구강작열감질환에 관한 고찰 및 의료분쟁 증례보고 (A Review of Burning Mouth Disorders)

  • Burning mouth disorders (sometimes referred to as burning mouth syndrome) are characterized by a burning sensation in the tongue or other oral sites, usually in the absence of clinical and laboratory findings. Affected patients often present with multiple oral complaints, including burning, dryness and taste alterations. Burning mouth complaints are reported more often in women, especially after menopause. Typically, patients awaken without pain, but report increasing symptoms through the day and into the evening. Conditions that have been reported in association with burning mouth syndrome include chronic anxiety or depression, various nutritional deficiencies, diabetes and changes in salivary function. However, these conditions have not been consistently linked with the syndrome, and their treatment has had little impact on burning mouth symptoms. Recent studies have pointed to dysfunction of several cranial nerves associated with taste sensation as a possible cause of burning mouth disorders. The most common central mechanism that likely explains burning mouth disorders is a centrally mediated continuous neuropathic pain. Given in low dosages, benzodiazepine, tricyclic antidepressants or anticonvulsants may be effective in patients with burning mouth disorders.

  • The main purpose of this thesis is the Korean version of Gwang On Yeok Ron(廣瘟疫論). Gwang On Yeok Ron is a specialized book about infectious diseases(瘟疫). The book was compiled by Dae Cheon jang(戴天章) in 1722 who was a medical man in the Qing dynasty. The book consists of four volumes and an appendix. "Gwang On Yeok Ron Vol. I(廣瘟疫論卷之一)" is for the demonstration of colds(傷寒) and infectious diseases through a patient's odor(氣), skin tone(色), tongue(舌), mental state(神) and pulse(脈). Also, it describes the complications(兼證, 夾證) of infectious diseases. "Gwang On Yeok Ron Vol. II(廣瘟疫論卷之二)" describes the external symptoms(表證) of infectious diseases and the remedies. "Gwang On Yeok Ron Vol. III(廣瘟疫論卷之三)" describes the internal symptoms(裏證) of infectious diseases and the remedies. "Gwang On Yeok Ron Vol. IV(廣瘟疫論卷之四)" presents principles for the remedy of infectious diseases, and describes the sequelae(遺證) after the recovery of infectious diseases. "Appendix of Gwang On Yeok Ron(廣瘟疫論方目)" presents 83 prescriptions for the remedy of infectious diseases.

  • 'Kangsim-San' is an added and subtracted prescription of 'Cheongsim-Yeonza-Tang' recorded in 'Dongeuy-Soose-Boweon' from which it has become one of the favorate prescriptions at the Oriental Medical Hospital, Kyung-Hee University. It is described in the book that the 'Cheongsim-Yeonza-Tang' can be effective to symptoms relating to fatigue, nocturnal emission, abdominal pain, tongue deviation and palsy, etc. However the 'Kangsim-San' has been used for neurotic syndrome, pulpitation, insomnia, constipation, dry-mouth, auorexia and arrythmias and so on, added to the above-mentioned symptoms for the 'Cheongsim-Yeonza-Tang' at the hospital. Nevertheless, the pharmacological research of fundamental basis is not completed so far, and we have attempted experiment on various animals to study the pharmacological effects of the medicine. The result was proved as follows; The prescription had a considerable effects on the sedation of central nervous system, antipyretic and analgesic action, and vasodilative action.

문진(聞診) 중 성음(聲音).언어(言語)에 대한 연구 (Study on Listening Diagnosis to Vocal Sound and Speech)

  • This study was written in order to help understanding of listening diagnosis to vocal sound and speech. The purpose of listening diagnosis is that we know states of essence(精), Qi(氣) and spirit(神). Vocal sound and speech are made by Qi and spirit. Vocal sound originates from the center of the abdominal region(丹田) and comes out through vocal organs, for example lung, larynx, nose, tongue, tooth, lip and so on. Speech is expressed by vocal sound and spirit. They are controled by the Five Vital organs(五臟). Various changes of vocal sound and speech observe the rules of yinyang. For example, if we consider patient likes to say or not, we can diagnose heat and coldness of illness. If we consider he speaks loudly or quietly, we can diagnose weak and severe of illness. If we consider he speaks clearly or thick, we can diagnose inside and outside of illness. If we consider he speaks damp or dry, we can diagnose yin and yang of illness. If we consider change of voice, we can diagnose new and old illness. Symptoms of changes of five voices, five sounds, dumbness and huskiness are due to abnormal vocal sound, and symptoms of changes of mad talk, mumble, sleep talking and so on are due to abnormal speech.

근위축성 측삭 경화증 환자 1례에 대한 임상증례 (Case of Amyotrophic Lateral Sclerosis(ALS) with Korean Medical Treatment Evaluated by K-ALSFRS-R and ALSSS)

  • Objective : This study is a report on treatment effect of Korean medical treatment to ALS(Amyotrophic Lateral Sclerosis) patient. The purpose of this case study is to improvement of Korean medical treatment and evaluate prognosis and clinical course of ALS patients. Methods : The patient was treated by acupuncture, Su-Gi therapy, herbal medication, physical treatment. To determine the effects of Korean medical treatment, we evaluated every four days used by Korean version of Amyotrophic Lateral Sclerosis Functional Rating Scale-Revised (KALSFRS-R), Amyotrophic Lateral Sclerosis Severity Scale(ALSSS). Results : Before mental contraction caused by everyday life nursing management, symptoms such as lumbar pain, getting hoarse, lower limb powerless & spasticity, upper limbs powerless, neckache, tongue-tied etc. were improved after above treatment. K-ALSFRS-R and ALSSS scores were also increased till patient's mental contraction Conclusions : The Korean medical treatment is effective on local symptoms of ALS, and there is also functional improvement of ALS diagnosed by external examination. It is necessary to have more examination about ALS.

Unilateral hypoglossal nerve palsy after mild COVID-19: a case report

  • Post-acute coronavirus disease (COVID-19) syndrome is defined as persistent symptoms or delayed complications after COVID-19. Several cases of cranial nerve invasion related to COVID-19 have been reported. However, to our knowledge, no cases of solitary unilateral hypoglossal nerve paralysis after mild COVID-19 without intubation have been reported to date. Herein, we report the case of a 64-year-old man with unilateral hypoglossal nerve palsy as a complication of COVID-19. He complained of dysarthria and tongue discomfort 2 weeks after COVID-19 onset. Brain and neck computed tomography, magnetic resonance imaging, ultrasonography, and blood tests ruled out other possible causes. The patient's nerve palsy was rapidly diagnosed and improved with early rehabilitation. Understanding of the pathology of COVID-19 is still limited. Physicians should focus on patients' symptoms and their relationship to COVID-19, and investigate complications immediately. This case highlights the importance of early detection and rehabilitation of post-acute COVID-19 syndrome.

성인의 생활스트레스가 구강증상에 미치는 영향 (Influence of Daily Stress to Oral Symptoms among Adults)

  • 생활스트레스는 점차 복잡하고 다양화되어 가는 현대사회에서 구강건강에 중요한 문제로 대두되고 있다. 본 연구는 2012년 7월 1일부터 10월 30일까지 460명의 성인을 대상으로 생활스트레스가 구강증상에 미치는 영향을 알아보고자 하였다. 연구결과는 다음과 같다. 1. 일반적인 특성에 따른 생활스트레스의 차이 결과 연령, 수입, 직종, 흡연유무에 유의한 차이를 나타냈다. 2. 일반적 특성에 따른 구강증상의 차이는 연령, 교육수준, 수입, 직종, 전신질환 음주와 흡연 습관에서 유의한 차이를 나타냈다. 3. 신체적, 행동상, 심리 감정상 스트레스의 수준에 따른 구강증상의 차이는 신체적, 심리 감정상 증후는 입술 볼, 혀, 치아 잇몸, 악관절 증상에서 유의한 차이를 나타냈으며, 행동상 증후는 입술 볼, 혀. 악관절 증상에서 유의한 차이를 나타냈다. 4. 생활스트레스가 구강증상에 미치는 영향은 신체적 증후 (p<0.01), 행동상 증후(p<0.05), 심리 감정상 증후(p<0.05)는 입술 볼, 혀 증상에 유의한 영향을 주는 것으로 나타났다. 신체적, 심리 감정상 증후는 치아 잇몸 증상에 유의한 영향을 주는 것으로 나타났으며, 심리 감정상(p<0.01), 신체적 징후(p<0.01)는 악관절 증상에 유의한 영향을 주는 것으로 나타났다. 이처럼 생활 스트레스는 입술 볼, 혀 및 구강점막에 더 많은 영향을 미치는 것으로 나타났다. 신체적, 심리 감정스트레스를 포함한 행동상 스트레스 증상까지 발생할 경우 다발성 구강질환이 더 많이 나타날 가능성을 본 결과를 통해 입증할 수 있었으며, 추후 스트레스와 구강점막 질환과의 관계를 좀 더 구체적으로 살펴 볼 필요가 있다.

A Case of Hypoglossal Neurilemmoma Resected Via Burr-hole Craniectomy

  • Hypoglossal neurilemmoma is extremely rare. Intracranial hypoglossal neurilemmoma has been reported to the present most commonly as a space-occupying lesion with symptoms of raised intracranial pressure. A 68-year-old women presented with deviation of the tongue to the left on protrusion. Preoperative radiological images revealed an extra-axial mass in and around the hypoglossal canal. The tumor was totally resected via retrosigmoid suboccipital approach with burrhole craniectomy. Histopathological examination verified a neurilemmoma. She had no neurologic abnormality except hypoglossal palsy which recovered completely in six months. Retrosigmoid suboccipital approach with burrhole craniectomy can be an useful approach in intracranial hypoglossal neurilemmoma without extracranial extension or with minimal extracranial extension into the hypoglossal canal.

Amoxicillin에 의한 비정상적 증상인 고혈압 (Unusual Symptom of Hypertension associated with Amoxicillin)

  • An 80-year-old Korean woman who previously experienced penicillin allergy presented to a dental hospital for treatment of a fractured tooth. A dentist extracted the tooth and prescribed her A-cillin (amoxicillin hydrate) 250 mg orally 3 times a day for 3 days and Carol-F® (ibuprofen arginine) orally 3 times a day for 3 days. She received the prescriptions, returned home, and took one dose of each drug. One hour later, she experienced hypertension, itching, shortness of breath, and was unable to move her tongue to pronounce words. Approximately 6 hours after drug administration, the symptoms persisted, and the patient was admitted to an emergency medical center (EMC). The patient was discharged from the EMC after a one-night stay in the hospital for symptomatic treatment.
