Result of Voice Analysis after Laryngeal Microsurgery for Vocal Polyp in Elderly (노인에서 성대 용종의 후두 미세수술 후 음성검사 결과)

  • Background and Objectives: Vocal polyps arc one of the most frequent benign laryngeal diseases. They arc usually found at the midpoint of the vocal fold. They are mainly caused by vocal overuse. Vocal polyps arc usually removed surgically. Generally, age-related changes to speech are attributed to change in anatomy and physiology of the speech mechanism. These changes result in increased variability in the acoustic properties of speech with age. Still, not 'all studies of age-related changes in speech have taken differences between the young group and adult group after laryngeal microsurgery into account. The aim of this investigation was to compare improvement of acoustic analysis in young patients and elderly patients with vocal polyps, before and after the laryngeal microsurgery. Materials and Method: One hundred and twenty-eight patients who underwent laryngeal microsurgery for vocal polyps from 2008 through 2011 were reviewed retrospectively. 105 of the 128 patients under age 60 were classified as adult group (AG), and remaining 23 patients as elderly group (EG). The speech of AG and EG were evaluated before and after surgery for identification of differences for age group across measures of fundamental frequency (F0), Jitter, Shimmer and Maximum phonation time (MPT). Results: There were not significant differences between two groups for improvement of F0, Jitter, Shimmer, NHR, and MPT before and after surgery. The findings suggest that elderly group compares quite well with adult group in effectiveness of surgery. However, comparison between elderly group and young group (Age under 40) there was significant difference of improvement in Jitter and Shimmer. Conclusion: In general, the results of the present research showed significant improvement in vocal quality after phonosurgery of vocal polyp in both elderly and adult group. However, comparison of improvement between elderly group and young group, there were significant differences of improvement in jitter and shimmer. Therefore, in treatment planning of elderly group, we should consider age related changes of vocal cord.

The Development of Signal Processing Software for Single-and Multi-Voxel MR Spectroscopy (단위용적 및 다용적 기법 자기공명분광 신호처리 분석 소프트웨어의 개발)

  • The aim of this study is to develop the $^1H$-MRS data postprocessing software for both single-voxel and multi-voxel technique, which plays and important role as a diagnostic tool in clinical field. This software is based on graphical user interface(GUI) under windows operating system of personal computer(PC). In case of single-voxel MRS, both of raw data in time-domain and spectrum data in frequency-domain are simultaneously displayed in a screen. Several functions such as DC correction, zero filling, line broadening, Lorentz-Gauss filtering and phase correction, etc. are included to increase the quality of spectrum data. In case of multi-voxel analysis, spectroscopic image reconstructed by 3-D FFT was displayed as a spectral grid and overlapped over previously obtained T1- or T2-weighted image for the spectra to be spatially registered with the image. The analysis of MRS peaks were performed by obtaining the ratio of peak area. In single-voxel method, statistically processed peak-area ratios of MRS data obtained from normal human brain are presented. Using multi-voxel method, MR spectroscopic image and metabolite image acquired from brain tumor are demonstrated.

Safety Evaluation of Concert Hall Floor Vibration Using Numerical Analysis Model (수치해석모델을 이용한 콘서트 홀 바닥진동 안전성 평가)

  • In this paper, the floor vibration of an example concert hall building was measured and floor safety criteria were analytically checked through comparison between experimental and analytical results. The floor bottom plate model was constructed considering the composite effect and the analytical model was modified to have the natural frequency identical to the measured one. Also, time history analysis was conducted using the dynamic loads induced by human rhythmic movement during a musical performance, and the analytically calculated floor accelerations were similar to the measured one. Based on this model, the floor vibration level due to the group activities of about 400 persons, maximum available persons for the concert hall, was estimated. It was confirmed that the human induced dynamic loads applied to the column and beam would be much lower than the design strength. In addition, the horizontal acceleration level is just 2% of the design seismic load, so the concert hall is safe in both vertical and horizontal excitations by human rhythmic movements.

  • SNS allowed users to serve not only as the content and message producer but as the consumer, with development into various types of SNS platforms. Instead of a traditional media structure of media-to-many and unspecified users, it also made it possible to achieve one-to-one or one-to-many interactions, regardless of time and space, through SNS platforms. Moreover, according to development of digital communication technology, IT, media contents and communication network have been mutually connected, though they were once separate. The changes in communication environments have caused rapid disorganization and reorganization in popular culture led by specific expert groups. Such trend has a greater influence on marketing strategies of enterprises. In other words, it will lead them to mostly introduce new information technologies, based on consumer market, and to strategically participate in SNS for promotion and marketing for their products and brands. Likewise, SNS has currently appeared as the main media affecting consumers' behaviors. In consideration of the importance of SNS features, which can stimulate responses of other users, analysis of main features affecting SNS user acceptance decision is required, as well as its utilization strategies. Accordingly, this study conducted division of SNS features into openness, quickness, interactiveness and economical efficiency to derive strategies for increasing the usage frequency of SNS and ultimately maximizing the expectation effect, in addition to an empirical analysis of effects of SNS features on usefulness, easiness and pleasure perceived in SNS, and SNS user intention.

Longitudinal Change in Health Status after the Sewol Ferry Accident among Bereaved Parents (세월호 참사 후 희생자 부모의 건강상태 변화)

  • Objective : The present study had examined the psychiatric symptoms and physical health consequences for the bereaved parents of the high school students who died in the 2014 Sewol ferry accident. Methods : Forty bereaved parents participated in the study. The authors administered self-report questionnaires about the parents' health behaviors and psychiatric symptoms. The authors also conducted laboratory tests to assess the parents' physical health at 18 and 30 months after the accident. Univariate descriptive statistics were performed to report the prevalence and severity of psychiatric symptoms and health-related behaviors. Paired t-test and Mcnemar test were performed to compare the 18-and 30-month findings. Correlation analysis between psychiatric symptoms and laboratory findings were performed to find a relationship between the two variables. Results : At 30 months after the accident, most of the bereaved parents still appeared to suffer from complicated grief (97.5%), post traumatic stress disorder (80%), insomnia (77.5%) and severe depression (62.5%) based on the scores on the Inventory of Complicated Grief (ICG), the PTSD Check List-5 (PCL-5), the Insomnia Severity Index (ISI) and the Patient Health Questionnaire-9 (PHQ-9). One quarter of the bereaved parents reported high-risk drinking, and 47.5% reported increased drinking amount and frequency after the accident. In objective laboratory results, 55% of the bereaved parents were obese as defined by body mass index ${\geq}25$. The parents' mean low-density lipoprotein shows a significant increase over time (118.5 mg/dL at 18 months. vs. 132.5 mg/dL at 30 months. paired t-test t=-4.061, $$p{\leq_-}0.001$$). Total cholesterol and low-density lipoprotein at 30 months after the accident were in clinically borderline high range. In correlation analysis, triglyceride was positively correlated with ISI. Conclusion : The loss of children in the Sewol ferry accident, a disaster caused by human error, continued to have considerable impact on the victims' parents' mental and physical health 18 and 30 months after the accident. A longitudinal study following the parents' physical health would be necessary to investigate the long-term effects of this traumatic experience on physical health.

  • Previous studies showed that epigallocatechin gallate(EGCG) have substantial effects of suppressing the N-methyl-N'-nitro-N-nitrosoguanidine(MNNG)-initiated cell transformation process on the bases of foci formation frequency and loss of anchorage dependency. In this study we tried to clarify the molecular mechanism of suppressing the cell transformation process. Mouse cell line balb/c 3T3 A31-1-1 was exposed 2 days to MNNG followed by 15 days 12-O-tetradecanoylphorbol-13-acetate(TPA) treatment for our transformation process. EGCG was added after the time point of 24 hours exposure to TPA and incubated for 19 days. 2029 genes were selected in our transformation process that showed fold change value of 1.5 or more in the microarray gene expression analysis covering the mouse full genome. These genes were found to be involved mainly in the cell cycle pathway, focal adhesion, adherens junction, TGE-$\beta$ signaling, apoptosis, lysine degradation, insulin signaling, ECM-receptor interaction. Among the genes, we focused on the 631 genes(FC>0.5) reciprocally affected by EGCG treatment. Our study suggest that EGCG down-regulate the gene expressions of up stream signaling factors such as nemo like kinase with MAPK activity and PI3-Kinase, Ras GTPase and down stream factors such as cyclin D1, D2, H, T2, cdk6.

Seismic Fragility Analysis of Curved Beam with I-Shape Section (I-Shape 단면을 갖는 곡선 보의 지진 취약도 분석)

  • Purpose: This study was to the fragility evaluation of I-shape curved beam structure subjected to strong ground motions including Gyeongju and Pohang earthquakes Method: In particular, to conduct the analytical model, ABAQUS and ANSYS platform was used in this study. Furthermore, the analytical model using 3D Finite Element Model (FEM) was validated, in comparison to the theoretical solutions at the location of 025L, 05L, and 0.75L in static loading condition. In addition, in order to evaluate the seismic fragility of the curved beam structure, 20 seismic ground motions were selected and Monte-Carlo Simulation was used for the empirical fragility evaluation from 0.2g to 1.5g. Result: It was interesting to find that the probability of the system failure was found at 0.2g, as using 190 MPa limit state and the probability of the failure using 390 MPa limit state was starting from 0.6g. Conclusion: This study showed the comparison of the theoretical solution with analytical solution on I-shaped curved beam structures and it was interesting to note that the system subjected to strong ground motions was sensitive to high frequency earthquake. Further, the seismic fragility corresponding to the curved beam shapes must be evaluated.

University Students Impact of Martial-Arts Training on Self-Realization among Security Majors (경호전공 대학생의 무도수련이 자아실현에 미치는 영향)

  • The subjects in this study were 500 students who were selected from a population that consisted of security majors who took courses in martial arts in four different four-year-course universities. After a survey was conducted, the answer sheets from 451 students were analyzed except 49 incomplete ones. The collected data were analyzed with SPSS Ver. 12.0 program. Frequency analysis, t-test and one-way ANOVA were utilized, and LSD and regression analysis were employed to make a post-hoc comparison. All the hypotheses formulated in this study were verified at the a=.05(Chronbach's alpha) level of significance. The findings of the study were as follows: First, as for relations between demographic characteristics and self- realization, the college students investigated were statistically different according to gender in three subvariables of self-realization that included ability development, ability display and attainment of ideal. Age and academic year made a significant difference to their ability development and attainment of ideal, and they differed statistically significantly in terms of ability development according to black lebel test. Second, concerning connections between the degree of martial-arts training and self-realization, training term, one of the subvariables of the degree of martial-arts training had a positive correlation to their ability development and attainment of ideal at the 5% level of significance, and training time was positively correlated to their ability development at the 5% level of significance. Third, training term, one of the subvariables of the degree of martial- arts training, had an impact on ability development, and that exerted a firsthand influence on attainment of ideal as well.

  • The mechanisms of $CH_4$ uptake into and release from the ocean are not well understood due mainly to complexity of the biogeochemical cycle and to lack of regional-scale and/or process-scale observations in the marine boundary layers. Without complete understanding of oceanic mechanisms to control the carbon balance and cycles on a various spatial and temporal scales, however, it is difficult to predict future perturbation of oceanic carbon levels and its influence on the global and regional climates. High frequency, high precision continuous measurements for carbon isotopic compositions from dissolved $CH_4$ in the surface ocean and marine atmosphere can provide additional information about the flux pathways and production/consumption processes occurring in the boundary of two large reservoirs. This paper introduces recent advances on optical instruments for real time $CH_4$ isotope analysis to diagnose potential applications for in situ, continuous measurements of carbon isotopic composition of dissolved $CH_4$. Commercially available, three laser absorption spectrometers - quantum cascade laser spectroscopy (QCLAS), off-axis integrated cavity output spectrometer (OA-ICOS), and cavity ring-down spectrometer (CRDS) are discussed in comparison with the conventional isotope ratio mass spectrometry (IRMS). Details of functioning and performance of a CRDS isotope instrument for atmospheric ${\delta}^{13}C-CH_4$ are also given, showing its capability to detect localized methane emission sources.

Body mass index(BMI) and lipid metabolism in patients admitted in long-term care hospitals (요양병원 입소 환자의 신체질량지수와 지질대사)

  • This is a non-experimental and retrospective study aimed at determining the effects of long-term hospitalization on the body mass index (BMI) and lipid metabolism in long-term hospitalized patients. The study subjects included 120 patients aged 40-65 years who were hospitalized for >3 months in 2 long-term care hospitals in Gyeonggi-do, South Korea. In this study, the BMI and levels of total cholesterol, triglycerides (TG), high-density lipoprotein (HDL), and low density lipoprotein (LDL) at admission and 3 months after hospitalization were compared and analyzed, and the related changes over time were followed up. The general characteristics of the subjects were analyzed by using descriptive statistics and frequency analysis. In addition, logistic regression analysis was performed to determine the effects of the general characteristics on the BMI and Dyslipidemia. The changes in the BMI and blood lipid levels between admission and 3 months after hospitalization were analyzed using the paired t-test. The results showed that with regard to the changes in the blood lipid levels, the triglyceride levels significantly increased 3 months after hospitalization (p<.05). These findings imply that long-term hospitalization for care and rehabilitation after acute-phase treatment should be considered a potential high-risk factor for dyslipidemia, which could be prevented or alleviated by providing the patients with health education, including exercise and dietary education.
