The Application of Vacuum-Assisted Closures According to the Texas Staging System in the Treatment of Infective Diabetic Foot Ulcers (감염성 당뇨 족부 궤양 치료에서 Texas 기준 체계에 따른 음압치료 적용)

  • Purpose: A group of patients who were hospitalized for diabetic foot ulcers was classified according to the University of Texas Staging System for Diabetic Foot Ulcers, and we attempted to evaluate whether this staging system could be a criterion for treatment success using vacuum-assisted closure (VAC) technique. Materials and Methods: A total of 32 patients were diagnosed with diabetic foot ulcers according to the University of Texas Staging System for Diabetic Foot Ulcers. Of these, 24 patients who were evaluated as stage B according to the staging system were classified as Group 1, and 8 patients in stage D were classified as Group 2. After applying VAC, the treatment success rate was compared by evaluating the size and severity of ulcers between the two groups. Results: The grade of granulation after VAC was on average 3.75±0.53 in Group 1 and 2.25±0.71 in Group 2. There was better granulation after VAC application in Group 1 (p<0.01). The success rate of the treatment was 22 cases (91.67%) in Group 1 and one case (12.5%) in Group 2. Thus there were statistically significant differences in the success rate of treatment between groups 1 and 2 (Pearson's chisquare test, p=0.01; odd ratio 77.00, 95% confidence interval [CI] 1.26~14.66; relative risk 4.30, 95% CI 1.26~14.66). Conclusion: These results suggest that there was a higher success rate of treatment with VAC in stage B patients. The University of Texas Staging System for Diabetic Foot Ulcers can thus be an index for applying VAC to patients with infective diabetic foot ulcers.

Treatment Outcomes of Patients with Multidrug-Resistant Tuberculosis: Comparison of Pre- and Post-Public-Private Mix Periods

  • Background: This study compared the treatment outcomes of patients with multidrug-resistant tuberculosis (MDR-TB) before and after the implementation of public-private mix (PPM). Factors affecting treatment success were also investigated. Methods: Data from culture-confirmed pulmonary MDR-TB patients who commenced MDR-TB treatment at Pusan National University Hospital between January 2003 and December 2017 were retrospectively reviewed. Patients were divided into two groups in terms of PPM status: pre-PPM period, patients who commenced MDR-TB treatment between 2003 and 2010; and post-PPM period, patients treated between 2011 and 2017. Results: A total of 176 patients were included (64 and 112 in the pre- and post-PPM periods, respectively). 36.9% of the patients were resistant to a fluoroquinolone or a second-line injectable drug, or both. The overall treatment success rate was 72.7%. The success rate of post-PPM patients was higher than that of pre-PPM patients (79.5% vs. 60.9%, p=0.008). Also, loss to follow-up was lower in the post-PPM period (5.4% vs. 15.6%, p=0.023). In multivariate regression analysis, age ≥65 years, body mass index ≤18.5 kg/m2, previous TB treatment, bilateral lung involvement, and extensively drug-resistant (XDR)- or pre-XDR-TB were associated with poorer treatment outcomes. However, the use of bedaquiline or delamanid for ≥1 month increased the treatment success. Conclusion: The treatment success rate in MDR-TB patients was higher in the post-PPM period than in the pre-PPM period, particularly because of the low rate of loss to follow-up. To ensure comprehensive patient-centered PPM in South Korea, investment and other support must be adequate.

Long-term analysis of tropospheric delay and ambiguity resolution rate of GPS data

  • Long-term GPS data analysis was performed in order to analyze the seasonal variation of tropospheric delay and the success rate of the ambiguity resolution. For this analysis, a total of 57 stations including 10 IGS stations in East Asia were processed together with double-differenced observables using Bernese GPS Software V5.0. The time span for this study ranges from 2002.0 to 2012.5 (10.5 years). The average baseline length is 339.0 km and the maximum reaches up to 2,000 km. The analysis is focused on two things: the annual variation of the tropospheric delay and the ambiguity resolution rate. The tropospheric delay is closely related to the weather condition, especially relative humidity, therefore it was estimated that the maximum would be in summer, while reaching its minimum in winter with the apparent seasonal variations. On the contrary, however, the success rate of the ambiguity resolution shows the opposite pattern: its maximum was in winter and minimum in summer. The fact seems to be induced by the surrounding conditions; that is, the trees thick with leaves near the GPS antenna interfere with GPS signals in summer. This seems to confirm partly that there is a distinct trend in the decreasing success rate since 2006 because the trees are growing every year. It is necessary to eliminate the factors that degrade the GPS quality and the tropospheric modeling for Korea needs to be studied further.

  • In this paper, we proposed some valuable analysis results of cultivation campaign marketing in A mobile telephone company. The cultivation campaign marketing is a new outbound TM campaign strategy composed of 4 steps for new subscribers. During the analyses, attrition rate of A mobile company, outbound TM rejection rate, contact rate/call success rate and additional service attrition rate after subscription are measured as an effect of cultivation campaign. The results showed that there were no statistical significance in improving retention rate between customers experienced 4 step cultivation campaign and control group. However, the cultivation campaign had a better effect on improving response rate and success of selling additional service product of mobile company compared to general selling campaign. This is because cultivation campaign group had been taking care of from subscription time. It is suggested that cultivation campaign had better confined customers more likely to buy additional service to outbound TM for the sake of financial efficiency.

  • When a loss of coolant accident (LOCA) occurs in a nuclear power plant, accident scenarios which can prevent core damage are defined based on break size. Current probabilistic safety assessment evaluates that core damage can be prevented under small-break LOCA (SBLOCA) and steam generator tube rupture (SGTR) with rapid cool down (RCD) strategy when all safety injection systems are unavailable. However, previous research has pointed out a limitation of RCD in terms of initiation time. Therefore, RCD success criteria estimation based on allowable coping time under a SBLOCA or SGTR when all safety injection systems are unavailable was performed based on time-line and thermal-hydraulic analyses. The time line analysis assumed a single emergency operating procedure flow, and the thermal hydraulic analysis utilized MARS-KS code with variables of break size, cooling rate, and operator allowable time. Results show while RCD is possible under SGTR, it is impossible under SBLOCA at the APR1400's current cooling rate limitation of 55 K/hr. A success criteria map for RCD under SBLOCA is suggested without cooling rate limitation.

Clinical Features and Factors Affecting Success Rate of Air Reduction for Pediatric Intussusception (공기 정복술을 시행 받은 소아 장중첩증 환자들의 치료 결과 및 성공률에 영향을 미치는 요인)

  • Air reduction is a safe, effective, and fast initial treatment for pediatric intussusception. There is low dose radiation exposure. Factors affecting outcomes of air reduction were analyzed by reviewing the clinical features and results of treatment. A total of 399 out of 485 patients with pediatric intussusceptions were treated at the Seoul National University Children's Hospital from 1996 to 2009. All of the patients received air reduction as the first line of treatment. Clinical features such as gender, age, seasonal variation, symptoms, signs, types, pathologic leading point, and treatment results including success rate, complication, recurrence, NPO time, and duration of hospitalization were reviewed. The Pearson chi-square, student T-, and logistic regression tests were used for statistical analysis. P-value less than 0.05 was considered to be statistically significant. The prevalent clinical features were: male (65.4 %), under one-year of age (40.3 %), ileocolic type (71.9 %), abdominal pain (85.4 %), and accompanying mesentery lymph node enlargement (2.2 %). The overall success rate for air reduction was 78.4 % (313 of 399 patients), and the perforation rate during reduction was 1.5 %. There were 23 recurrent cases over 21.6 months. All were successfully treated with re-do air reduction. Reduction failures had longer overall NPO times (27.067hrs vs. 43.0588hrs; p=0.000) and hospitalization durations (1.738d vs. 6.975d; p=0.000) compared to the successful cases. The factors affecting success rates were fever (p=0.002), abdominal distension (p=0.000), lethargy (p=0.000) and symptom duration (p=0.000) on univariate analysis. Failure rates were higher in patients with symptom durations greater than 24 hours (p=0.023), and lethargy (p=0.003) on multivariate analysis. Air reduction showed high success rates and excellent treatment outcomes as the initial treatment for pediatric intussusception in this study. Symptom duration and lethargy were significantly associated with reduced success rates.

A Study of Influencing Factors on World Handball Win-Loss using the Decision Tree Analysis (의사결정나무 분석을 통한 세계핸드볼 승패결정요인 분석)

  • The purpose of this study is to collect official records of the 2019 Men's and Women's Handball World Championships to identify important shooting variables that determine the team's record of winning or losing. After collecting 192 games of men's and women's national teams from 24 countries and verifying the difference in competition records according to the winning and losing groups, the decision tree method, one of the data mining techniques, is analyzed. According to the analysis, the 9m shooting success rate and Near shooting success rate were the most important factors for both men and women. Men win 83.3% if the 9m shooting success rate is 32.5% or higher and the Near shooting success rate is 67.5%, and women win 75% if the 9m shooting success rate is 75% or more and the Near shooting success rate is 51%. Also, the women's yellow cards are considered important variables that determine victory or defeat. In conclusion, both men and women were able to identify the factors of winning and losing decision shooting, but follow-up studies are needed considering the relativity of various record variables and performance in future handball.

  • Purpose : This research was designed to provide basic data for advanced pre-hospital airway management by comparing the ease of intubation, the success rate and the time for intubation performed with Macintosh Laryngoscope, Gum Elastic Bougie and Pentax AirWay Scope on the floor and table. Methods : Intubation was performed 30 times in total, including 5 times for each of intubation using Macintosh Laryngoscope, Gum Elastic Bougie and Pentax Airway Scope on the table and floor, only on subjects who passed the practice test of the national exam for paramedics. Data were collected by measuring the ease of intubation, the success rate and the time for intubation, and then analyzed by descriptive analysis, paired t-test, ANOVA using SPSS 18.0. Results : 1. There was a significant difference in the ease of intubation according to intubation devices. 2. There was a significant difference in the ease of intubation according to table height for intubation. 3. There was a significant difference in the success rate according to intubation devices. 4. There was no significant difference in the success rate according to table height for intubation. 5. There was a significant difference in the intubation time according to intubation devices. 6. There was a partially significant difference in the intubation time according to table height for intubation. Conclusion : Sufficient training not only with Macintosh Laryngoscope but also with Gum Elastic Bougie and Pentax Airway Scope could improve the survival rate by intubation in patients with difficult airway.

Clinical study on success rate of microscrew implants for orthodontic anchorage (교정치료 고정원으로서 Microscrew implants의 성공률에 대한 임상적 연구)

  • To provide some guideline for microscrew implants, 73 patients that received a total of 180 mini- or microscrew implants were scrutinized. The overall success rate was $93.3\%$ (168 among 180 mini- or microscrew implants) and the mean period of utilization was 15.8 months. Microscrew implants in the UB group (maxillary buccal area) succeeded at a rate of $94.6\%$ (87 among 92), mini- or microscrew implants in the LB group (mandibular buccal area) succeeded $96.6\%$ of the time (56 out of 58), while microscrew implants in the UP group (maxillary palatal area) had a $100\%$ success rate (11 out of 11), and mini- or microscrew implants in the LR group (retromolar area) succeeded in $73.7\%$ of cases (14 among 19). This study might indicate that microscrew implants can be used successfully as orthodontic anchorage in daily orthodontic practice.

  • Lateral canal benign paroxysmal vertigo (BPPV) causing dizziness is a common cause is not found while continuing to appeal for vertigo is a typical disease. It is characterized by acute stand up, brief and rotatory vertigo attacks provoked by change in head position. Treatment requires only one treatment visit in most patients. However, there are significant numbers of patients who require multiple treatment visits for relief. The purpose of this study benign paroxysmal positional vertigo treatment of type affect is to analyze the cause. Dizziness and vertigo patient's in patients admitted to the dizziness center of lateral canal benign paroxysmal positional vertigo were classified. In patients with lateral canal benign paroxysmal positional vertigo and accompanying lateral 15 treatment affects disease were investigated. March 2008 to November 2010 lateral canal benign paroxysmal positional vertigo 166 people cure rate of patients was investigated. First time the success rate of 74.1%, twice times the success rate of 12.0%, three times the success rate of 9.6%, more than three times the success rate was 4.2%. Affecting factor treatment of benign paroxysmal positional vertigo in post-traumatic, medicine disease, headache, cerebral infarction, small vessel disease, vestibulopathy, (p<0.05). Statistical analysis using SPSS (version 12K) in coefficient measure through descriptive statistical of cross table.
