  • For the fault tree analysis (FTA) analysis of the packaged hydrogen filling station, the composition of the charging station was analyzed and the fault tree (FT) diagram was prepared. FT diagrams were created by dividing the causes of events into external factors and internal factors with the hydrogen event as the top event. The external factors include the effects of major disasters caused by natural disasters and external factors as OR gates. Internal factors are divided into tube tailer, compressor & storage tank, and dispenser, which are composed of mistakes in operation process and causes of accidents caused by parts leakage. In this study, the purpose was to improve the hydrogen station. The subjects of this study were domestic packaged hydrogen stations and FTA study was conducted based on the previous studies, failure mode & effect analysis (FMEA) and hazard & operability study (HAZOP). Top event as a hydrogen leaking event and constructed the flow of events based on the previous study. Refer to "Off shore and onshore reliability data 6th edition", "European Industry Reliability Data Bank", technique for human error rate prediction (THERP) for reliability data. We hope that this study will help to improve the safety and activation of the hydrogen station.

  • The British railway safety research group has developed a risk assessment model for the railway infrastructure and major railway accidents. The major hazardous factors of the railway infrastructure were identified and classified in the model. The frequency rates of critical top events were predicted by the fault tree analysis method using failure data of the railway system components and ratings of railway maintenance experts, The consequences of critical top events were predicted by the event tree analysis method. They classified the Joss of accident due to railway system into personal. commercial and environmental damages. They also classified 110 hazardous event due to railway system into three categories. train accident. movement accident and non-movement accident. The risk assessment model of the British railway system has been designed to take full account of both the high frequency low consequence type events (events occurring routinely for which there is significant quantity of recorded data) and the low frequency high consequence events (events occurring rarely for which there is little recorded data). The results for each hazardous event were presented in terms of the frequency of occurrence (number of events/year) and the risk (number of equivalent fatalities per year).

  • A HAZID is a brainstorming workshop to identify hazards in an early phase of a project. It should be flexible to capture all probable accidents allowing experienced participants to exploit their expertise and experiences. A bowtie analysis is a graphical representation of major accident hazards elaborating safety measures i.e. barriers. The result of these workshops should be documented in an organized manner to share as good as possible details of the discussion through the lifetime of the project. Currently results are documented using a three-step representation of an accident; causes, top event and consequences, which cannot capture correctly sequence of events leading to various accidents and roles of barrier between two events. Another problem is that barriers would be shown repeatedly leading to a misunderstanding that there are an enough number of safety measures. A new bowtie analysis method is proposed to describe an accident in multiple steps showing relations among causes or consequences. With causes and consequences shown in a format of a tree, the frequencies of having the top event (Fault tree analysis) and various consequences (Event tree analysis) are evaluated automatically based on the frequency of initiating causes and the probabilities of failure of barriers. It will provide a good description of the accident scenario and help the risk to be assessed transparently.

  • DNA topoisomerases catalyze changes in DNA topology through cycles of transient DNA strand breakage and religation. During this process. the active site tyrosine in human DNA topoisomerase Ⅰ(Top Ⅰ) becomes covalently linked to the 3'-ends of a single-stranded nick in the DNA duplex, Stabilization of the Top Ⅰ-DNA cleavable complex is the common initial event leading to the cytotoxicity of top 1 inhibitors. • Probabilistic safety assessment is widely used to quantify the risks of nuclear power plants and their uncertainties. When the lognormal distribution describes the uncertainties of basic events, the uncertainty of the top event in a fault tree is approximated with the sum of lognormal random variables after minimal cutsets are obtained, and rare-event approximation is applied. As handling complicated analytic expressions for the sum of lognormal random variables is challenging, several approximation methods, especially Monte Carlo simulation, are widely used in practice for uncertainty analysis. In this study, a theoretical approach for analyzing the sum of lognormal random variables using an efficient numerical integration method is proposed for uncertainty analysis in probability safety assessments. The change of variables from correlated random variables with a complicated region of integration to independent random variables with a unit hypercube region of integration is applied to obtain an efficient numerical integration. The theoretical advantages of the proposed method over other approximation methods are shown through a benchmark problem. The proposed method provides an accurate and efficient approach to calculate the uncertainty of the top event in probabilistic safety assessment when the uncertainties of basic events are described with lognormal random variables.

  • This paper is investigated of hardness and adhesiveness of plasma sprayed coating steels by AE(Acoustic Emission) testing when loading a tensile. AE Parameters used are Event, Count, Energy and Amplitude. Test specimens are carbon steel(S45C) with sprayed coating layers of Ni-4.5wt.%Al(bond coating) and $TiO_2$(top coating), and carry out heat treatment at $800^{\circ}C\;and\;1000^{\circ}C$, respectively. The micro-hardness of the heat treatment specimen have been improved more than that of non-heat treatment. On the tensile test, the process and occurence of the exfoliation of the sprayed coating layer can be estimated by AE Characteristics of AE parameters, such as event, count, amplitude and energy, on the layer exfoliation are shown the similar aspects. The exfoliation of bond coating occure at about 20% of strain and top coating is about 5% of strain.

  • This paper describes the application of fault tree technique to analyze of compressor failure. Fault tree analysis technique involves the decomposition of a system into the specific form of fault tree where certain basic events lead to a specified top event which signifies the total failure of the system. In this research. fault trees for failure analysis of screw type air compressor are made. This fault trees are used to obtain minimal cut sets from system failure and system failure rate for the top event occurrence can be calculated. It is Possible to estimate air compressor reliability by using constructed fault trees through compressor failure example. It is Proved that FTA is efficient to investigate the compressor failure modes and diagnose system.

  • In conventional probability-based human reliability analysis, the basic human error rates are modified by experts to consider the influences of many factors that affect human reliability. However, these influences are not easily represented quantitatively, because the relation between human reliability and each of these factors in not clear. In this paper, the relation is expressed quantitatively. Furthermore, human reliability is represented by error possibilities proposed by Onisawa, which is a fuzzy set on the interval [0,1]. Fuzzy reasoning is used in this method in order to obtain error possibilities. And, it is supposed that many basic events affected by the above factors are connected to the top event through Fault Tree structure, and an estimate of the top event expressed by a member- ship function is obtained by using the fuzzy measure and fuzzy integral. Finally, a numerical example of human reliability analysis obtained by this method is given.
