Background : The atopic dermatitis patient and their families are looking for safer herb-medicine treatments that possess therapeutic effects, but without the recurrence of symptoms and long-term harmful consequences that can result from other treatment. However, for many reasons, including methodological difficulties and lack of high quality study like randomized clinical study and systematic review, there are no consistent clinical guide line for atopic dermatitis in Traditional Korean Medicine. Objectives : Evidence-based clinical practice guidelines support clinical decision-making by making recommendations to guide clinical practice. The purpose of this study was to develop Traditional Korean Medicine clinical practice guideline for atopic dermatitis Result : The future guide should be based on scientific evidence and include the followings: (1) diagnosis of atopic dermatitis (2) the pattern identification of atopic dermatitis (3) classification of Sasang Constitution (4) efficacy assessment (5) treatment guideline (6) education for patients and care givers Conclusion : The development of Traditional Korean Medicine clinical practice guideline for atopic dermatitis is needed.

Background and Objectives Surgery for harvesting costal cartilage is often required for revision septorhinoplasty due to a lack of septal cartilage in patients with a severely contracted nose, and postoperative scarring on the anterolateral rib cage commonly requires additional treatment. This study aimed to evaluate the therapeutic efficacy and safety of combined polydeoxyribonucleotide (PDRN) and microlens array (MLA)-type nanosecond-domain neodymium (Nd):yttrium-aluminum-garnet (YAG) laser treatment for postoperative scars after costal cartilage harvest surgery. Materials and Methods Nine Korean patients with scars after costal cartilage harvest surgery treated with PDRN injections and MLA-type Nd:YAG laser treatments were retrospectively reviewed. Results Most of the scar lesions exhibited clinical improvement at 2 weeks after PDRN and MLA-type nanosecond-domain laser treatments, and the lesions further improved after adding more treatment sessions. The median Vancouver Scar Scale (VSS) score decreased from 6 (interquartile range [IQR]: 6-7) before combined intralesional PDRN injection and MLA-type, nanosecond-domain Nd:YAG laser treatments to 3 (IQR: 2-4) thereafter. Patient satisfaction after the combination treatments was rated as satisfactory. None of our patients reported major adverse events. Conclusion This case series study demonstrated that combined PDRN and MLA-type, nanosecond-domain Nd:YAG laser treatments are effective and safe for treating scars from costal cartilage harvest surgery.
아동에게 있어서 말기 질환의 범위는 악성종양을 비롯하여 선천성 대사이상, 염색체 이상, 선천성 기형, 신경근육계 질환, 기타 다른 불치의 상태까지 포함한다. 병원환경에서 이러한 말기 질환 아동은 질병의 치료 과정과 관련하여 공포심, 분노, 좌절감을 경험할 수 있으며 통제된 병원 환경안에서 자율성을 제한받기 때문에 통제감을 상실하고 무기력해질 수 있다. 본 연구에서는 환경적 지지 모델(Contextual Support Model of Music Thterapy)을 중심으로 음악치료가 병원환경에 있는 말기질환자들의 정서 및 기능 변화에 긍정적 변화를 줄 수 있는지를 알아보았다. 환경적지지 모델 음악치료는 음악 환경의 구조적인 특성(structure), 자발성 지지(autonomy support) 그리고 참여(involvement)라는 세 가지 요소로 구성된다. 그리고 이러한 치료적 구성 요소로 이루어진 음악 환경은 아동의 활동을 지지하고 격려하도록 도와준다. 이 세 가지 치료적 요소를 중심으로 하여 본 연구는 말기질환 아동과의 음악치료사례를 분석하여 말기 질환 아동의 행동이 환경적 지지 역할을 하는 음악 안에서 어떠한 유형으로 나타나는지에 대해서 알아보았다. 본 연구의 대상자는 만 10살의 급성 림프성 백혈병 여아로 총 9회의 세션을 통한 음악 치료 활동 내용을 질적 연구방법으로 사용하여 내용분석을 하였다. 내용분석은 환경적 지지모델의 세 가지 치료적 요소 중심으로 본 연구자가 설정한 행동들을 기준으로 이루어졌다. 환아와의 주 음악 치료 활동은 악기 연주하며 노래부르기, CD플레이어로 노래 들으면서 노래 부르기, 노래 만들기, 노래 가사 탐색하기 활동으로 이루어졌다. 본 연구의 결과는 다음과 같다. 환경적인 구조적 특성(Structure) 측면에서는 음악치료는 노래를 부를 수 있는 환경과 악기를 연주할 수 있는 환경을 제공해주고 음악 만들기 활동을 할 수 있는 준비를 해줌으로써 아동이 성공적으로 음악 활동을 할 수 있도록 방향을 제시해주었다. 자발성 지지(Autonomy support) 측면에서 음악치료는 노래를 부르거나 악기를 연주할 때 여러 가지 선택의 기회를 제공함으로써 아동이 자발적으로 참여할 수 있도록 하였다. 참여(Involvement)의 측면에서는 치료사가 아동에게 긍정적인 강화를 제시함으로써 아동이 긍정적으로 경험하고 공유된 활동 안에서 치료사와 상호 협력적으로 음악활동을 진행하는 모습을 볼 수 있었다. 이와 같이 음악치료는 환경적으로 지지 역할을 하며 정서적으로 위축되어있는 아동에게 자발성과 독립성을 개발시켜주고 병원 환경이라는 제약된 환경 안에서 자율성을 촉진시켜줄 수 있음을 보여주었다. 이러한 음악 치료의 효과를 근거로 하여 입원 환경에 있는 아동들을 위한 더 다양한 접근 방법의 음악치료 연구가 이루어지기를 기대한다.
The role of the neonatal nurse specialist has been well established over the past decade and now reform in 21st century. Neonatal nurse specialists responsibilities in caring for critically and long-term chronically ill infants and their families are very important. Neonatal nurse specialists have a two fold responsibility in caring for these infants. First, through acquiring advanced practice education in complex neonatal care and diagnostic skills, neonatal nurse specialists meet the physiologic needs of the infant. Second, neonatal nurse specialists provide a more holistic approach to their care through evaluating the family in treatment plans and involving the family in discharge planning for the infant. In some institutions, neonatal nurse specialists are directly involved in institutional and/or home follow-up care and case management also. It is the neonatal nurse specialists responsibility to function collaboratively with the multidisciplinary team in managing critically or chronically ill infants from admission to discharge. The role of the neonatal nurse specialist case manager can be described as one that focuses on individualized care of the infant, while providing continuity of care to both the infant and family. The neonatal nurse specialist's role will vary depending on the neonatal intensive care unit(NICU). Therefore, the multidisciplinary collaborative approach to long-term management of infants in the NICU is extremely important to provide successful transition to home or to long-term rehabilitative care facilities because care for the chronically ill infant is complex and multifaceted. I suggest the role of neonatal nurse specialist in 21st century are as follows. 1. Diagnostic/patient assessment 2. Management of patient health/illness 3. Administering/monitoring therapeutic interventions and regimens 4. Monitoring/ensuring quality of health care practices 5. Organization and work role 6. Helping role 7. Teaching/coaching role 8. Management of rapidly changing situations 9. Consulting role The advanced practice nursing model of care delivered by neonatal nurse specialist's in the NICU incorporates medical and nursing role functions and emphasizes holism, caring, and a health perspective for critically and chronically ill neonates and their families.
본 연구는 의사로부터 고혈압으로 진단받은 사람들을 대상으로 개인 건강행태 및 지역 보건의료수준이 고혈압의사진단에 미치는 영향을 파악하고자 실시하였다. 연구는 2010년도 지역사회건강조사대상 만 19세 이상 성인 229,229명을 대상으로 개인특성에 따른 고혈압 의사진단의 차이 비교는 카이제곱 검정을 사용하였으며, 개인 및 지역수준 변수를 고려한 고혈압 의사진단의 관련지표의 지역 간 변이 파악은 다수준 로지스틱회귀분석를 실시하였다. 연구결과 개인건강행태수준에서 연령수준이 높을수록, 최종학력이 높을수록, 기초생활수급이 무일 때, 체질량지수가 높을수록, 일회음주량이 많을수록 고혈압 의사진단 경험률이 높았다. 또한 보건의료기관을 이용한 사람이, 한 달 동안 혈압측정횟수가 높을수록, 주관적 스트레스 수준이 높을수록, 우울감 경험이 있을 때 고혈압 의사진단 경험률이 높았다. 반면에 고용형태가 임금근로자인 경우, 고용주 및 자영업자에서, 걷기일수가 많을수록, 현재 흡연여부 경험이 적을수록, 민간의료보험에 가입한 사람에서 고혈압 의사진단 경험률이 낮았다. 건강행태 및 지역의 보건의료수준에 따라, 고혈압의사진단에 미치는 영향이 크다는 것을 알 수 있었으며, 추후 많은 연구에서 지역수준을 고려한 다수준 분석이 필요할 것으로 사료된다.

Autism spectrum disorder (ASD) is a neurodevelopmental disorder marked by impaired social communication and repetitive, restricted behaviors and activities. The prevalence of ASD has been increasing for the past 2 decades, but evidence-based therapeutic approaches are lacking for patients with ASD. To date, there is no cure for the core symptoms of ASD, and the existing treatments focus on improving the patient's function and adaptation by using behavioral intervention methods. Behavioral interventions have been proven to show the greatest effect when applied before the age of 2 years, for at least 40-60 hours per week. Many clinicians and ASD families are unfamiliar with the treatment methods, and consequently, may seek unproven and potentially hazardous methods. The purpose of this article was to present an extensive and updated review on evidence-based ASD behavioral interventions that are commonly used in clinical settings.
Purpose: The purpose of this study is to examine how well patients who had hip arthroplasty comply with medical regimens given to them after the operation. Method: The subjects of the study were patients who had arthroplasty at P Hospital between April 1, 2001 and August 30, 2002. 20 patients of the subjects experienced complications after the operation and the other 20 did not. Data from a survey using the qustionnaire were statistically analyzed in terms of real number, percentage point, mean and standard deviation by using $X^2$-test, t-test and ANOVA. Result: 1) the surveyed patients were significantly different in the compliance of medical regimen among them according to their education background as one of the subjects general characteristics. 2) It was found that the group of complication was higher in the compliance of medical regimen than that of non-complication. The two groups showed statistically significant difference with each other in the degree of compliance with therapeutic instructions than the experimental group in terms of the maintenance of abduction after the operation, training instructions on step-by-step basis, urination cotrol on bed, accurate use of crutch, compliance with medication, balance among medical treatment, training, leisure, rest and nutrition, instructions by physicians, nurses and physical therapists, use of low armchairs and toilet bowels and no bending of the body forward, and use of a non-operated leg in case of go upstairs or downstairs. Conclusion: It seems necessary to develop systematic and sessional education programs for improving the compliance of medical regimen, ultimately reducing complications following hip arthroplasty.
Objectives: Epilepsy is a chronic disease that requires long-term treatment and intervention from health workers. Medication adherence is a factor that influences the success of therapy for patients with epilepsy. Therefore, this study aimed to analyze the role of pharmacists in improving the clinical outcomes of epilepsy patients, focusing on medication adherence. Methods: A scoping literature search was conducted through the ScienceDirect, PubMed, and Google Scholar databases. The literature search included all original articles published in English until August 2023 for which the full text was available. This scoping review was carried out by a team consisting of pharmacists and neurologists following the Preferred Reporting Items for Systematic reviews and Meta-Analyses (PRISMA) Extension for Scoping Reviews and the Joanna Briggs Institute guidelines, including 5 steps: identifying research questions, finding relevant articles, selecting articles, presenting data, and compiling the results. Results: The literature search yielded 10 studies that discussed pharmacist interventions for patients with epilepsy. Five articles described educational interventions involving drug-related counseling with pharmacists. Two articles focused on similar pharmacist interventions through patient education, both verbal and written. Three articles discussed an epilepsy review service, a multidisciplinary intervention program involving pharmacists and other health workers, and a mixed intervention combining education and training with therapy-based behavioral interventions. Conclusions: Pharmacist interventions have been shown to be effective in improving medication adherence in patients with epilepsy. Furthermore, these interventions play a crucial role in improving other therapeutic outcomes, including patients' knowledge of self-management, perceptions of illness, the efficacy of antiepileptic drugs in controlling seizures, and overall quality of life.
Home visiting as a Public Health Horsing function is believed to be of therapeutic value to, the patient. However, home visiting is time consuming and expensive. Is the gain in knowledge and treatment for patients with Tuberculosis in Korea enough to make the necessary outlay in finances and personnel worthwhile\ulcorner While this study does not attempt to completely answer this question it does, under the following objectives, attempt to answer part of the question. The objectives of the study were to l) ascertain if there is a difference between patients, who receive home visits from the public Health Nurse and those who do not, in the following areas: a) their compliance with medical regimen, b) their ability to answer general questions about Tuberculosis, and c) their compliance with medical advice concerning prevention (B.C.G. immunization) and early diagnosis (contact X-rays), and 2) to determine if there is any correlation between the patient's answers to questions about Tuberculosis and his action both in the areas of treatment and prevention. The patients participating in the study were all newly diagnosed patients at Kwangju Christian Hospital. A control group and an experimental group were selected. The patients in the control group were seen according to the regular schedule at the Kwangju Christian Hospital except that they received no home visits from the Public Health Nurses. The patients in the experimental group were visited on an average of three times during the first two months of their treatment by the investigator, a Public Health Nurse. At the end of two months the patients in the two groups were compared as to compliance both in the treatment and preventive areas. They were also compared according to their answers to a questionnaire regarding both the prevention and treatment of Tuberculosis. The following results were obtained : 1. Patients in the experimental group (68.2%) showed a significantly higher compliance rate for medical treatment than patients in the control group (43.2%). 2. Patients in the experimental group (87.5%) showed a higher compliance rate for B.C.G. immunization than those in the control group (40%). 3. Women patients in the experimental group showed a higher mean score (7.2$\pm$2.6) on the questionnaire than did women in the control group (6.2$\pm$3.4). The results of this study seem to indicate that while home visiting is beneficial to the patient with Tuberculosis as far as treatment is concerned, something more concrete needs to be done if home visiting is to help the patient learn more about Tuberculosis and its prevention. Further study is indicated in the following areas: 1) A similar type of study over a longer period of time involving more subjects and using Korean Public Health Nurses to make the home visits. 2) Study to develop an adequate approach to education considering the problems unique to patients with Tuberculosis living in Korea.
골다공증 환자들의 치료순응도와 그 관련요인을 알아보기 위하여 1999년 4월에서 6월 사이에 골다공증으로 진단된 고령지역의 3개 보건진료소와 보건소 관할지역에 거주하는 40세에서 69세 사이의 여성 140명을 대상으로 골밀도 검사 사업을 실시한지 약 1년 후인 2000년 4월에서 5월 사이에 약물치료순응도와 치료순응도 관련 이론적 모형 변수에 대한 설문조사를 실시하였다. 연구 대상자 중 규칙적 치료군은 12.1%, 간헐적 치료군은 53.6%, 미치료군은 34.3%였다. 단순분석 결과, 일반적 특성 중 연령이 높을수록, 본인이 인지하는 질병의 정도가 심하다고 생각할수록, 의사의 치료권유를 받은 경우에 치료순응률이 높았다(p<0.05). 환자역할수행요인 중 합병증(골절)의 가능성과 골다공증의 심각성이 치료 순응과 유의한 관련성이 있었다(p<0.05). 그리고 치료가 유익하다고 생각할수록, 치료의 장애도가 낮을수록 치료경험율이 높았다(p<0.01). 경로분석 결과 모형의 적합도는 양호하였다. 합병증(골절)의 가능성 인지에는 연령, 학력, 골다공증에 대한 매스컴 접촉/보건교육 경험, 치료에 대한 가족의 관심 등이 정(正)의 직접효과를 미쳤으며, 골다공증의 심각성 인지에는 연령, 학력, 의사의 치료권유, 치료에 대한 가족의 관심 등이 정(正)의 직접효과를 미쳤다. 치료의 유익성 인지에는 의사의 치료권유와 치료에 대한 가족의 관심이 정(正)의 직접효과를 미쳤으며, 치료의 장애도 인지에 대하여는 연령, 경제적 수준, 의사의 치료권유와 치료에 대한 가족의 관심이 부(負)의 직접효과를 미쳤다. 골다공증 치료순응도에는 골다공증의 심각성이 정(正)의 직접효과, 치료의 장애도가 부(負)의 직접효과를 미쳤다. 수정요인 중 본인이 인지한 질병의 정도는 치료순응도에 정(正)의 직접효과를 미쳤으며, 연령, 경제적 수준, 학력, 의사의 치료권유, 가족의 치료에 대한 관심 등은 환자역할 수행요인에 대한 영향을 통해 치료순응도에 정(正)의 간접효과를 미쳤다. 치료순응도에 총 효과 가장 큰 요인은 치료의 장애도였으며, 그 다음으로 본인이 인지하는 질병의 정도, 연령, 의사의 치료권유, 치료에 대한 가족의 관심, 골다공증의 심각성 순이었다. 치료의 장애도는 연령, 경제적 수준, 의사의 치료권유와 치료에 대한 가족의 관심의 영향을 받았다. 또한 낮은 연령일수록 치료순응도가 낮았다. 향후 골다공증 환자의 치료순응도를 향상시키기 위하여는 자신의 질병상태를 올바르게 인식하도록 하고, 의사의 치료 권유와 치료에 대한 가족의 관심 등 골다공증 치료에 지지적인 환경을 조성하여 심리적 및 현실적 장애를 감소시키는 것이 중요하리라 생각되며, 효율적이고 지속적인 환자관리체계의 개발이 필요하겠다.

저작권법 제136조에 따라 5년 이하의 징역 또는 5천만 원 이하의 벌금에 처해질 수 있습니다.
제 14 조 (유료서비스)
① 당 사이트 및 협력기관이 정한 유료서비스(원문복사 등)는 별도로 정해진 바에 따르며, 변경사항은 시행 전에

제 5 장 계약 해지 및 이용 제한

일시정지, 이용계약 해지 할 수 있습니다.
제 17 조 (전자우편주소 수집 금지)
회원은 전자우편주소 추출기 등을 이용하여 전자우편주소를 수집 또는 제3자에게 제공할 수 없습니다.
제 6 장 손해배상 및 기타사항
제 18 조 (손해배상)
당 사이트는 무료로 제공되는 서비스와 관련하여 회원에게 어떠한 손해가 발생하더라도 당 사이트가 고의 또는 과실로 인한 손해발생을 제외하고는 이에 대하여 책임을 부담하지 아니합니다.
제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.
[부 칙]
1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
