Background: Isolated infraspinatus tear is very rare and clinical features are not as well known, therefore the purpose of this study was to evaluate clinical characteristics and outcomes of isolated infraspinatus tear that authors experienced. Methods: Authors reviewed 288 cases of full-thickness rotator cuff tear involving infraspinatus between 2010 and 2015, and retrospectively analyzed six cases of isolated infraspinatus tear. Perioperative clinical characteristics, postoperative functional outcomes of 6 months were investigated. Functional evaluation included visual analogue scale (VAS), range of motions, American Shoulder and Elbow Surgeons (ASES) score, and Constant score. Results: Calcific tendinitis was accompanied in 4 cases (66.7%). Three of them received steroid injection or aspiration or extracorporeal shockwave therapy. Mean preoperative pain VAS was 7.1 (range, 5-9), and mean postoperative pain VAS at 6 months later was 1.6 (range, 0-5). Preoperative muscle strength by isokinetic muscle performance test showed 52% deficit of abduction and 37.6% deficit of external rotation. All 6 patients had arthroscopic repair of the infraspinatus tendon. All the patients at the 6 months follow-up exhibited clinical improvement in the Constant score (67.8 [range, 45-77] to 89.3 [range, 81-100], p=0.029), and ASES score (52.3 [range, 30-77] to 90.0 [range, 80-100], p=0.002). There was no healing failure on imaging. Conclusions: Isolated infraspinatus tendon tear was frequently accompanied by calcific tendinitis, but pathophysiologic relationship should need more study. To rule out neurogenic etiology, magnetic resonance imaging and electromyography would be helpful. Arthroscopic infraspinatus tendon repair and supraspinatus debridement showed relatively good result in painful shoulder.
Objectives : The purpose of this study is to investigate the latest trend of studies on treatments of postoperative rotator cuff tear patients which are published in Korea. Methods : We investigated the studies on the treatments of postoperative rotator cuff tear patients via searching 6(six) Korean web databases(NSDL, RISS, Korean traditional knowledge portal, OASIS, KISTI, KCI). As a result, 16(sixteen) studies were found. We classified the papers by publication date, published journal, method of study, type of treatment, evaluation scale and referencing research ethics. Results : 16(sixteen) papers were published from 2006 to 2017. The papers were published in The journal of Korean shoulder & elbow society, The journal of korean acupuncture & moxibustion medicine, etc. 5(five) randomized controlled trials, 5(five) non-randomized controlled trials, 4(four) case reports and 2(two) Retrospective report have been found. For types of treatment, 6(six) exercise therapies, 3(three) Korean medicine treatments and 7(seven) other treatments have been found. Main evaluation tools were VAS, ROM, NRS and Muscle strength. Conclusions : In this study, we analyzed the latest trend of studies on treatments of postoperative rotator cuff tear patients. Reviewing the international trends of studies on treatments for postoperative rotator cuff tear are essential for the future studies.
This study is a continuation of the previous experimental analysis and is mostly focused on handsheet strength properties. Four completely different fibers, which were Hw-BKP, SW-BKP, White ledger, and OCC, were selected to investigate the effect of mechanical pre-treatment by Hobart mixer on handsheet strehgth properties. After equal time mechanical pre-treatment, the fibers were refined with laboratory valley beater for 10, 20 and 30 minutes, and handsheets were prepared from the fibers for physical strength comparison. Handsheets from SW-BKP and OCC showed 5-30% increase of breaking length, burst index, tear index, and compression index while handsheets from HW-BKP and White ledger no increase except tear index. In Hobart mixer pre-treatment, HW-BKP and White ledger fibers were easily attached to the wall of the mixer bowl and mechanical action was not effectively applied. The fiber length of Hw-BKP and White ledger were 0.837 mm and 1.591 mm, respectively, while SW-BKP and OCC were 2.744 mm and 2.033 mm, respectively, in weight weighted length. The effective mechanical pre-treatment seems to be related to the fiber length. Tear indexes of the pre-treated furnish were much higher than no pre-treatment at the same breaking length level.
The purpose of this study was to compare the outcomes between arthroscopir repair and mini-open repair of medium and large rotator cuff tears in which arthroscopic repair was technically unsuccessful. We evaluated 76 patients of full-thickness rotator cuff tears, among them 42 patients had all-arthroscopic and 34 patients had mini-open salvage repairs. Patients who had acromioclavicular arthritis, subscapularis tear, or instability were excluded. There were 39 males and 37 females with mean age of 56 years (range,42 to 75 years). At a mean follow-up of 39 months (range, 24 to 64 months), the results of both groups were compared with regard to the UCLA and ASES shoulder rating scale s. Shoulder scores improved in all ratings in both groups (p > 0.05). Overall, sixty-six patients showed excellent or gr)of and ten patients showed fair or poor scores by the UCLA scale. Seventy-two patients satisfactorily returned to prior activity. Four showed unsatisfactory return. The range of motion, strength, and patient's satisfaction were improved postoperatively. There were no difference in shoulder scores, pain, and activity return between the arthroscopic and mini-open salvage groups (p > 0.05). However, Patients with larger size tear showed lower shoulder scores and less predictive recovery of the strength and function (p < 0.05). Postoperative pain was not different with respect to the size of the tear (p : 0.251). Arthroscopic repair of medium and large full-thickness rotator cuff tears had iln equal outcome to technically unsuccessful arthroscopic repairs, which were salvaged by conversion to a mini- open repair technique. Surgical outcome depended on the size of the tear, rather than the method of repair.
Mechanical Properties, such as tensile strength, elongation, and tear strength, decreased according to increasing the load of ATH, volume resistivity, AC break down strength, and tracking resistance for HVI SC contained ATH treated by vinyl Silane were better than those for HVI SC were contained ATH treated by other surface treatment agent, such as stearic acid and acryl silane.
In order to investigate the applicability of lyocell fiber to filter paper, papper sheets were manufactured with the addition of lyocell fibers in various length(1.5 denier: 2, 3, 4mm) and content(10, 30, 50%) and their physical characters, such as fibrilation rate, adsorption efficiency of methylene blue(MB), paper formation, and crimping ability, etc, were tested. The level of main fibrilation from lyocell fiber was higher in wet beating process than that in free beating because of the higher strength of lyocell fiber compared with wood fiber. Fibrilation could be observed at the degree of beating over 30$^{\circ}$ SR in wet beating with Valley beater. The air permeability and tear factor of the paper increased and the paper formation index decreased according to the increase of fiber length. The weak binding force of lyocell fiber in spite of its higher fiber strength, might be a limitng factor in addition of lyocell fiber to the natural wood pulp in manufacturing the paper having the needed physical properties. High contents of wood pulp decreased air permeability, the breaking length, tear factor, the bursting strength, and paper formation index in paper sheets. As the contents of lyocell increased from 10% to 100%, the adsorption efficiency of MB was elevated to 1.7-7.9 times compared with that in 100% wood pulp. But the length of lyocell fiber did not affect the MB adsorption.
Adsorption of xylans extracted from birchwood and oat spelt on Hw-BKP were analyzed using HPLC. The effect of xylan adsorption on paper properties such as tensile, tear index and brightness was also investigated. The constituents of xylan was analyzed with HPLC after hydrolysis with dilute sulfuric acid. It was shown that xylose was the major constituent and small amounts of glucose and galactose were contained in the xylan samples. Adsorption of xylan on hardwood fibers was evaluated using acid hydrolysis and HPLC techniques. Results showed that the adsorption of negatively charged xylan on the fiber surface was negligible probably because electrostatic repulsion between these two materials. Pretreatment of the fiber with alum increased xylan adsorption. The amount of adsorption increased up to 30 mg/g. With the increase of xylan adsorption both tensile and tear strength of the handsheet increased suggesting xylan can be a very effective strength agents for papermaking. Brightness of the handsheets decreased, however, with the use of xylan.
Wet compaction test (WCT) is a fiber evaluation method where wet fibers are compressed at one side of a cylinder and water drains out from the other side. The consistency of the fiber furnishes and their pressures are recorded during the test. In the previous study we found that WCT results always gave better coefficients of determination in fiber furnish drainage, and paper properties (density, tensile, tear, and burst strength) than those of WRV (water retention value). Fiber freeness and fiber length correlated well with drainage and tear strength of the furnishes, respectively; however, their correlations were very much improved by combining the WCT results. In this study, we used the WCT test for fractionated fiber furnishes to see whether improvement of the WCT is possible. We found that strength properties such as breaking length and burst index were correlated better with the fractionated long fiber furnishes. Drainage was greatly affected by the presence of short fiber furnishes. We used bleached chemical pulps (SwBKP, HwBKP), recycled pulp (OCC), and mechanical pulp (BCTMP) as fiber furnishes in this study. Fiber fractionation can be performed on-line in these days by using multifractor and WCT can be used as an on-line test in papermachine in the future.
목적: 광범위 회전근 개 파열은 파열단의 내측 퇴축, 근위축 및 지방 변성과 함께 주변 조직과의 유착 등으로 해부학적 봉합이 불가능한 경우가 많다. 본 종설에서는 광범위 회전근 개 파열의 여러 가지 치료 방법 중 봉합술에 대하여 문헌 고찰과 함께 임상 결과 향상과 재파열율을 감소시키고 치유력을 높일 수 있는 방법을 알아보고자 한다. 대상 및 방법: 광범위 회전근 개 파열 봉합술의 선택은 환자의 나이, 동반 질환, 통증, 운동 범위 감소, 근력 약화 등의 임상 증상 및 재활 의지 등 환자와 관련된 요인과, 회전근 개 파열 기간, 크기, 퇴축 및 지방 변성 정도 등은 회전근 개와 관련된 요인을 고려하여 선택해야 한다. 결과: 조직의 가동성이 떨어져 해부학적 봉합이 어려운 광범위 회전근 개 파열은 주변 조직 유리술과 간격 활주 방법으로 가동성을 증가시켜 봉합할 수 있다. 주변 조직 유리술을 시행하고 회전근 개의 가동성을 증가시켜도 해부학적 봉합이 불가능한 광범위 회전근 개 파열은 부분 봉합, 변연 수렴 술식 및 상완 이두건 절제 및 고정술 등의 대체 술식을 고려할 수 있다. 광범위 회전근개 파열 환자에서 견봉하 감압술 및 회전근 개 봉합술에 관한 여러 보고는 통증 감소, 견관절 기능 및 근력 회복 등 만족할만한 임상 결과를 보고하고 있으나 장기 추시 결과 높은 재파열율이 관찰되고 있다. 결론: 광범위 회전근 개 파열의 치료는 아직까지 높은 재파열 발생율로 해결해야 할 과제가 많으나 광범위 파열의 병인, 진행 과정 및 임상 양상에 대한 이해와 재파열 예후 인자 분석 등을 통해 향상된 치료 성적을 얻을 수 있을 것으로 사료된다.
본 연구는 골판지고지의 가장 중요한 물성인 강도적 성질을 증대시키기 위하여 섬유의 전처리 방법으로서 Hobart mixer를 사용하여 섬유 내부의 결합제거 및 섬유표면에 Microcompression을 형성하여 개질처리 하였으며, 4가지 방법의 고해방법(Valley beater, Kady mill, Impact refining)을 채택하여 골판지고지의 최적고해 방법을 찾는데 그 목적을 두고 이 연구를 수행하였다. 그 결과는 다음과 같다. 1. 섬유장감소를 적게 유발하는 고해방법으로는 Kady mill과 PFI mill의 고해방법 이었으며 고해시 Curl을 펴주는 고해방법을 Kady mell과 Valley beater의 고해방법이 적절하였다. 2. 열단장은 Valley beating의 고해방법에 의하여 강도가 가장 많이 증가되었으며 인열강도는 가장 많이 감소하였다. 섬유전처리를 한 Comp-1과 Comp-2는 고해방식과 상관없이 열단장을 전체적으로 약 10% 증대시켰다. 3. 인열강도는 Comp-1, Comp-2의 전처리방법과 PFI mill 고해로 증대할수 있었다. 4. 파열강도는 Comp-1, Comp-2의 전처리방법과 Valley beater 고해로 증대할수 있었다. 5. 인열강도를 우지한채로 열단장을 가장 높이는 방법은 섬유전처리 방식인 Comp-1이나 Comp-2를 사용하여 Valley beating을 이용하는 방법으로 판단되었다.
