This study describes the clinical steps taken in the treatment of a patient who had an avulsed right upper central incisor that presented with incomplete root development and chronic apical periodontitis. A 7-year-old boy was referred from a private dentist to a dental office specializing in endodontics. The tooth had remained in a dry environment for 20 minutes, and tooth replantation was performed at an emergency appointment. After clinical and radiographic examinations, root canal decontamination was performed, followed by several changes in intracanal calcium hydroxide medication. Blood clot formation was attempted, but bleeding within the root canal was insufficient; therefore, we opted for an intracanal medication change to stimulate mineralized tissue formation in the apical region. Root obturation was performed 45 days after the last change of intracanal medication, and clinical, radiographic, and tomographic follow-up examinations were performed at 3, 6, 18, and 40 months after the endodontic intervention. The increase in thickness and length of the root structure and the absence of root resorption were verified through follow-up examinations. Therefore, it was concluded that the procedures used were successful for tooth replantation.
We analyzed 121 age estimation clients who had visited oral medicine clinic, Chonbuk National University Hospital, from January 2000 to December 2007 to evaluate its characters in Jeonllabuk-do. The obtained results were as follows : 1. There was distinct difference in sex distribution(Male : 58.7%, Female : 41.3%) and more than half percent(55.4%) was over 50's. 2. The difference between registered and alleged age was the most in more than 5 years, and most of them were over 50's. 3. There were more clients who wanted to increase their age than to decrease and was no prominent difference in sex. Age estimation clients who wanted to decrease their age were predominant under 10 years old and in 20's, and those wanted to increase were predominant in 10's and over 30's. 4. The most reasons to correct age were related to welfare benefit. The purpose of age estimation was different according to each age groups; welfare benefit was the most over 50's, occupation in 40's and 50's, friendship in 30's and 40's, sibling-related in 50's, employment in 10's and 20's. 5. Age was estimated by the attrition of permanent tooth and pulp/tooth ratio from 20 years, root apex closure for 10's and calcification of permanent tooth under 10 years old. In cases that were difficult to estimate by use of the attrition of permanent tooth and pulp/tooth ratio, age was estimated by missing time of permanent tooth and the change of mandibular angle with age. 6. The estimated ages were close to alleged age in 77.7% of clients but the rest(22.3%) was close to registered age.
Objective: To compare the positions of the mandibular premolars in Angle Class I subjects according to vertical facial type. The results will provide a theoretical basis for predicting effective tooth movement in orthodontic treatment. Methods: Cephalometric parameters were determined using cone-beam computed tomography in 120 Angle Class I subjects. Subjects were categorized as short, normal, and long face types according to the Frankfort mandibular angle. Parameters indicating the position of the mandibular right premolars and the mandible were also measured. Results: The angle between the mandibular first premolar axis and buccal cortex, the distance between the root apex and buccal cortex, angle of vestibularization, arc of vestibularization, and root apex maximum movable distance were significantly greater in the short face type than in the long and norm face types. The angle between the mandibular second premolar axis and buccal cortex, the distance from root apex to buccal cortex, and the arc of vestibularization were significantly greater in the short face type than in the normal face type. Conclusions: There are significant differences in the mandibular premolar positions in Class I subjects according to vertical facial type.
The purpose of this study was to compare the effects of preparation with GT files and profiles .04 in shaping of root canals and reconstruct the three-dimensional root canal system using micro computed tomography 40 canals of the extracted human mandibular molars were used, and randomly distributed into two experimental groups. In group 1. canals were prepared by GT files. In group 2, Profiles .04. were used. Apical preparation size was #30. For each tooth pre and post operative cross-sectional images were obtained by the micro CT at 50 micron intervals. Pre and post operative cross-sectional images of 1, 2, 3, 5, and 8mm from the apex were compared. For each section. canal area and centering ratio were determined. For each tooth pre- and post-operative root canal volume from the furcation to the apex of the roots was calculated by three-dimensional image software. Following results were obtained: 1. At 8mm from the apex, area of dentin removed by GT rotary file was significantly larger than that by Profile .04. And at the other levels there was not a significant difference. 2. There was a trend for GT rotary file to remain more centered in the canals than Profile .04 at all levels. But at 3mm level. there was a statistically significant difference. 3. In root canal volume increments after instrumentation, there was no significant difference between two groups.

Autotransplantation is the transplantation of embeded, impacted, or unerupted tooth, into extraction socket or surgically prepared socket in the same individual. Clinically, successful autotransplantation must show radiolucent space(periodontal ligament space) between transplanted tooth and supporting bone, lamina dura, no root resorption, no ankylosis, no inflammatory change, and physiologic tooth mobility. It is important that procedure is atraumatic, and the instruments should not contact the root surface during procedure. We performed autotransplantation of impacted mandibular canine that transversely located beneath the apices of the mandibular incisors with uncompletely developed apex. In radiographs and clinical evaluation, this transplant showed successful clinical finding except irregularity of mesial root surface after 14 months. It is conclued that transplantation of canine with $\frac{1}{2}{\sim}\frac{3}{4}$ root development provides a good chance of pulp survival, limited risk of root resorption and ensures sufficient final length, and is thus recommended.
In the infected immature tooth with periapical involvement, the pulp is considered to hardly exist in the canal and periapical area. Such a tooth receives apexification procedure, because revascularization of the pulp chamber is in principle not expected. Apexification is beneficial to induce further development of an apex to close the foramina, but does not promote the thickness of the entire canal wall dentin. It may be possible for the pulp to be only partially necrotic and infected when an extremely large communication from the pulp space to the periapical tissues exists with a very young tooth. If this were the case, vital pulp in the apical part of the canal could proliferate new pulp into the coronal pulp space by the successful removal and disinfection of the necrotic infected coronal pulp.(omitted)

Impaction of permanent incisor occurs rare than the canine & third molar. But it's often observed in school age child. The causes of impaction are trauma, space deficiency, mesiodens, infections of root apex, etc. In spite of elimination of cause, normal eruption of impacted tooth is rare. Though eruption is normal, the position of tooth will be incorrect. Because the impacted tooth results in malocclusion, root resorption of adjacent tooth, pathologic cystic change, it should be confirmed the precise position by clinical and radiographic exam and found the correct location by appropriate treatment plan. In case of pathologic change of impacted tooth and injury to adjacent tooth, it will be extracted. But through orthodontic retraction, the function and esthetics of tooth can be restored. It is important that impacted tooth should be detected early and diagnosed correctly, and appropriate treatment plan should be made. Before impacted tooth is retracted, the considerations of space for alignment and anchorage should be preceded and through appropriate force and mechanics, the side effects, for example, a root resorption should be minimized. In this study, we guided impacted tooth to normal position by using a forced eruption.
Purpose : The purpose of this study was to examine the patient- and treatment-related etiologic factors of external root resorption. Materials and Methods : This study consisted of 163 patients who had completed orthodontic treatments and taken the pre- and post-treatment panoramic and lateral cephalometric radiographs. The length of tooth was measured from the tooth apex to the incisal edge or cusp tip on the panoramic radiograph. Overbite and overjet were measured from the pre- and post-treatment lateral cephalometric radiographs. The root resorption of each tooth and the factors of malocclusion were analyzed with an analysis of variance. A paired t test was performed to compare the mean amount of root resorption between male and female, between extraction and non-extraction cases, and between surgery and non-surgery groups. Correlation coefficients were measured to assess the relationship between the amount of root resorption and the age in which the orthodontic treatment started, the degree of changes in overbite and overjet, and the duration of treatment. Results : Maxillary central incisor was the most resorbed tooth, followed by the maxillary lateral incisor, the mandibular central incisor, and the mandibular lateral incisor. The history of tooth extraction was significantly associated with the root resorption. The duration of orthodontic treatment was positively correlated with the amount of root resorption. Conclusion : These findings show that orthodontic treatment should be carefully performed in patients who need the treatment for a long period and with a pre-treatment extraction of teeth.
The endodontically treated tooth is generally restored with post and core, owing to the brittleness and the loss of large amount of tooth structure. Although there have been lots of studies about the endodontically treated teeth, the three-dimensional quantitative studies about the stress distribution of them are in rare cases. In this study, it was assumed that the coronal portion of the upper incisor had extensively damaged. After the root canal therapy it was post cored, and restored with PFG crown. The three-dimensional model, in which the root was supported with a normal alveolar bone, was constructed. Force was applied to the centric stop point with the angle of 135 degrees to the long axis of the tooth. Force was assumed to be 250N as functional maximum bite force of upper central incisors. The results analyzed with three-dimensional finite element method were as follows : 1. Stress was concentrated on the middle portion of the labial side dentin and the apical portion of the dentin. 2. Stress in the post was more concentrated on the post apex. 3. The displacement of the post at the post-cement interface was almost symmetrical la-bio-lingually. 4. It assumed that restoring extensively damaged tooth with a post-core and PFG crown is an adequate method of restoration.

Dens evaginatus is a dental anomaly involving an extra cusp or tubercle that protrudes from the occlusal surface of the affected tooth. The prevalence of dens evaginatus is 1 to 4 percent, and dens evaginatus is observed most commonly in premolars. It can be worn or fractured easily, resulting in pulpal exposure, pulpal infection, loss of vitality, facial infection and osteomyelitis. Since the tooth frequently has the immature apex when the tubercle is fractured, there is difficulty in treatment. Although calcium hydroxide is widely used for pulp treatment of an immature permanent tooth, several alternatives have been suggested to reduce patient's appointments. Mineral trioxide aggregate is considered biocom-patible and has excellent marginal sealing ability. In addition, it can minimize patient's visits. In this case report, apexification with MTA was attempted on the immature premolars in patients with cellulitis patient, caused by pulp necrosis due to dens evaginatus. Favorable clinical and radiologic results were achieved. In one case, continued root formation was observed.
