Sonogram of coccygeus muscle in dairy cows with different gestational ages

  • Background: The change in size and weight of the female reproductive organs during gestation and birth might be affect the perineal muscles and this condition in dairy cow not been reported. This study aimed to assess the ultrasonographic image of coccygeus muscle in 11 inseminated dairy cows with different gestational ages and postpartum. Methods: Gestational age was calculated based on the record of artificial insemination and confirmed by using transrectal brightness mode ultrasonography. Perineal hair between the sacrum and ischium bones was shaved along 3-5 cm before being ultrasound. The images of perineal area were obtained by transcutaneous ultrasound using a 5.0 MHz transducer. The thickness and intensity of the coccygeus muscle were measured and analyzed by gestational status and postpartum to show the differences. Results: The results showed that the thickness of coccygeus muscle increased with the increase in gestational age. Muscle intensity only increased at young gestational age. However, it decreased with the increase in gestational age (P < 0.05). Conclusions: The ultrasound image of coccygeus muscle was affected by gestational status, thus this method may be used as one of the new methods of indirect gestational detection on dairy cows.

Roles of Sonography and Hysteroscopy in the Detection of Premalignant and Malignant Polyps in Women Presenting with Postmenopausal Bleeding and Thickened Endometrium

  • Background: To assess the role of sonographic endometrial thickness and hysteroscopic polyp size in predicting premalignant and malignant polyps in postmenopausal women. Materials and Methods: A total of 328 postmenopausal women with abnormal uterine bleeding and thickened endometrium underwent operative hysteroscopy due to detection of endometrial polyps were included in this retrospective study. Preoperative endometrial thickness measured by transvaginal ultrasonography and polyp size on hysteroscopy were noted. Hysteroscopic resection with histology was performed for endometrial polyps. Endometrial thickness and polyp size were evaluated on the basis of final diagnosis established by histologic examination. Receiver operator characteristic curves were calculated to assess the sensitivity, specificity, positive predictive value, negative predictive value and diagnostic accuracy of endometrial thickness and polyp size for detecting pemalignant and malignant polyps. Results: Premalignant and malignant polyps were identified in 26 (7.9%) of cases. Sonographic measurement showed a greater endometrial thickness in cases of premalignant and malignant polyps when compared to benign polyps. On surgical hysteroscopy, premalignant and malignant polyps were also larger. Endometrial thickness demonstrated a sensitivity of 53.8%, specificity of 85.8%, PPV of 24.6% and NPV of 95.6% at a cut-off limit of 11.5 mm with diagnostic accuracy of 83.2%. Polyp size has a diagnostic accuracy of 94.8% with a sensitivity of 92.3%, specificity of 95.0%, PPV of 61.5% and NPV of 99.3% at a cut-off point of 19.5mm. Conclusions: Endometrial thickness measured by transvaginal ultrasonography is not sufficient in predicting premalignant and malignant endometrial polyps in postmenopausal women with abnormal uterine bleeding and thickened endometrium. Polyp size on hysteroscopy is a more accurate parameter, because of better sensitivity and specificity. However, while polyp size ${\geq}19.5mm$ seems to have a great accuracy for predicting premalignancy and malignancy, histologic evaluation is still necessary to exclude premalignant and malignant polyps.

경동맥 초음파로 진단된 죽상동맥경화증과 위험인자의 관련성 분석 (Correlation Analysis of Clinical Risk Factors and Atherosclerosis Diagnosed by Carotid Artery Ultrasonography)

  • 본 연구는 혈관질환을 예측하는 데 있어서 일차 선별검사로서 경동맥 초음파 검사와 혈액학적 검사를 실시한 환자를 대상으로 경동맥 내중막 두께의 변화를 확인하고 죽상경화반의 발현에 위험인자들이 미치는 영향을 알아보고자 하였다. 일개 병원에서 건강검진을 목적으로 내원하여 경동맥 초음파 검사를 실시한 건강한 성인 469명을 대상으로 후향적으로 분석하였다. 그 결과 경동맥 내중막 두께, 연령, 체질량지수, 허리둘레, 수축기 혈압, 총콜레스테롤, 고비중지단백 콜레스테롤, 저비중지단백 콜레스테롤, 공복혈당이 죽상경화반의 유의한 예측인자로 분석되었다(p<0.001). ROC curve 분석을 통해 결정된 죽상경화반 위험인자들의 Cut off value를 기준으로 위험비를 산출하였으며 경동맥 내중막 두께는 8.06배, 연령은 7.53배, 허리둘레는 3.97배, 공복혈당은 2.02배 순으로 높게 나타났다. 따라서 본 연구에서는 죽상경화반의 유무에 영향을 주는 임상학적 위험요인들의 한국인 기준치를 마련할 수 있었으며, 초음파를 이용한 경동맥의 관찰은 심뇌혈관 질환을 조기에 진단하거나 예측하는데 도움이 될 것으로 생각된다.

Effects of Lumbar Stabilization Using Pressure Biofeedback Unit During Hip Abduction in Side-Lying in Patients With Low Back Pain

  • Background: Lumbar stabilization (LS) improve the thickness of the quadratus lumborum (QL) muscle and muscle activity of the gluteus medius (GM) muscle during hip abduction in a side-lying position in patients with low back pain (LBP). Objects: The purpose of this study was to assess the effects of LS on muscle thickness of QL and muscle activity of GM during hip abduction in side-lying in patients with LBP. Methods: The study included 32 patients with LBP, who were randomly divided into the control group and experimental group, each with 16 patients. All subjects performed $35^{\circ}$ preferred hip abduction (control group) and $35^{\circ}$ hip abduction with LS (experimental group) during side-lying. An ultrasonography and a surface electromyography were used to measure the thickness of the QL muscle, and the muscle activities of the GM muscle respectively. Independent t-test was used to compare the muscle thickness of the QL and the muscle activity of the GM muscle, respectively. Results: Anterio-posterior diameter in the muscle thickness of QL muscle was decreased significantly in hip abduction with LS more than in preferred hip abduction (p<.001), but medio-lateral diameter in the muscle thickness of QL muscle was not significantly different between in preferred hip abduction and in hip abduction with LS (p=.06). The muscle activity of GM was increased significantly in hip abduction with LS more than in preferred hip abduction (p<.001). Conclusion: These findings suggest that hip abduction with LS could be recommended as a hip abduction for LS and a prevention unwanted compensatory pelvic lateral tilting movement.

요통유무에 따른 복부 드로우인과 골반바닥근 수축훈련이 배가로근과 골반바닥근에 미치는 영향 (The effect of abdominal drawing-in maneuver and pelvic floor muscle contraction in individuals with and without low back pain)

  • PURPOSE: This study aimed to identify differences in the thickness of the transverse abdominis (TrA) and pelvic floor muscles (PFM) between those with (LBP) and without low back pain (non-LBP). The standardized methods of contraction for the TrA and PFM were used to perform the abdominal draw-in maneuver (ADIM) and pelvic floor muscle contraction (PFC), respectively. METHODS: This study included 27 young men and women, who were verbally instructed regarding the maneuvers (LBP, n=14; non-LBP, n=13). For all subjects, TrA and PFM thickness were evaluated by ultrasonography during ADIM and PFC. RESULTS: The results of this study showed that TrA thickness increased during ADIM and PFC to a greater degree in the non-LBP group than in the LBP group (p < 0.01). PFM thickness increased more during PFC in the non-LBP group than in the LBP group (p < 0.05). Both groups showed greater increases in TrA thickness during ADIM than during PFC (p < 0.01), and greater increases in PFM thickness during PFC than during ADIM (p < 0.05). CONCLUSION: These results suggest that receiving verbal instructions on how to use each muscle for a specific maneuver was more beneficial than other verbal instruction. Further studies are needed determine how our results may be applied beneficially in research on this topic.

외복사근과 다열근에 대한 초음파 영상과 표면 근전도 측정방법의 신뢰도와 타당도 (Reliability and Validity of Ultrasound Imaging and sEMG Measurement to External Abdominal Oblique and Lumbar Multifidus Muscles)

  • The purpose of this study was to investigate intra-rater reliability and determine the validity of electromyography (EMG) measurements to represent muscle activity and ultrasonography (US) to represent muscle thickness during manual muscle testing (MMT) to external abdominal oblique (EO) and lumbar multifidus (MF). Twenty healthy subjects were recruited for this study and asked to perform MMT at differing levels. The subjects' muscle activity using EMG was measured by a ratio to maximum voluntary contraction (MVC) and root mean square (RMS) methods. The subjects' muscle thickness using US was measured by raw muscle thickness and change ratio of thickness to maximum (MVC) or resting condition. In three trials, measurements were performed on each subject by one examiner. The intra-rater reliability of measurements of EMG and US to EO and MF was calculated using intra-class coefficients. The intra-rater reliability of all measurements was excellent (ICC=.75~.98) in EMG and US. The conduct validity was calculated by one-way ANOVA with repeated measurements to compare whether the EMG and US measurements were different between MMT at different levels. There was only a significant difference between all grades at %MVC thickness measurement of US. These results suggest that a %MVC thickness measurement of US was a more sensitive and discriminate in all manual muscle testing grades. This information will be useful for the selection of US measurement and analysis methods in clinics.

엉덩관절 모음근의 수축을 동반한 플랭크 운동이 중심 근육의 두께에 미치는 영향 (The Effects of Plank Exercises with the Contraction of the Adductor Muscle of the Hip Joint on Core Muscle Thickness)

  • Purpose: The purpose of this study was to investigate the effects of plank exercises involving the contraction of the adductor muscle of the hip joint on core muscle thickness and to propose an effective plank exercise method. Methods: A total of 30 healthy young adults (17 males, 13 females) voluntarily participated in the study. The subjects were randomized to the prone plank exercise with hip adductor contraction (n=10), the prone plank exercise (n=10), and the supine plank exercise (n=10) groups. Muscle thickness measurements were taken prior to starting the exercise program and after completing the program at the end of a 4-week period. The muscle thickness of the rectus abdominis (RA), multifidus (MF), external oblique (EO), internal oblique (IO), and transverse abdominis (TrA) muscles were measured using ultrasonography. Each group performed the stipulated plank exercise five times a week as follows: 5 sets of 20 seconds during the first week, 5 sets of 30 seconds in the second week, 5 sets of 40 seconds in the third week, and 5 sets of 40 seconds in the last week. Results: The three different types of plank exercises all showed significantly increased thickness of the RA, MF, EO, IO, and TrA (P<0.05). Furthermore, changes in the thickness of both the MF and TrA were statistically more significant in the groups that did the prone plank exercise with the hip adductor contraction and the supine plank exercise than in the prone plank exercise group (P<0.05). Conclusion: The study results suggest that the prone plank exercise with hip adductor contraction is a more effective method for overall activation of the RA, MF, EO, IO, and TrA than the prone plank exercise and the supine plank exercise.

Comparison of Muscle Thickness of Abdominal Muscles According to Various Types of Abdominal Crunch Exercise

  • Purpose : The purpose of this study was to compare the effect of additional isometric contraction of trunk, shoulder, and hip muscles during abdominal crunch exercise on abdominal muscle thickness and to identify the most effective intervention for core muscle activation. Methods : This study was conducted on 22 healthy male adults. Subjects performed three types of crunch exercises (abdominal crunches accompanied by internal and external isometric rotation of the hip, horizontal shoulder adduction and abduction, and rotation of the trunk). The thickness of the transverse abdominis (TrA), internal oblique (IO), and external oblique (EO) were evaluated using ultrasonography. The collected data used one-way repeated ANOVA statistics. Wilcoxon signed-rank test of nonparametric statistics was used for post-test analysis. Results : The IO thickness was significantly lower than general abdominal crunch when shoulder adduction was added (p<.05). The crunch with shoulder abduction, hip external rotation, and ipsilateral trunk rotation was significantly higher than the abdominal crunch (p<.05). The EO thickness was significantly greater in the crunch with hip external rotation than in the abdominal crunch (p<.05). Conclusion : The level of contraction in abdominal muscles appears to vary when isometric contractions of the trunk, shoulder, and hip muscles are added to the abdominal crunch exercise. Therefore, the use of isometric contractions of other joints to selectively induce contraction of the abdominal muscles may be considered.

Effect of Electrical Muscle Stimulation Training With and Without Superimposed Voluntary Contraction on Rectus Femoris and Vastus Intermedius Thickness and Knee Extension Strength

  • Background: The superimposed technique (ST) involves the application of electrical muscle stimulation (EMS) during voluntary muscle action. The physiological effects attributed to each stimulus may be accumulated by the ST. Although various EMS devices for the quadriceps muscle are being marketed to the general public, there is still a lack of research on whether ST training can provide significant advantages for improving quadriceps muscle strength or thickness compared with EMS alone. Objective: To compare the effects of eight weeks of ST and EMS on the thicknesses of the rectus femoris (RF) and vastus intermedius (VI) muscles and knee extension strength. Methods: Thirty healthy subjects were recruited and randomly assigned to either the ST or EMS groups. The participants underwent ST or EMS training for eight weeks. In all participants, the thicknesses of the RF and VI muscles were measured before and after the 8-week intervention by ultrasonography, and quadriceps muscle strength was measured using the Smart KEMA tension sensor (KOREATECH Co., Ltd.). Results: There were significant differences in the pre- and post-intervention thicknesses of the RF and VI muscles as well as the quadriceps muscle strength in both groups (p < 0.05). RF thickness was significantly greater in the ST group (F = 4.294, p = 0.048), but there was no significant difference in VI thickness (F = 0.234, p = 0.632) or knee extension strength (F = 0.775, p = 0.386). Conclusion: EMS can be used to improve quadriceps muscle strength and RF and VI muscle thickness, and ST can be used to improve RF thickness in the context of athletic training and fitness.

베개 높이에 따른 SCM과 Upper trapezius의 근 두께, 근 긴장도, 근 활성화 측정비교 (Comparison of Thickness, Tension, and Activation of the Scm and Upper Trapezius Muscles According To The Pillow Height)

  • PURPOSE: The study aimed to analyze the effect of pillow height on upper trapezius and sternocleidomastoid (SCM) muscle activity and overall human body comfort to determine the optimal pillow height. METHODS: The study included 32 healthy college students, of both genders, who provided prior consent. It examined three pillow heights (flat, 6cm, 12cm) and measured the upper trapezius and SCM muscles using ultrasonography, myotone, and electromyography(EMG). Muscle activation, thickness, and fatigue were assessed. RESULTS: The study found significant differences in muscle thickness and muscle tone based on the pillow heights (flat, 6cm, and 12cm) with a p < .05. The SCM and upper trapezius muscles were thinnest at a pillow height of 6cm. Muscle tone in the SCM was lower at both 6cm and 12cm pillow heights. Post-hoc measurements showed significant differences in both the SCM and upper trapezius muscles thickness at the 6cm pillow height (p < .05). Also, significant differences in muscle tone were observed only in the SCM, particularly between the 6cm and 12cm pillow heights. CONCLUSION: The ultrasonography and myotone measurements showed a significant difference in muscle thickness and muscle tension, both of which were above 6cm, while there was no difference in muscle activation. Based on the strong correlation between muscle tension and muscle thickness with pillow height, this study concluded that the human body feels comfortable with a 6cm pillow height. Therefore, it is recommended to use a pillow height of 6cm when lying in a supine position.
