Epicondylitis (상과염)

  • Epicondylitis, as a tendinopathy characterized by fibroblast and microvascular hyperplasia, is a common musculoskeletal problem especially related with repetitive hand and wrist motion. It has a prevalence of between 0.2% and 5% in general population depending on the amount of exposure to manual labor jobs. Although it is known that the pathological lesions lie in the flexor or extensor common tendons, there could be collateral ligament lesions and/or reactive synovitis accompanied, which may make a case unresponsive to the treatment aimed only at the tendinopathy. Epicondylitis is easy to diagnose with typical pain, tenderness, and positive provocation tests. However, many conditions can mimic epicondylitis that further imaging or electrodiagnostic studies should be undertaken to exclude other possible problems. Ultrasonography provides information about the existence and extent of tendinopathy with relatively high specificity. Magnetic resonance imaging is often required to rule out other problems and confirm the diagnosis of the cases intractable to long term treatment. Many options of treatment are available for epicondylitis while numerous conflicting evidences have been noted, debating one treatment method is better than the others. Since it was reported that over 80% of epicondylitis improved within a year no matter what was done as treatment, it is a challenge to make accurate diagnosis and combine effective therapeutic regimens for the 20% of intractable cases.

A Study on Concept and Design Characteristics of 『Platform Architecture』 in the Architecture of Sverre Fehn (스베르펜(Sverre Fehn) 건축에 나타난 『플랫폼 건축(Platform Architecture)』 개념과 디자인 특성에 관한 연구)

  • The Norwegian architect, Sverre Fehn who was born in 1924 and died in 2009 designed various buildings in his country. Since he studied architecture, he had been interested in the domestic/local architectural environment as well as international modern movement such as CIAM. Pavilion of the Nordic Nations designed in 1960s gave him world reputation for the peculiar design characteristic in which Scandinavian tradition and modern design were integrated. Fehn introduced his unique architectural philosophy that is called 'Platform Architecture'. This concept is based on the Fehn's long-term interest in the philosophy of Genius Loci and place-specificity. It consists of three theoretical elements: Tectonic of Place, Horizontality of Space and Visual-Perception by Light. This paper focuses on three case projects: Villa Busk in 1990, Norwegian Glacier Museum in 1991, Aukrust Museum in 1993. It is aimed to discover differences as well as similarities of the projects, and ultimately to summarize the cases under the light of 'Platform Architecture'. As a conclusion, it is found that each case project has unique spatializing method to organize the given place and program by 'Platform' architectural design. 'Platform Architecture' is still relevant in the contemporary architecture and interior design because of its deep consideration on place, environment and human.

Review of Wind Energy Publications in Korea Citation Index using Latent Dirichlet Allocation (잠재디리클레할당을 이용한 한국학술지인용색인의 풍력에너지 문헌검토)

  • The research topics of more than 1,900 wind energy papers registered in the Korean Journal Citation Index (KCI) were modeled into 25 topics using latent directory allocation (LDA), and their consistency was cross-validated through principal component analysis (PCA) of the document word matrix. Key research topics in the wind energy field were identified as "offshore, wind farm," "blade, design," "generator, voltage, control," 'dynamic, load, noise," and "performance test." As a new method to determine the similarity between research topics in journals, a systematic evaluation method was proposed to analyze the correlation between topics by constructing a journal-topic matrix (JTM) and clustering them based on topic similarity between journals. By evaluating 24 journals that published more than 20 wind energy papers, it was confirmed that they were classified into meaningful clusters of mechanical engineering, electrical engineering, marine engineering, and renewable energy. It is expected that the proposed systematic method can be applied to the evaluation of the specificity of subsequent journals.

Analysis and Prediction of Behavioral Changes in Angelfish Pterophyllum scalare Under Stress Conditions (스트레스 조건에 노출된 Angelfish Pterophyllum scalare의 행동 변화 분석 및 예측)

  • The behavior of angelfish Pterophyllum scalare exposed to low and high temperatures was monitored by video tracking, and information such as the initial speed, changes in speed, and locations of the fish in the tank were analyzed. The water temperature was raised from 26℃ to 36℃ or lowered from 26℃ to 16℃ for 4 h. The control group was maintained at 26℃ for 8 h. The experiment was repeated five times for each group. Machine learning analysis comprising a long short-term memory model was used to train and test the behavioral data (80 s) after pre-processing. Results showed that when the water temperature changed to 36℃ or 16℃, the average speed, changes in speed and fractal dimension value were significantly lower than those in the control group. Machine learning analysis revealed that the accuracy of 80-s video footage data was 87.4%. The machine learning used in this study could distinguish between the optimal temperature group and changing temperature groups with specificity and sensitivity percentages of 86.9% and 87.4%, respectively. Therefore, video tracking technology can be used to effectively analyze fish behavior. In addition, it can be used as an early warning system for fish health in aquariums and fish farms.

A study on the basic conception of the Saemangeum Agricultural Land Landscape Plan (새만금 농생명용지 경관계획 기본구상 연구)

  • This study aims to establish a landscape plan that can preserve the identity of the region while meeting the unique functions related to land use of the Saemangeum agricultural land. In addition, it was intended to prepare a mid- to long-term landscape plan basic concept for the formation and management of the Saemangeum agricultural land landscape. Therefore, in this study, landscape management by type for processing/storage facilities, production/processing facilities, complex/sales facilities, and research/experimental facilities for facilities scheduled to move in after setting the direction and five strategies for landscape management considering the specificity of agricultural land direction was set. In addition, the landscape management direction for 6 common landscape elements such as buildings, open spaces, advertisements, colors, public facilities, and night scenery was also presented. In particular, Agroworks, Agro City, and Agro Town, which are important landscape areas, are landscape-focused management areas, and the landscape management direction for the area has been established. This study is considered to have great utility value when setting landscape standards for public or private buildings and infrastructure, development projects, etc. to be moved in prior to establishing a landscape plan for agricultural land. It is expected that it can be used as a basis for deliberation and review of the landscape standards derived from this study in the future landscape-related licensing for agricultural land.

Comparison of Automated Breast Volume Scanning and Hand-Held Ultrasound in the Detection of Breast Cancer: an Analysis of 5,566 Patient Evaluations

  • Background: The purpose of this study was to compare the accuracy and effectiveness of automated breast volume scanning (ABVS) and hand-held ultrasound (HHUS) in the detection of breast cancer in a large population group with a long-term follow-up, and to investigate whether different ultrasound systems may influence the estimation of cancer detection. Materials and Methods: Institutional review board approval was obtained for this retrospective study, and informed consent was waived. From September 2010 to August 2011, a total of 1,866 ABVS and 3,700 HHUS participants, who underwent these procedures at our institute, were included in this study. Cancers occurring during the study and subsequent follow-up were evaluated. The reference standard was a combination of histology and follow-up imaging (${\geq}12months$). The recall rate, cancer detection yield, diagnostic accuracy, sensitivity, specificity, and positive (PPV) and negative (NPV) predictive values were calculated with exact 95% confidence intervals. Results: The recall rate was 2.57 per 1,000 (48/1,866) for ABVS and 3.57 per 1,000 (132/3,700) for HHUS, with a significant difference (p=0.048). The cancer detection yield was 3.8 per 1,000 for ABVS and 2.7 per 1,000 for HHUS. The diagnostic accuracy was 97.7% for ABVS and 96.5% for HHUS with statistical significance (p=0.018). The specificity of ABVS and HHUS were 97.8%, 96.7%, respectively (p=0.022). Conclusions: ABVS shows a comparable diagnostic performance to HHUS. ABVS is an effective supplemental tool for mammography in breast cancer detection in a large population.

Post-Medium and Postproduction: Contemporaneity of Contemporary Art (포스트-미디엄과 포스트프로덕션 : 포스트모더니즘 이후 현대미술의 '동시대성(contemporaneity)')

  • In recent studies of art historical methodology, such as Critical Terms for Art History and The Art of Art History, subjectivity, identity, abjection, and other terms have been placed safely in the genealogy of contemporary art history. This paper questions the contemporaneity in the story of contemporary art in our time in relation to two other critical terms that have been regularly cited by contemporary critics, not only in Euro-American fields but also in Korea. The terms are postmedium and postproduction, respectively, as used by Rosalind Krauss and Nicolas Bourriaud. This paper stems from the critical condition in which art criticism and theory have their power in the rise of neo-liberalism. But this paper does not deal with the contemporary as a chronological term for art history but rather examines the three critical terms-contemporaneity, post-medium, and postproduction-that have garnered scholarly attention. I would like to put aside postmodernism for the moment; I don't disregard the postmodern condition although the death of postmodern critical terms has resulted in the loss of its polemical power in art worlds such as in exhibitions, etc. To look at "the postproduction in the age of post-medium age after postmodernism," I first explore Krauss's notion of post-medium because, unlike media artists like Lev Manovich and Peter Weibel, Krauss's post-medium condition is different and insists on medium specificity. In this sense, Krauss has turned out to be another Greenberg in disguise. For her, photography and video are expanded mediums after Greenberg, because Krauss has spent her life explicating those mediums. Under the Cup, her recent publication, came out in 2011, and discusses her desire to defend medium-specificity against the intermedia of installation art found ubiquitously in international exhibitions and biennales. Her usage of post-medium has been taken up by Weibel as postmedia in a broader sense. But whether the post-medium condition or the postmedia age, we nonetheless enter the new age of the contemporary. Consequently, this paper questions what constitutes contemporaneity in our times. It is said that there is nothing new on earth, yet I find original artistic strategies among the younger generation in the postmedia age. The contemporary justifies its place in art fields and criticism by keeping its distance from postmodernism although we still find the remnants of postmodern artistic practices and theoretical foundations. By looking at materials written by Terry Smith, I would like to examine contemporaneity as a rhetoric where artists, critics, and curators endeavor to set up a new spirit of criticism, distant from the past of modernism and postmodernism. In discussions, modernism and postmodernism act as catalysts interacting with each other while justifying their own place. In conclusion, my paper reaches to delineate where the contemporary finds its place among artists' responses and working methods. It explores the postproduction of the Internet and the World Wide Web generations, where images become data rather than representation (of modernism) and appropriation (of postmodernism). This paper analyzes Bourriaud's text, as well as relevant artists like Pierre Huyghe, Liam Gillick, and others. By examining the aforementioned critical terms, I would like to reconsider our own contemporary art in Korea, especially among young artists influenced by digital media and the World Wide Web in the 1990s.

Validation of enzyme immunoassay for the quantitative measurement of human IgG antibodies specific for Haemophilus influenzae Type b capsular polysaccharide (Haemophilus influenzae type b 피막 다당질 특이 인간 IgG 항체의 정량적 측정을 위한 enzyme immunoassay의 타당성 연구)

  • Purpose : This study was conducted to validate enzyme immunoassay (EIA) for the quantitative measurement of human IgG antibodies specific for Haemophilus influenzae type b (Hib) capsular polysaccharide. Method : We evaluated specificity, repeatability, intermediate precision, accuracy, lower limit of quantification (LLOQ), and stability to validate standardized EIA for the quantitative measurement of human anti-polyribosylribitol phosphate (PRP) IgG antibodies. Results : The results indicated that this EIA showed specificity to HbO-HA antigen and repeatability and intermediate precision were within acceptance criteria (repeatability: $CV{\leq}15%$, intermediate precision: $CV{\leq}20%$). The EIA-derived results from this laboratory were equivalent to those obtained by the standard radioactive antigen binding assay (RABA) for quantitation of anti-PRP antibodies in the 28 sera. Spiking recovery result was within acceptance criteria ($100{\pm}20%$). The precision and accuracy of samples in LLOQ were from -14.7 to -4.7% in nominal values, which were within acceptance criteria (precision: $CV{\leq}25%$, accuracy: ${\pm}25%$). Freeze-thaw stability and short term temperature stability were within ${\pm}20%$ of acceptance criteria. Conclusions : The EIA which is performed at the Center for Vaccine Evaluation and Study Ewha Medical Research Institute, is an appropriate serologic assay which can be used for quantitation of anti-PRP IgG antibodies in human sera.

Non-neoplastic Myelopathies Mimicking Intramedullary Spinal Cord Tumors : Retrospective Analysis of 8 Surgically Proven Cases (척수내 종양과 감별을 요하는 비종양성 척수증 : 수술로 확진된 8례의 후향적 분석)

  • Objective : It is difficult to differentiate intramedullary spinal cord tumors preoperatively from non-neoplastic pathologies in patients presenting as non-compressive myelopathies in magnetic resonance imaging(MRI). In this report, the authors reviewed nonneoplastic intramedullary spinal cord lesions preoperatively diagnosed as tumors and discussed their clinical and radiological characteristics and usefulness of surgical intervention. Methods : From January, 1985 to January, 1999, authors experienced eight non-neoplastic pathologies mimicking intramedullary spinal cord tumors and analysed their medical records, radiological findings and histopathological specimens retrospectively. Results : There were five males and three females and the duration of symptoms were from two to 20 months(mean, 9.8 months). The location of lesions were four cervical, one cervicothoracic and three thoracic. All patients manifested sensory abnormality, seven motor weakness, and six bladder symptom. All cases had swollen spinal cords and increased signal intensities in spin-echo sequences. Six cases showed contrast enhancement : four cases were focal and two diffuse. Under the impression of intramedullary tumors, the patients were operated upon. Final diagnoses on the base of clinical and pathologic finding were : three subacute necrotizing myelopathies, two multiple scleroses, two myelopathy of unknown etiology. One case showed no gross abnormality in surgical field in spite of adequate exposure of the lesion, so biopsy was not performed. In that case, postoperative MRI revealed spontaneous resolution of the lesion. Conclusion : MRI is invaluable diagnostic tool in screening myelopathies. However, its high sensitivity and lack of specificity make difficulty in preoperative differential diagnosis of non-compressive myelopathies. Although no surgical morbidity occurred in our series, we sometimes failed to confirm definite diagnosis even with biopsy. In such a circumstance, long-term follow up is needed.

  • By Wikipedia, big data is a broad term for data sets so large or complex that traditional data processing applications are inadequate. Data mining is the computational process of discovering patterns in huge data sets involving methods at the intersection of association rule, decision tree, clustering, artificial intelligence, machine learning. Association rule is a well researched method for discovering interesting relationships between itemsets in huge databases and has been applied in various fields. There are positive, negative, and inverse association rules according to the direction of association. If you want to set the evaluation criteria of association rule, it may be desirable to consider three types of association rules at the same time. To this end, we proposed a balanced comparative confidence considering sensitivity, specificity, false positive, and false negative, checked the conditions for association threshold by Piatetsky-Shapiro, and compared it with comparative confidence and inversely comparative confidence through a few experiments.
