Empyema and Pericarditis by Salmonella Group D Complicating Malignant Thymoma with Pleural Metastasis (흉선종의 흉막전이에 동반된 Group D Salmonella 농흉 및 심낭염 -1예 보고-)

  • Non-typhoid salmonella infection frequently associated with bacteremia rarely been reported in immunocom-promized patients with malignant neoplasms, diabetes or extended use of corticosteroids. Especially, concomitant pleural empyema and pericarditis due to non-typhoid salmonella. infection is extremely rare. Here, we report a case of concomitant empyema and pericarditis in malignant thymoma with pleural. metastasis complicated by salmonella group D infection with brief review of literature.

  • A newly developed typhoid vaccine was tested for subcutaneous and intramuscular irritationin male Ner Zealand White rabbits. In subcutaneous and intramuscular irritation tests, there were no observed clinical signs, body weight changes and gross pathologic findings at doses of 1 mg/ml and 0.0125 mg/ml during experimental period. However, in positive control (0.75% acetic acid), we could find various lesions that had hemorrhage, necrosis and infiltration of inflammation cells in both subcutaneous and muscular tissues. From these results, we suggest that typhoid vaccine is not irritant in subcutaneous and muscular tissue of rabbits.

  • Fowl typhoid is a septicemic disease of turkeys and chickens. The antigen is Salmonella(S) gallinarum, which cross-reacts with S pullorum because of common antigen. The organism is maintained in the ovary of carrier birds and transmitted vertically in the york. It is also transmitted horizontally through feces and broken eggs. The symptoms are weakness, drowsiness, wing drooping, hyperexcitability, paresis and diarrhea. In the many cases, the birds are found dead in the morning before any symptoms have been noticed. this experiment was carried out to investigate the characteristics of S gallanarum and scrutinized about the control systems. 1. Fowl typhoid was came about in the winter than the other seasons. The average of mortality was 46.3%. And it was came about not only adult poultry but also chicks. 2. S gallinarum was isolated from liver and spleen in infected chickens. Total isolated strains were 60. 3. The organisms were all extinct at 60$^{\circ}C$, 30mins. 4. A survival rate was higher in underground water than a secondary distilled water. It was detected in underground water until 30 days. 5. When being exposed to the solar light, it became extinct almost within 24 hours. 6. It was survived in a refrigerator until 12 months. 7. It was extinct among 5 mins in 0.1% phenol and 2% formalin. 8. When the cultured organism fluid was inoculated in SD-rat abdominal cavity, rat died within 24 hours. 9. Pullorum-typhoid related program must disciplines in order to destroy the fowl typhoid, and breeding system must be converted to cage system.

Measurement of the Levels of IgG Subclasses Reactive to Salmonella typhi in the Sera of Patients with Typhoid Fever (장티푸스환자의 혈청내 Salmonella typhi에 대한 IgG subclass항체의 분포)

  • To diagnose the typhoid fever rapidly and accurately in clinically suspected patients, the levels of IgG subclass antibody were measured by enzyme-linked immunosorbent assay(ELISA). With symptom, blood culture and agglutination test, tested persons were categorized into 6 groups as typhoid fever, FUO, paratyphi A or B, other bacterial infctions, cancers, and control. ELISA was performed on the polyvinyl chloride plates coated with killed whole cell($10^8\;cell/ml$) of S. typhi 0901W by poly-L-lysine applied as binding substance (and polyvinyl chloride as solid phase). The distribution of the level of IgG subclass antibodies in each group was analyzed and compared with other groups. The results obtained were summarized as follow: 1. The optimal dilution of the sera from patients with typhoid fever was 1:160, and those of the sheep anti-human IgG subclass and the peroxidase conjugated rabbit anti-sheep IgG were 1:4000 and 1:5000, respectively. 2. The absorbance levels of IgG subclass in the sera of typhoid fever patients were as follows; a) IgG1 value is $0.439{\pm}0.110$ b) IgG2 value is $0.416{\pm}0.165$ c) IgG3 value is $0.449{\pm}0.145$ d) IgG4 value is $0.525{\pm}0.154$ IgG subclass levels in the sera of typhoid patients were much higher than in control group and patient with paratyphi A or B as well as other infectious diseases. The sensitivity and the specificity in differential diagnosis of typhoid fever and other febrile diseases were 92% and 79% in the assay of IgG1 respectively, whereas those in the assay of IgG2 were 97% and 72%, respectively (above absorbance 0.3). 3. The absorbance levels of IgG subclass in the serial sera of typhiod fever patients tend to decrease to the level of absorbance 0.3 in 10 months from the onset of illness. 4. The order of absorbance levels of IgG subclass in the serum of each group were typhoid fever, paratyphi A or B, other infectious diseases, control and cancer. 5. For the serodiagnosis of typhoid fever against other febrile diseases, the sensitivity and the specificity in the assay of IgG2 activity were 76% and 93% in absorbance 0.4, respectively. 6. In the distribution of the level of each IgG subclass in the sera of FUO patients which were suspected of typhoid fever, the positive rate was ranged from 36% to 82%. This suggest that more than 50% of FUO patients are caused by S. typhi.

  • Typhoid fever can cause serious complications, such as enterobrosia, meningitis, pneumonia, myocarditis, hepatitis, osteomyelitis, and disseminated intravascular coagulation in 10-15% of the patients. Kidney complications are very rare, and a few cases have been reported in children. We are reporting a case of childhood typhoid fever complicated with acute nephritis present with albuminuria, hypertension, and renal failure.

  • To develop a live vaccine strain against fowl typhoid and paratyphoid caused by Salmonella serovar Gallinarum biovar Gallinarum (Salmonella Gallinarum) and Salmonella serovar Enteritidis (Salmonella Enteritidis), respectively, several nalidixic acid resistant mutants were selected from lipopolysaccharide (LPS) rough strains of Salmonella Gallinarum that escaped from fatal infection of a LPS-binding lytic bacteriophage. A non-virulent and immunogenic vaccine strain of Salmonella Gallinarum, SR2-N6, was established through in vivo pathogenicity and protection efficacy tests. SR2-N6 was highly protective against Salmonella Gallinarum and Salmonella Enteritidis and safer than Salmonella Gallinarum vaccine strain SG 9R in the condition of protein-energy malnutrition. Thus, SR2-N6 may be a safe and efficacious vaccine strain to prevent both fowl typhoid and paratyphoid.

  • Typhoid fever is an acute systemic febrile disease caused by Salmonella typhi. The classic picture of the disease consists of prolonged continuous or remitting fever, abdominal pain, diarrhea, rose spots and delirium. Salmonella infection can lead to diffuse organ involvement., including bone, lung, thyroid, kidney, liver, spleen, heart, pericardium, intestine and skin and cause a variety of complications. Pulmonary manifestations occur in only 1 percent of the patients. Mild cough with sticky sputum is the earliest symptom and bronchitis, pneumonia and lung abscess were presented. Recently we experienced a case of typhoid fever complicated by bronchitis, dysplasia in a 37-year-old male physician who was improved with ceftriaxone and ciprofloxacin We report this case with a review of the literature.

  • In a flocks of broiler, 29 days old, was submitted to Iksan-branch, Livestock Development and Research Institute of Chonbuk province. Grossly, their feathers were ruffled and multiful hemorrhage were presented in the thigh and pectoral muscles. There were multifocal foci on the liver surface and enlarged and edematous on bursa of Fabricius. The coagulative materials, which were blood-tinged, were filled in cecum, and Eimeria oocysts were detected in the cecal contents. Microscopically, macrogametes were noted in the cecum, degeneration and necrosis of lymphocytes in the medullary area of bursal follicles, and multifocal necrosis of hepatocytes. There were positive in the infectious bursal disease(IBD) through selologic test and Salmonella gallinarum were detected. This report is a mixed infection case of coccidium, IBD and fowl typhoid in a flocks of broiler.
