  • Objectives : I would find out the interpretation of cong Yin yin Yang(從陰引陽) cong Yang yin Yin(從陽引陰) in Su Wen(素問) Yin Yang Yin Xiang Da Lun(陰陽應象大論) and its clinical meanings. Methods : I had searched the opinions of annotators about this paragraph and compared treatment principles that TCM clinicians, such as Li Dong-yuan(李東垣), Zhang Jie-bin (張介賓), Yu Jia-yan(喩嘉言) developed. Results : This paragraph is related to treatment principles of the paragraph Yang bing zhi Yin(陽病治陰) Yin bing zhi Yang. Li Dong-yuan applied it to treatment principle of internal injuries(內傷病), Zhang Jie-bin extended its clinical meanings to the opposite treatment principles(反治) such as gu han wu xie(求汗於血) sheng Qi wu Jing(生氣於精), yin huo gui yuan(引火歸源) and na Qi gui shen(納氣歸腎), Yu Jia-yan used it as treatment principle of tuo zheng(脫症). Conclusion : This paragraph can mean several messages and be applied to several treatment principles.

An Evaluation of Significance of Herbal Acupuncture on Treating Limb Impediment Among the Children with Development Disabilities (발달장애아동의 수족장애에 대한 약침요법의 유의성 평가)

  • Objectives : This study was conducted to establish a standard and classify suitability in the treatment of limb impediment among the children with development disabilities using bee venom and eight principles herbal acupuncture. Methods : 10 patients with pediatric development disabilities with limb impediment as the main symptoms were chosen in this study. Bee venom herbal acupuncture and eight principles herbal acupuncture treatments were rendered and evaluated responses as well as pursuing most proper treatment methods. Results : 1. Bee venom herbal acupuncture showed a significant effects when used as supplement treatment technique for the children with partial movement, but insignificant for the children whom were unable to move. 2. Bee venom was effective for severe limb impediment and limb asthenia, whileas eight principles herbal acupuncture effective for mild limb impediment and spasticity. 3. Bee venom herbal acupuncture used in conjunction with eight principles herbal acupuncture for treating limb impediment among the children with development disabilities showed general improvement by intensifying muscular strength.

  • Anterior Cruciate Ligament (ACL) plays an important biomechanical role for the stability of knee joint. ACL injury often leads to injuries of articular cartilage, menisci, or other supporting structures, and subsequent development of degenerative arthritis. Controversies still exist in the best treatment modalities of ACL injuries. hut the author considers it most important to make the appropriate patient selection for operative reconstruction or nonoperative treatment. and describes the treatment principles of ACL injury, including diagnosis, patient selection and the treatment modalities for successful treatment of ACL injury.

  • A case of chronic dry cough which was tried to be treated through so many medications in a period of two months, responded very well to a simple ayurvedic remedy composing of a common formulation chosen as per the ayurvedic principles of pathogenesis and management. This case may be an eye opener to the concurrent ayurvedic clinical practice and invites ayurvedic practitioners to stick to their own principles of disease management for obtaining better responses.

A Study on the Zhongfeng Treatment of Zhang Shanlei Based on the Zhongfeng Jiaoquan (장산뢰(張山雷)의 중풍(中風) 치료법(治療法)에 관한 연구(硏究) - 『중풍각전(中風斠詮)』을 중심으로 -)

  • Objectives : This paper aims to study the characteristics of zhongfeng treatment by examining the eight principles of zhongfeng treatment in the Zhongfeng Jiaoquan of Zhang Shanlei along with Zhang Bolong's treatment of 'Yangxu Leizhongfeng[Yang deficiency pseudo Wind damage]' which is missing from the eight principles. Methods : The treatment methods in the Zhongfeng Jiaoquan was organized in the order of cause, characteristic, symptom, treatment, and precautions, in order to analyze features that were emphasized by Zhang in zhongfeng treatment. Results : First, treatment for bizheng is to 'open and close', then apply methods of 'qianyang jiangqi(潛陽降氣)' and 'zhenni huatan(鎭逆化痰)' while that for tuozheng is to 'lianyin yiye(戀陰益液)' accompanied by medicinals that 'qianzhen xutang(潛鎭虛陽)'. Second, treatment for ganyang shangnizheng is to 'qianzhen rougan', while for tanzian yongsezheng, one must 'dangdi(蕩滌)' for those who are strong in qi, 'xiehua(泄化)' for those who are weak in qi, while for those who have qinizheng[qi reverse syndrome] to 'shunqi(順氣).' Third, for deficiency in xinye and ganyin, one must 'yuyin yangxue[育陰養血]', while for deficiency in shenyin, one must first 'qianjiang shena[潛降攝納]' then slowly apply the method of 'ziyang shenyin[滋養腎陰]' if there is no phlegm turbidity. Fourth, in order to communicate the meridians and unfold collaterals, if the pathogen is external, apply the method of 'yangxue tongluo[養血通絡]', while if the pathogen is internal, calm by doing 'qianyang zhenni[潛陽鎭逆].' Fifth, in order to treat pseudo zhongfeng caused by yang deficiency, one must 'lianyin gutuo[戀陰固脫]' while using medicinals that 'jiangxiang[潛降]'. Conclusions : Treatment of zhongfeng in the Zhongfeng Jiaoquan diverged from 'wenjing sanhan', the usual approach to zhongfeng which sees it as external, and established the 'qianjiang zhenshe [潛降鎭攝]' treatment method based on the internal wind theory. It suggests a new Korean Medical pathology based on theories of Western medicine, and introduces eight principles in treating zhongfeng, which would influence the treatment of zhongfeng in the future.

  • Objectives The purpose of this study is to analyze the types of childhood simple obesity and suggest clinical symptoms, treatment principles, and traditional Chinese medicine (TCM) treatments for each type. Methods All kinds of literature published by the China National Knowledge Infrastructure (CNKI) up to August 20, 2022 were analyzed. We extracted information about types of childhood simple obesity, relevant clinical symptoms, treatment principles and TCM treatments. Results 25 studies were included. Spleen deficiency with dampness obstruction, gastrointestinal dampness-heat, internal excess of phlegm-dampness were the most reported. Spleen deficiency with dampness obstruction has symptoms of powerless, heavy limbs, pale tongue, teeth-marked tongue, sunken and slippery pulse. As a treatment, herbal medicine (HM) like modified Banggihwanggitang and acupoint like Joksamri were mainly reported. Gastrointestinal dampness-heat has symptoms of thirst, constipation, edacity, rapid hungering, heavy limbs, red tongue, slippery and rapid pulse. HM like Modified Xiehuangsan to clear heat was mainly reported. Internal excess of phlegm-dampness has symptoms of heavy limbs, lack of strength, tongue with white slimy fur, slippery pulse. Modified Ijintang to dry dampness to resolve phlegm was mainly reported. Conclusions This study analyzed types of pattern, clinical symptoms, treatment principles, and TCM treatments of childhood simple obesity. Based on this study, it is necessary to derive a standardized dialectical information that reflects the domestic situation.

Oral management of Stevens-Johonson syndrome, toxic epidermal necrolysis patients (스티븐 존슨 증후군과 중독성 표피 괴사 융해증 환자의 구강위생관리)

  • Steven-Johnson syndrome (SJS) and toxic epidermal necrolysis(TEN) are severe mucocutaneous reaction which are most frequently caused by drugs. Although the incidence of SJS and TEN is known to be relatively low, outcomes may be fatal. A systematic approach is required because morbidity rate is currently increasing and oral lesion is frequent. We investigated the clinical features and outcomes of 6 patients diagnosed as SJS and TEN and referred from the department of dermatology, Chonnam National University Hospital for oral care. Ketoconazol, Ofloxacin, Chlorphenesin, Amoxicillin, Pontal, Harnal, and Ciprofloxacin were suspected as the causative drugs. Average treatment period was 3.2 weeks, and two patients were referred to 'burn-patients' hospital. Most of oral lesion were cured be normal tissue, but scares with discoloration were observed. For intraoral management, antibiotic disinfection and steroid application were performed according to systemic treatment principles. Additionally, ingestion of zinc, antioxidants, and vitamin was recommended. The establishment of oral treatment principles is demanded because it has not been yet. Also, through investigation of drug side effect and careful prescription are required.

  • Saam acupuncture has an evident principles of prescription based on acupuncture tonification and purgation of Nan-gyeong but many prescriptions of Saam acupuncture are not typical patterns. Therefore it is difficult to understand principles of those prescriptions. Understanding the principles of any prescription means understanding the physiology and pathology of that prescription and it have an important role in selecting the best prescription and taking the most effective treatment. This paper studies principles of prescriptions of the king fire and the premier fire. The theoretical authorities of analysis are Nan-gyeong, works of Joo dan-gye and Li dong-won and Uihakimmun. The damage of fire to mentality is very intensive and severe. The prescriptions of the king fire and premier fire have a good effect on diseases by fire. The fire of human body are two types, the king fire and the premier fire but the water of human body is only one and then inferior to fire. The fire superior to the water is unstable and the fire influences rapidly and intensively on four elements, wood, earth, metal, water. So the control of the water is the key to treat the disease by fire. Considering the attack and the treatment of disease, heart and liver are attacked with a disease by fire in its early stage and kidney and lung are the key to treatment. Consequently the pathology of the disease by fire is corresponding to the theory- Yang Sufficient, Yin Deficient- of Joo dan-gye. It seems that the theory of Joo dan-gye influenced on the medical theory of Saam Acupuncture. On that account I believe that the study of medical theories of the Geum and Won Dynasty that various medical theories appeared is very available.

  • Inspiratory pneumoniais one of the common complications in stroke patients who have dysphagia. Pneumonia due to Klebsiella bacteria shows higher fatality than other bacteria. For this reason, K. pneumoniae can be fatal to immune deficient patients. The purpose of this paper is to report the adapting pattern identification according to principles of defense, Gi, nutrients and blood on K. pneumoniae treatment. In this case, improvement was seen in a stroke patient with dysphagia diagnosed with K. pneumoniae by X-ray, sputum culture, and by using Cheonghobyulgaptang as a way of nourishing Eum and clearing heat, because in this case K. pneumoniae was presumed to cause Eumdeficiency due to the chronic low fever, cough, sputum and weakness. This paper proposes that adapting pattern identification according to principles of defense. Gi, nutrients and blood can increase the efficiency of K. pneumoniae treatment.

  • Purpose : The purpose of this study is to report the effect of oriental treatments for the abnormal uterine bleeding. Methods : We treated a 30-year-old female who had been suffering from abnormal uterine bleeding for one month by Herb medication(Yikweseungyangtang, Chokyungjongoktang, Jahagehwan), acupuncture therapy and moxibustion therapy at the same time. And these teatments were based on 塞流([sailiu]-stanch blood), 澄源([chengyuan]-correct origin), 復舊([fuliu-restore) which are principles of treatment of 崩漏([benglou]-metrorrhagia). Results : After oriental medical teatment, abnormal uterine bleeding was disappeared and recovered the normal menstrual cycle. Conclusion : This case study shows that the oriental medical treatment in accordance with three principles of oriental treatment has an useful effect on abnormal uterine bleeding patient's treatment and recovery.
