  • 최근 생명 연장과 삶의 질에 관한 많은 관심에 따라서 의료 부문에 있어서도 Ubiquitous 환경을 도입하는 노력이 시도되고 있다. 그 중 Ubiquitous Hospital은 여러 가지 병원 서비스 부분의 편의성을 제공하고자 분야로써 이것을 이용하여 시간과 장소의 제약 없이 각종 의료서비스와 건강관리를 제공 받을 수 있는 Ubiquitous Healthcare 서비스에 대한 관심이 증대되고 있다. 본 논문에서는 이 Ubiquitous Healthcare 서비스 제공 system 구현을 위한 framework을 제안한다. 이 framework에서는 사용자가 되는 의사와 환자에 적합한 인터페이스를 제공하고 환자에 대한 추적 관찰과 증상의 판단 및 환자 진료시의 지원 서비스를 제공한다. 또한 보험회사나 병원과 같은 외부시스템과 연계하여 필요한 데이터의 공유가 가능하도록 한다. 그리고 제안된 framework를 이용하여 전립선 환자를 위한 BPH 환자관리 시스템을 구현한다.

Improvement of Personalized Diagnosis Method for U-Health (U-health 개인 맞춤형 질병예측 기법의 개선)

  • Applying the conventional machine-learning method which has been frequently used in health-care area has several fundamental problems for modern U-health service analysis. First of all, we are still lack of application examples of the traditional method for our modern U-health environment because of its short term history of U-health study. Second, it is difficult to apply the machine-learning method to our U-health service environment which requires real-time management of disease because the method spends a lot of time in the process of learning. Third, we cannot implement a personalized U-health diagnosis system using the conventional method because there is no way to assign weights on the disease-related variables although various kinds of machine-learning schemes have been proposed. In this paper, a novel diagnosis scheme PCADP is proposed to overcome the problems mentioned above. PCADP scheme is a personalized diagnosis method and it makes the bio-data analysis just a 'process' in the U-health service system. In addition, we offer a semantics modeling of the U-health ontology framework in order to describe U-health data and service specifications as meaningful representations based on this PCADP. The PCADP scheme is a kind of statistical diagnosis method which has characteristics of flexible structure, real-time processing, continuous improvement, and easy monitoring of decision process. Upto the best of authors' knowledge, the PCADP scheme and ontology framework proposed in this paper reveals one of the best characteristics of flexible structure, real-time processing, continuous improvement, and easy monitoring among recently developed U-health schemes.

  • Various kinds of studies for building up U-Korea are now underway. Especially, RFID comes into the spotlight as a core technology of Ubiquitous. Though, the development of higher levels of application is far behind the hardware-based technology in RFID application system. For that reason, In this paper suggests technology using RFID on Ubiquitous and embodies the effective health care application which is one of the application services of user-centered environment It realizes the system which enables us to manage and send data about changing prescriptions and locations of patients effectively under the basis of RFID tag and mobile devices. It also provides the high quality of practical effectiveness of entire system throughout the realization of applications of application server and mobile device. on the basis of framework for providing the various kinds of services and physical elements.

How to Improve Influenza Vaccination Rates in the U.S.

  • Annual epidemics of seasonal influenza occur during autumn and winter in temperate regions and have imposed substantial public health and economic burdens. At the global level, these epidemics cause about 3-5 million severe cases of illness and about 0.25-0.5 million deaths each year. Although annual vaccination is the most effective way to prevent the disease and its severe outcomes, influenza vaccination coverage rates have been at suboptimal levels in many countries. For instance, the coverage rates among the elderly in 20 developed nations in 2008 ranged from 21% to 78% (median 65%). In the U.S., influenza vaccination levels among elderly population appeared to reach a "plateau" of about 70% after the late 1990s, and levels among child populations have remained at less than 50%. In addition, disparities in the coverage rates across subpopulations within a country present another important public health issue. New approaches are needed for countries striving both to improve their overall coverage rates and to eliminate disparities. This review article aims to describe a broad conceptual framework of vaccination, and to illustrate four potential determinants of influenza vaccination based on empirical analyses of U.S. nationally representative populations. These determinants include the ongoing influenza epidemic level, mass media reporting on influenza-related topics, reimbursement rate for providers to administer influenza vaccination, and vaccine supply. It additionally proposes specific policy implications, derived from these empirical analyses, to improve the influenza vaccination coverage rate and associated disparities in the U.S., which could be generalizable to other countries.

Recovery of People Living with Mental Illness: A Concept Analysis (정신질환의 회복[Recovery] 개념분석)

  • Purpose: The purpose of this study is to analyze the concept of recovery in relation to those living with mental illness and provide a better understanding to the definition, perspectives, and paradigm of recovery in phenomenon, as a conceptual knowledge. Methods: A literature review was conducted to define the concept of recovery from a mental illness by using key words, "recovery", "mental health and illness", "concept analysis" and "recovery-oriented nursing", and searching the Cumulative Index to Nursing and Allied Health Literature, PubMed, Cochrane library and RISS4U database. Concept analysis of recovery was done, by using the Walker and Avant's framework of concept analysis. Results: Attributes of recovery for those living with mental illness included regain, life reconstruction, hope, adjustment, and health. Antecedents of recovery from mental illness included instilling hope, recovery vision, belief, peer support, recovery-oriented services, empowerment, personal accountability, education, human rights and culture. The consequences, as meanings of recovery included self-esteem, hopeful life, positive adjustment, and healthy life. Conclusion: Concept of recovery is important for a nurse to understand when caring for a person living with mental illness. This concept of recovery from mental illness may apply to future studies to develop a recovery-oriented nursing intervention.

A Review of the Korean Nursing Research Literature with Focus on Quantitative Measurement of Caring (돌봄 측정 관련 국내 간호학 연구 문헌고찰: 양적 연구를 중심으로)

  • Purpose: The purpose of this study is to review the quantitative research literature on measuring caring in order to identify overall trends in measuring caring. Methods: Fifty three papers were selected from four databases including RISS4U, DBpia, KISS, and Korea Med. Results: The number of measuring caring papers has increased since 2000. Approximately 60 % of the total papers were descriptive and correlative design researches with convenience sampling. Jean Waston's theory was the most popular conceptual framework, but much of the research tended to be conducted without any conceptual framework. In that kind of research, 'caring' terms were used without definition. The most frequently used term for the concept of caring was nurses' caring behaviors. Also, 'nurses' was one of the most popular subjects. Thirty six measuring caring instruments were used. Twenty were developed in foreign countries and translated into Korean. The others were developed originally in Korean. Interpersonal Caring Technique - Communication Skills Scale, based on the interpersonal process model, was the most frequently used tool. Among the translated instruments, Coates' Caring Efficacy Scale was the most popular. Some instruments were used without validation. Conclusion: These results provide basic data on measuring caring and indicate directions for further research. In particular, validation studies of measuring caring instruments are needed.

A Study on the Determinants of Private Long-Term Care Insurance and First Home Care Use in the United States: Using Discrete Time Model (미국의 민간장기요양보험 가입과 재가요양서비스 이용의 결정요인에 관한 연구: 이산시간모델을 사용하여)

  • Using the 1998-2004 Health and Retirement Study(HRS), this study explored the determinants of private long-term care insurance(LTCI) ownership and the first home care use. To account for the interaction between LTCI purchase and home care use, this study used two-period utility model as theoretical framework. Discrete time model was used as an empirical model to incorporate the time-dependent feature of LTCI ownership. And this study accounted for the endogeneity of LTCI ownership and home care use by employing full information maximum likelihood estimation. This study indicated insignificant effects of private LTCI ownership and Medicaid eligibility on the home care use. Also, the effects of income and assets on home care utilization were negligible. Those who have poor health condition and who do not have potential informal caregivers were more likely to use home care. For private LTCI ownership, income and assets have positive relationship with LTCI purchase, and poor health status and age were negatively related to LTCI purchase. The elderly living with children and those who have more siblings were less likely to have private LTCI, and those who lived with spouses with no children were more likely to buy private LTCI. Based on the findings, this study provides implications to design long-term care(LTC) policies in the U.S. and to develop LTC planning education programs.

Trends in Research on Caregivers Hospitalized Children in Korea-Focus on Knowledge Type (입원아동 보호자 대상 연구논문 분석-지식체 유형을 중심으로)

  • Purpose: This study was designed to analyze recent trends in research about caregivers of hospitalized children in Korea and to suggest future research directions in this area. Methods: Eighty one studies selected from http://www.kan.or.kr, www.childnursing.or.kr, www.riss4u.net, and www.ndsl.kr published from 1995 to 2011 were used. The analysis framework of concepts was derived from client domain (Kim, 2000) and knowledge type (Kim et al., 2004). Results: In terms of research design, nonexperimental studies (82.7%) were the most frequent, followed by experimental studies (14.8%) and qualitative studies (2.5%). Mothers were the most frequent caregivers, and hospitalization was the most frequent health problem of the children. In terms of categories of the concepts, 35 (39.3%) studies included essentialistic concepts like coping and adaptation, 15 (16.9%) studies included problematic concepts like anxiety and uncertainty, and 39 (43.8%) studies included health-care experiential concepts like educational needs and nursing needs. In term of knowledge types, there were 35 (39.3%) studies of the explanatory knowledge type, 44 (49.5%) descriptive ones, and 10 (11.2%) prescriptive ones. Conclusion: The results indicate that further research is necessary on problematic concepts and prescriptive knowledge types for child health nursing practice which will lead to expanding nursing knowledge.

A Hybrid Adaptive Security Framework for IEEE 802.15.4-based Wireless Sensor Networks

  • With the advent of ubiquitous computing society, many advanced technologies have enabled wireless sensor networks which consist of small sensor nodes. However, the sensor nodes have limited computing resources such as small size memory, low battery life, short transmission range, and low computational capabilities. Thus, decreasing energy consumption is one of the most significant issues in wireless sensor networks. In addition, numerous applications for wireless sensor networks are recently spreading to various fields (health-care, surveillance, location tracking, unmanned monitoring, nuclear reactor control, crop harvesting control, u-city, building automation etc.). For many of them, supporting security functionalities is an indispensable feature. Especially in case wireless sensor networks should provide a sufficient variety of security functions, sensor nodes are required to have more powerful performance and more energy demanding features. In other words, simultaneously providing security features and saving energy faces a trade-off problem. This paper presents a novel energy-efficient security architecture in an IEEE 802.15.4-based wireless sensor network called the Hybrid Adaptive Security (HAS) framework in order to resolve the trade off issue between security and energy. Moreover, we present a performance analysis based on the experimental results and a real implementation model in order to verify the proposed approach.

The Trends and Prospects of Health Information Standards : Standardization Analysis and Suggestions (의료정보 표준에 관한 연구 : 표준화 분석 및 전망)

  • Ubiquitous health care system, which is one of the developing solution technologies of IT, BT and NT, could give us new medical environments in future. Implementing health information systems can be complex, expensive and frustrating. Healthcare professionals seeking to acquire or upgrade systems do not have a convenient, reliable way of specifying a level of adherence to communication standards sufficient to achieve truly efficient interoperability. Great progress has been made in establishing such standards-DICOM, IHE and HL7, notably, are now highly advanced. IHE has defined a common framework to deliver the basic interoperability needed for local and regional health information networks. It has developed a foundational set of standards-based integration profiles for information exchange with three interrelated efforts. HL7 is one of several ANSI-accredited Standards Developing Organizations operating in the healthcare arena. Most SDOs produce standards (protocols) for a particular healthcare domain such as pharmacy, medical devices, imaging or insurance transactions. HL7's domain is clinical and administrative data. HL7 is an international community of healthcare subject matter experts and information scientists collaborating to create standards for the exchange, management and integration of electronic healthcare information. The ASTM specification for Continuity of Care Record was developed by subcommittee E31.28 on electronic health records, which includes clinicians, provider institutions, administrators, patient advocates, vendors, and health industry. In this paper, there are suggestions that provide a test bed, demonstration and specification of how standards such a IHE, HL7, ASTM can be used to provide an integrated environment.
