The Effects of Self-efficacy and Self-stigma on Self-care in People with Diabetes

  • Purpose: This descriptive study investigated the effects of self-efficacy and self-stigma on self-care in people with diabetes. Methods: The study included a total of 377 patients with diabetes enrolled in university hospitals in D city and public health centers in S city. Data were collected from 1 July to 31 August, 2017, and were analyzed using descriptive statistics, t-test, analysis of variance, Pearson's correlation coefficient, and hierarchical multiple regression. Results: Diabetes self-care was positively correlated with diabetes self-efficacy, whereas it was negatively correlated with diabetes self-stigma. Participants' education level, marital status, perceived health status, type of medication, self-efficacy, and self-stigma explained 42.4% of the variance in diabetes self-care. Conclusion: The findings indicate that diabetes self-efficacy and self-stigma are important factors for improving self-care in patients with diabetes. Therefore, systematic programs for enhancing self-efficacy and reducing self-stigma of these individuals should be developed.

제2형 당뇨병 환자의 평소 식사 중 당질섭취량과 glycemic index 관련 연구 (Carbohydrate Consumption and Glycemic Index of the Usual Diet in Type 2 Diabetes Mellitus Patients)

  • The possibility that high, long-term intake of carbohydrates that are rapidly absorbed as glucose may increase the risk of type 2 diabetes has been long-standing controversy. A high consumption of carbohydrates with a high glycemic index produces greater insulin resistance than did the intake of low glycemic index carbohydrates. This study was designed to evaluate the cabohydrate intake status include glycemic index and correlation carbohydrtae intake status with anthropometry factors & other nutrients in usual diet of the Korean type 2 diabetes mellitus. In 104 tpye 2 diabetes mellitus patients(mean age : 51.8yr, male=44.femal=60), we determined carbohydrte intake status include glycemic index with 24hr recall method and measured anthropometry. Mean daily carbohydrtae intakes and glycemic index were 307.3g(male 323.1g, female 295.5g) and 90.7(male 93.4, female 88.8), respectively. We found a strong and statistically significant association between carbohydrate ratio and glycemic index in obese factors, other nutrient. But carbohydrate intake/kg of body weight was low a significant differences in obese factors, other nutrient. Also glycemic index was effected by total energy intake and carbohydrate ratio than carbohydrate intake/kg of body weight. In conclusion, emphasis for dietary modification should be total energy intake and carbohydrate ratio in diabetes mellitus patient.

제2형 당뇨병 성인 남성 환자의 약물치료 이행과 혈당 조절 수준 간에 치료 스트레스의 매개효과 (The Mediating Effect of Regimen Distress in the Relationship between Medication Adherence and Glycemic Control in Men with Type 2 Diabetes Mellitus)

  • Purpose: The purpose of this study was to examine the mediating effect of regimen distress in the relationship between medication adherence and glycemic control in men with type 2 diabetes. Methods: A total of 116 patients with type 2 diabetes was recruited for the cross-sectional survey design. Data were analyzed using descriptive statistics, independent t-test, ANOVA, Pearson's correlation coefficient, and multiple regression using Baron and Kenny steps for mediation. Results: The mean score for medication adherence was 6.32, and the mean glycemic control level (HbA1c) was 7.47%. The mean score was 2.37 for regimen distress. There were significant correlations among medication adherence, regimen distress, and HbA1c. Regimen distress had a partial mediating effect (${\beta}$=.22, p=.005) in the relationship between medication adherence and HbA1c (Sobel test: Z=2.47, p=.013). Conclusion: Regimen distress was found to be associated with glycemic control in men with type 2 diabetes. Based on the findings of this study, nursing intervention programs focusing on decreasing regimen distress are highly recommended to improve level of glycemic control in patients with type 2 diabetes.

  • Objective: This study reported the effectiveness of Korean medicine for a patient with Type-1 diabetes mellitus after pancreatectomy with a compression fracture of the thoracic spine. Methods: A patient diagnosed with Type-1 diabetes mellitus after a pancreatectomy was treated with acupuncture, pharmacopuncture, Gami-hwanggigyejiomul-tang (modified Huangqi Guizhi Wuwu decotion), Jindang-won, etc. The conventional treatment was maintained using an insulin pump and insulin injection. Results: Blood glucose (FBS/PP2hrs), ESR and CRP decreased after one month of treatment. Conclusion: Gami-hwanggigyejiomul-tang and Jindang-won may be effective treatments for Type-1 diabetes mellitus. Further studies are needed to confirm the findings of this case.

Effect of Bambusae caulis in Liquamen on Streptozocin-Induced Diabetic C57BL/6 Mice

  • Bambusae caulis in Liquamen is one of the important herbal medicine produced by heating bamboo indirectly and is used for treatment of stroke, hypertension, and diabetes etc. Recently the mechanism of clinical effects on Bambusae caulis in Liquamen has been studied. This experiment was conducted to confirm the clinical effects of Bambusae caulis in Liquamen on type 1 diabetes and its related mechanism. We divided C57BL/6 mice into 3 groups and induced them to be type 1 diabetes by injection of streptozocin into peritoneum. The dosage of each group was 150 mg/kg once only, 140 mg/kg once only and 40 mg/kg for 5 days respectively. The two groups injected streptozocin for once took orally Bambusae caulis in Liquamen after the induction of diabetes, and the other one group was given Bambusae caulis in Liquamen during the diabetes inducing period. As the result, the two diabetes-induced groups showed blood glucose decreasing effect by Bambusae caulis in Liquamen on an average, but they didn't show the signiftcant differences statistically. But Bambusae caulis in Liquamen showed the anti-diabetic effect suppressing blood sugar rising trend during the diabetes inducing peried (P<0.05). The anti-oxidative effect of Bambusae caulis in Liquamen was measured with the hypoxanthine/xanthine oxidase (HX/XOD) system. The quantity of ROS was measured using DCFDA reagent indirectly. As the result, $10\%$ solution of Bambusae caulis in Liquamen showed anti-oxidative effect by scavenging $93.4\%$ superoxide as compared with control group. It is suspected that the anti-oxidative effect of Bambusae caulis in Liquamen suppressed the increase of blood glucose in the diabetes-inducing group. These results could be useful data to understand the effect of Bambusae caulis in Liquamen on type 1 diabetes and type 1 diabetes developing because ROS were closely connected with the induction and complications of diabetes.

Understanding of type 1 diabetes mellitus: what we know and where we go

  • The incidence of type 1 diabetes mellitus (T1DM) in children and adolescents is increasing worldwide. Combined effects of genetic and environmental factors cause T1DM, which make it difficult to predict whether an individual will inherit the disease. Due to the level of self-care necessary in T1DM maintenance, it is crucial for pediatric settings to support achieving optimal glucose control, especially when adolescents are beginning to take more responsibility for their own health. Innovative insulin delivery systems, such as continuous subcutaneous insulin infusion (CSII), and noninvasive glucose monitoring systems, such as continuous glucose monitoring (CGM), allow patients with T1DM to achieve a normal and flexible lifestyle. However, there are still challenges in achieving optimal glucose control despite advanced technology in T1DM administration. In this article, disease prediction and current management of T1DM are reviewed with special emphasis on biomarkers of pancreatic ${\beta}-cell$ stress, CSII, glucose monitoring, and several other adjunctive therapies.

제2형 당뇨환자 기능성게임 개발을 위한 식이교육 콘텐츠 효과 (The Effects of Diet Education Contents for the Customized Serious Game Development in Type II Diabetes)

  • 본 연구는 제2형 당뇨환자에게 맞춤형 식이교육을 제공하기 위하여 기능성게임을 개발하기 위한 콘텐츠 효과를 검증한 연구이다. 기존의 기능성 게임이 제1형 당뇨에 중점을 두고 개발되었기 때문에 본 연구에서는 제2형 당뇨환자와 보건교육자가 상호작용을 통해 맞춤형 교육을 실시할 수 있도록 설계되었다. 자료수집 기간은 2015년 11월 7일부터 12월 26일까지였으며, 연구대상자는 보건소에 내원하여 당뇨 식이교육을 신청한 제2형 당뇨환자 중 선정기준에 적합하고 8회의 교육을 꾸준히 이수한 총 26명이었다. 기능성 게임 프로토타입으로 맞춤형 식이교육을 제공받은 후 대상자의 자가간호행위, 당화혈색소가 통계적으로 유의하게 상승하였다(p<.05).

Frequency of Self-Monitoring of Blood Glucose during the School Day Is Associated with the Optimal Glycemic Control among Korean Adolescents with Type 1 Diabetes

  • Background: This study aimed to evaluate the relationship between the frequency of self-monitoring of blood glucose (SMBG) and glycosylated hemoglobin (HbA1c) levels among Korean adolescents with type 1 diabetes mellitus (T1DM). Factors affecting the SMBG frequency were analyzed in order to improve their glycemic control. Methods: Sixty-one adolescents aged 13 to 18 years with T1DM were included from one tertiary center. Clinical and biochemical variables were recorded. Factors associated with SMBG frequency were assessed using structured self-reported questionnaires. Results: Average total daily SMBG frequency was $3.8{\pm}2.1$ and frequency during the school day was $1.3{\pm}1.2$. The mean HbA1c level was $8.6%{\pm}1.4%$. As the daily SMBG frequency increased, HbA1c levels declined (P=0.001). The adjusted odds of achieving the target HbA1c in participants who performed daily SMBG ${\geq}5$ significantly increased 9.87 folds (95% confidence interval [CI], 1.58 to 61.70) compared with those performed SMBG four times a day. In the subjects whose SMBG frequency <1/day during the school day, an 80% reduction in the adjusted odds ratio 0.2 (95% CI, 0.05 to 0.86) showed compared to the group with performing two SMBG measurements in the school setting. The number of SMBG testing performed at school was significantly high for individuals assisted by their friends (P=0.031) and for those who did SMBG in the classrooms (P=0.039). Conclusion: Higher SMBG frequency was significantly associated with lower HbA1c in Korean adolescents with T1DM. It would be necessary to establish the school environments that can facilitate adequate glycemic control, including frequent SMBG.

Aerobic and Graduated Treadmill Exercise Decreases Blood Glucose Levels, Lipid Levels and Oxidative Stress in an Animal Model of Type 1 Diabetes Mellitus

  • Purpose: Exercise has been shown to be a simple and economical therapeutic modality that may be considered as an effective aid for diabetic mellitus. For example, exercise training increases insulin sensitivity in type 2 diabetes. But we found no reported of how exercise affect type 1 diabetes. This study investigated the impact of aerobic and graduated treadmill exercise regimens on body weight, glucose and insulin concentrations, lipid profiles, and oxidative stress indicators in rats with streptozotocin (STZ) induced diabetes. Glycosylated hemoglobin ($HbA_{1c}$) was determined as an indicator of glucose control during exercise. Methods: In our study, a total of 40 rats were used. Three groups of 10 rats each were given STZ to induce diabetes. The remaining 10 rats became the normal group. After 28 days we determined biochemical parameters such as glucose, glycosylated hemoglobin ($HbA_{1c}$), insulin concentration, serum total cholesterol (TC), triglycerides (TG), and high-density lipoprotein (HDL). Superoxide dismutase (SOD) and catalase activities were also measured. Results: Concentrations of blood glucose and $HbA_{1c}$ in the moderated exercise groups were significantly decreased after 28 days compared with the control group (p<0.05). There was a significant reduction in serum TC and TG in the experimental groups. The activity of SOD increased significantly by 17.70% and 48.25% respectively. Conclusion: These results indicate that physical training and exercise training affects body weight, fasting blood glucose, $HbA_{1c}$, insulin, lipid profiles, and antioxidant status in rats with streptozotocin-induced diabetes. We suggest that graduated treadmill exercise may have therapeutic, preventative, and protective effects against diabetes mellitusby improving glycemic control, oxidant defenses, and lipid metabolism.

  • Patients with diabetes mellitus show delayed wound healing and increased susceptibility to infection. Therefore, the effects of diabetes on pulpal and periodontal healing should be taken into consideration when treating diabetic dental traumatized patients. This case presents the treatment for dental traumatized 20 yr old female with uncontrolled type II diabetes. The traumatized upper central incisors had showed pulpal healing in early days. However, 7 mon after the trauma, the teeth had been diagnosed with pulp necrosis with apical abscess. Eventually, non surgical root canal treatment on the teeth had been performed.
