Minimizing the extra-oral time in autogeneous tooth transplantation: use of computer-aided rapid prototyping (CARP) as a duplicate model tooth

  • Objectives: The maintenance of the healthy periodontal ligament cells of the root surface of donor tooth and intimate surface contact between the donor tooth and the recipient bone are the key factors for successful tooth transplantation. In order to achieve these purposes, a duplicated donor tooth model can be utilized to reduce the extra-oral time using the computer-aided rapid prototyping (CARP) technique. Materials and Methods: Briefly, a three-dimensional digital imaging and communication in medicine (DICOM) image with the real dimensions of the donor tooth was obtained from a computed tomography (CT), and a life-sized resin tooth model was fabricated. Dimensional errors between real tooth, 3D CT image model and CARP model were calculated. And extra-oral time was recorded during the autotransplantation of the teeth. Results: The average extra-oral time was 7 min 25 sec with the range of immediate to 25 min in cases which extra-oral root canal treatments were not performed while it was 9 min 15 sec when extra-oral root canal treatments were performed. The average radiographic distance between the root surface and the alveolar bone was 1.17 mm and 1.35 mm at mesial cervix and apex; they were 0.98 mm and 1.26 mm at the distal cervix and apex. When the dimensional errors between real tooth, 3D CT image model and CARP model were measured in cadavers, the average of absolute error was 0.291 mm between real teeth and CARP model. Conclusions: These data indicate that CARP may be of value in minimizing the extra-oral time and the gap between the donor tooth and the recipient alveolar bone in tooth transplantation.

흰쥐의 악골에 동종 이식한 태아 치아싹의 장기간 발육 (THE LONG-TERM GROWTH OF HOMOGENEOUS EMBRYO TOOTH GERM TRANSPLANTED INTO THE MAXILLA OF A RAT)

  • 치아우식증이나 치주질환으로 치아를 상실한 경우 여러 가지 방법으로 수복 치료할 수 있으나 그 중에서 임플란트와 치아 이식에 대한 관심이 높아지고 있다. 최근에는 치아 이식에 대한 성공률을 높이기 위해서 치아를 형성시키고 발육시키는 과정에 관한 많은 연구가 이루어지고 있다. 치아싹 이식은 생체 내와 생체 외에서 연구되고 있고, 생체 내 이식은 쥐와 생쥐, 고양이, 개 등 여러 동물에서 시행되고 있다. 이러한 생체 내 이식은 대부분 구강 외에서 시행되고 있으며 구강 내 이식에 대한 연구는 드물다. 본 연구는 악골 내에 이식한 치아싹이 발육되고 석회화되는지 관찰하기 위하여 성숙한 흰쥐의 상악 제 1 구치를 발치하고 그 발치와에 임신 13.5일 된 태아쥐에서 모상기의 하악 제 1 구치의 치아싹을 이식하고 2, 6 개월 후 희생하여 방사선학적 그리고 조직학적으로 관찰하여 다음과 같은 결과를 얻었다. 12 개월과 6개월 동안 악골 내에 이식한 치아싹은 석회화된 치아 조직이 형성 되었고 치아 조직에서 상아질과 백악질, 치수 조직이 관찰되었으며 법랑질 공간 주변은 상피로 둘러싸여 있었다. 2. 6개월 동안 구강 상피 하방에 위치한 치아 조직은 상피로 둘러싸여 있었고 주위에 치주인대 및 결합조직이 관찰되었다. 3. 이식한 시간이 경과함에 따라 치아 형성이 진전되었으나 치아 조직은 크기가 작았고 형태학적으로 완전하지 못하였다.

Autotransplantation combined with orthodontic treatment: a case involving the maxillary central incisors with root resorption after traumatic injury

  • Traumatic dental injury can result in avulsion of anterior teeth. In young patients, it is a challenge to the dental professional because after replantation, late complications such as ankylosis require tooth extraction. Although prosthetic and orthodontic treatment, and implant placement have been described as the options for intervention, autogenous tooth transplantation could be an effective procedure in growing patients if there is a suitable donor tooth available. This case presents the treatment of a patient who suffered a traumatic injury at 9 years old with avulsion of tooth 21, which had been replanted, and intrusion of tooth 11. Both teeth ankylosed; thus they were removed and autotransplantation of premolars was carried out. After transplantation, the tooth underwent root canal treatment because of pulpal necrosis. Orthodontic treatment began 3 months after transplantation and during 7 years' follow-up the aesthetics and function were maintained without signs of resorption.

  • Autotransplantation is a viable option for the treatment of a missing tooth or for replacement of avulsed and traumatized tooth when there is a donor tooth available. The maintenance of healthy periodontal ligament cells in the donor tooth is one of the most important factors for successful tooth transplantation. The purpose of this study was to evaluate the prognosis and the cause of failure in 182 auto-transplanted teeth. A total of 182 cases which received the auto-transplantation in Department of Conservative dentistry, Yonsei Dental Hospital were included in this study.(omitted)

  • The autogenous tooth transplantation is an alternative treatment replacing a missing tooth when a suitable donor tooth is available. It is also a successful treatment option to save significant amount of time and cost comparing implants or conventional prosthetics. These cases, which required single tooth extraction due to deep caries and severe periodontal disease, could have good results by transplanting non-functional but sound donor tooth to the extraction site.

치근단 완성된 치아의 자가이식 (Autogenous transplantation of tooth with complete root formation)

  • Purpose: Autogenous transplantation of teeth can be defined as transplantation of teeth from one site to another in the same individual, involving transfer of impacted or erupted teeth into extraction sites or surgically prepared sockets". Successful autogenous transplantation of teeth depends upon a complex variety of factors. Such factors include damage to the periodontal ligament of the donor tooth, residual bone height of the recipient site, extra-oral time of tooth during surgery. Schwartz and Andreasen previously reported that autogenous transplantation of teeth with incomplete root formation demonstrated higher success rate than that of teeth with complete root formation. Gault and Mejare yielded similar rate of successful autogenous transplantation both in teeth with complete root formation and in teeth with incomplete root formation when appropriate cases were selected. This case report was aimed at the clinical and radiographic view in autogenous transplantation of teeth with complete root formation. Materials and Methods: Patients who presented to the department of periodontics, Chonnam National University Hospital underwent autogenous transplantation of teeth. One patient had vertical root fracture in a upper right second molar and upper left third molar was transplanted. And another patient who needed orthodontic treatment had residual root due to caries on upper right first premolar. Upper right premolar was extracted and lower right second premolar was transplanted. Six months later, orthodontic force was applied. Results: 7 months or 11/2 year later, each patient had clinically shallow pocket depth and normal tooth mobility. Root resorption and bone loss were not observed in radiograph and function was maintained successfully. Conclusion: Autogenous transplantation is considered as a predictive procedure when it is performed for the appropriate indication and when maintenance is achieved through regular radiographic taking and follow-up.

  • The purpose of this paper is to discuss the indication. treatment procedure. prognosos and complication of autogenous transplantation in treating impacted teeth of orthodontic patient. Autogenous transplantation is indicated, in cases of rejecting orthodontic treatment due to the visible orthodontic appliance, the relatively long treatment time, unfavorable tooth position for orthodontic repositioning, unrestorable advanced detal caries. advanced periodontitis and ankylosed tooth. Most process related to the decision of the prognosis is dependent on the careful surgical technique. In comparison to other orthodontic and surgical procedure, the application of the autotransplantation is limited, although its success rate is markedly increased today. Therefore we must we must pay attention to the treatment planning and cooperation with other specialties is needed.

노인 환자에 있어서 치아 재식술 및 이식술의 임상적 고찰 (Clinical evaluation of tooth replantation and transplantation of old patients)

  • The purpose of this article is to discuss the effect of aging on the treatment of replantation and transplantation of teeth in old patients. I case of replantation and 2 cases of transplantations in old patients are reported in this article. Patients aged 65 and over are included. They had several problems such as periapical lesion, crack or foot fractures. In those cases, the replantation and transplantation were treatment of choice. White ProRoot MTA was used in apical retrograde filling and perforation repair. After replantation and transplantation of teeth, follow-up visits showed signs of healing in 3 cases. Considerations and indications for replantation and transplantation are not much affected by age. Old patients are aware that replantation and transplantation can be performed comfortably and that age is not a factor in prediction success.

매복 하악 견치의 자가이식술을 이용한 치험례 (Autotransplantation of impacted mandibular canine)

  • Autotransplantation is the transplantation of embeded, impacted, or unerupted tooth, into extraction socket or surgically prepared socket in the same individual. Clinically, successful autotransplantation must show radiolucent space(periodontal ligament space) between transplanted tooth and supporting bone, lamina dura, no root resorption, no ankylosis, no inflammatory change, and physiologic tooth mobility. It is important that procedure is atraumatic, and the instruments should not contact the root surface during procedure. We performed autotransplantation of impacted mandibular canine that transversely located beneath the apices of the mandibular incisors with uncompletely developed apex. In radiographs and clinical evaluation, this transplant showed successful clinical finding except irregularity of mesial root surface after 14 months. It is conclued that transplantation of canine with $\frac{1}{2}{\sim}\frac{3}{4}$ root development provides a good chance of pulp survival, limited risk of root resorption and ensures sufficient final length, and is thus recommended.

치근단 미완성 치아의 자가치아이식 (AUTOTRANSPLANTATION OF TOOTH WITH IMMATURE ROOT FORMATION)

  • 자가치아이식이란 자신의 치아를 구강 내의 한 위치에서 다른 발치와나 외과적으로 형성된 치조와로 이동시키는 술식이다. 통상적으로 이 술식은 상실되거나 보존하기 힘든 제1대구치를 다른 치아로 대체하거나, 매복된 견치를 악궁의 정상 위치로 이동시키거나, 전치부와 같이 심미적으로 중요한 부위의 상실된 치아를 소구치로 대체하는 경우에 많이 사용된다. 자가치아이식을 성공적으로 수행하기 위해서는 세심한 수술 기법과 더불어 적절한 증례 선택이 중요한데, 이식할 치아는 치근 발육이 1/2~3/4 정도 이루어진 미성숙 치아가 유리하고 근단공이 넓을수록 치수 재혈관화의 가능성도 높아져 이후의 근관 치료의 필요성도 줄일 수 있다. 본 증례들은 소아에서 치근단 미완성 치아의 자가치아이식을 통해 임상적, 방사선학적으로 양호한 치유 결과를 얻었고, 별도의 근관 치료 없이 양호한 치수 치유 결과를 보였기에 보고하는 바이다.
