문명의 발달로 나날히 복잡해지는 사회 환경속에서 우리는 생명을 위협하는 수 많은 사고를 당하게 되며, 이럭 경우에 상기도를 유지하기 위한 기관절재술을 많이 시행하게 되는데, 때로는 기관협착등 이에 따른 여러가지 합병증을 유발하게 되는 경우가 있으며, 임상가들에게도 곤란한 문제를 갖어다 주고있다. 그러나 이러한 합병증은 수술방법의 개선과 항생제의 출현등으로 그 빈도가 많이 감소되기는 하였으나, 1969년 Lindholm은 술후성 기관협착증의 빈도를 1.5%내지 10%라고 했으며, 1969년 Donnelly, Mulder와 Rubush, 그리고 1971년 Andrew와 Pearson 등은 이들 원인의 대부분이 cuff 가 있는 기관삽입관이나, 기관케뉼(tracheal canula)의 부적합한 사용이나, 또는 감염에 의한 압박괴사(pressure necrosis)로 형성된다고 하였고, 협착부위로서 1972년 Bryce는 절개공(stoma)주위가 가장 많으며, 응급기관절개술시에 의사의 부정요법(mal-practice)도 원인이 될 수 있다고도 주의한 바 있다. 기관협착증의 치료로서 협착이 경미한 경우에는 일차적으로 세심한 관찰을 실시하면서 실리콘관(silicon tube)이나 스텐트(stent)를 사용하거나, 비강을 통한 삽관법(naso tracheal intubation)으로 기계적인 확장을 시도하며 (Schmiegelow, 1929, Montgomery, 1965), 육아조직이 형성되었을 경우에는 기관경검사하에서 이를 제거한 후에 steroid를 병용하는 편이 좋은데(Birck, 1970) 그밖에도 기관개찰술(Fenestration method, 백·홍 1974)이나 재수술(Revision)을 하기도 한다. 이러한 방법으로서도 치료가 불가능한 경우에는 그 협착 부분을 절제한 후에 단단문합술(End-to-End Anasto-mosis)을 시행하는 수도 있다. 저자들은 1967년 10월부터 1977년 3월까지 10연년간 세브란스병원에서 기관절개술을 받았던 1514례를 대상으로 일련의 조사를 실시하여 이들 중에서 기관협착증을 유발한 23례를 치료하였으며, 여기서 몇가지 지견을 얻었기에 보고하는 바이다.
선천성 기관 협착증은 아주 드문 질병이지만 영아나 유소아에 있어서 치명적인 질병이다. 하부기관을 침습한 선천성 기관 협착 환아 2명에게 Peter Goldstraw가 고안한 슬라이드 기법을 이용하여 기관 성형술을 시행하였다. 기관 성형술은 기관 협착부위 중앙을 절단한 후 협착 부위의 상하를 종 절개하여 서로 문합했다. 증례 1은 현재 건강한 상태이다. 증례 2는 술후 4일째 문합부위 파열로 사망하였다.
악성 종양에 의한 기관침윤이나 양성 기관협착이 기관의 하부나 전면에 존재할 경우에는 통상의 기관절개술이나 기관 내 삽관이 곤란한 경우가 있으며, 이러한 상황에서는 기관의 측면이나 하부를 통한 기관절개술이 불가피할 것이다. 저자들은 2001년 5월부터 2005년 5월까지 기관의 하부 혹은 전면의 병변으로 유발된 기관협착증 환자 6예에서 기관의 측면이나 하부를 절개하는 변형된 기관절개술을 시행하여 만족스러운 결과를 얻었기에 보고하는 바이다.
A 37 year old male patient was suffered from severe labored breathing caused by post tracheostomy stenosis, which was localized at the mediastinal trachea [cuffed tracheal stenosis] and ranged 1.5 cm in length and approximately 3 ram. in diameter on tracheogram. After dilation of tracheal stenosis with dilator, endotracheal intubation was tried for induction of anesthesia and control of respiration during operation. A tube was placed just beyond the tracheal stenosis without respiratory difficulty. Under the endotracheal anesthesia, circumferential resection of the mediastinal trachea containing the stenosis, approximately 2 cm in length [4 tracheal rings}, was carried out and primary direct end to end anastomosis was performed with interrupted submucosal sutures [3-0 Dexon] and mobilization of trachea Postoperative tracheostomy was not performed. The patient was completely relieved from dyspnea immediately after operation. Post-operative convalescence was entirely uneventful and at present, about 3 months after operation, he is now conducting a usual life. From the literature and our experience, the etiology and treatment of post-tracheostomy stenosis were discussed.
We report on the application of a venovenous type extracorporeal membrane oxygenator (ECMO) in high-risk tracheal procedures in six cases consisting of five patients with tracheal stenosis. An ECMO should be helpful for preventing respiratory emergency during a tracheal procedure.
Objectives: Endoscopic airway dilatation and stenting has been developed to treat the airway stenosis without potential morbidities of open surgery. We report the clinical results of endoscopic airway dilatation with silicone stenting in patients with posttuberculous bronchial stenosis(PTBS) and with severe main tracheal stenosis who have poor general conditions Methods : A prospective observation study of five patients, who have undergone endoscopic airway dilatation and silicone stenting between Feb 2007 and Feb 2009. A total of twelve patients were treated with endoscopic airway dilatation, among them 5 patients were included in this study. three patients were treated with newly designed silicone stent (Natural stent: TNO Co., Seoul, South Korea) because of poor surgical conditions and longer stenotic segment Results: 3 patients were grade III PTBS, and the other 2 patients were grade IV post tracheotomy main tracheal stenosis. One patient of PTBS were treated with silicone stent following endoscopic dilatation because of longer stenotic segment. Two patients of main tracheal stenosis patients were treated with silicone stent because of tracheal lumen collapse. There was no severe postoperative complications except mild granulation tissue formations Conclusions : Endoscopic dilatation including silicone stenting could be a useful method for treating patients with PTBS, and for main tracheal stenosis patients with poor general surgical conditions
A total of 55 patients underwent surgical managements for postintubation tracheal stenosis from July 1975 through March 1997. All but 8 had received ventilatory assistance. The patients had S cuff lesions, 17 stoma lesions, 7 at both levels, 5 at subglottic lesions. Thirty two patients underwent the sleeve tracheal resection and end-to-end anastomosis. Five patients performed a wedge resection and end-to-end anastomosis. Twenty two patients received the Montgomery T-tube for relief of airway obstruction. Simple excision of granulation tissue was done in 7 patients. Rethi procedures(anterior division of cricoid cartilage, partial wedge resection of lower thyroid cartilage and T-tube molding) were performed in 2 subglottic stenosis patients. And the other subglottic patient was received permanent tracheal fenestration at 1975. The tracheoesophageal fistula patient was done sleeve tracheal resection and end-to-end anastomosis with interrupted double layer closure of esophageal fistula site. Cervical approach was used in 49 cases, cervicomediastinal in 13 cases and median stemotomy In 6 cases. Techniques for obtaining tension-free anastomosis included a cervical neck flexion(15-30$^{\circ}$) in all sleeve resection patients and laryngeal release in one. The length of resection was 1.5 to 5.0 on A total of 41 patients(74.5%) had good(24 patients) or satisfactory(17 patients) results. But in ten cases, the restenosis of anastomosis site which is the most common complication was developed Two of them underwent a second reconstruction and 8 patients required T-tube insertion for airway maintenance. Three patients(5.4%) died. The causes of death were tracheo-innominate artery fistula(2) and sudden obstruction of airway(1).
Background: Tracheal resection and end-to-end anastomosis are the preferred treatment for tracheal tumor or tracheal stenosis. Absorbable suture material has been commonly used in case of tracheal anastomosis. But anastomosis technique is controversial. This experimental study compared between interrupted suture and continuous suture using Polydioxanone (PDS) absorbable suture material after tracheal resection and end-to-end anastomosis in rabbits. Methods : Twenty-four rabbits were used, divided into two groups of 12 animals each. The trachea was resected and then anastomosed with continuous 6-0 polydioxanone (PDS) (group 1), and intermittent 6-0 polydioxanone (PDS) (group 2). The animals were followed up for 6 months. Stenosis of the trachea was assessed at the time of death. Results : In two groups, anteroposterior diameter, transverse diameter, cross sectional area, and perimeter were greater in group 2 than in group 1 but, not significantly different. In addition, histologic findings were not different in two groups. Conclusions : This study suggest that stenosis after tracheal resection and end-to-end anastomosis using absorbable polydioxanone (PDS) suture material be not significantly different in both groups.
Tracheal neurilemoma, an extremely rare benign tracheal tumor that there has been only one case reported in 1996 throughout the nation, is a slowly progressing disease that obliterates the upper airway, delays diagnosis for its symptom similarity to asthma, and makes intubation for operation difficult. Bronchoscopic is therefore needed for diagnosis. There are two options for the treatment methods, a bronchoscopic resection or open surgical resection; however if intubation is difficult, then the bronchoscopic resection is used first to keep the airway open for the surgical resection. In this case, the severe tracheal stenosis impeding intubation made the surgical resection of the primary tracheal neurilemoma with extratracheal mass impossible; therefore, bronchoscopic laser resection was applied first to optain the airway passage for endotracheal intubation, followed by a successful open surgical resection.
