The combination of canagliflozin and omega-3 fatty acid ameliorates insulin resistance and cardiac biomarkers via modulation of inflammatory cytokines in type 2 diabetic rats

  • The present study was carried out with the hypothesis that combination of canagliflozin and omega-3 fatty acid may have potential effect on insulin level, insulin resistance, cardiac biomarkers, inflammatory cytokines and histological studies in type 2 diabetes mellitus (DM). Type 2 DM was induced by injecting nicotinamide (120 mg/kg, i.p.) 15 min before STZ (60 mg/kg) injection. Canagliflozin (5 and 10 mg/kg) and omega-3 fatty acid (300 mg/kg) were given for 28 days after confirmation of diabetes. Biochemical estimations revealed elevated levels of glucose, insulin, HOMA-R and inflammatory cytokines in diabetic group. Daily dosing of alone canagliflozin and omega-3 fatty acid slightly reduced elevated levels of glucose, insulin, HOMA-R and inflammatory cytokines ($IL-1{\beta}$, IL-2, and $TNF{\alpha}$), whereas canagliflozin and omega-3 fatty acid combination has reduced these biochemical parameters significantly when compared with diabetic group. Similarly in diabetic group the levels of cardiac biomarkers such as lipid profile, LDH, CKMB and troponin were significantly increased. Elevated levels of cardiac biomarkers were significantly reduced after daily dosing of alone canagliflozin and omega-3 fatty acid. Canagliflozin and omega-3 fatty acid combination has offered better improvement in cardiac biomarkers compared to alone canagliflozin and omega-3 fatty acid. Histopathological analysis also supported the above hypothesis that combination therapy (canagliflozin and omega-3 fatty acid) offered better protection against degenerative changes in ${\beta}-cells$ of pancreas as compared to alone treatment with these drugs. Thus the present study revealed that canagliflozin and omega-3 fatty acid can be used as potential combination therapy in type 2 DM along with cardiac complication.

Combined transcriptome and proteome analyses reveal differences in the longissimus dorsi muscle between Kazakh cattle and Xinjiang brown cattle

  • Objective: With the rapid development of proteomics sequencing and RNA sequencing technology, multi-omics analysis has become a current research hotspot. Our previous study indicated that Xinjiang brown cattle have better meat quality than Kazakh cattle. In this study, Xinjiang brown cattle and Kazakh cattle were used as the research objects. Methods: Proteome sequencing and RNA sequencing technology were used to analyze the proteome and transcriptome of the longissimus dorsi muscle of the two breeds of adult steers (n = 3). Results: In this project, 22,677 transcripts and 1,874 proteins were identified through quantitative analysis of the transcriptome and proteome. By comparing the identified transcriptome and proteome, we found that 1,737 genes were identified at both the transcriptome and proteome levels. The results of the study revealed 12 differentially expressed genes and proteins: troponin I1, crystallin alpha B, cysteine, and glycine rich protein 3, phosphotriesterase-related, myosin-binding protein H, glutathione s-transferase mu 3, myosin light chain 3, nidogen 2, dihydropyrimidinase like 2, glutamate-oxaloacetic transaminase 1, receptor accessory protein 5, and aspartoacylase. We performed functional enrichment of these differentially expressed genes and proteins. The Kyoto encyclopedia of genes and genomes results showed that these differentially expressed genes and proteins are enriched in the fatty acid degradation and histidine metabolism signaling pathways. We performed parallel reaction monitoring (PRM) verification of the differentially expressed proteins, and the PRM results were consistent with the sequencing results. Conclusion: Our study provided and identified the differentially expressed genes and proteins. In addition, identifying functional genes and proteins with important breeding value will provide genetic resources and technical support for the breeding and industrialization of new genetically modified beef cattle breeds.

히스티딘을 함유한 결정성 심정지액(Histidine-Tryptophan Ketoglutarate solution)과 혈성 심정지액을 사용한 선천성 심기형 환자에서의 심근보호 효과에 대한 전향적 비교연구 (A Prospective Clinical Trial of Histidine-Tryptophan Ketoglutarate Solution in Congenital Heart Surgery)

  • 소아에서 사용되는 혈성 심정지액 혹은 결정성 심정지액의 심근보호 효과의 상대적 우월성에 대해서는 아직까지 논란의 여지가 있다. 본 연구에서는 소아에서 히스티딘을 함유한 결정성 심정지액(HTK 용액)과 혈성 심정지액의 심근보호 효과를 비교해보고자 하였다. 대상 및 방법: HTK 용액을 사용한 15명의 환자들(1군)과 혈성 심정지액을 사용한 15명의 환자들(2군)을 대상으로 전향적인 연구를 시행하였다. 모든 환자들을 대상으로 술 전, 수술 직후, 술 후 1일째, 2일째의 혈중 심관효소들(troponin I, CK-MB, LDH)의 농도를 측정하였고 술 전, 술 중, 그리고 술 후의 임상적 자료들을 비교 분석하였다. 결과: 두 군 간에 연령, 체중, 성비 등의 차이는 없었다. 질병의 종류는 1군의 경우 심실중격결손(n=4), 심방중격결손(n=1), 팔로씨 사징증(n=4), 기타 복잡심기형(n=6) 등이었고, 2군의 경우 심실중격결손(n=7), 심방중격결손(n=5), 팔로씨 사징증(n=3) 등이었다. 체외순환 시간은 1군이 99.1$\pm$48.1분, 2군이 69.3 $\pm$27.3분이었고(p=0.02), 대동맥 차단 시간은 1군이 52.1 $\pm$23.6분, 2군이 37.9$\pm$20.5분이었다(p=0.07). 대동맥 차단 제거 직후 자발적 제세동이 이루어지지 않았던 경우는 두 군 모두에서 발생하지 않았다. 두 군 모두에서 수술 사망은 없었다. 두 군 간에 수술 후 2일째까지 측정한 심근효소들의 농도의 유의한 차이는 관찰되지 않았다. 두 군 간에 강심제 사용시간, 인공호흡기 이탈까지의 시간의 차이는 없었다. 결론: HTK 용액은 결정성 심정지액이 가지는 사용의 용이함, 깨끗한 수술시야의 확보와 같은 장점 외에 완충제로서 다량의 히스티딘이 포함되어 있어 대동맥 차단시간이 긴 경우 주기적으로 심정지액을 투여해야 하는 번거로움을 피할 수 있는 장점을 가지고 있다. 본 연구에서는 현재 널리 사용되고 있는 혈성 심정지액과 비교하여 심근보호 효과의 유의한 차이가 없음을 확인할 수 있었다.

심장수술시 저체온 체외순환과 정상체온 체외순환의 전신 효과에 관한 연구 (The Systemic Effects of Hypothermic and Normothermic Cardiopulmonary Bypass in Cardiac Surgery)

  • 저자득은 최근 논의되고 있는 정상체온 체외순환의 임상적 유용성과 생리학적 영향을 규명하기 위해 저체온 체외순환과의 비교 연구를 시행하게 되었다. 대상 및 방법: 심장수술이 계획된 36명의 성인 환자들을 연구목적에 따라 무작위로 중등도 저 체온 체외순환군(이하 저체온군, 비인두 온도$26\~28^{\circ}C,\;n=18)$과 정상체온 체외순환군(이하 정상체온군, 비인두 온도>,$35.5^{\circ}C,\;n=18)$으로 나누었다. 전체 환자들로부터 체외순환 전, 체외순환 중, 체외순환 종료 직후에 각각 혈액을 채취하여 총 백혈구 수, neuron-specific enolase 증가율(NSE), interleukin-6 증가율(IL-6), endothelin-1 (ET-1), cortisol 증가율, troponin I (TNI), aspartate aminotransferase (AST), alanine aminotransferase (ALT), creatinine, blood urea nitrogen (BUN)을 측정하였다. 그 외 수술 중 소변 배출량, 수술 후 페지수$(PaO_{2}/FiO_{2}),$ 기계호흡 보조시간, 중환자실 치료기간, 합병증, 재원일수 등을 조사하여 양 군 간에 비교 평가하였다 결과: 정상체온군이 저체온군에 비해 심장수술 동안 총 백혈구 수, NSE 증가율, IL-6 증가율, cortisol 증가율은 유의하게 더 높았으나 소변 배출량은 더 낮았다. 또한 기계 호흡 보조 시간, 중환자실 치료기간, 재원일수 역시 정상체온군이 저체온군보다 유의하게 더 길었다. 결론: 저자들의 연구 결과를 볼 때 정질 냉각 심정지액을 이용한 심장 수술 동안 정상체온 체외순환은 중등도저체온 체외순환보다 더 높은 염증 및 스트레스 반응을 유도하여 생리학적으로 유해할 것으로 생각된다. 그러나 좀더 많은 환자 수를 대상으로 한 추가적 연구가 필요할 것으로 판단된다.

개심술 중 심폐기 이탈에 실패한 환자에게 적용한 경피적 심폐순환 보조장치 (Percutaneous Cardiopulmonary Support (PCPS) for Patients with Cardioppulmonary Bypass Weaning Failure during Open Heart Surgery)

  • 배경: 최근 경피적 심폐순환 보조장치는 여러 원인의 심인성 쇼크나 심정지의 치료에 매우 활발하게 적용되고 있다. 하지만 개심술 후 체외순환 이탈에 실패한 경우 적용하는 경피적 순환보조는 아직 예후가 매우 불량하다고 알려져 있다. 본 연구는 개심술 후 체외순환 이탈에 실패하여 경피적 심폐순환 보조장치를 적용한 환자들의 임상성적을 알아보고, 이들의 예후에 영향을 미치는 요인들에는 어떠한 것들이 있는지 알아 보고자 시행하였다. 대상 및 방법: 2005년 1월부터 2008년 12월까지 개심술 후 심폐기 이탈에 실패하여 경피적 순환 보조장치(CAPIOX emergent bypass system, $EBS^{(R)}$, Terumo Inc, Tokyo, Japan)를 적용한 10명의 환자들의 의무기록을 후향적으로 분석하였다. 결과: 평균 연령은 $60.2{\pm}16.5$세($19{\sim}77$세)였고, 평균 체외순환보조기간은 $48.7{\pm}64.7$시간($4{\sim}210$시간)이었다. 순환 보조 장치를 이탈할 수 있었던 경우는 6예(장치 이탈율 60%), 생존퇴원은 5예(생존율 50%)였다. 합병증은 5명(50%)에서 발생하였다. 단변량 분석에서 수술 중 대동맥 겸자시간이 길었던 경우, 순환 보조기간 중 종격동 출혈이 있었던 경우, 순환 보조장치 삽입전의 Troponin-I 수치가 높았던 경우가 사망에 영향을 미치는 인자로 조사되었다. 퇴원한 환자는 모두 생존하여 평균 $34{\pm}8.6$개월($23{\sim}48$개월)간 추적 관찰 중이다. 결론: 개심술 후 체외순환 이탈에 실패하게 되는 경우 매우 높은 사망률을 보이는 바 이들 환자에 대하여 적극적인 경피적 순환 보조장치의 적용으로 향상된 생존율을 기대할 수 있었다. 앞으로 지속적인 생존을 향상 및 합병증 감소를 위한 보다 많은 경험 및 임상연구가 필요할 것으로 생각된다.

Screening for candidate genes related with histological microstructure, meat quality and carcass characteristic in pig based on RNA-seq data

  • Objective: The aim of the present study was to identify genetic variants based on RNA-seq data, obtained via transcriptome sequencing of muscle tissue of pigs differing in muscle histological structure, and to verify the variants' effect on histological microstructure and production traits in a larger pig population. Methods: RNA-seq data was used to identify the panel of single nucleotide polymorphisms (SNPs) significantly related with percentage and diameter of each fiber type (I, IIA, IIB). Detected polymorphisms were mapped to quantitative trait loci (QTLs) regions. Next, the association study was performed on 944 animals representing five breeds (Landrace, Large White, Pietrain, Duroc, and native Puławska breed) in order to evaluate the relationship of selected SNPs and histological characteristics, meat quality and carcasses traits. Results: Mapping of detected genetic variants to QTL regions showed that chromosome 14 was the most overrepresented with the identification of four QTLs related to percentage of fiber types I and IIA. The association study performed on a 293 longissimus muscle samples confirmed a significant positive effect of transforming acidic coiled-coil-containing protein 2 (TACC2) polymorphisms on fiber diameter, while SNP within forkhead box O1 (FOXO1) locus was associated with decrease of diameter of fiber types IIA and IIB. Moreover, subsequent general linear model analysis showed significant relationship of FOXO1, delta 4-desaturase, sphingolipid 1 (DEGS1), and troponin T2 (TNNT2) genes with loin 'eye' area, FOXO1 with loin weight, as well as FOXO1 and TACC2 with lean meat percentage. Furthermore, the intramuscular fat content was positively associated (p<0.01) with occurrence of polymorphisms within DEGS1, TNNT2 genes and negatively with occurrence of TACC2 polymorphism. Conclusion: This study's results indicate that the SNP calling analysis based on RNA-seq data can be used to search candidate genes and establish the genetic basis of phenotypic traits. The presented results can be used for future studies evaluating the use of selected SNPs as genetic markers related to muscle histological profile and production traits in pig breeding.

심장수술 시 황산마그네슘 투여의 임상효과 (The Clinical Effect of Administration of Magnesium Sulfate in Cardiac Surgery)

  • 배경: 심폐바이패스를 이용한 심장수술에 따른 혈중 마그네슘 결핍 현상은 흔히 발생하는 합병중으로 이 연구의 목적은 심장수술시 인위적으로 첨가한 황산마그네슘의 유익한 임상 효과를 규명함에 있다. 대상 및 방법: 심폐바이패스를 이용하여 심장수술을 시행하기로 예정된 환자들 35명을 무작위로 황산마그네슘 투여군(n=15)과 투여하지 않은 군(n=20)으로 분류한 뒤 마그네슘군은 심폐기 충전액에 황산마그네슘 1g과 심정지액에 황산마그네슘 1g을 각각 첨가하였다. 요골 동맥으로부터 혈액을 채취하여 $Mg^{++}$ 농도, 전해질농도, 동맥혈액 가스분석, 혈액학적 분석, 혈청 총 단백 농도와 albumin 농도, blood urea nitrogen (BUN), creatinine, alanine amino transferase (ALT), aspartate aminotransferase (AST), total bilirubin 농도, tumor nerosis factor-${\alpha}$ $(TNF-{\alpha})$, interleukin-6 (IL-6), interleukin-10 (IL-10), creatine phosphokinase (CpK), creatine kinase-MB (CK-MB), lactate dehydrogenase (LDH), troponin-I (TNI) 농도, prothrombin time (PT)과 activated partial thromboplastin time (aPTT) 등도 측정하였다. 또한 수술 전과 후 정맥 혈액을 채취하여 activated clotting time (ACT)을 측정하였다. 결과: $Mg^{++}$ 농도는 수술 동안 및 후 시기에 마그네슘군이 대조군보다 유의하게 높게 유지되었다(p<0.05). 수술 전 부정맥이 없었던 환자들(대조군=10명, 마그네슘군=17명) 중 수술 후 부정맥 발생 수는 대조군이 10명(100%) 마그네슘군이 8명(46.4%)으로 마그네슘군이 대조군보다 유의하게 적었다(p=0.050). 결론: 심장수술 시 저용량의 황산마그네슘의 투여는 수술 중과 수술 후 저마그네슘 혈증의 예방과 부정맥의 빈도를 낮춰줄 것으로 생각한다.

Clinical Comparison of Low-dose and High-dose Steroid in Pediatric Cardiac Surgery with Cardiopulmonary Bypass

  • Cardiopulmonary bypass (CPB) for cardiac surgery triggers the production and release of numerous chemotactic substances and cytokines, ensuing systemic inflammatory response that leads to postoperative major organ dysfunction. Traditionally, corticosteroids (steroid) have been administered to patients undergoing cardiac surgery to ward off these detrimental physiologic alterations. However, the majority of the studies have been performed on adult patients with high-dose steroid. We carried out a randomized, prospective, double-blind study to compare the efficacy of low-dose steroid with that of high-dose steroid and to determine the adequate dose of pretreated-steroid for prophylactic effects in pediatric cardiac surgery. Thirty pediatric patients scheduled for elective cardiac surgery were randomly assigned to two groups; fifteen patients received low-dose methylprednisolone (10mg/kg intravenously, n=15, low-dose group) and the others received high-dose methylprednisolone (30mg/kg intravenously, n=15, high-dose group) 1 hour prior to CPB. Arterial blood samples were taken before CPB (Pre-CPB), 10 minutes after start of CPB (CPB-10), and immediately after CPB-end (CPB-OFF) for measuring total leukocyte counts (T-WBC) and diff-counts, platelet counts, interleukin-6 (IL-6), myeloperoxidase (MPO), total antioxidant (TAO), neuron-specific enolase (NSE), troponin I (TNI), aspartate aminotransferase (AST), alanine aminotransferase (ALT), creatinine, and blood urea nitrogen (BUN) levels. Other parameters such as volumes of urine output, pulmonary index $(PI,\;PaO_2/FiO_2)$, mechanical ventilating period, intensive care unit (ICU)-staying period, postoperative complications (fever, wound problem), postoperative 24 hrs and total volumes in blood loss, and hospitalized days were also assessed. All parameters were compared between two groups. There were no significant differences in T-WBC counts, monocyte fraction, platelet counts, TA levels, NSE levels, creatinine levels, BUN levels, the volumes of total urine output, PI, the incidences of fever and wound problem, postoperative 24hrs- and total-blood loss volumes and ICU-staying period between two groups (P>0.05). At CPB-OFF, neutrophil fraction, MPO level, TNI level, and AST level were higher in the high-dose group than in the low-dose group (P<0.05). IL-6 level at CPB-10 was higher in the high dose-group than in the low-dose group (P<0.05). Furthermore, mechanical ventilating periods and hospitalized days of the high-dose group were significantly longer than those of low-dose group (P<0.05). The high-dose group had significantly low lymphocyte fi-action at CPB-OFF compared with the low-dose group (P<0.001). These findings suggest that pretreatment of high-dose steroid is not superior to that of low-dose steroid regrading its potential benefits in pediatric cardiac surgery. Therefore, the conventional strategy of steroid treatment, high-dose pretreatment, should be modified in the cardiac surgery with CPB. However, further studies must be performed on the larger number of patients in as much as small number of patients in this study.

Myocardial Injury Following Aortic Valve Replacement for Severe Aortic Stenosis: Risk Factor of Postoperative Myocardial Injury and Its Impact on Long-Term Outcomes

  • Background: As hypertrophied myocardium predisposes the patient to decreased tolerance to ischemia and increased reperfusion injury, myocardial protection is of utmost importance in patients undergoing aortic valve replacement (AVR) for severe aortic valve stenosis (AS). Methods: Consecutive 314 patients (mean age, $62.5{\pm}10.8$ years; 143 females) with severe AS undergoing isolated AVR were included. Postoperative myocardial injury (PMI) was defined as 1) maximum postoperative creatinine kinase isoenzyme MB or troponin-I levels ${\geq}10$ times of reference, 2) postoperative low cardiac output syndrome or episodes of ventricular arrhythmia, or 3) left ventricular ejection fraction of less than 55% and decrease in left ventricle (LV) ejection fraction of more than 20% of the baseline value. Results: There were 90 patients (28.7%) who developed PMI. There were five cases of early death (1.6%), all of whom had PMI. On multivariable analysis, the use of histidine-tryptophan-ketoglutarate (HTK) solution instead of blood cardioplegia (odds ratio [OR], 3.06; 95% confidence interval [CI], 1.63 to 5.77; p=0.001), greater LV mass (OR, 1.04; 95% CI, 1.01 to 1.07; p=0.007), and increased cardiac ischemic time (OR, 1.13; 95% CI, 1.05 to 1.22; p<0.001) were independent predictors for PMI. Patients who had PMI showed significantly inferior long-term survival than those without PMI (p=0.049). Conclusion: PMI occurred in a considerable proportion of patients undergoing AVR for severe AS and was associated with poor long-term survival. HTK cardioplegia, higher LV mass, and longer cardiac ischemic duration were suggested as predictors of myocardial injury.

Double-Chambered Right Ventricle in an Old Standard Poodle Dog

  • A 12-year-old Standard Poodle presented with intermittent weakness and occasional dyspnea at the Veterinary Medicine Teaching Hospital of Kangwon National University. A grade of 4 out of 6 systolic murmur with an irregular tachycardic rhythm was auscultated on both sides of the chest. Systolic blood pressure was 140 mmHg. Panting was noticed in the hospital, but there was no crackle sound. Blood analysis revealed mild increases in liver panel levels (alanine aminotransferase 149 [reference interval, 19-70] U/L; and alkaline phosphatase, 185 [reference interval, 15-127] U/L) and severe increases in cardiac biomarker levels (n-terminal pro-brain natriuretic peptide, 4169 [reference interval, 50-900] pmol/L; and cardiac troponin I, 0.22 [reference interval, 0.03-0.12] ng/mL). On electrocardiography, irregularly irregular supraventricular tachycardic rhythm with an f-wave and no distinct p-wave was observed. Generalized cardiomegaly with an enlarged right atrium and left ventricle was confirmed on thoracic radiography. Moreover, hepatomegaly and an enlarged caudal vena cava were observed. Echocardiographic evaluation revealed a fibromuscular diaphragm in the right ventricle. Because of the obstructive lesion in the right ventricle, the right atrium and ventricle were enlarged (right atrial area index, 38.82 cm2/m2 [reference interval, 4.2-10.2 cm2/m2]; right ventricle end-diastolic area index, 14.152 cm2/m2 [reference interval, 4.9-10.92 cm2/m2]). Accordingly, the patient was diagnosed with double-chambered right ventricle (DCRV). Pimobendan, furosemide, enalapril, diltiazem, and S-adenosylmethionine (SAMe) were prescribed, and all symptoms were relieved. DCRV is a right-sided congenital heart defect resembling pulmonic valve stenosis. If symptoms are not severe, medical therapy can be facilitated without surgery or the balloon dilation.
