PURPOSE. The aim of this work is to evaluate different types of materials used for making implant abutments, by means of an in vitro study and a review of the literature, in order to identify the indications for a better choice of an implant-supported restoration in the anterior section. MATERIALS AND METHODS. 5 implant abutments were tested in a random order in the superior anterior maxilla of pig gingiva (n = 8): titanium dioxide (Nobel Biocare); zirconium dioxide, Standard BO shade (Nobel Biocare, Kloten, Switzerland); zirconium dioxide, Light BI shade (Nobel Biocare); zirconium dioxide, Intense A 3.5 shade (Nobel Biocare); and aluminium oxide. Each abutment was tested for 2 mm and 3 mm thickness. To determine color variation, VITA Easyshade Advance spectrophotometer (Vita Zahnfabrik, Bad Sackingen, Germany) was used. RESULTS. Results showed that the color variation induced by the abutment would be affected by the abutment material and gingival thickness, when the gingival thickness is 2 mm. All materials except zirconium dioxide (Standard shade) caused a visible change of color. Then, as the thickness of the gingiva increased to 3 mm, the color variation was attenuated in a significant manner and became invisible for all types of abutments, except those made of aluminium oxide. CONCLUSION. Zirconium dioxide is the material causing the lowest color variation at 2 mm and at 3 mm, whereas aluminium oxide causes the highest color variation no matter the thickness.
골흡수가 심한 환자에서 임플란트를 이용한 전악 수복의 경우 치은과 치아의 외형을 심미적이면서도 기능적으로 회복하기 위해서는 보철물 제작에 있어 신중을 기해야 한다. 이러한 전악 수복 증례에서 CAD/CAM 시스템이 높은 정확성을 가지는 보철물 제작을 가능하게 함으로써 사용이 증가하고 있다. 최근에 임상에서 티타늄이나 지르코니아의 단점을 보완하면서 높은 생체적합성을 가지고 CAD/CAM 시스템으로 제작할 수 있는 팔라듐-은(Pd-Ag) 합금이 개발되어 사용되고 있다. 본 증례에서는 상악 전부 무치악 환자에서 임플란트를 식립하고 도재의 chipping이나 파절의 위험을 감소시키기 위해 Pd-Ag 합금을 이용해 제작된 임플란트 고정성 보철물로 수복한 환자에 대해 보고하고자 한다.

PURPOSE. In this study, a temporal abutment fixation screw, designed to fracture in a controlled way upon application of an occlusal force sufficient to produce critical micromotion was developed. The purpose of the screw was to protect the osseointegration of immediate loaded single implants. MATERIALS AND METHODS. Seven different screw prototypes were examined by fixing titanium abutments to 112 Mozo-Grau external hexagon implants (MG Osseous$^{(R)}$; Mozo-Grau, S.A., Valladolid, Spain). Fracture strength was tested at $30^{\circ}$ in two subgroups per screw: one under dynamic loading and the other without prior dynamic loading. Dynamic loading was performed in a single-axis chewing simulator using 150,000 load cycles at 50 N. After normal distribution of obtained data was verified by Kolmogorov-Smirnov test, fracture resistance between samples submitted and not submitted to dynamic loading was compared by the use of Student's t-test. Comparison of fracture resistance among different screw designs was performed by the use of one-way analysis of variance. Confidence interval was set at 95%. RESULTS. Fractures occurred in all screws, allowing easy retrieval. Screw Prototypes 2, 5 and 6 failed during dynamic loading and exhibited statistically significant differences from the other prototypes. CONCLUSION. Prototypes 2, 5 and 6 may offer a useful protective mechanism during occlusal overload in immediate loaded implants.
Osteoblast is one of cells related with osseointegration and many research have conducted the adhesion of osteoblast onto the surface of implant. In the osseointegration, biocompatibility of the implant and cell adhesion to the surface are important factors. The researches related to cell adhesion have a direction from micro-scaled surface roughness to nano-scaled surface roughness with advancing nanotechnology. A cell reacts and sense to stimuli from extracellular matrix (ECM) and topography of the ECM [1]. Thus, for better osseointegration, we should provide an environment similar to ECM. In this study, we synthesize TiO2 nanowires using hydrothermal reaction because TiO2 provides inertness to titanium on its surface and enables it used as an implant material for the orthopedic treatment such as fixation of the bone fracture [2]. Ti substrate is immersed into NaOH aqueous solution. The solution are heated at $140{\sim}200^{\circ}C$ for various time (10~720 minutes). After heat treatment, we take out the sample and immerse it into HCl aqueous solution for 1 hour. The acid treated sample is heated again at $500^{\circ}C$ for 3 hours [3]. Then, we culture osteoblast on the TiO2 nanowires. For investigating cell adhesion onto nanostructured surface, we conduct several tests such as MTT assay, ALP (Alkaline phosphatase) activity assay, measuring calcium expression, and so on. These preliminary results of the cell culture on the nanowires are foundation for investigating cell-material interaction especially with nanostructure interaction.
Alloplastic implants have been used to repair orbital wall fractures in most cases. Orbital hemorrhage is a rare complication of these implants and has been reported rarely in Korea. The purpose of this article is to report a late complication case focusing on their etiology and management. A 20-year-old male patient underwent open reduction with Medpor (porous polyethylene) insertion for bilateral orbital floor fractures. The initial symptom occurred with proptosis in the right side as well as vertical dystopia, which had started 4 days earlier, 8 months after surgery. Any trauma history after the surgery was not present. We performed an exploration and removal of hematoma with Medpor titanium meshed alloplastic implant. A case of delayed orbital hematoma following alloplastic implant insertion was identified. It occurred within the pseudocapsule of the implant. One week after surgery, overall symptoms improved successfully, and no complications were reported during the 11-month follow-up period. Although rare, orbital hemorrhage is a potential complication of alloplastic orbital floor implants, which may present many years after surgery. As in the case presented, delayed hematoma should be included in the differential diagnosis of late proptosis or orbital dystopia.
연구 목적: 본 연구의 목적은 골 유착된 임플란트에 인위적 비틀림을 가하여 골 유착을 파괴시킨 뒤 시간 경과에 따른 골 재유착력의 변화와 임플란트와 골과의 계면에서의 골 반응을 조직형태학적으로 분석하는 것이다. 연구 재료 및 방법: 표면처리 하지 않은 실험용 임플란트 (cp titanium, ${\phi}3.75\;mm{\times}4\;mm$)를 제작하였다. 3.5 kg이상의 뉴질랜드산 흰색 암컷 토끼 12마리의 좌우 경골에 제작한 임플란트를 2 - 3개씩 식립한 후, 6주의 골유착 유도한 기간을 부여한 후 비틀림 제거력을 측정하여 1차 측정치로 기록한 다음, 골 재유착을 얻기 위하여 다시 침하시켰다. 시간 경과에 따른 골유착의 변화를 살펴보기 위해 각각 4일 (I군), 1주 (II군), 2주 (III군), 3주 (IV군), 4주 (V군), 및 5주 (VI군)의 치유기간 후에 2차 비틀림 제거력을 측정하였다. 형광 조사 하에서 골 재형성여부를 검사하기 위해서 각군의 토끼에게 테트라사이클린 15 mg/kg을 근육 주사하였다. 2차 비틀림 제거력을 측정한 후 토끼를 희생시키고 각 실험군당 2 - 3개씩 총 16개를 제작하였다. 광학 현미경을 이용하여 형광 조사 하에서 골재형성여부를 관찰하였다. 측정된 각 실험군의 1차 비틀림 제거력과 2차 비틀림 제거력의 증감율의 평균을 구하였다. 그리고 각 실험군에서 그리고 실험군간에서 2차 비틀림 제거력이 1차 비틀림 제거력에 대하여 유의한 변화가 있는지 알아보기 위하여 paired t-test, one-way ANOVA, 그리고 DUNCAN 다중비교법으로 통계처리하였다. 결과: 실험 I군과 II군에서는 2차 비틀림 제거력이 감소하였으며 특히 실험 I군은 유의성 있게 감소하였다. 실험 III, IV, V, 그리고 VI군에서는 2차 비틀림 제거력이 유의성있게 증가하였다. 실험군 간 차이 비교에서, 실험 I, II군과 실험 III, IV, V, VI군 사이에 유의성있는 차이를 보였다. 형광 조사 검사에서 1주째 시편부터는 임플란트와 골과의 계면 부위에서 형광띠로 나타나는 광물화가 시작되는 부분이 관찰되었으며, 2주째 시편 이후부터는 더욱 분명하게 골생성이 이루어짐을 볼 수 있었다. 결론: 임플란트에 의원성 동요가 발생된 후 힘을 가하지 않은 상태로 일정시간 유지하게 되면 임플란트와 골 사이 계면에 다시 골유착이 발생되며 토끼의 경골에서는 높은 골유착력을 2주 만에 얻을 수 있었다는 것을 알 수 있었다.
연구 목적: 본 연구의 목적은 골 유착된 임플란트에 인위적 비틀림을 가한 후 골 유착이 다시 일어나는지, 혹은 일어나지 않는지, 일어난다면 이전의 골유착보다 강화되는지, 약화되는지에 대해 알아보기 위함이다. 연구 재료 및 방법:상업적으로 순수한 티타늄 (commercial pure titanium 99%)으로 직경 3.75 mm, 길이 4 mm의 표면처리 없는 실험용 임플란트를 제작하였다. 3.0 kg이상의 뉴질랜드산 흰색 암컷 토끼 7마리의 좌우 경골에 제작한 임플란트를 2개씩 식립 후, 6주의 골유착 유도 기간 부여 후 비틀림 제거력을 측정한 경우를 I군으로, 다시 임플란트 고정체를 재위치시키고 봉합하여 4주간의 치유기간을 더 두고 측정한 경우를 II군으로 분류하였다. 광학현미경을 이용한 조직 형태학적 분석을 위하여, 1차 비틀림 제거력 측정 직후 1마리의 토끼를 희생시키고, 2차 비틀림 제거력 측정 직후 2마리의 토끼를 희생시켜 각각 4개, 7개의 시편을 제작하여, 광학현미경 (${\times}20$) 분석에서 골-임플란트 접촉 (Bone-Implant contact, BIC, %)비율과 치밀골 부위에 위치하는 임플란트의 나사산 수를 측정하여 CBa (Bone area in the cortical passage)비율을 측정하였다. 비틀림 제거력과 BIC및 CBa비율에서의 I군과 II군의 통계적 유의성 ($\alpha$=.05)을 평가하였다. 결과: 비틀림 제거력 측정에서 I군은 $10.8{\pm}3.6$ NCm로 측정되었으며, II군은 $20.2{\pm}9.7$ NCm로 측정되었으며, I군보다II군의 비틀림 제거력이 평균 98.1% 증가되었다 (P<.05). 조직 형태학적 분석에서 BIC와 CBa 비율은 I군과 II군 사이에 통계적 유의성을 나타내지 않았고 (P>.05), RT/BIC와 RT/CBa 값은 I군과 II군 사이에 통계적 유의성을 나타냈다 (P<.05). 결론: 임플란트 고정체에 의원성 비틀림 동요가 발생한 경우에 초기 골유착을 얻기 위해 필요로 했던 치유기간보다 짧은 기간 내에 이전보다 견고한 골유착을 얻을 수 있을 것으로 생각된다.
무릎 임플란트에서 접촉압력이 마모에 큰 영향을 미친다. 본 연구에서는 본 연구기관에서 개발한 무릎 임플란트 모델을 이용하여 유한요소해석을 하였다. 연구와 산업에서 실제로 사용하는 조합을 이용하여 총 10가지의 무릎 임플란트의 재료조합에 대한 접촉압력을 분석하였다. 무릎이 30도, 45도 60도 기울어져있을 때의 하중을 가하여 접촉압력을 구하였다. 접촉압력을 계산한 결과 티타늄합금-UHMWPE 조합에서 가장 작은 접촉압력이 나왔다. UHMWPE의 경우 대퇴골부에 어떠한 재료를 사용하여도 접촉압력이 크게 변하지 않았다. 접촉압력이 가장 큰 조합과 작은 조합을 비교하였을 때 0.77% 차이를 보였다. 반면에 Carbon/PEEK 복합재료의 경우 접촉압력이 가장 큰 경우와 작은 경우를 비교하였을 때 5.3% 차이를 보였다. 이를 통해 Carbon/PEEK 복합재료를 베어링부로 사용할 경우 대퇴골부의 재료가 마모에 영향을 미침을 알 수 있다. 본 연구는 무릎 임플란트 마모예측과 마모를 최소화 연구에 도움이 될 것이라 생각한다.
임플란트 주위조직과 치아 주위조직은 조직학, 형태학적으로 유사한 면이 있다. 특히 functional ankylosis라 불리는 티타늄과 연조직의 직접적인 상피부착은 치아주위 조직의 접합상피처럼 염증세포의 치근단으로의 이행 및 그로 인한 골흡수를 막는 역할을 한다. 그러나 반복적인 지대주의 착탈은 임플란트 주위 연조직의 mucosal barrier를 파괴하여 골흡수를 야기할 수 있다. 만일 지대주의 반복적인 착탈 없이 수술 시에 보철 지대주를 채결한다면 골흡수의 양을 줄일 수 있을 것이다. 이는 기존의 수술방법으로는 한계가 있었으나 Cone Beam Computed Tomography(CBCT)를 이용한 guided surgery를 이용함으로써 임플란트 식립의 3차원적 정확성의 비약적인 향상으로 이런 술식이 가능하게 되었다. 본 증례는 정밀한 CBCT를 통한 분석 후 지르코니아 맞춤 지대주를 미리 제작하고 수술 시에 즉시 지대주와 임시보철물을 연결한 경우로 임상적으로 만족할만한 결과를 얻어 이를 보고하는 바이다.
뼈의 성장에 미치는 많은 요소들 중에서 implant의 상대적인 미세운동(relative micromotion)은 뼈의 implant와의 접합을 방해하는 것으로 알려져 왔다. 그런데 이러한 상대적인 운동 및 spinal stability에 직접적으로 영향을 주는 하중조건, spinal material의 물성치, spinal geometry 및 뼈와 implant의 접촉면에서의 마찰계수를 고려하기 위하여, 하나의 titanium interbody cage 가 삽입된 human lumbar segments (L4-L5)의 유한요소 모델이 개발되었다. 이러한 유한요소 모델의 해석을 통하여 상대적인 미세운동, Posterior의 수직적인 변위, von Mises 응력 및 마찰력이 예측되었다. Cancellous bone. annulus fibers 및 ligaments의 기계적인 물성치의 감소 또는 접촉면에서의 마찰계수의 감소는 상대적인 미세운동 (relative micromotion or slip distance)을 증가 시켰다. 접촉면에서의 normal force는 뼈의 밀도 (cancellous bone density) 가 감소하거나 접촉마찰계수가 증가하면 감소했다. 특히 하중조건에 있어서, compressive preload에 대한 torsion의 추가는 접촉면의 anterior부위에서 상대적인 미세운동을 증가 시켰다. 하지만 디스크면적이 증가할수록 상대적인 미세운동은 감소했다. 결론적으로, 접촉면의 기계공학적 거동 (Relative micromotion, stress response, posterior axial displacement and contact normal force)은 접촉면의 마찰계수 뼈의 밀도, 하중조건 및 노화에 따른 형상/물성의 변화에 매우 민감함을 보이고있다.
