A Study on the Therapeutic Shoes for Diabetic Patients (당뇨병 환자의 치료용 신발에 대한 연구)

  • Purpose: To investigate the characteristics of the patients and therapeutic shoes for diabetic patients. Materials and Methods: Forty two diabetic patients who had their own therapeutic shoes which were prescribed somewhere else were studied from March 2003 to December 2003. There were 27 males and 15 females, and the mean age was 62.1 years (range, 49-72 years). Duration of diabetes was average 14 years (range, $6{\sim}30$ years), all had type 2 diabetes. Sensation was examined with 5.07 nylon monofilament. The route of purchasing the shoes, compliance to the prescribed shoes were investigated by interview. The shape of shoe, stiffness of upper, conformity of insole to the shape of the foot were recorded. In-shoe plantar pressure was measured in 15 patients. Results: Eighteen patients were insensate to the monofilament. Seven patients did not wear the therapeutic shoes, and only 18 of 35 patients were wearing the therapeutic shoes more than 6 hours a day. The shoes of 17 patients were prescribed by medical doctor and the rest were purchased by the recommendation of acquaintances or advertisement. Ulcer recurred in four of five patients to whom the shoe was prescribed by medical doctor and the cause of three recurrences were evident by just observing the foot and shoe. The therapeutic shoes were made from 11 different makers. Eight shoes were adequate for diabetic patients with respect to the material, shape of insole, type of shoe. In-shoe plantar pressure was examined in 15 patients and was less than 300 kPa in all patients. Conclusion: The therapeutic shoes for the diabetic patients need to be prescribed by medical doctor for selective patients with neuropathy or previous history of ulcer and follow-up examination is important to monitor the compliance of the patients and adequacy of the shoes.

Diagnostic and Therapeutic Laparoscopy for Abdominal Trauma: A Single Surgeon's Experience at a Level I Trauma Center

  • Purpose: Laparoscopy has various advantages over laparotomy in terms of postoperative recovery. The number of surgeons using laparoscopy as a diagnostic and therapeutic tool in abdominal trauma patients is increasing, whereas open conversion is becoming less common. This report summarizes a single surgeon's experience of laparoscopy at a level I trauma center and evaluates the feasibility of laparoscopy as a diagnostic and therapeutic tool for abdominal trauma patients. Methods: In total, 30 abdominal trauma patients underwent laparoscopy by a single surgeon from October 2014 to May 2020. The purpose of laparoscopy was categorized as diagnostic or therapeutic. Patients were classified into three groups by type of surgery: total laparoscopic surgery (TLS), laparoscopy-assisted surgery (LAS), or open conversion (OC). Univariate analysis was performed to determine the advantages and disadvantages. Results: The mechanism of injury was blunt in 19 (63.3%) and penetrating in 11 patients (36.7%). Eleven (36.7%) and 19 patients (63.3%) underwent diagnostic and therapeutic laparoscopy, respectively. The hospital stay was shorter for patients who underwent diagnostic laparoscopy than for those who underwent therapeutic laparoscopy (5.0 days vs. 13.0 days), but no other surgical outcomes differed between the groups. TLS, LAS, and OC were performed in 12 (52.2%), eight (34.8%), and three patients (13.0%), respectively. There was no significant difference in morbidity and mortality among the three groups. Conclusions: Laparoscopic surgery for selected cases of abdominal trauma may be feasible and safe as a diagnostic and therapeutic tool in hemodynamically stable patients due to the low OC rate and the absence of fatal morbidity and mortality.

Percutaneous Radiofrequency Facet Rhizotomy for Cervical Dorsal Ramus Syndrome

  • Objective : Radiofrequency facet rhizotomy[RFFR] has been widely performed for treatment of chronic neck pain caused by cervical dorsal ramus syndrome[CDRS]. To evaluate the therapeutic effectiveness of RFFR in the patients with CDRS, we analyzed patients with various cervical pathologic conditions. Methods : The therapeutic results in forty-four patients who underwent RFFR for CDRS from January, 2000 to December, 2002 were analyzed according to the underlying pathologic conditions causing CDRS. The pathologic conditions were sprain [33 cases], herniated nucleus pulposus [6], foraminal stenosis [4], and compression fracture [1]. The therapeutic results were evaluated one month after the operation and graded as excellent, good, fair of poor. Treatments were considered successful if the therapeutic results were graded as either excellent of good. Results : The overall success rate in all patients was 72.7%. The success fate for treatment of cases with cervical sprain was 87.9%, but treatment of cases with herniated nucleus pulposus, foraminal stenosis, and fracture showed unsatisfactory results [mean success rate was 27.3%]. Some patients complained of transient hypesthesia [4 cases] of transient dull pain at the electrode insertion sites [2 cases]. Conclusion : RFFR is an effective and safe treatment for CDRS caused by cervical sprain, regardless of the patient's age, the duration of the symptoms, and the presence of radiating pain.

Utilization Characteristics and User Satisfaction by Type of Physical Therapy Service in Senior Centers

  • Purpose: This study was conducted to identify the characteristics of physical therapy service utilization and user satisfaction depending on the experience with therapeutic exercise or ultrasound intervention among elderly persons using senior centers. Methods: The subjects were adults aged over 60 years (total 215) recruited in 40 senior centers located in each province in South Korea. Subjects responded to questions concerning overall demographics factors, utilized characteristics of physical therapy service and 12 variety user satisfactions with effectiveness, facilities, and therapist using a survey instrument. The collected data were analyzed by Fishers' exact tests and t-tests using the SPSS 21.0 program to compare the results of elderly persons who had or had not experienced exercise therapy or ultrasound therapy. Results: The participants that experienced ultrasound therapy or therapeutic exercise reported significantly higher overall results pertaining to effectiveness of physical therapy and a clear explanation from physical therapist's satisfaction than those who were non-experienced. Satisfaction with pain relief was significantly higher among elderly who experienced therapeutic exercise than those who did not. Those who underwent ultrasound therapy showed significantly higher satisfaction with facilities and location than those who did not. Conclusion: The results of this study suggest that satisfaction among users differs by type of physical therapy. In the future, physiotherapy services provided in senior centers needs to be designed to improve the effectiveness of physical therapy, professionalism of physical therapists and comfort of facility.

A Study on the development therapeutic environmental rating scales for the elderly people with dementia in nursing homes - Focused on the TESS-NH environmental rating scale Analysis (치매성 고령자를 위한 노인요양시설의 치료적 환경평가척도 개발에 관한 연구 - TESS-NH 환경평가척도 분석을 중심으로)

  • Purpose: AIn recent years, the rapid progress of aging and the increase in the elderly people with dementia is becoming a big social issue. Therefore, this paper studies the development of therapeutic environmental rating scales for the elderly people with dementia in korean nursing homes. Methods: We identify the development process and structure of TESS-NH and understand the details of its nature and features. Then, based on the detailed items, we provide major scale factors which do influence on therapeutic environmental rating scales using PCA. Results: In the results, TESS, in the case of the initial stage of TESS-NH, has an advantage to evaluate on physical environments for a short period of time. TESS-2 + sets a large rage of activities of the elderly people with dementia, and tries to evaluate ongoing cares. TESS-NH has a complete set of well-thought-out assessment features for improving quality of care (Quality of Care) On the other hand, the main factors affecting the therapeutic environmental rating scales in nursing homes are "a consideration for facility management (Care for facilities' maintenance)", "a consideration for spatial cognition (Care for spatial cognition) ", and "a consideration for the safety (Care for safety)". Implications: In the future, to develop the therapeutic environmental rating scales on Korea's dementia nursing home, we should actively consider cares of spaces and facility utilization which the residents (the elderly people with dementia) are more secure and comfortable.

Effects of reimbursement restriction on pharmaceutical expenditures : A case of Ginkgo biloba (은행잎 제제의 급여제한 정책효과 분석)

  • Since May 1st in 2008, the products of ginkgo biloba extract have had to be used with the patient's out-of-pocket payment due to reimbursement restriction guidelines. This study aims to analyze the policy effects of reimbursement restriction on pharmaceutical expenditures using interrupted time series(ITS) analysis. We retrieved monthly NHI claims data for the period between May, 2005 and December 2009. The ingredients identified as a substitute for ginkgo biloba have similar indications based on the similar pharmacological activities. The effects of changes in reimbursement scope were evaluated both for all relevant pharmaceuticals within the same therapeutic class and for 2 separate groups : ginkgo biloba's and its substitutes. According to the study results, restrictions on reimbursement scope resulted in savings of the drug expenditures in the targeted therapeutic class. Direct restriction on ginkgo biloba was associated with a decrease in expenditure level by 60.1% and changes in trend from an average increase rate of 1.4% to an average decrease rate of 1.5% for the therapeutic class, with a dramatic decrease in expenditure level(-191.5%) for ginkgo biloba itself, but with an increased expenditure level(+50.1%) and changes in trend from an average increase rate of 2.0% to an average decrease rate of 1.0% for the substitute group. Further policy to restrict nicergoline was associated with additional decrease in expenditure level for the therapeutic class. Additionally, we could identify the balloon effect - a new policy squeezing one part results in bulging out elsewhere. After the restriction of ginkgo biloba, the utilization of and expenditures on its substitutes increased significantly. In conclusion, we demonstrated that consecutively introduced policies effectively reduced overall expenditures on the therapeutic class of interest. Some ingredients played as a substitute while others did not. Further studies need to be conducted to identify which factors determine a substitute.

A Case Study on Therapeutic Music Making for Enhancement of Communications in Single-Mother Families (여성 한부모 가족의 의사소통 증진을 위한 치료적 음악 만들기 사례연구)

  • The purpose of this study is to discover how therapeutic music making affects the way of communications in single-parent families. To do this, the study conducted a therapeutic music making program in two families consisting of a mother and a child. Songwriting, Ostinato-creating, improvisation imitation, and ensemble were used as musical interventions. The results of this study are following. First, the participant group B improved from 59 to 69.5 point after the program. However, the group A decreased from 81 to 75.5 point mainly due to frequent absences, differences in applying music. Second, the analysis of the results of verbal, nonverbal and musical responses from the program suggests that the group B gradually improved functional communications as each session develops. For the case of group A, at the initial stage, dysfunctional communications in verbal and nonverbal ones could be found. After the eighth session however, functional communications had increased. The result of the study shows that overall therapeutic music making has been an effective musical intervention in improving communication in single-parent families.

Retrospective Statistical Analysis on Patients Admitted to a Korean Medicine Hospital by Traffic Accident (교통사고로 한방병원에 입원한 환자에 대한 후향적 통계 분석)

  • Objectives: The purpose of this study was to investigate characteristics of patients who were admitted to an oriental medicine hospital by traffic accident. Methods: The medical charts of 346 patients admitted to an oriental medicine hospital from June 1, 2017 to May 31, 2018 were analyzed. The Numbering Rating Scale (NRS) and duration of hospitalization were used to evaluate characteristics of the patients. Results: Acupuncture, Moxibustion, Infralux were used to treat all the patients. The most frequently used herbal medication was Danggwisu-san(22.25%). 87 patients(25.14%) visited the outpatient department after being discharged from the hospital. The most frequent complaint in terms of pain was cervical pain(82.7%) and of systemic symptom was headache(23.7%). Men and younger aged patients showed higher therapeutic effect than women and older ages. The most common duration of hospitalization was 2~4 days(42.73%) and positively correlated with therapeutic effect. The most frequent interval between time of injury and visit to the hospital was from 0-1 days(68.90%) and showed no relationship with therapeutic effect. The most frequent admission pathway was "Directly to the hospital"(57.51%). Admission pathway was proportionally associated with duration of hospitalization and treatment results were not. The most common vehicle type involved in the traffic accidents was a sedan(72.25%), accident type was a rear-end collision(43.64%) and showed no relationship with therapeutic effect. Conclusions: In this study, therapeutic effects were highly correlated among men, younger ages, and duration of hospital stay, and was not for interval days, admission pathway, vehicle type, and accident type.

The Effects of Programs Using Strategies for Promoting Self Efficacy in Patients with Lung Cancer (폐암극복을 위한 자기효능 증진 프로그램의 효과)

  • Purpose: This study investigated the effects of a 'overcoming cancer program' on knowledge, self efficacy, and quality of life, therapeutic compliance for patients with lung cancer. Method: Research design of this study was a nonequivalent control group quasi-experimental study. Subjects for this study were 16 lung cancer patients for the control group, and 12 lung cancer patients for the experimental group. The experimental group participated in the program once a week for 4 weeks. Data were collected before and after the program. Nonparametric statistics were used to analyze the data. Results: The results of this study were as follows: In the pretest, there were no significant differences in general characteristics, knowledge, self efficacy and quality of life between the two groups. In the posttest, there were significant differences in knowledge, self efficacy between the experimental and the control groups. But there were no significant differences in therapeutic compliance and quality of life between the two groups. Conclusion: From the results above, it can be concluded that program was effective to improve knowledge and self-efficacy for patients with lung cancer.

Radiation Therapy of Head and Neck Cancer with CO-60 HDR Transcatheteric Irradiation (고선량율 강내 조사를 이용한 두경부암의 방사선 치료)

  • The basic strategy of irradiation is to deliver a dose to the cancer that is high enough to make cancer cells incapable of reproduction, while keeping the doses to the various healthy tissues below tolerable levels. In order to improve local control and survival, as a boost therapy after external radiotherapy, high dose rate transcatheteric irradiation using remote control afterloading system(RALSTRON-20B) was used for twelve patients with head and neck cancers. Present results showed complete remission of cancer in 9 out of 12 patients without treatment related complications. Although this procedure is easy to operate, well trained skillful hand is essential for good results. Furthermore our experience suggested that meticulous treatment planning should be developed for better results.
