  • Past treatment of the single machine maintenance problem has shown that preventive maintenance may be desirable for equipment for which failures are caused at least partially by wear-out factors. In all previous treatment, however, the size of the maintenance-repair crew has been held constant and the optimal maintenance period has then been determined. This paper suggests a simultaneous solution for the maintenance-repair crew size and the optimal maintenance period. The optimal maintenance period is seen to shift as the size of the maintenance-repair crew varies.

  • A knee joint's longevity depends on the proper integration of structural components in an axial alignment. If just one of the components is abnormally off-axis, the biomechanical system fails, resulting in arthritis. The complexity of various failures in the knee joint has led orthopedic surgeons to select total knee replacement as a primary treatment. In many cases, this means sacrificing much of an other-wise normal joint. Here, we review novel computational approaches to describe knee physiotherapy by introducing a new dimension of foot loading to the knee axis alignment producing an improved functional status of the patient. New physiotherapeutic applications are then possible by aligning foot loading with the functional axis of the knee joint during the treatment of patients with osteoarthritis.

Magnetic NDE for Sensitization of Inconel 600 Alloy

  • Inconel 600 alloy, Ni base alloy, is widely used for steam generator tubings where sensitization occurs at grain boundaries and sensitization will induce tubing failures. This alloy has usually paramagnetic property, however, it transforms into ferromagnetic property along grain boundaries when sensitization occurs: this means NDE using magnetism for sensitization is possible. Therefore, in this study, Inconel 600 alloys were heat treated at 873 K from 0 to 400 hours so as to generate sensitization and their magnetic properties were investigated in detail. The saturation and the residual magnetization increase with increasing heat treatment time and take a maximum. On the other hand, the coercive force decreases with the increase in time of heat treatment. We confirmed that characteristics at only grain boundaries change into ferromagnetic phase by a MFM observation. As a trial for industrial application, heat treated Inconel 600 alloy was scanned by a magnetic field sensor, and the variations in magnetization were obtained nondestructively. The results indicate a feasibility of magnetic NDE for sensitization of Inconel 600 alloy.

  • Prognostics and health management (PHM) is recently converging throughout the industry, one of the trending issue is to detect abnormal conditions at decanter centrifuge during water treatment facilities. Wastewater treatment operation produces corrosive gas which results failures on attached sensors. This scenario causes frequent sensor replacement and requires highly qualified manager's visual inspection while replacing important parts such as bearings and screws. In this paper, we propose anomaly detection by measuring the vibration of the decanter centrifuge based on the video camera images. Measuring the vibration of the screw decanter by applying the optical flow technique, the amount of movement change of the corresponding pixel is measured and fed into the LST M model. As a result, it is possible to detect the normal/warning/dangerous state based on LSTM classification. In the future work, we aim to gather more abnormal data in order to increase the further accuracy so that it can be utilized in the field of industry.

The impact of radiotherapy on clinical outcomes in parameningeal rhabdomyosarcoma

  • Purpose: Radiotherapy (RT) is considered a mainstay of treatment in parameningeal rhabdomyosarcoma (PM-RMS). We aim to determine the treatment outcomes and prognostic factors for PM-RMS patients who treated with RT. In addition, we tried to evaluate the adequate dose and timing of RT. Materials and Methods: Twenty-two patients with PM-RMS from 1995 to 2013 were evaluated. Seven patients had intracranial extension (ICE) and 17 patients had skull base bony erosion (SBBE). Five patients showed distant metastases at the time of diagnosis. All patients underwent chemotherapy and RT. The median radiation dose was 50.4 Gy (range, 40.0 to 56.0 Gy). Results: The median follow-up was 28.7 months. Twelve patients (54.5%) experienced failure after treatment; 4 local, 2 regional, and 6 distant failures. The 5-year local control (LC) and overall survival (OS) were 77.7% and 38.5%, respectively. The 5-year OS rate was 50.8% for patients without distant metastases and 0% for patients with metastases (p < 0.001). Radiation dose (<50 Gy vs. ${\geq}50Gy$) did not compromise the LC (p = 0.645). However, LC was affected by ICE (p = 0.031). Delayed administration (>22 weeks) of RT was related to a higher rate of local failure (40.0%). Conclusion: RT resulted in a higher rate of local control in PM-RMS. However, it was not extended to survival outcome. A more effective treatment for PM-RMS is warranted.

Effect of Hydrofluoric Acid Etching on Shear Bond Strength between Resin Cement and Zirconia cores (표면 불산처리가 레진시멘트와 지르코니아 하부구조물의 전단결합강도에 미치는 영향)

  • The purpose of this study was to evaluate the influence of hydrofluoric acid etching treatment on the bonding strength of yttria-stabilized tetragonal zirconia polycrystal(Y-TZP). Four groups of zirconia-resin cement specimens were prepared; 1) ZGS group (zirconia, no treatment), 2) ZGSH group (zirconia, hydrofluoric acid etching treatment) 3) H-ZGS group (Hybrid zirconia, no treatment) 4) H-ZGSH group (Hybrid zirconia, hydrofluoric acid etching treatment). The shear bond strength between zirconia and porcelain was measured using a Instron Universal Testing Machine(Model DBBP-500, Instron Corporation, Kyonggi, Korea). Data were statistically analyzed using independent t-test and two-way ANOVA(${\alpha}=0.05$). The ceramic-resin cement bonding strength was affected by hydrofluoric acid etching treatment(p<0.05). Digital microscope examination of the fracture surface showed mixed failures with adhesive and cohesive types in hydrofluoric acid etching treatment with treated zirconia and hybrid zirconia groups.

Plasmodium falciparum Genotype Diversity in Artemisinin Derivatives Treatment Failure Patients along the Thai-Myanmar Border

  • Genetic characteristics of Plasmodium falciparum may play a role in the treatment outcome of malaria infection. We have studied the association between diversity at the merozoite surface protein-1 (msp-1), msp-2, and glutamate-rich protein (glurp) loci and the treatment outcome of uncomplicated falciparum malaria patients along the Thai-Myanmar border who were treated with artemisinin derivatives combination therapy. P. falciparum isolates were collected prior to treatment from 3 groups of patients; 50 cases of treatment failures, 50 recrudescences, and 56 successful treatments. Genotyping of the 3 polymorphic markers was analyzed by nested PCR. The distribution of msp-1 alleles was significantly different among the 3 groups of patients but not the msp-2 and glurp alleles. The allelic frequencies of K1 and MAD20 alleles of msp1 gene were higher while RO33 allele was significantly lower in the successful treatment group. Treatment failure samples had a higher median number of alleles as compared to the successful treatment group. Specific genotypes of msp-1, msp-2, and glurp were significantly associated with the treatment outcomes. Three allelic size variants were significantly higher among the isolates from the treatment failure groups, i.e., $K1_{270-290}$, $3D7_{610-630}$, $G_{650-690}$, while 2 variants, $K1_{150-170}$, and $3D7_{670-690}$ were significantly lower. In conclusion, the present study reports the differences in multiplicity of infection and distribution of specific alleles of msp-1, msp-2, and glurp genes in P. falciparum isolates obtained from treatment failure and successful treatment patients following artemisinin derivatives combination therapy.

Medical Treatment of Laryngopharyngeal Reflux (인후두역류의 약물치료)

  • Otolaryngological manifestations of acid reflux include a wide range of pharyngeal and laryngeal symptoms ; and the constellation of symptoms has been called laryngopharyngeal reflux (LPR). In the absence of definite diagnostic criteria, LPR disease remains a subjective entity. A diagnosis of LPR is usually based on response of symptoms to empirical treatment. Investigative modalities such as pH monitoring and, more recently, impedance studies are generally reserved for treatment failures. LPR usually requires more aggressive and prolonged treatment to achieve regression of both symptoms and laryngeal findings. The suppression of gastric acid and secretion with anti-secretary agents has been the mainstay of medical treatment for patients with acid-related disorders. The suppression of gastric acid secretion achieved with Hz-receptor antagonist $(H_2RA)$ has proved suboptimal for relief of reflux symptoms. The rapid development of tolerance and rebound acid hypersecretion after the with-drawal of $H_2RA$ limit their clinical use. Proton pump inhibitors (PPI) have been proved to be very effective for suppressing intragastric acidity, but the optimal dose and duration is unknown. Current evidence indicates that pharmacologic intervention should include, at a minimum, a 3 month trial of twice daily PPI. Symptoms of LPR improve over 2 months of therapy. The physical findings of LPR resolve more slowly than the symptoms and this continues through out at least 6 months of treatment. For most patients with LPR, twice daily dosing with a PPI is usually recommended for an initial treatment for a period of no less than 6 months treatment, and lifetime treatment may be required.

Nephroprotective Effects of Daebuncheong-eum Aqueous Extracts on Cisplatin-induced Acute Renal Failure in Rats (대분청음(大分淸飮)이 Cisplatin으로 유발(誘發) 된 흰쥐의 급성신부전(急性腎不全)에 미치는 영향(影響))

  • Purpose : This study was to observe the nephroprotective effects of the traditional prescription, Daebuncheong-eum (DBCE). DBCE has generally been used for treating various renal diseases, including renal failure. Methods : Three different dosages of DBCE extract were orally administered once a day for 28 days. At the 23rd day after DBCE extract treatment, cisplatin was also treated. Then, 5 days after cisplatin treatment, all rats (6 groups of 8 rats each) were sacrificed. Changes on the body weight, kidney weight, serum BUN and creatinine levels were observed, along with changes to the kidney MDA and GSH contents. The results were compared with captopril 100mg/kg, from which the effects on cisplatin-induced acute renal failures have already been confirmed. Results : Cisplatin induced ARF are induced by oxidative stress and related lipid peroxidation in the present study. However, these ARFs and inhibition of antioxidant effects induced by cisplatin were dose-dependently reduced by treatment of all three different dosages of DBCE extracts. Conclusion : This study suggests that DBCE extracts show favorable effects on cisplatin-induced rat ARF.

Prognosis following dental implant treatment under general anesthesia in patients with special needs

  • Background: This study retrospectively investigated outcomes following dental implantation in patients with special needs who required general anesthesia to enable treatment. Method: Patients underwent implant treatment under general anesthesia at the Clinic for the Disabled in Seoul National University Dental Hospital between January 2004 and June 2017. The study analyzed medical records and radiographs. Implant survival rates were calculated by applying criteria for success or failure. Results: Of 19 patients in the study, 8 were males and 11 were females, with a mean age of 32.9 years. The patients included 11 with mental retardation, 3 with autism, 2 with cerebral palsy, 2 with schizophrenia, and 1 with a brain disorder; 2 patients also had seizure disorders. All were incapable of oral self-care due to serious cognitive impairment and could not cooperate with normal dental treatment. A total of 27 rounds of general anesthesia and 1 round of intravenous sedation were performed for implant surgery. Implant placement was performed in 3 patients whose prosthesis records could not be found, while 3 other patients had less than 1 year of follow-up after prosthetic treatment. When the criteria for implant success or failure were applied in 13 remaining patients, 3 implant failures occurred in 59 total treatments. The cumulative survival rate of implants over an average of 43.3 months (15-116 months) was 94.9%. Conclusion: For patients with severe cognitive impairment who are incapable of oral self-care, implant treatment under general anesthesia showed a favorable prognosis.
