압축강화형 복합레진($TESCERA^{TM}$ ATL)을 지르코니아 framework에 효과적으로 전장하기 위하여, 표면처리 방법을 여러 가지로 달리한 지르코니아 표면에 압축강화형 복합레진을 전장하여 전단결합강도를 실험적으로 비교하였다. 지르코니아의 표면적 증가를 도모하기 위해 최종소결 전 지름 1.1mm round bur를 사용하여 지르코니아 표면에 pockmark (honey-comb concept)를 형성하였고, 화학적으로는 Primer (Zr-plus primer, Bisco, Inc., Shaumurg, USA)를 도포하여 결합강도 증가를 도모하였다. 지르코니아에 porcelain을 적층하여 대조군으로 삼고 각 군을 5도와 55도 사이에서 10,000회 thermocycling시켜 simulated aging 결과 또한 비교하였다. 전단결합강도를 측정하였고 주사전자현미경을 통해 파절단면을 관찰하였다. Pockmark를 형성한 실험군에 압축강화형 복합레진($TESCERA^{TM}$ ATL) 전장 시 도재 전장과 유사한 전단결합 강도를 발휘하였으며,(p>0.05) 동일 시편을 24시간 수중 보관했을 때에 비해 thermocycling 했을 때의 전단결합강도가 약간 감소하였으나 유의차는 보이지 않았다.(p>0.05) 지르코니아 표면에의 Primer의 도포가 압축강화형 복합레진 ($TESCERA^{TM}$ ATL)과의 결합강도를 증가시키지는 못하였다. 지르코니아 소결 전 표면에 pockmark를 주어 표면적을 증가시키고 요철의 효과를 주는 것은 $TESCERA^{TM}$ ATL과의 결합강도를 증가시켰으며, 이는 임상적으로 지르코니아-포세린의 결합강도에 필적하는 강하고 내구성 있는 결합강도를 가져온다고 판단되었다.
연구목적: 간접 복합레진 수복은 직접 수복의 여러 문제점들을 극복하는 대안으로 적용 빈도가 증가하고 있지만 간접 수복물의 특성상 치아 삭제 전 술자에 의해 결정된 색상을 기공 과정 동안 재현해야하기 때문에 기준이 되는 shade guide와 실제 간접수복용 레진간의 색상의 일치성이 요구되며, 치아의 다양한 표면의 광학적 특성을 부여하기위한 법랑질 특성의 복합레진의 광학적 특성에 대한 정보가 필요하다. 연구 재료 및 방법: 본 연구에서는 이러한 간접 복합레진 수복재료의 색상 특성을 알아보기 위해 간접수복용 레진인 Tescera ATL (Bisco, USA)의 Body composite의 색상과 동일한 색상 기호의 Vitapan shade guide 간의 색차를 알아보고, Body composite와 Incisal composite의 적층 수복 시 색상 변화 양상을 분석하고자 시행하였다. 직경 5 mm, 두께 3 mm의 원기둥형 Body composite 시편을 각 색상 당 9개씩 제조사의 지시에 따라 제작하고, 표면 연마 후 분광광도계(NF999, Nipon Denshuku, Japan)을 이용해 색상(CIE $L^*a^*b^*$)을 측정하고 Vitapan shade guide의 색상을 측정하였다. 색상 측정이 끝난 Body composite에 9 색상의 Incisal composite를 1 mm 두께로 적층하고 중합, 연마 한 후 적층면의 색상을 측정하였으며, Incisal composite를 0.5 mm 두께만큼 연마한 후 다시 색상을 동일한 측정 기기로 측정하였다. 결과: Tescera ATL과 Vitapan shade guide의 동일한 색상 간에 최소 6.88, 최대 12.80의 큰 색차(${\Delta}E^*$)를 보였으며, 모든 색상에서 시각적으로 확연한 차이를 보였다. 모든 색상의 Body composite에서 적층한 Incisal composite의 색상에 관계없이 Incisal composite의 두께가 두꺼워 질수록 Body composite 과의 색차가 증가하였다(p < 0.05). 또한 $L^*$값과 $b^*$값은 Incisal composite의 두께가 두꺼울수록 감소하는 양상을 보인 반면 $a^*$값은 큰 변화를 보이지 않았다. 결론: 이상의 결과로 Tescera ATL을 이용한 간접 복합레진 수복 시 색상 결정에 있어 기존의 shade guide의 사용은 부적절하며, 동일한 재료로 제작한 개별적인 shade guide가 필요하리라 사료되며, Incisal composite 적용 시 색상 변화에 대한 정확한 정보를 바탕으로 신중한 적용이 필요하리라 사료된다.
Statement of problem: Second-generation indirect composite resins have been improved flexural strength, compressive strength, hydrolytic degradation resistance, wear resistance compared to first-generation indirect composite resins, but there are still some problems as hydrolysis and low wear resistance. Some manufacturers claim that wear resistance of their materials has been improved, but little independent study has been published on wear properties of these materials and the properties specified in the advertising materials are largely derived from in-house or contracted testing. Purpose: This study was to evaluate the wear of indirect composite resins (SR Adore, Sinfony, Tescera ATL) and gold alloy against the human enamel. Material and method: Extracted human incisors and premolars were sectioned to $2{\times}2{\times}2mm$ cube and embedded in the clear resin and formed conical shaped antagonist to fit the jig of pin-on-disk tribometer. Total 20 antagonists were stored in distilled water. Five disk samples, 24mm in diameter and 1.5mm thick, were made for each of three groups of indirect composite resins and gold alloy group, and polished to #2,000 SiC paper on auto-polishing machine. Disk specimens were tested for wear against enamel antagonists. Wear test were conducted in distilled water using a pin-on-disk tribometer under condition (sliding speed 200rpm contact load 24N, sliding distance 160m). The wear of the enamel was determined by weighing the enamel antagonist before and after test, and the weight was converted to volumes by average density. The wear tracks were analyzed by scanning electron microscopy and surface profilometer to elucidate the wear mechanisms. Statistical analysis of the enamel wear volume, wear track depth and wear tract width of disk specimens were accomplished with one-way ANOVA and the means were compared for significant differences with Scheffe's test. Results: 1. The enamel wear was most in gold alloy, but there were no statistically significant differences among all the groups (P>.05). 2. In indirect composite resin groups, the group to make the most shallow depth of wear tract was Sinfony, followed by Tescera ATL, SR Adoro (P<.05). Gold alloy was shallower than Sinfony, but there was no statistically significant difference between Sinfony and gold alloy (P>.05). 3. The width of wear tract of SR Adore was larger than the other groups (P<.05), and there were no statistically significant differences among the other groups (P>.05). 4. SEM analysis revealed that Sinfony and gold alloy showed less wear scars after test, Tescera ATL showed more wear scars and SR Adore showed the most. Conclusion: Within the limits of this study, Sinfony and gold alloy showed the least wear rates and showed similar wear patterns.
STATEMENT OF PROBLEM: The discoloration of anterior teeth restoration is one of the material problems demanding retreatment. OBJECTIVES: To evaluate the color stability and affecting factors on esthetic restorative materials when subjected to accelerated aging. MATERIAL AND METHODS: This study was conducted using porcelain disks (IPS Empress 2-glazed, IPS Empress 2-polished), direct restorative resin disks (SYNERGY Duo) and indirect restorative resin disks (Sinfony, TESCERA ATL). Accelerated aging was done by precipitating the specimens in 38.C distilled water and irradiating with xenon light, and the total irradiation was 397.98 KJ/mm. Color and microhardness change of the specimens were measured before accelerated aging and after 100 hours, 200 hours and 300 hours of accelerated aging, and Surface of the specimens were examined with SEM before and after 300 hours of accelerated aging. RESULTS: 1. After 300 hours' accelerated aging, a ${\Delta}E$ value was 3.3 or lower in IPS Empress 2-glazed, IPS Empress 2-polished and Sinfony. 2. After 300 hours' accelerated aging, gloss was lost and surface changes including microcracks were observed in TESCERA ATL and SYNERGY Duo, and color changes of them ranged between 3.58 and 6.40 ${\Delta}E$ units. 3. During 300 hours' accelerated aging, the microhardness of surface was increased by 3.21 - 19.64% in all kinds of composites resin. CONCLUSION: After 300 hours' accelerated aging, SEM images IPS Empress 2-glazed, IPS Empress 2-polished and Sinfony showed little morphological change and their color changes were considered to be clinically acceptable. And there was significant correlation between microhardness changes and color changes of composites (P < .05).

Composite inlays are indicated for large cavities and different approaches have been proposed to improve the adaptationof Class II restorations, including applying base. The purpose of this study was to compare in vitro the marginal adaptation of class II(MOD) composite inlays(Tescera ATL system, Bisco) made with or without bases, having different physical properties. Extracted human lower molars were used for this study. The base was made from Aeliteflo(Bisco), Dyract AP(Bisco) or Fuji II LC improved(GC) respectively and the comtrol group has no base.(omitted)
본 연구는 복합레진 인레이 브릿지에서 강화재의 표면 처리와 사용 방법이 파괴 강도에 미치는 영향을 평가하였다. 본 연구에서 사용한 강화재료는 I Beam, U Beam, 1 + U Beam이었으며, 표면처리 방법은 Silane, Sandblast, Hole형성 (U beam)이었다. 강화 재료의 구성과 표면 처리 방법에 따라 총 11개의 실험군을 설정하였다. 상악 인공치 모형에서 제2소구치의 발거 상태를 가정하고 복합레진 인레이 브릿지 제작을 위하여 인접한 제1소구치에 DO, 제1대구치에 MO 와동을 형성하였다. 와동이 형성된 인공치 모형을 고무 인상체를 이용하여 석고로 제작하고, 각 실험군 별로 강화재료와 강화 재료의 표면 처리 방법에 따라 Tescera ATL (BISCO Inc. IL, USA) 복합레진을 사용하여 복합레진 인레이 브릿지를 제작하였다 그 후 시편을 복제모형에 인산아연시멘트로 합착하고 Universal testing machine (EZ Test, Shimadzu, Japan)을 이용하여 flexural stress를 가하여 파괴 강도를 측정하였으며 95% 유의 수준에서 one-way ANOVA/ Scheffes post-hoc test를 시행하여 통계 분석하였다. 다음과 같은 결론을 얻을 수 있었다. 1. 내부 강화재 I beam을 사용한 실험군이 유의성 있게 높은 파괴 강도 값을 보였다 (P<0.05). 2. 표면 처리 방법에 따른 차이는 나타나지 않았다 (P>0.05). 3. 복합레진 인레이 브릿지의 파괴는 강화 재료를 사용 시에는 복합레진과 강화 재료간에 분리 파괴가 나타났으며 사용하지 않은 경우에는 수직파괴 경향이 나타났다. 4. U beam에 유지 hole을 형성한 경우 파괴 강도 증가를 시키지 않았다.
본 연구는 상아질 접착제와 레진 시멘트의 적합성 여부를 알아보고자 시행하였다. 발거한 제3대구치의 상아질 표면을 노출시킨 후 Tescera ATL 복합레진 시편을 상아질 접착제 [All Bond 2 (Bisco), Clearfil SE Bond (Kuraray), Adper Prompt L-POP (3M), One-Up Bond F (Tokuyama)]와 레진시멘트 [Choice (Bisco), Panavia F (Kuraray), RelyX ARC (3M), Bistite II DC (Tokuyama)]로 접착하고 미세인장 결합강도 및 주사전자 현미경 관찰을 시행한 결론은 다음과 같다. 1. Clearfil SE Bond와 All-Bond 2가 Prompt L-Pop과 One-Up Bond F 보다 높은 미세인장 결합강도를 보였다(p<0.05). 2. Clearfil SE Bond와 All-Bond 2 사용시 1-step 상아질 접착제보다 혼성층이 두껍고 레진 tag가 길었다.
본 연구에서는 현재 임상에 사용되고 있는 opaque shade 복합레진의 opacity와 배경색 차단 능력을 평가하였다. 24개의 배경 시편 (직경 5.5 mm, 두께 3 mm)을 Tescera ATL Root dentin Mustard로 제작하고 배경판 레진 상에 7종의 opaque shade 복합레진 (Universal composite, Filtek Z350, Charisma, Clearfil ST, Palpaque Estelite, Esthet-X, Metafil Flo)과 통상적인 복합레진인 Estelite Sigma를 3.0 mm 적층 수복하였다. 적층된 opaque shade 복합레진의 표면 색상을 측정한 후, 적층한 두께가 0.5 mm가 될 때까지 0.3 mm 간격으로 적층된 면을 연마하면서 각 두께에서의 표면 색상을 측정하였다. 연마 후 각 두께에서 측정된 opaque shade 복합레진의 표면 색상과 적층 전의 배경판 색상, 그리고 연마전의 표면 색상 간의 색차를 계산하여 두께에 따른 배경색 차단 능력을 평가하였다. 또한 1 mm 두께의 시편을 제작하고 opacity를 측정하여 다음의 결과를 얻었다. Opaque shade 복합레진의 opacity는 Clearfil ST, Metafil Flo, Filtek Z350, Palpaque Estelite, Universal composite, Charisma, Esthet-X 순으로 감소하였다 (p < 0.05). Opaque shade 복합레진의 두께의 감소에 따라 $L^*$ 값은 감소하는 양상을, $a^*$ 값은 증가하는 양상을 보였다. 연마 전 표면 색상과 연마 후 각 두께에서의 색차 (${\Delta}E^*$)는 Clearfil ST 군은 모든 군에서, Metafil Flo 군은 1.0 mm 이상에서 3 이하의 색차를 보였다. 배경 색상에 대한 각 opaque shade 복합레진의 색차는 모든 두께에서 10 이상의 색차를 보였으며, Clearfil ST와 Metafil Flo가 가장 큰 색차를 보였다 (p < 0.05). 현재 임상에 사용되고 있는 opaque shade 복합레진들은 제조사에 따라 서로 다른 opacity와 색상을 가진다. 따라서 재료의 선택에 있어 적용하고자 하는 병소의 특성에 따라 opaque shade 복합레진의 opacity와 색상에 대한 충분한 고려가 필요하리라 사료된다.
