Thoracic Hyperkyphosis affects Scapular Orientation and Trunk Motion During Unconstrained Arm Elevation

  • Background: Shoulder function is achieved by the coordinated movements of the scapula, humerus, and thoracic spine, and shoulder disorders can be associated with altered scapular kinematics. The trunk plays an important role as the kinematic chain during arm elevation. Objects: The purpose of this study was to determine the effects of thoracic hyperkyphosis on scapular orientation and trunk motion. Methods: Thirty-one subjects (15 in the ideal thorax group and 16 in the thoracic hyperkyphosis group) performed right-arm abduction and adduction movements in an unconstrained plane. The scapular orientation and trunk motion were recorded using a motion analysis system. Results: Those subjects with thoracic hyperkyphosis displayed greater scapular posterior tilting at a $120^{\circ}$ shoulder elevation, greater scapular internal rotation throughout the arm raising phase, and greater trunk axial rotation at the upper ranges of the shoulder elevation, compared to those subjects with an ideal thorax (p<.05). Conclusion: Thoracic hyperkyphosis can cause scapular instability, greater trunk rotation and greater scapular posterior tilting, and may contribute to preventing the achievement of a full range of humeral abductions in an unconstrained plane.

The Influence that the Closed Chain Breathing Exercise Affects on Thorax Circumference Increase and ROM of Shoulder Rotation on the Cervical Vertebral Cord Injury Patients (닫힌 사슬 호흡 운동법이 상위 경수 손상 환자의 흉곽둘레길이와 어깨관절의 회전가동범위에 미치는 영향)

  • Purpose : The purpose of this study was to investigate the effects of closed chain breathing exercise on thorax circumference increase and ROM of shoulder rotation for cervical vertebral cord injury patient's. Methods : 12 cervical vertebral cord injury patients volunteer to participate in this study. closed chain breathing exercise group of 4 subjects performed exercise three time a week. The deep breathing exercise group and general exercise group of 4 subjects performed exercise three time a week. We analyaed the descriptive statistics and $3{\times}2$ repeated measures ANOVA by SPSS 12.0 for window. Results : In comparison of rotation of shoulder ROM between pre and post value, the increase of rotation ROM of shoulder was significant in the closed chain breathing exercise group(p<.05). Conclusion : The closed chain breathing exercise helped to increased rotation ROM of shoulder.

Surgical Removal of Intrathoracic Lipoma from Herniated Omentum through the Diaphragmatic Defect in an Abyssinian Cat

  • A nine-year-old neutered male cat was presented with chronic cough and vomiting. Thoracic radiography showed regions of fatty opacity in the right caudoventral region. On positive contrast celiography, contrast agent did not move into thoracic cavity. Computed tomography revealed 7-mm diameter of defect at the right diaphragmatic crus and a 2-mm diameter defect at the left ventral diaphragmatic crus. Through the right diaphragmatic defect omental herniation was confirmed by the presence of contrast enhanced omental vessel running across the diaphragm. On exploratory thoracotomy, the omentum protruded into the thorax through the right diaphragmatic defect, and it contained a yellowish lipomatous mass. The protruded omentum containing a mass in the thorax was removed, and the right diaphragmatic defect was closed. Histopathologic examination revealed that the protruded omentum showed normal omental structure and the adipose mass showed lipoma surrounded by fibrous tissue. In conclusion, a thorough examination is necessary to confirm the origin of the mass located near the diaphragm.

  • Timely and accurate diagnosis of lung diseases using Chest X-ray images has been gained much attention from the computer vision and medical imaging communities. Although previous studies have presented the capability of deep convolutional neural networks by achieving competitive binary classification results, their models were seemingly unreliable to effectively distinguish multiple disease groups using a large number of x-ray images. In this paper, we aim to build an advanced approach, so-called Ensemble Knowledge Distillation (EKD), to significantly boost the classification accuracies, compared to traditional KD methods by distilling knowledge from a cumbersome teacher model into an ensemble of lightweight student models with parallel branches trained with ground truth labels. Therefore, learning features at different branches of the student models could enable the network to learn diverse patterns and improve the qualify of final predictions through an ensemble learning solution. Although we observed that experiments on the well-established ChestX-ray14 dataset showed the classification improvements of traditional KD compared to the base transfer learning approach, the EKD performance would be expected to potentially enhance classification accuracy and model generalization, especially in situations of the imbalanced dataset and the interdependency of 14 weakly annotated thorax diseases.

Treatment Results of Increased Dose External Beam Radiation Therapy for Unresectable Esophageal Cancer (절제 불가능한 식도암에서 고선량 외부조사 방사선 치료의 결과)

  • Purpose : To evaluate the treatment outcome for patients with locally advanced, unresectable esophageal cancer treated with relatively high dose radiation therapy(RT). Materials and Methods : From January 2000 to December 2008, 32 patients with locally advanced unresectable or medically inoperable esophageal cancer were treated with radiation therapy(RT) with or without concurrent chemotherapy. Ten patients were excluded from analysis because of distant metastasis and drop off. Patient distributions according to AJCC stages II, III IVa were 7(31.8%), 12(54.6%), 3(13.6%) respectively. The locations of tumor were cervical/upper thorax 3 (13.6%), mid thorax 13(59.1%), and lower thorax/abdominal 6(27.3%), respectively. Eleven patients received RT only, and 11 patients received cisplatin based concurrent chemoradiotherapy(CCRT). Median radiation dose was 65 Gy(range 57.6~72 Gy). Results : The median follow-up was 9.1 months(range 1.9~43.8 months). The response rates for complete response, Partial response, stable disease and Persistent disease were 6(27.3%), 11(50.0%), 4(18.2%) and 1(4.5%), respectively. Two patients(9.1%) suffered from esophageal stenosis and stents were inserted. Two patients(9.1%) had Grade 3 radiation pneumonitis and one of them expired due to acute respiratory distress syndrome(ARDS) at 36 days after completion of radiation therapy. The recurrence rate was 11(50.0%). The patterns of recurrence were persistent disease and local progression in 5(22.7%), local recurrence 3(13.7%) and concomitant local and distant recurrence in 3(13.7%). The overall survival(OS) rate was 32.1% at 2 years and 21.4% at 3 years(median 12.0 months). Disease free survival(DFS) rate was 17.3% at 2 and 3 years. All patients who had no dysphagia at diagnosis showed complete response after treatment and 100% OS at 3 years(p=0.0041). The OS for above 64.8 Gy group and 64.8 Gy or below group at 3 years were 60.6% and 9.1%(p=0.1341). The response to treatment was the only significant factor affecting OS(p=0.004). Conclusion : Relatively high dose radiation therapy in unresectable esophageal cancer tended to have a better outcome without increased complication rate. Further study with more patients is warranted to justify improved result.

Education Evaluation of Basic CPR on Guard Major Collegian in Gwangju and Jeonnam Region (광주·전남지역 경호학과 대학생의 심폐소생술(CPR) 교육평가)

  • This study is focused on guard major collegians who are composed of factor in medical emergency system. In the case of cardiac failure, it is to make its basic data develop its education program of CPR which can increase the patient's survival rate before his hospitalization. The subject of study is composed of 120 persons who are 94 boy-collegians(78.3%) and 26 girl-collegians(21.7%) in sex and 66 first-grade collegians(55.0%) and 54 second-grade collegians(45.0%) in a school year. There is significant difference in the practices of artificial respiration and the thorax pressure after the education of basic CPR in sex(p<0.01). The practices of artificial respiration in boy-collegians($93.72{\pm}4.21$) is higher than in girl-collegians($82.31{\pm}6.36$) and the practices of thorax pressure in boy-collegians($92.45{\pm}4.44$) is higher than in girl-collegians($88.08{\pm}6.49$). The satisfaction degree of boy-collegians($4.33{\pm}0.59$) is higher than that of girl-collegians($3.73{\pm}0.67$) after theory & practice education of basic CPR(p<0.01). It is necessary to develop the education program and educate its knowledge & technology in proportion to collegians characteristics of sex and school year. Also, education authorities should develop a subject of the accident provided the practical education of CPR for guard major collegians.

A Simulation study on the Cardiac Current Density distribution during the Defibrillation Shock (제세동 쇼크에 의한 심장 전류밀도 분포에 관한 시뮬레이션 연구)

  • This paper is about to simulate the defibrillation situations using 3D FE(finite element) thorax model and describes the effects of three clinical electrodes' positions and size and organ's resistivity used in simulation on the characteristics of current density distribution over myocardium. The model was constructed with a eillipsoidal cylinder for the thorax and the 2D Visible Human images for remains. And, the distributions of current density were computed by a commercial program ANSYS 5.4. The electrical shock of the AP(anterior-posterior ) electrode provided more current flows with heart than the others and that of the LL(lateral-lateral) electrode showed the most uniform current density distribution. However, the electrode size had little effect on the current density distribution. In the evaluation of model's sensitivity to tissue resistivity variation, the variation of the myocardium's resistivity most affected the minimum, average and maximum current densities.

  • This is a case report of superior vena cava syndrome due to thyroglossal duct cyst, which was surgically treated. The patient was 61 year old male who progressively complained exertional dyspnea for about 5 months and right ptosis, facial flushing, and nasal stuffiness for about 1 month before admission. The CT scan of the thorax revealed the right paratracheal cystic mass that compressed and displaced the trachea to leftward and SVC to rightward. The resection of the cystic mass was performed through the right posterolateral thoracotomy. The pathologic result was compatible with thyroglossal duct cyst. The postoperative status of the patient was uneventful.

Pulmonary Blastoma -A Case Report- (폐아세포종 -수술치험 1례-)

  • We experienced a very rare case of pulmonary blastoma in a 29 year old female. She complained of left chest pain and dyspnea for 1 month. The characteristic feature of this tumor is it`s biphasic pattern consisting of a spindle cell stroma containing glandular structures. A serial check of simple chest X-ray and computed tomography revealed a growing huge lung mass occupying whole left thorax. We successfully removed the upper lobe of left lung with a huge tumor mass. Pathologic study revealed this tumor as pulmonary blastoma. We report a case with review of literature.

  • Primary intrathoracic neoplasms occur frequently, and the most frequent intrathoracic tumors are of pulmonary or bronchogenic origin. Second in frequency are mediastinal tumors, and least frequency or comparatively rare are the neoplasms arising from the wall of the thorax and extending into the thoracic cavity. Of all intrathoracic tumors, fibroma is extremely rare. Recently, we have experienced a huge rapid growing intrathoracic fibroma originating from thoracic wall, which is removed successfully and confirmed histopathologically. Authors present one case of fibroma and discussion with a brief review of the relevant literatures.
