickness and peri-implant bone defect are changed. Materials and methods: To simulate osseointegration, the fixture was fixed to an aluminum mold with a screw. Acrylic resin powder and liquid were poured into the mold for polymerization. The engaged resin thickness with implant was controlled. Simulated cortical bone thicknesses were 1, 3, 5 and 10 mm. Additional 1, 3 and 5 mm peri-implant bone defects were simulated. Three types of implants were used; 4 mm diameter implants of straight shape, 4 mm diameter implants of tapered shape and 5 mm diameter implants of tapered shape. Five fixtures per each type were tested in respective bone condition. Resonance frequency analysis and Periotest were evaluated in all bone conditions. Peak removal torque was measured at simulated cortical bone thicknesses of 1 and 3 mm. The statistical analysis was performed with the Kruskal-Wallis test, Mann-Whitney U test, and Spearman test using a 95% level of confidence. Results: With increasing engaged bone depth, the Implant Stability Quotient (ISQ) values increased and the Periotest values (PTVs) decreased (P<.001, P<.001). With increasing peri-implant bone defect, ISQ values decreased and PTVs increased (P<.001). When the diameter of implant increased, ISQ values increased and Periotest values (PTV) decreased (P<.001). There was a strong correlation between ISQ values and PTVs (r = -0.99, P<.001). Furthermore, the peak removal torque values had weak correlations with both ISQ values and PTVs (r = 0.52, P<.001 ; r = -0.52, P<.001). Conclusion: This study confirmed favorable implant stability with increasing engaged bone depth and implant diameter and decreasing peri-implant bone defect. ISQ values and PTVs showed strong correlation with each other and not with the peak removal torque values.

Geochemical Occurrence of Uranium and Radon-222 in Groundwater at Test Borehole Site in the Daejeon area (대전지역 시험용 시추공 지하수내 우라늄 및 라돈-222의 지화학적 산출특성)

  • A drilling project was undertaken to characterize the geochemical relationship and the occurrence of radioactive materials at a test site among public-use groundwaters previously known to have high occurrence of uranium and radon-222 in the Daejeon area. A borehole (121 m deep) was drilled and core rocks mainly consist of two-mica granite, and associated with pegmatite and dykes of intermediate composition. The groundwater samples collected at six different depths in the borehole by a double-packed system showed the pH values ranging from neutral to alkaline (7.10-9.3), and electrical conductivity ranging from 263 to 443 ${\mu}S/cm$. The chemical composition of the borehole groundwaters was of the $Ca-HCO_3(SO_4+Cl)$ type. The uranium and Rn-222 contents in the groundwater were 109-1,020 ppb and 9,190-32,800 pCi/L, respectively. These levels exceed the regulation guidelines of US EPA. The zone of the highest groundwater uranium content occurred at depths of 45 to 55m. The groundwater chemistry in this zone (alkaline, oxidated, and high in bicarbonate) is favorable for the dissolution of uranium into groundwater. The dominant uranium complex in groundwater is likely to be $(UO_2CO_3)^0$ or $(UO_2HCO_3)^+$. Radon-222 content in groundwater shows an increasing trend with depth. The uranium and thorium contents in the core were 0.372-47.42 ppm and 0.388-11.22 ppm, respectively. These levels are higher values than those previously been reported in Korea. Microscopic observations and electron microprobe analysis(EPMA) revealed that the minerals containing U and Th are monazite, apatite, epidote, and feldspar. U and Th in these minerals are likely to substitute for major elements in crystal lattice.

Prognostic Factors for Local Control in Early Glottic Cancer Treated with Radiation Therapy (방사선치료를 받은 조기 성문암 환자의 국소 종양 제어에 관한 예후 인자)

  • Purpose :This study was performed to find out the prognostic factors affecting local control in early glottic cancer treated with radiation therapy alone. Materials and Methods :We analysed 37 patients of histologically confirmed early glottic cancer treated at Chonnam National University Hospital between July Im and December 1995, retrospectively. Age of patients ranged from 30 to 73 years (median; 59 years). Thirty-five (95$\%$) patients were male. Histological type was all squamous cell carcinoma. According to the staging system of 1997 American Joint Committee on Cancer, 37 patients were restaged as follows: Tla; U (73$\%$), Tlb; 3 (8$\%$), 72: 7 (19$\%$). Radiation therapy was done using 6 MV X-ray of linear accelerator The range of total radiation dose delivered to the glottic lesion was between 5,040 cGy and 7,020 cGy (median; 6,600 cGy). Median follow-up period was U months. local control rates were calculated by Kaplan-Meier method. Generalized Wilcoxon test was used to evaluate the difference of control rates between comparable groups. Multivariate analysis using Cox proportional hazard model was done to find out prognostic factors affecting local control. Results:5 year survival rate of 37 patients was 89$\%$. Local control rate of 37 patients was 74$\%$ in 5 years. We included age, 7-stage, anterior commissure involvement, fraction size, total radiation dose, treatment time of radiotherapy as potential prognostic factors in univariate and multivariate analysis. As a result, treatment time had statistical significance in local control rate in both univariate (p=0.026) and multivariate (p=0.017) analysis. Complication was not recorded except one patient with hypothyroidism. Conclusion :This study revealed that overall treatment time of radiation was a significant factor affecting local control rate.

The Effect of Temporary Cement Cleaning Methods on the Retentive Strength of Cementation Type Implant Prostheses (임시 시멘트 제거방법이 시멘트 유지형 임플란트 보철물의 유지력에 미치는 영향)

  • The remnant of temporary cement on the intaglio surface of cast restoration may have a negative effect on the retentive strength of permanent cement. This study was to evaluate the effect of temporary cement cleaning methods on the retentive strength of cementation type implant prostheses. Prefabricated implant abutments - height 5.5mm, diameter 4.5mm, 6 degree axial wall taper with chamfer margins were used. Forty copings-abutment specimens were divided into four groups(each n=10) according to the cleaning methods for temporary cement(Temp-$Bond^{(R)}$) as follows : no temporary cementation(the control group), orange solvent, ultrasonic cleaning, air borne-particle abrasion. After the application of temporary cement and the separation, the cleaning procedure was performed according to the protocol of each group. The specimens were cemented with $Premier^{(R)}$ Implant $Cement^{TM}$. After the permanent cementation, the specimens were subjected to thermocycling and pulled out from the specimens with a universal testing machine at a cross-head speed of 0.5mm/min. After the retentive strength test, all the specimens were cleaned using ultrasonic cleaning, abraded with air borne-particles, and steam-cleaned. Likewise, the specimens were temporarily cemented(Temp-$Bond^{(R)}$ NE), cleaned according to the protocol of each group, cemented with $Premier^{(R)}$ Implant $Cement^{TM}$ and subjected to thermocycling and measurement of their retentive strength. The mean of group with orange solvent were significantly lower than those of other groups(p<0.05). There was no significance between group with ultrasonic cleaning and group with air borne-particle abrasion. Group with ultrasonic cleaning and group with air-particle abrasion were no significance at control group. There was no significance between group cemented with Temp-$Bond^{(R)}$ and group cemented with Temp-$Bond^{(R)}$ NE. Within the limitation of this study, it can be concluded that the temporary cement cleaning method with only orange solvent may have a negative effect on the retentive strength of permanent cement. Ultrasonic cleaning and air borne-particle abrasion methods are recommended for the temporary cement cleaning method on cementation type implant prostheses.

A Study on the Effect of Aroma Therapy on Anxiety and Pain Before and After Tooth Extraction (향기흡입법이 발치 전·후에 미치는 불안과 통증에 관한 연구)

  • The purpose of this study was to examine the effect of aroma therapy on anxiety and pain before and after tooth extraction. The subjects in this study were 60 patients who felt severe anxiety and pain due to tooth extraction. The experiment was conducted from January through March, 2004, by organizing an experimental group and a control group with 30 patients each. The experimental group was asked to keep wearing lavender-containing necklaces from two days before tooth extraction to inhale lavender, and no such an action was taken to the control group. The collected data were analyzed with SPSS 10.0 program to obtain statistical data. and ${\times}2$ test and t-test were implemented. The findings of this study were as follows: 1. Regarding whether or not the experimental and control groups were homogeneous, men outnumbered women, and the largest number of the patients were in their 30s. Those who were married were more than the others who were unmarried in number, and those who lived with their spouses under the same roof together outnumbered the others who didn't, as the rate of the former stood at 65 percent in the experimental group and 86.4 percent in the control group. They expressed high satisfaction at their spouses, since 45 percent of the experimental group and 31 percent of the control group did it, but the difference between them and those who were unsatisfied was insignificant (pE0.347). By occupation, the largest number of people in the experimental group, which numbered 16.7 percent, were self-employed, and lots of patients in the control group were government workers. As for blood type, type A was most prevailing, which recorded 43.3 percent. By religion, 43 percent of the experimental group had no religion, whereas 36.7 percent of the control group were Christian. The most common monthly income ranged from 2 million to 2.5 million won. 2. There was little disparity in past pain experience between the two groups before aroma therapy was applied. The experimental group underwent more pain (6.15) than the control group (5.78), but the difference wasn't significant. The experimental group (90%) experienced more anxiety and fear than the control group(83%), but the difference was insignificant. This fact showed that there was little gap between the two groups in anxiety and fear caused by tooth extraction. Contrary to earlier expectation that pre-anxiety might not be the same. little significant difference was found. 3. After aroma therapy was applied, 50 percent of the experimental group and 23.3 percent of the control group suffered significantly less anxiety and fear about tooth extraction(${\times}2$=4.59, pE.05). And the experimental group exposed to aroma therapy was less nervous(3.0) than the control group(4.39), and the gap between the two was significant (t=13.37, pE.001). Therefore, aroma therapy had a good effect on alleviating their anxiety. During tooth extraction. 73.3 percent of the experimental group and 93.3 percent of the control group felt pain. The former group suffered Significantly less pain(${\times}2$=4.32, PE.05). Concerning the extent of pain, the experimental group(2.53) found it less painful to have their teeth extracted than the control group(5.50), and the gap between the two was significant(t=5.89, PE.05). 4. As to the effect of aroma therapy on alleviating anxiety or fear, the experimental group(33.3%) felt that aroma therapy let them more relieved. Every member of that group was willing to use aroma therapy again in the future, and 86.7 percent of that group perceived that aroma therapy made a difference to dental treatment. The experimental group responded to aroma therapy favorably, as every member of it had an intention to advise others to use that therapy.

SURFACE ROUGHNESS OF COMPOSITE RESIN ACCORDING TO FINISHING METHODS (복합레진 표면의 연마방법에 따른 표면조도)

  • The purpose of this study was to evaluate the difference of surface roughness of composite resin according to composite resin type, polishing methods, and use of resin sealant. Two hundred rectangular specimens, sized $8{\times}3{\times}2mm$, were made of Micro-new (Bisco, Inc., Schaumburg, IL, U.S.A) and Filtek Supreme (3M ESPE Dental Products, St. Paul, MN, U.S.A.), and divided into two groups; Micronew-M group, Filtek Supreme-S group. Specimens for each composite group were subdivided into five groups by finishing and polishing instruments used; M1 & S1(polyester strip), M2 & S2 (Sof-Lex disc), M3 & S3 (Enhance disc and polishing paste), M4 & S4(Astropol) and M5 & S5 (finishing bur), Polished groups were added letter B after the application of resin surface sealant (Biscover), eg, M1B and S1B. After specimens were stored with distilled water for 24hr, average surface roughness (Ra) was taken using a surface roughness tester. Representative specimens of each group were examined by FE-SEM (S-4700: Hitachi High Technologies Co., Tokyo, Japan). The data were analysed using paired t-test, ANOVA and Duncan's tests at the 0.05 probability level. The results of this study were as follows ; 1. The lowest Ra was achieved in all groups using polyester strip and the highest Ra was achieved in M5, S5 and M5B groups using finishing bur. On FE-SEM, M1 and S1 groups provided the smoothest surfaces, M5 and S5 groups were presented the roughest surfaces and voids by debonding of filler on the polished specimens. 2. There was no significant difference in Ra between Micronew and Filtek Supreme before the application of resin sealant, but Micronew was smoother than Filek Supreme after the application of resin sealant. 3. There was significant corelation between Ra of type of composite resin and polishing methods before the application of resin sealant (p=0.000), but no significant interaction between them after the application of resin sealant. On FE-SEM, most of composite resin surfaces were smooth after the application of resin sealant on the polished specimens. 4. Compared with before and after the application of resin sealant in group treated in the same composite and polishing methods, Ra of M4B and M5B was statistically lower than that of M4 and M5, and S5B was lower than that of S5, respectively (p<0.05). In conclusion, surface roughness by polishing instruments was different according to type of composite resin. Overall, polyester strip produced the smoothest surface, but finishing bur produced the roughest surface. Application of resin sealant provided the smooth surfaces in specimens polished with Enhance, Astropol and finishing bur, but not provided them in specimens polished with Sof-Lex disc.
