위축성 질염에 대한 한약 투약의 효과 : 체계적 문헌 고찰 (Application of Traditional East Asian Herbal Medicine for Atrophic Vaginitis: A Systematic Review of Randomized Controlled Trials)

  • Objectives: To evaluate the effectiveness and safety of application of Traditional East Asian Herbal Medicine (TEAM) in the treatment of Atrophic Vaginitis (AV). Methods: Randomized Controlled Trials (RCTs) were obtained from PubMed, Cochrane Library, Embase, CNKI, RISS, NDSL, and KISS. The risk of bias was assessed by using Cochrane's risk of bias tool, and RevMan 5.3 software was used. Results: 26 RCTs with 3,162 patients were identified and reviewed. Among them, 21 RCTs observe the effect of integrated traditional Chinese and Western medicine. 23 RCTs reported treatment groups was statistically effective than control groups in the study. Also, the recurrence rate was estimated in 10 RCTs and was lower than control groups. 12 studies observed adverse events (AEs) and severe AEs were not reported. Conclusions: This review suggested that TEAM was safe and effective in the treatment of AV. TEAM may also decrease the recurrence rate. However, this could not be proven conclusively. To ensure evidence-based clinical practice, well-designed trials with larger sample sizes are needed.

소아 구내염의 한약치료에 대한 임상연구 동향: 중의학 무작위대조군 임상시험을 중심으로 (A Review of Recent Clinical Studies of Herbal Medicine Treatment for Children's Stomatitis : Focusing on Randomized Controlled Trials in Traditional Chinese Medicine)

  • Objectives This study aims to find out the recent trend of the herbal medicine treatment in children's stomatitis in China, and propose better methods to treat children's stomatitis in Korea. Methods The randomized controlled trials (RCT) relevant to 'Herbal medicine treatment on children's stomatitis' were searched using China Academic Journal (CAJ). The search method we used included search formula, (SU = '口內炎' + '口腔炎' + 'stomatitis') and (SU = '儿童' + '小儿' + 'children' + 'child') from 2000 to September 2021. Results A total of 9 RCTs were selected and analyzed. All reports showed that oral administration of herbal medicine were effective and improved clinical symptoms. The most-used herbal medicine category was Heat-clearing medicine (淸熱藥). Conclusions The literatures from China have shown that herbal medicine improves children's stomatitis. These outcomes may be helpful for the treatment of children's stomatitis in Korea. However, further studies are needed to develop treatment guidelines for children's stomatitis.

만성 췌장염의 한약 치료에 대한 최근 중의 임상 연구 동향 고찰 (A Review of Recent Chinese Clinical Studies on Herbal Medicine Treatments for Chronic Pancreatitis)

  • Objectives: This study summarizes current trends and results in clinical studies on herbal medicine treatments for chronic pancreatitis. Methods: Studies published from 2014 to 2023 were searched for on the China National Knowledge Infrastructure (CNKI), PubMed, and KMbase using the keywords "chronic pancreatitis", "herbal medicine", "herb", "만성 췌장염", and "한약". The studies were analyzed according to year, study design, characteristics of the patients, and traditional Chinese medicine (TCM) intervention. Results: In total, 23 studies were selected. There were 21 randomized controlled trials and 2 case-control studies. The most common herbal prescriptions were Qingyi-tang (淸胰湯), Radix Bupleuri (柴胡), Radix Paeoniae Alba (白芍藥), Radix Glycyrrhizae (甘草), Radix Scutellariae (黃芩), and Rhei Rhizoma (大黃). In all the studies, herbal medicine treatments were effective in the treatment of chronic pancreatitis, and efficacy was assessed using clinical symptoms, physicochemical examination, radiological examination, and relapse. Conclusions: Herbal medicine treatment can be an effective treatment for chronic pancreatitis and has potential in clinical practice.

한약물 단독치료로 호전된 갑상선기능항진증 치험 1례 (Clinical Case Report of Hyperthyroidism Patient Treated with Only Traditional Korean Herbal Medicine)

  • This study was designed to evaluate the effects of traditional korean herbal medicine(Jaumkunbi-tang) therapy on a hyperthyroidism patient. Clinical data was analyzed on a patient with hyperthyroidism whose main symptoms were fatigue, palpitation, up flame, hyperhidrosis, hypersomnia and dyspepsia. The patient visited our hospital 9 times from November 10, 2011 and remianed until January 30, 2012. He was treated with Jaumkunbi-tang, the traditional korean herbal medicine. After treatment, improvement was seen in clinical symptoms(such as fatigue, palpitation, up flame, hyperhidrosis, hypersomnia and dyspepsia) and thyroid function test. The study suggests that Jaumkunbi-tang, one of traditional korean herbal medicines, could be effective on the treatment of hyperthyroidism.

한약제제 허가기준 개선방안 연구(I) (A Study on Amendment of Approval Specifications for Traditional Korean Medicines)

  • This study was conducted to improve the current regulation guidelines for developing traditional Korean medicines with effectiveness and productivity, in order to cope with the upcoming ICH on specifications on oriental herbal drugs. Also, major purposes of this study are to motivate R&D and to pioneer foreign markets for domestic herbal drug companies. First, after examining concepts of traditional Korean medicines and comparing the numbers and differentiation of herbal drugs registered on Pharmacopeia among Korea, China, and Japan, the current new drug development requirements for traditional Korean medicines were reviewed in detail, followed by comparison of foreign regulation systems including USA, EU, China, and Japan. Second, empirical cases on failure of development for new traditional Korean medicines under the current regulation system in the domestic companies including Dong-A, Kwangdong, and Samchondang, were collected and analyzed. As a result, hanbangsaengyak, the new category for traditional Korean medicines was newly developed on the basis of scientification of data between saengyak and hanyak, from the perspectives of harmonization between oriental medicine and western medicine and of balance between food and drug, in terms of industrialization, publicity, modernization, and effectiveness of administration. In addition, the new regulation requirements for the new hanbangsaengyak preparations were discussed by establishing principles of reinforcing preclinical test and of simplifying clinical trials in Korea. Finally, the further researches to articulate the complete specifications for pre-clinical and clinical requirements for traditional Korean medicines were strongly suggested.

신생아 황달의 예방적 치료에 대한 연구동향 (A Review of the Treatment to Prevent Neonatal Jaundice - Based on Traditional Chinese Medicine)

  • Introduction Jaundice is one of the most common conditions found in neonatal period. Phototherapy is one of the main treatments for neonatal jaundice. However, several adverse effects of the phototherapy have been reported, including DNA damage recently. Therefore, a variety of treatments have been conducted to shorten the duration of phototherapy. Meanwhile, it has been hardly tried to prevent neonatal jaundice, but diverse approaches have been tried in traditional Chinese medicine (TCM) for many years. Therefore, this study aims to analyze the studies for the treatments to prevent neonatal jaundice based on TCM. Materials and Methods Various literatures have been searched via CNKI, and PubMed using the terms "neonatal jaundice" (新生儿黄疸, 胎黃, 胎疸) in category of 'Traditional Chinese Medicine', 'Traditional Chinese Medicinal Herbs' and 'Combination of Traditional Chinese Medicine With Western Medicine'. The search range included randomized controlled trials (RCTs), controlled clinical trials (CCTs), case reports, reviews and animal experiments published from 2013 to 2015. Results A total of 104 studies were found. 93 articles were excluded by reviewing the titles and abstracts. Out of the remaining 11 studies, 9 articles were RCTs, 2 articles were CCTs. The treatment used in neonatal babies in the studies were, using herbal remedies (meditation 5, bath 3 and retention enema 1) and the acupressure. All treatments were initiated within the first 24-48 hours after their birth. Then, there was a study that the subjects taken the herbal medicine were women during pregnancy. The outcome assessments used were the serum total bilirubin (TB), transcutaneous bilirubin measurement (TCB), the incidence of the hyperbilirubinemia, the duration time of the jaundice. Especially, TB, TCB and the incidence of the hyperbilirubinemia have decreased significantly more than that of the control group in the most of the results. No severe adverse events were reported in all articles reviewed. Conclusions Conventional treatment such as, herbal medicine in TCM, seems to be the effective way to prevent neonatal jaundice or hyperbilirubinemia. Therefore, the conventional treatment may be favorable choice for preventive treatment for neonatal jaundice.

한약재내의 미량원소의 의의와 치료효과에 대한 고찰 (Therapeutical Effect with Trace Elements in Herbal Medicine)

  • Trace element are involved in enzymatic activities, immunological reactions. physiological mechanisms. Deficiency in some trace elements, such as iron and iodine. is still an important health problem, The role of trace elements deficiency is suspected in various clinical situations and is now confirmed by well designed supplementation studies. However, the importance of trace elements as chinese herbal constituents is not sufficiently appreciated by the oriental medical profession, although in recent years a significant increase of new finding on their essential character in chinese herbal medicine occurred. It is well known that herbal medicine contains a variety of trace elements which would show therapeutic effects with active components in herbal medicine . In china, recent work showed some positive correlation between trace element and traditional chinese medicine (TCM) in terms of therapeutic effects even if their role in therapeutic effects is still obscure. In korea, not much attention has been on the therapeutic importance of trace element contained in herbal medicine Here, the therapeutic effects of trace element in TCM were reviewed and summarized. 1. Iron, copper, zinc and manganese are mainly contained in TCM. In addtion, chromium, magnesium, molybdenum, nickel, alminium, cobalt, arsenic and selenium has been studied for their therapeutic effects 2. Zinc, is decreased in patients who have deficiency of kindney(腎虛) and chronic disease. Fe is decreased in patients who have deficiency of blood(血虛). However copper is increased in patients who have chronic disease and hepatic disease.3 Iron concentration is high in herbs used for tonifying and nourishing yin or blood(補陰補血藥) Zinc concentration is high also in herb used for tonifying kidiney and vital essence(補腎補精藥). In addition. copper concentration Is high in herb used for replenishing qi(補氣藥) 4 In herbal drugs, the therapeutic substances in TCM are not only organic but also inorganic. It seems that trace elements would be one of components in herb for its therapeutic effects. This indicates that therapeutic effects of TCM should be extended not only to herb itself, bur also to trace elements contained in herb.

골다공증의 한약 치료 효과: 국내 및 국외 동물 실험 및 임상 연구 논문에 대한 문헌 고찰 (Effect of Herbal Medicine on Osteoporosis: A Review of Animal Experiment, Clinical Studies Published in Korea and Overseas)

  • Objectives This study was designed to review the effect of herbal medicine on animal model and patients with osteoporosis. Methods The databases in Korea and overseas were searched with term as osteoporosis, Chinese medicine, Korean medicine, herbal medicine. Among the results, we reviewed studies published in 2011 to 2020. 420 articles were primarily selected and after processing through flowchart, finally 72 studies were selected. Results Above 72 studies, 71 studies showed that the herbal medicine was effective on treating osteoporosis, using macroscopical, osteological, histological, hematological analysis and urinalysis. Conclusions Using herbal medicine for treating osteoporosis is effective in various pathways. But further study should be conducted to clarify its treatment mechanisms and effectiveness.

Trends and Future Direction of the Clinical Decision Support System in Traditional Korean Medicine

  • Objectives: The Clinical Decision Support System (CDSS), which analyzes and uses electronic health records (EHR) for medical care, pursues patient-centered medical care. It is necessary to establish the CDSS in Korean medical services for objectification and standardization. For this purpose, analyses were performed on the points to be followed for CDSS implementation with a focus on herbal medicine prescription. Methods: To establish the CDSS in the prescription of Traditional Korean Medicine, the current prescription practices of Traditional Korean Medicine doctors were analyzed. We also analyzed whether the prescription support function of the electronic chart was implemented. A questionnaire survey was conducted querying Traditional Korean Medicine doctors working at Traditional Korean Medicine clinics and hospitals, to investigate their desired CDSS functions, and their perceived effects on herbal medicine prescription. The implementation of the CDSS among the audit software developers used by the Korean medical doctors was examined. Results: On average, 41.2% of Traditional Korean Medicine doctors working in Traditional Korean Medicine clinics manipulated 1 to 4 herbs, and 31.2% adjusted 4 to 7 herbs. On average, 52.5% of Traditional Korean Medicine doctors working in Traditional Korean Medicine hospitals adjusted 1 to 4 herbs, and 35.5% adjusted 4 to 7 herbs. Questioning the desired prescription support function in the electronic medical record system, the Traditional Korean Medicine doctors working at Korean medicine clinics desired information on 'medicine name, meridian entry, flavor of medicinals, nature of medicinals, efficacy,' 'herb combination information' and 'search engine by efficacy of prescription.' The doctors also desired compounding contraindications (eighteen antagonisms, nineteen incompatibilities) and other contraindicatory prescriptions, 'medicine information' and 'prescription analysis information through basic constitution analyses.' The implementation of prescription support function varied by clinics and hospitals. Conclusion: In order to implement and utilize the CDSS in a medical service, clinical information must be generated and managed in a standardized form. For this purpose, standardization of terminology, coding of prescriptions using a combination of herbal medicines, and unification such as the preparation method and the weights and measures should be integrated.

망막정맥폐쇄증의 한약치료 임상연구에 대한 문헌 고찰 - 중국논문을 중심으로 (Literature Review of Clinical Research Herbal Treatment on Retinal Vein Occlusion - Focusing on Traditional Chinese Medicine's Journals -)

  • The objective of this study was to investigate the diagnosis and treatment of Retinal vein occlusion such as syndrome differentiation and herbal medicine by reviewing Chinese traditional medicine`s journals. The journal search was carried out using China National Knowledge Infrastructure and Pubmed from January 2000 to December 2014. Searching key words were the various combination of "Retinal vein occlusion", "traditional chinese medicine", "syndrome differentiation". The final selection of 11 studies were selected and summarized by researchers. The effective rate was significantly higher than control but, methodological quality of the trials was generally low questioning their liability. The syndrome differentiation was classified as hyperactivity of liver yang and blood stasis. The most frequently prescribed herbal medication was Hyulbuchookeotang, Boyanghwanotang and Cheomagudeungum. This finding could be widely utilized in future clinical practice and form the basis for the development of clinical practice guidelines in advance.
