Prognostic Factors of Renal Scarring on Follow-up DMSA Scan in Children with Acute Pyelonephritis

  • Purpose: Early diagnosis and treatment of urinary tract infection have been emphasized to prevent renal scarring. If untreated, acute pyelonephritis could cause renal injury, which leads to renal scarring, hypertension, proteinuria, and chronic renal failure. The purpose of this study was to assess risk factors of renal scarring after treatment of acute pyelonephritis (APN). Methods: The medical records of 59 patients admitted at Daegu Fatima Hospital because of APN between March 2008 and April 2015 whose renal cortical defects were confirmed by using initial technetium-99m dimercaptosuccinic acid (DMSA) scans were reviewed retrospectively. We divided 59 patients into 2 groups according to the presence of renal scar and assessed risk factors of renal scar, including sex, age at diagnosis, feeding method, hydronephrosis, bacterial species, vesicoureteral reflux, and vesicoureteral reflux grade. Results: Of 59 patients (41%), 24 showed renal scar on follow-up DMSA scan. No significant differences in sex, hydronephrosis, bacterial species, and fever duration were found between the renal-scarred and non-scarred groups. As for age at diagnosis, age of >12 months had 5.8 times higher incidence rate of renal scarring. Vesicoureteral reflux (VUR) affected renal scar formation. VUR grade III or IV had 14.7 times greater influence on renal scar formation than VUR grade I or II. Conclusion: Our data suggest that the presence of VUR and its grade and age at diagnosis are risk factors of renal scar on follow-up DMSA scan after APN.

Study on the Development of 340MPa Grade Super Formable High Strength Steel Sheets (340MPa 급 초고성형성 고강도강판 개발에 관한 연구)

  • The demand for formable high-strength steel (HSS) sheets has recently increased to reduce the weight of automotive bodies. The 340MPa (Tensile Strength) grade steel sheets are widely used for body inner and outer panels. Especially, super formable 340MPa grade steel sheets with high r-value have an excellent deep drawability compared with the other 340MPa grade steel sheets. It is very available for a part such as rear floor, center floor and dash panels used conventional mild steels up to now. We developed a super formable HSS by optimization of chemical composition, texture control and heat treatment control. It has good mechanical properties with excellent formability (tensile strength: 343MPa, elongation: 41.1% and $\bar{r}=2.1$).

A Descriptive Analysis on Health Education in Elementary School after Introduction of 7th Curriculum Modification (7차 교육과정 시행 후 초등 보건교사의 보건교육 실태)

  • Purpose : This study was undertaken in order to find out the actual condition and barriers of health education in elementary school after introduction of 7th curriculum modification. Method: A questionaire was sent by mail on November 1, 2003, and received a total of 230 till November 30, 2003. Among them, 198 were included in fmal analysis. The collected data was analyzed through frequency, percentage, mean and standard deviation. Results : The results of this study were as follows: Among of the school health task, the most time spending task was emergency treatment and medication. The health education content which now being taught was sex education and prevention of drug abuse in the 6th grade, prevention of obesity in the 5th grade, emergency treatment, safety education in the 4th grade and prevention of disease of teeth in the 3rd grade. The most difficult problem in health education was pointed to no subject, no hour about health education by policy. Conclusion : It was necessary to formalize the health education subject, to ensure of health education hour by policy and to establish of extra health education room to improve health education at elementary school.

  • Impurities removal from waste carbon black was carried out to produce high-grade carbon black. A lot of hydrophilic carbon black is produced as a byproduct of the hydrogen production process by flame decomposition of water. Due to its impurity content such as sulphur, iron, ash and etc., it can only be used as low-grade carbon or burnt out. High-grade hydrophilic carbon black is 3-5 times more expensive than oil-based carbon black because of its process difficulties and requires pollutant treatment. Hydrophilic carbon is normally used far conductive materials for batteries, pigment for plastics, electric wire covering, additives for rubber, etc.. In these applications, hydrophilic carbon must maintain its high purity. In this study magnetic separation, froth flotation and ultrasonic treatment were employed to remove impurities from the low-grade hydrophilic carbon black. As results, the ash, iron and sulphur content of product decreased to less than 0.01wt.%, 0.0lwt.% and 0.3wt.% respectively, and the surface area of product was about 930 $m^2$/g.

A case of Adult Whole-body Atopic Dermatitis treated by Traditional Korean Treatment - Focus on the effect of Oryeong-san (한방치료를 통한 성인 전신성 아토피 피부염 치험 1례 - 오령산(五苓散) 사용 효과를 중점으로)

  • Objetives : The purpose of this study is to report the effect of Oryeong-san on adult whole-body atopic dermatitis. Methods : We treated a 21-year-old male patient who have adult whole-body atopic dermatitis by Korean medicine. This treatment was enforced from 2019.5.25 to 2019.6.29. And we have confirmed the improvement of itching, rashes and oozing. We recorded photos that includes change of symptoms. Results : After the treatments was done, the grade of VAS and SCORAD was decreased. The VAS grade was decreased from grade 8 to grade 2. The SCORAD score was decreased from 72.1 to 52.1. And itching, rashes, oozing were clearly decreased. Conclusions : Oryeong-san has effect on decreasing itching, rashes and oozing. Also, sleeping time reduced due to itchness was normalized. Thus, It is able that Oryeong-san works on skin desease like atopic dermatitis.

  • The grade of glioma is important information related to survival and thus is important to classify the grade of glioma before treatment to evaluate tumor progression and treatment planning. Glioma grading is mostly divided into high-grade glioma (HGG) and low-grade glioma (LGG). In this study, image preprocessing techniques are applied to analyze magnetic resonance imaging (MRI) using the deep neural network model. Classification performance of the deep neural network model is evaluated. The highest-performance EfficientNet-B6 model shows results of accuracy 0.9046, sensitivity 0.9570, specificity 0.7976, AUC 0.8702, and F1-Score 0.8152 in 5-fold cross-validation.

Endoscopic Assessment of Esophagitis with Transnasal Esophagoscopy in the Prediction of Treatment Response

  • Objectives : The purpose of this study was to investigate the endoscopic evidence of esophagitis in laryngopharyngeal reflux disease (LPRD) patients using transnasal esophagoscopy (TNE) and to correlate these findings with treatment response. Methods : Fifty patients underwent TNE at Korea University Anam Hospital from July 2007 to Feb 2009. Participants were selected from patients that presented with various laryngeal symptoms. One experienced otolaryngologist assessed esophagitis according to the Los Angeles classification system using the TNE findings. Results : Fifteen of 50 LPR patients (30%) were found to have esophagitis (12 patients with Grade A, 3 patients with Grade B, no patients with grade C/ D esophagitis). Among the 15 patients positive for esophagitis based on the endoscopic findings, 12 (80%) showed symptom improvement after pharmacological therapy. Symptom improvement was correlated with evidence of esophagitis (p=0.002) but not with RFS (p=0.749). Conclusion : Endoscopic evaluation of esophagitis using TNE is a potentially valuable tool for predicting treatment response in LPRD patients.

A Case Report of Diabetic Retinopathy Vitreous Hemorrhage (당뇨망막병증성 유리체출혈 환자 치험 1례 보고)

  • Objectives : The aim of this study is to report the effect of Korean medicine treatment on diabetic retinopathy vitreous hemorrhage. Methods : A 66-year-old female who had diabetes for over 20 years showed vitreous hemorrhage of Grade 4 at initial visit. For about 2 months, acupuncture, moxibustion and herbal medicine treatment were performed. Prognosis was confirmed by funduscopic examination, automatic refractive examination and visual acuity test. Results : In the fundus photographs, we can observe the recovery of turbidity in Grade 4, which decreases to Grade 1, and the result of refraction in the right eye and visual acuity that was not measured when the turbidity is severe. Conclusions : This study was a case of a patient with diabetic retinopathy vitreous hemorrhage complaining of decreased visual acuity. Patients were given herbal medication, acupuncture and moxibustion. Through the funduscopic examination, the treatment effect on the absorption of vitreous hemorrhage was confirmed.

  • Hemifacial spasm is a facial nerve disorder characterized by episodic involuntary facial muscle contraction. In this study, the effect of Acupuncture and Electro- acupuncture therapy for two patient with hemifascial spasm were evaluated. Two patients with hemifacial spasm were treated with Acupuncture and Electro-acupuncture and evaluated with Scott's scale and Treatment satisfaction grade. In Case 1, the grade of spasm intensity classified by Scott's description improved from 3 to 1. In Case 2, the grade improved from 4 to 2. Patients were satisfied with the treatment in both cases. Acupuncture and Electro-acupuncture seem to be effective in the treatment of hemifacial spasm.

Diagnosis and Treatment of Acute Ankle Sprain (족관절 급성 염좌의 진단과 치료)

  • Acute sprain of the ankle requires comprehensive history taking and physical examination in diagnosing the type of severity and deciding on the plan of treatment. Literature supports functional treatment as the treatment of choice for grade I and II injuries. During the acute phase, the goal of treatment focuses on controlling pain and swelling. PRICE (protection, rest, ice, compression, and elevation) is a well-established protocol at this phase. There is some evidence that application of ice and use of nonsteroidal anti-inflammatory drugs improves healing and speeds recovery. Then the functional treatment (motion restoration and strengthening exercises) is administered to progress the rehabilitation appropriately in order to facilitate healing and restore the mechanical strength and proprioception. Early mobilization has been shown to result in more rapid return to work and daily activities than immobilization. Grade III injuries still generate controversy in terms of the best management available, and more studies on early mobilization, cast immobilization, or surgery are needed. Even the Cochrane reviews published to date are not conclusive.
