A Clinical Research of Obesity by the Acupuncture Therapy (비만증(肥滿症)의 침치료효과(鍼治療孝果)에 관(關)한 임상적(臨床的) 관찰(觀察))

  • Clinical studies were done on 109 peoples(9males. 100females) who were treated with the auricular and body acupuncture therapy to decrease body weight in the Dept. of the Acupuncture and Moxibution. Hospital of Oriental Medicine in Won Kwang University. from Feberuary 1, 1992 to March 31, 1994. The results were obtained as follows: 1. Total judgement of efficacy: significant(33.0%). good(50.4%). unchanged(16.5%). 2. Loss of appetite and decrease of meal quantum after auricular and body acupuncture treatment: loss of appetite(61.4%). decrease of meal quantum(68.8%). 3. Degree of average body weight loss during 1 week: 0{\sim}1.0kg(64.2%), $1.0{\sim}2.0kg(29.3%),\;2.0{\sim}3.0kg(6.4%)$. 4. Body weight loss during auricular and body acupuncture treatment : $0{\sim}0.5kg(16.5%),\;1{\sim}1.5kg(34.8%),\;2{\sim}2.5kg(17.4%),\;3{\sim}3.5kg(l1.0%),\;4{\sim}4.5kg(11.0%),\;5{\sim}5.5kg(5.5%),\;6{\sim}6.5kg(0.9%).\;7{\sim}7.5kg(2.7%)$. 5. The men who were $10.0{\sim}20.0%$ in the grade of obesity resulted in 79.1%. and men who were more than 20.0% in the grade of obesity resulted in 84.7%. The percentage of whole efficiency of the men who had been treated the auricular and body acupuncture therapy to decrease the body weight resulted in 83.4%, and of the men who were more than 20.0% in the grade of obesity resulted in 84.7% and of the man who belonged to simple obesity resulted in 87.8%.

  • The mucoepidermoid carcinoma is classified as either well, moderately, or poorly differentiated. The criteria used to classify the lesions are discussed, and pathologic features are illustrated. The most important factors in prognosis are : 1. degree of histologic differentiation, and 2. presence or abscence of tumor on the lines of surgical excision. Recurrences rates are correlated with histologic differentiation. Stewart, Foote, and Becker in 1945 coined the term "mucoepidermoid tumor" to discribe an unusual salivary neoplasm containing epidermoid and mucus-secreting cells which was thought to arise in salivsary gland ducts. The treatment of the mucoepidermoid carcinoma is chiefly surgical, although recent data have shown favorable responses to radiation therapy. Currently, surgery followed by radiation treatment is recommended for intermediate-grade and high-grade tumors ; low-grade tumors can be managed by surgery alone. Authors present a case of mucoepidermoid carcinoma managed with wide surgical resection and postoperative irradiation and showing a good clinical result with review of literatures.

Evaluation of Un-fasted Pig Stomach Spent Feed as a Substitute in Finishing Pigs Diet

  • Insufficient pre-slaughter fasting leaves serious amount of feed-like contents (designated here as un-fasted stomach spent feed, USSF) in the eviscerated pig stomach. This study was intended to evoke economical and environmental seriousness of USSF discharge by estimating its value as pig feed. For finishing pigs feeding trial, three levels (0, 5, and 10%) of USSF were blended with pig feed to prepare control and two treatment diets, respectively. A total of 42 (21 males, 21 females) crossbred (Landrace ${\times}$ Yorkshire ${\times}$ Duroc) finishing pigs weighing $81.5{\pm}8.0$ kg were employed to 28d feeding trial and in vivo digestibility trial by $Cr_2O_3$ indicator method with 7 males and 7 female pigs per treatment. In vitro total tract digestion of USSF showed 70.5% and 57.6% of DM and OM digestibilities, respectively which were poorer (p<0.05) than those of pig diet. There were no differences in body weight gain, daily feed intake and feed conversion ratio among treatments although 10% USSF substitution exerted relatively poor performance. In vivo apparent digestibilities of diets containing USSF 5% and 10% were lower (p<0.05) than that of 100% pig feed. There were no differences (p>0.05) in dressing percentage and carcass grade among treatments. Results of this study showed that 5% USSF substitution in finishing pigs diet did not exert any disadvantage in terms of production performance and carcass grade. This study implied that un-fasted slaughter causing excessive excretion of USSF should be avoided. If not avoidable, the USSF should not be wasted in abattoir but could be recycled as pig feed.

  • As for the current state of college taekwondo athletes' sport injury, times of occurrence and major causes showed differences in weight class and career, while situations of occurrence had differences in grade, weight class, and career. As for the measures to improve their coping ability, first aid showed differences in grade, while prevention of injury had differences in grade and career. Treatment methods turned out to have no differences. For college taekwondo athletes, sport injury occurred during exercise, mostly when they fell. In addition, lack of skill was the greatest major cause of sport injury. As for the sport injury coping ability, ice massage was used as first aid and self-massage was performed. For prevention of injury, sufficient fatigue recovery was considered to be the priority of prevention of injury.

Micropapillary Variant of Urothelial Carcinoma of the Urinary Bladder: Report of a Case with Cytologic Diagnosis in Urine Specimen (방광의 미세유두형 요로상피암종의 세포소견 -1예 보고-)

  • A micropapillary variant of urothelial carcinoma (MPC) is a distinct entity with an aggressive clinical course. It has a micropapillary configuration resembling that of ovarian papillary serous carcinoma. Its cytologic features have rarely been reported. We report a case of MPC detected by urine cytology. A woman aged 93 years presented with a chief complaint of macroscopic hematuria. Cytology of her voided urine showed clusters of malignant cells in a micropapillary configuration. Each tumor cell had a vacuolated cytoplasm, a high nuclear:cytoplasmic ratio, and irregular hyperchromatic nuclei. An ureteroscopic examination revealed exophytic sessile papillary masses extending from the left lateral wall to the anterolateral wall of the urinary bladder. A transurethral resection of the tumor was carried out. The tumor was characterized by delicate papillae with a thin, well-developed fibrovascular stromal core and numerous secondary micropapillae lined with small cuboidal cells containing uniform low- to intermediate-grade nuclei and occasional intracytoplasmic mucinous inclusions. These tumor cells infiltrated the muscle layers of the bladder, and lymphatic tumor emboli were frequently seen. Recognizing that the presence of MPC components in urinary cytology is important for distinguishing this lesion from low-grade papillary lesions and high-grade urothelial carcinomas can result in early detection and earlier treatment for an improved treatment outcome.

A Case Report of Liposarcoma on the Neck in an Adult (성인에 발생한 경부 지방육종의 증례보고)

  • Purpose: Although liposarcoma is the second most common soft tissue sarcoma in adults, the incidence of liposarcoma of the head and neck is rare. There is only one reported case in Korea and moreover, only in adolescence. We report a case of liposarcoma on the neck in a 32-year-old male in adult. Methods: The patient had a slow growing, none tender mass on the posterior neck without lymphadenopathy, which has been present for 3 years and recurred twice during that time. MRI showed a 1.5 cm sized ovoid, well demarcated mass that was located in the subcutaneous layer of the posterior neck. Results: The mass was surgically removed. The resection margin was free of tumor on frozen biopsy and histopathologic examination indicated myxoid and round cell liposarcoma. The whole body F-18 FDG PET-CT applied on the fourteenth day postoperatively, revealed a moderate FDG-uptaking soft tissue lesion showing postoperative wound healing process on the posterior neck region and there was no distant metastasis. Conclusion: Liposarcoma is the second most common soft tissue sarcoma in adults. But, it rarely involves the head and neck region. Prognosis is principally dependent on histologic subtype and grade. Low grade liposarcoma such as well differentiated and myxoid liposarcoma tend to recur locally, rarely metastasize. On the other hand, high grade liposarcoma such as round cell and pleomorphic liposarcoma have higher rates of local recurrence and distant metastasis. Complete surgical excision provides the most effective means of treatment. Radiotherapy or chemotherapy can be used as an asjunctive treatment modality.

Current Radiopharmaceuticals for Positron Emission Tomography of Brain Tumors

  • Brain tumors represent a diverse spectrum of histology, biology, prognosis, and treatment options. Although MRI remains the gold standard for morphological tumor characterization, positron emission tomography (PET) can play a critical role in evaluating disease status. This article focuses on the use of PET with radiolabeled glucose and amino acid analogs to aid in the diagnosis of tumors and differentiate between recurrent tumors and radiation necrosis. The most widely used tracer is $^{18}F$-fluorodeoxyglucose (FDG). Although the intensity of FDG uptake is clearly associated with tumor grade, the exact role of FDG PET imaging remains debatable. Additionally, high uptake of FDG in normal grey matter limits its use in some low-grade tumors that may not be visualized. Because of their potential to overcome the limitation of FDG PET of brain tumors, $^{11}C$-methionine and $^{18}F$-3,4-dihydroxyphenylalanine (FDOPA) have been proposed. Low accumulation of amino acid tracers in normal brains allows the detection of low-grade gliomas and facilitates more precise tumor delineation. These amino acid tracers have higher sensitivity and specificity for detecting brain tumors and differentiating recurrent tumors from post-therapeutic changes. FDG and amino acid tracers may be complementary, and both may be required for assessment of an individual patient. Additional tracers for brain tumor imaging are currently under development. Combinations of different tracers might provide more in-depth information about tumor characteristics, and current limitations may thus be overcome in the near future. PET with various tracers including FDG, $^{11}C$-methionine, and FDOPA has improved the management of patients with brain tumors. To evaluate the exact value of PET, however, additional prospective large sample studies are needed.

Combined Effects of Gami-sipjeondaebo-tang Administration, Acupuncture, and Wet Dressing in a Patient with Grade 4 Pressure Ulcer: A Case Report (4도 욕창 환자에 대한 가미십전대보탕 투여 및 침치료와 습윤드레싱의 병행 효과 : 치험 1례)

  • Introduction: Pressure ulcers, often found in patients with mobility issues, particularly elderly patients, are increasing in prevalence. Their treatment is complex, with recovery more challenging in advanced stages, and certain factors can delay healing. Case Presentation: An 87-year-old male developed a grade IV pressure ulcer on his left greater trochanter following lumbar fractures and subsequent percutaneous vertebroplasty in 2021. Despite potential surgical recommendations, his treatment consisted of daily wet dressings, acupuncture from July 2021 to February 2022, and Gami-sipjeondaebo-tang beginning August 2021. The ulcer, initially measuring 6 cm×6 cm and showing a red wound without necrosis, healed completely and showed no signs of recurrence as of August 2023. Conclusion: A grade IV pressure ulcer was effectively treated using wet dressing, acupuncture, and Gami-sipjeondaebo-tang, demonstrating no recurrence over a 1.5-year period.

  • Impurity removal from metallurgical grade silicon by acid washing at $50^{\circ}C$ was investigated by employing sulfuric, nitric acid and the mixture of hydrochloric and hydrofluoric acid. Acid washing treatment had no effect on the removal of boron and the concentration of this clement after treatment was rather increased. In our experimental range, the removal percentage of phosphorus was 60%. In the acid washing with sulfuric and nitric acid, the removal percentage of major impurities was below 50%, which indicates that refining effect was not great with these acids. Acid washing with the mixture of hydrochloric and hydrofluoric acid led to removal percentage of higher than 90%. Data on the purity of silicon after acid washing at various conditions are reported.

Outcomes in the Management of Spontaneous Cerebellar Hemorrhage

  • Objective : The operative indications on cerebellar hemorrhage have been controversial especially when the patient condition is grave. Therefore we investigated whether it can be justifiable if we perform the surgery in poor clinical grade. Methods : Clinical records and computerized tomography[CT] films of the 89 patients, who were undergone hospital treatment due to spontaneous cerebellar hemorrhage between May 1997 and May 2004, were retrospectively researched. Results : The study population consisted of 36 males and 53 female patients. The mean age was 65years [range $23{\sim}89$]. As a result of treatment, the patients, whose Glasgow coma scale[GCS] score were higher, showed better outcomes [p=0.001]. 13 patients [14.6%] were below 5 in GCS score and 10 patients of these were operated. Among 10 patients, 4 patients [40%] showed good outcome and 5 patients [50%] had been dead. 3 patients [60%] of these dead patients had the findings of intraventricular hemorrhage, fourth ventricular obliteration and hydrocephalus in CT scan. Conclusion : This study suggests that operation may be justifiable in clinically poor grade patient with spontaneous intra cerebellar hemorrhage.
