갑상선암의 경부 림프절 전이 진단을 위한 미세침세척액 티로글로불린 측정법 (Thyroglobulin Measurement in Fine Needle Aspirates for Diagnosing Cervical Lymphnode Metastasis from Differentiated Thyroid Malignancy)

  • 갑상선암 환자의 경부 림프절에서 미세침흡인생검술(fine needle aspiration biopsy, FNAB)을 시행하며 얻은 미세침세척액의 티로글로불린(thyroglobulin, Tg)측정법의 진단적 성적과 유용성을 알아보고자 FNAB 세포검사의 결과와 비교하였다. 갑상선암의 경부림프절 전이가 의심되어 초음파 시행한 총 37명의 환자 48개의 FNAB 검체를 대상으로 하였고, 수술 후 조직검사결과 또는 장기간 영상 추적검사를 기준으로 하여 세포검사와 미세침세척액 티로글로불린(FNAB-Tg)측정법을 비교하여, 각 검사의 특이도, 민감도 및 정확도를 평가하였다. 추가적으로 Tg-항체가 양성일때 FNAB-Tg 검사에는 어떠한 영향을 미치는지 Tg-항체 검사를 같이 시행한 34명을 대상으로 분석하여 보았다. FNAB 검체의 세포검사에서 전이 음성인 36개 검체 중 35개 검체에서 음성을 보였고, 전이 양성인 12개 검체 중 9개 검체에서 양성을 보였다. FNAB-Tg측정법의 경우 전이 음성인 36개의 검체 중 32개의 검체에서 음성을 보였고, 전이 양성인 12개의 검체 중 12개 검체 모두 양성을 보였다. 따라서 세포검사의 민감도와 특이도는 각각 75.0% (9/12)와 97.2% (35/36)였고, 위음성이 25% (3/12)였다. 반면에 FNAB-Tg 측정법의 민감도와 특이도는 각각 100% (12/12)와 88.9% (32/36)였고, 위양성이 11.1% (4/36)였다. 정확도는 두 검사 모두 91.7% (44/48)였다. Tg-항체에 대한 검사 결과를 얻을 수 있는 34명을 따로 분석한 결과, Tg-항체의 존재 유무가 FNAB-Tg 측정법의 진단 성능에 영향을 주지 않음을 확인할 수 있었다. FNAB 검체를 이용한 세포검사는 특이도가 높고 반면, FNAB-Tg 측정법은 민감도가 높은 검사임을 확인할 수 있었다. 따라서 림프절 전이가 의심되는 갑상선환자에서 FNAB를 시행할 때 세포검사와 FNAB-Tg 측정법은 동시에 시행되어야 할 상호보완적인 검사들이다.

영건의궤(營建儀軌)에 실린 목부재용어(木部材用語)의 용례(用例)와 변천(變遷)에 관(關)한 연구(硏究) (A study on the examples and changes of wooden member terms in Yeonggeon-euigwe)

  • This study examines the examples and changes of wooden member terms in Yeonggeon-euigwe(營建儀軌) in the era of Joseon Dynasty. As a result of examining examples, about 240 wood member terms were found on the basis of phonetic value and examples different from today's term use were also confirmed. Wood member terms were derived in variety and synonym and different style, that is, coexistence or transition of several notations as the term indicating the same member was found. Derivation of detail terms has the characteristic increasing on the basis of morpheme and formation of different notation followed Chinese notation or was caused by complex coinage features like a coined word of Korea by the meaning of a word and borrowed character notation borrowing sound and it is also related to the specificity of that time which had dual language system. The typical examples without different style for long were pillar, rafter, door and window. Examples with active generation and selection of different styles included beam, capital and bracket-system terms. Different styles were caused by the combination of several notations including borrowed character in the process of Chinese character notation borrowing sound, Korean unique character emphasizing and limiting combination of 木 (wood) with side of character and Chinese. Period showing remarkable change of example notation was the compilation of ${\ll}$the Hwa-Sung-Sung-Euk-Eui-Gue${\gg}$ 華城城役儀軌. ${\ll}$the Hwa-Sung-Sung-Euk-Eui-Gue${\gg}$ is the representative type uigwe made by printed type not by handicraft. Printing by type accompanies unification of the shape of a character necessarily and it was considered that it resulted in the unification of character of different style, the number of strokes and minute difference of strokes, and it was interpreted that common use of intentional notation with the unification of the shape of a character was achieved.

P53 and MDM2 Over-expression and Five-year Survival of Kidney Cancer Patients Undergoing Radical Nephrectomy - Iranian Experience

  • Background: Relatively little is known with certainty about the status and role of p53 or MDM2 in predicting prognosis and survival of renal cell carcinoma. The present study aimed to determine the value of P53 and MDM2 over-expression, alone and simultaneously, to predict five-year survival of patients with kidney cancer in Iran. Materials and Methods: Patients with kidney cancer referred to Hasheminejad Kidney Center between 2007 and 2009, underwent radical nephrectomy and had pathology reports of clear cell, papillary or chromophobe renal cell carcinoma were included in our cohort study. Other histological types of renal cell carcinoma were not included. The patients with missed, incomplete or poor quality paraffin blocks were also excluded. Overall ninety one patients met the inclusion and exclusion criteria. To assess the histopathological features of the tumor, immunohistochemical (IHC) staining of formalin fixed, paraffin-embedded tumor samples were performed. The five-year survival was determined by the patients' medical files and telephone following-up. Results: In total, 1.1% of all samples were revealed to be positive for P53. Also, 20.8% of all samples were revealed to be positive for MDM2.The patients were all followed for 5 years. In this regard, 5-year mortality was 30.5% and thus 5-year survival was 85.3%. According to the Cox proportional hazard analysis, positive P53 marker was only predictor for patients' 5-year survival that the presence of positive p53 increased the risk for long-term mortality up to 2.8 times (HR=2.798, 95%CI: 1.176-6.660, P=0.020). However, the presence of MDM2 could not predict long-term mortality. In this regard, analysis by the ROC curve showed a limited role for predicting long-term survival by confirming P53 positivity (AUC=0.610, 95%CI: 0.471-.750, P=0.106). The best cutoff point for P53 to predict mortality was 0.5 yielding a low sensitivity (32.0%) but a high specificity (97.9%). In similar analysis, measurement of MDM2 positivity could not predict mortality (AUC=0.449, 95%CI: 0.316-.583, P=0.455). Conclusions: The simultaneous presence of both P53 and MDM2 markers in our population is a rare phenomenon and the presence of these markers may not predict long-term survival in patients who undergoing radical nephrectomy.

안양시 공공도서관 중장기 발전 계획 수립을 위한 현황 분석 및 운영 전략 연구 (A Study on the Current Status Analysis and Operation Strategies for Establishing the Mid- to Long-term Development Plans for Anyang Municipal Libraries)

  • 본 연구는 안양시의 정체성과 특수성을 고려한 도서관 중장기 종합 발전 계획 수립을 위한 운영 전략들을 도출하고자하는 목적으로 수행되었다. 이를 위해 본 연구에서는 다음과 같은 내용으로 진행되었다. 첫째, 안양시의 대내외 환경과 지역적 특수성 등을 파악하기 위해 안양시와 관련된 각종 문헌 자료 및 통계 자료들을 수집·분석해 정리하였다. 둘째, 「국가도서관통계시스템」 및 「경기도 공공도서관 연감」, 각종 도서관 관련 법규 등의 자료를 토대로 안양시 공공도서관의 운영 현황에 대해 파악하였다. 셋째, 안양시 공공도서관에서 근무하고 있는 사서 26명을 대상으로 안양시 정체성부터 현재의 도서관 운영 전반에 대한 의견을 수렴하기 위해 주관식 개방형 질문으로 구성한 설문조사를 실시해 분석하였다. 마지막으로, 앞서 정리한 현황 분석 내용과 최근의 도서관 트렌드, 정책 및 사회문화 환경 등을 반영해 향후 안양시 공공도서관의 중장기 발전 계획 수립의 토대가 될 수 있는 구체적인 운영 전략들을 ① 조직체계 및 인력 구성, ② 지역 간 균형 발전을 위한 시설 계획, ③ 장서 개발 및 보존 방향, ④ 특화주제 서비스 방안, ⑤ 협력체계 구축, ⑥ 홍보 방안의 여섯 가지 영역으로 구분해 제안하였다.

  • The Lifelong Learning City Project has made quantitative expansion as well as qualitative growth since 2001 but the project has been criticized by academic scholars and field practitioners. The Lifelong Learning City Project is a national policy project which has been promoted by the Ministry of Education and Human Resources Development and should be required to make production profits proportional to the amount of public finance. The Lifelong Learning City Project is a community development project intended to promote growth and progress by supporting the community in lifelong learning endeavors. Therefore, the community development theory could offer guidelines to the Lifelong Learning City Project. Based on this assumption, this study intends to investigate the Lifelong Learning City Project at the national, city, and county levels using the community development theory. The improvement methods of the Lifelong Learning City Project are role allotment between national and wide level projects supporting organizations, and the establishment of a system and a long term project policy. In addition, the project is to have a more systematic performance. It is to enhance opportunities for community members' participation, and practice in planning, performance of learning, and the proper performance in regard to the community conditions and specificity. The most important goal of the Lifelong Learning City Project is to support the empowerment of community members by making opportunity planning, practicing and sharing lifelong learning more accessible.

Accuracy Test of Coring for Measuring Annual Increment in Quercus mongolica, Kalopanax septemlobus, and Pinus densiflora

  • Coring has been widely used to measure annual increment in temperate forest ecosystems. This method is attractive because cores can be taken in just one visit. However, the accuracy of this method has not been tested. We expected coring to be less accurate than band dendrometers because of the eccentricity of tree growth. We studied 41 trees at the Long Term Experimental Forest in Mt. Gyebang, which has been monitored with band dendrometers since 1996. We collected two tree cores from the south and north face of each tree, 10 cm below the growth band. Increment cores were measured to 0.01 mm under stereomicroscopy. Annual growth from 1997 to 2006 was 3.2 mm $yr^{-1}$ for Quercus mongolica, 3.5 mm $yr^{-1}$ for Kalopanax septemlobus, and 5.7 mm $yr^{-1}$ for Pinus densiflora. The difference between the two methods was 10% for Q. mongolica, 14% for K. septemlobus, and 4% for P. densiflora. Compaction in the corer and shrinkage during drying decreased diameter increment by 5.6% and 1.0% on P. densiflora, respectively. This study suggests that the two methods for annual increment measurement are very similar, but species specificity should be concerned for direct comparison.

Discovery of Novel DUSP4 Inhibitors through the Virtual Screening with Docking Simulations

  • Dual specificity protein phosphatase 4 (DUSP4) has been considered a promising target for the development of therapeutics for various human cancers. Here, we report the first example for a successful application of the structure-based virtual screening to identify the novel small-molecule DUSP4 inhibitors. As a consequence of the virtual screening with the modified scoring function to include an effective molecular solvation free energy term, five micromolar DUSP4 inhibitors are found with the associated $IC_{50}$ values ranging from 3.5 to $10.8{\mu}M$. Because these newly identified inhibitors were also screened for having desirable physicochemical properties as a drug candidate, they may serve as a starting point of the structure-activity relationship study to optimize the medical efficacy. Structural features relevant to the stabilization of the new inhibitors in the active site of DUSP4 are discussed in detail.

Disaster Cooperation Strategies between North and South Korea based on Sendai Framework

  • TThe cooperation on disaster prevention between the two Koreas has been mostly focused on cooperation at the recovery stage, such as emergency relief supplies and equipment support. This study aims to articulate future practical development plans by analyzing disaster cooperation between North and South Korea in accordance with the changing paradigm of international disaster cooperation. Considering the specificity of inter-Korean cooperation, the Framework for Inter-Korean Disaster Prevention Cooperation was established centering on the Sendai Framework for Disaster Risk Reduction. The framework consists of understanding and sharing disaster risks, building back better, strengthening community resilience, and supporting regional disaster action plans for continuous action. Inter-Korean disaster prevention cooperation requires sharing information about North Korean disasters. It is necessary to accurately identify and support social vulnerability to North Korean disasters. Above all, it is supporting the development of disaster action plans from a humanitarian perspective needs. A medium-to-long term resilience reinforcement plan that North Korea can resolve on its own is also needed. Since North Korea is also deeply interested in international disaster cooperation, it should be based on the direction of the Sendai Framework. Considering the uniqueness of inter-Korean cooperation, a path for North Korea to actively participate in international disaster cooperation should be established.

Biological Control Strategy of Uzi Fly in Sericulture

  • Uzi fly (Exorista bombycis Louis) is one of the major larval endo-parasitoid of silkworm (Bombyx mori). It causes extensive damage to sericulture industry. The application of synthetic organic pesticides has tremendous impact on minimizing pest population but their overuse and frequent misuse and high sensitivity towards the silkworms, has forced the entomologists to search for alternatives to chemical control, which is safe to silkworm, environment and farm workers. Biological control continues to offer exciting possibilities for the control of fly pest population. It is environmentally safe alternative to chemical control and offering a long-term protection. Several potential hymenopteran parasitoids have been screened. Among successful natural enemies, Nesolynx thymus, Trichomalopsis apanteloctena, Trichopria sp., Brachymeria lasus, Pediobius sp., Spalangia sp., Spilomicrus karnatakensis and Dhirhinus sp. are important. It is essential to predict accurately the efficacy of these natural enemies in a new habitat prior to its introduction. The important desirable attributes of these potential parasitoids viz., host searching capacity, specificity, power of increase and fitness and adaptability of the parasitoid in new environment has been recorded. Results of the host parasitoid interaction indicate that the aging of the host function as a factor that influence the host finding efficiency of the parasitoid. It is highly scored with 15-20 hrs old pupa of the host. However, aging of the parasitoid does not significantly affect it. The sex ratio is female biased which is advantageous from biological control point of view, Biological suppression methods involving conservation and utilization of natural enemies have been discussed in detail.

언더라이팅시 흔하게 접하는 심전도 소견과 위험 평가 (Common ECG pattern and underwriting risk assessment)

  • ECG is included in certain medical examinations of insurance application, ECG has low specificity and sensitivity. So ECG is not usually used to diagnose specific diseases. But, ECG is not invasive and costs low. So ECG is usually used in underwriting. Actually in underwriting we meet various ECG patterns and diagnosises. Understanding of various ECG patterns is different between insurance medicine and clinical medicine. So We have to learn various ECG patterns and effects on mortality and morbidity. First considerations of ECG readings are age, sex, blood pressure, family history, smoking historyalcohol history and hyperlipidemia. These are predictors for possibility of disease. Also it is important to review recording ECG with proper skill. In this review I consider several ECG diagnosises that we meet frequently, which is, LVH, RVH, ST abnormalities, LBBS, RBBB, A-B blocks, several kinds of arrhythmia. We have to consider long term mortalities and morbidities of specific ECG patterns although applicants have no symptom and sign. And then we have to make underwriting manual according to specific ECG diagnosises and patterns and underwrite precisely ECG patterns according to insurance products. Nowadays coronary heart disease and other heart diseases are increasing in Korea. So we have to learn various ECG patterns and research mortalities and morbidities of abnormal ECG patterns. Also we have to apply to more broad, precise underwriting skills about ECG patterns and diagnosises.
