An Attention-based Temporal Network for Parkinson's Disease Severity Rating using Gait Signals

  • Parkinson's disease (PD) is a typical, chronic neurodegenerative disease involving the concentration of dopamine, which can disrupt motor activity and cause different degrees of gait disturbance relevant to PD severity in patients. As current clinical PD diagnosis is a complex, time-consuming, and challenging task that relays on physicians' subjective evaluation of visual observations, gait disturbance has been extensively explored to make automatic detection of PD diagnosis and severity rating and provides auxiliary information for physicians' decisions using gait data from various acquisition devices. Among them, wearable sensors have the advantage of flexibility since they do not limit the wearers' activity sphere in this application scenario. In this paper, an attention-based temporal network (ATN) is designed for the time series structure of gait data (vertical ground reaction force signals) from foot sensor systems, to learn the discriminative differences related to PD severity levels hidden in sequential data. The structure of the proposed method is illuminated by Transformer Network for its success in excavating temporal information, containing three modules: a preprocessing module to map intra-moment features, a feature extractor computing complicated gait characteristic of the whole signal sequence in the temporal dimension, and a classifier for the final decision-making about PD severity assessment. The experiment is conducted on the public dataset PDgait of VGRF signals to verify the proposed model's validity and show promising classification performance compared with several existing methods.

뇌졸중 환자에 대한 기능적 보행평가, 버그 균형척도, 일어나 걸어가기 검사의 동시 타당도 (Concurrent Validity of the Functional Gait Assessment, Berg Balance Scale, and Timed Up and Go Test in Patients With Stroke)

  • Balance is a complex motor skill that depends on interactions between multiple sensorimotor processes and environmental and functional contexts. Many rehabilitation specialists believe that balance assessment under multitask conditions may be a more sensitive indicator of balance problems and falls than balance assessment in a single-task context. Functional Gait Assessment has many tasks that allow for testing under multitask conditions. The purpose of this study was to determine the concurrent validity between the Functional Gait Assessment (FGA), Berg Balance Scale (BBS), and Timed "Up & Go" Test (TUG) in patients with stroke. One hundred and five participants with at least 3 months post stroke and able to walk at least 6 m with or without a mono cane, participated in this study. Concurrent validity between the FGA, BBS, and TUG was assessed using Spearman rank order correlation. The FGA correlated with the BBS (r=.80, p<.01) and TUG (r=-.77, p<.01). The good and moderate correlation between the FGA, BBS, and TUG establishes the concurrent validity of the FGA in patients with stroke. These measures provide clinicians with valuable information about patients' functional balance capabilities.

  • GA 기반의 4족 보행로봇의 걸음새의 속도와 안정성을 최적화하는 걸음새의 자동 생성 방법을 구현한다. 기존에 사용된 걸음새 파라미터 집합에서 중요 파라미터의 영향을 분석하였고, 이를 통해 효율적인 탐색 방향을 설정하였다. 또한 속도 위주의 기존 연구와는 달리 반 정확도가 수반된 결과를 얻도록 하였다. 제안된 기법의 검증을 위하여 SONY Aibo 4족 보행 로봇에 대해서 ODE 기반의 물리적 특성을 포함한 정교한 시뮬레이션이 가능한 Webots 을 이용하여 실험을 수행하였고, 속도와 안정성 면에서 향상된 결과를 얻었다.

50세 이상 성인의 보행 시 스마트폰 사용에 따른 자세 조절 전략 (Postural Control Strategies on Smart Phone use during Gait in Over 50-year-old Adults)

  • Objective: The aim of this study was to investigate postural control strategies on smart phone use during gait in over 50-year-old adults. Method: 8 elderly subjects (age: $55.5{\pm}3.29yrs$, height: $159.75{\pm}4.20cm$, weight: $62.87{\pm}8.44kg$) and 10 young subjects (age: $23.8{\pm}3.19yrs$, height: $158.8{\pm}5.97cm$, weight: $53.6{\pm}5.6kg$) participated in the study. They walked at a comfortable pace in a gaitway of ~8 m while: 1) reading text on a smart phone, 2) typing text on a smart phone, or 3) walking without the use of a phone. Gait parameters and kinematic data were evaluated using a three-dimensional movement analysis system. Results: The participants read or wrote text messages they walked with: slower speed; lesser stride length and step width; greater flexion range of motion of the head; more flexion of the thorax in comparison with normal walking. Conclusion: Texting or reading message on a smart phone while walking may pose an additional risk to pedestrians' safety.

Correlation among Functional Leg Length Discrepancy, Muscle Activity, Muscle Contraction Onset Time and Vertical Ground Reaction Force during Simple Lifting Task

  • Purpose: Leg length discrepancy causes the posture deformation, gait asymmetry, and lower back pain. The purpose of this study is to investigate the correlation among functional leg length discrepancy (FLLD), muscle activity, muscle contraction onset time and vertical ground reaction force (vGRF) during simple lifting task. Methods: Thirty-nine subjects participated in this study. FLLD was measured from the umbilicus to medial malleolus of left and right leg using a tape. The subjects performed to lift a 10 kg box from the floor to chest. The muscle activity and muscle contraction onset time of rectus abdominis, erector spinae and rectus femoris was measured using EMG system and vGRF was measured by two force plate. Pearson correlation was used to fine out the correlation among FDDL, muscle activity, muscle contraction onset time and vGRF during simple lifting task. Results: Correlation between FLLD and difference of muscle activity of short-long side was very high (r>0.9) during simple lifting task. Correlation between FLLD and difference of muscle contraction onset time of short-long side was very high (r>0.9) during simple lifting task. And correlation between FLLD and difference of vGRF of short-long side was high (r>0.7) during simple lifting task. Conclusion: This study suggests that there is high correlation between FLLD and muscle activity, muscle contraction onset time, and ground reaction force during simple lifting task. Therefore, FLLD could negatively affect the postural balance.

무릎관절 추적훈련이 편마비 환자의 보행에 미치는 영향 (Effects of Knee Tracking Training on Gait in Stroke Patients)

  • Manual tracking is an experimental paradigm that can be used to study information processing in continuous movements involving accurate, ongoing control of motor performance. The purpose of this study was to identify the effects of knee tracking training, using the paretic side, on gait in stroke patients. Nine patients with hemiplegia participated in the study. The timed 10 m gait speed test and tracking test were administrated. The tracking test was composed with ranges of $-20^{\circ}$ to $20^{\circ}$ and $0^{\circ}$ to $60^{\circ}$. The tracking training consisted of five times every week for 4 weeks. The data were analyzed by non-parametric paired sign test of Wilcoxon. The flexion/extension error of the tracking test was significantly reduced on the paretic side, while the nonparetic side was not statistically significant. The transfer of the skill to the functional activity was shown in the significant improvement at timed 10 m gait speed test. This study shows that individuals with chronic who have impaired knee movement can be trained to improve their knee control through intensive practice at a knee movement tracking task and that the skill learned from such training is transfered to a more functional gait speed.

The Influence of Restricted Arm Swing on Symmetry, Movement of Trunk and Pelvis Rotation according to Using a Mobile Phone

  • Purpose: This study was conducted to investigate the effects of variations in arm swing during gait on movement of the trunk and pelvis. During the gait task, the angle of the trunk and pelvic rotation were analyzed according to arm swing conditions. Methods: Seventeen healthy males participated in this study. All subjects were analyzed for gait on a treadmill three times each under three different types of arm swing conditions - natural arm swing, restricted arm swing using a phone, restricted swing in both arms. 3-D motion analysis systems were used to collect and analyze the kinematic data of trunk and pelvic movements, and repeated one-way ANOVA was used to compare the trunk and pelvic kinematic data and symmetry index. The level of significance was ${\alpha}=0.05$. Results: The results showed kinematic differences in trunk and pelvic during gait based on the arm swing conditions. Specifically, there were significant differences in trunk rotation, left and right trunk rotation and symmetry index of trunk rotation during gait among the three arm swing conditions. ROM was used to calculate a symmetry index (SI) based on the average left and right trunk rotation in which a value closer to zero indicated better balance. The SI obtained for arm swing restricted with the phone was closer to -1 than the other conditions. Conclusion: Restricted arm swing due to use of a phone had the possibility to induce instability of postural control while walking, which could be seen to suggest a risk of falling during gait.

동작관찰훈련프로그램이 편마비환자의 보행 및 균형에 미치는 영향 (Effects of Action-observational training Program on Gait and Balance of Patients with Hemiplegia)

  • 본 연구는 만성뇌졸중 편마비 환자에게 적용한 동작관찰 훈련이 보행과 균형에 미치는 영향을 알아보고자 하였다. 연구에 동의한 만성 뇌졸중 편마비 환자 4명중 동작관찰훈련을 시행한 실험군 2명 일반과제지향훈련을 시행한 대조군 2명에게 총 4주간 하루 30분씩 훈련하였다. 훈련효과를 알아보기 위해 훈련전 보행은 10m검사를 시행하였고 균형은 버그균형척도로 검사하고 훈련 후 다시 재측정하였다. 이에 동작 관찰 훈련군이 일반과제지향훈련군에 비해 보행과 균형에서 유의미한 차이를 보였다. 이에 동작관찰훈련이 만성뇌졸중 편마비환자의 보행과 균형에 효과적인 프로그램임을 알 수 있다.

시설 노인을 위한 기능적 그룹활동 프로그램의 개발 및 운영 평가 (Development and Evaluation of Functional Group Activity Program on Institutionalized Aged)

  • Purpose: The purpose of this study was to identify the changes of physical function, perceptual and cognitive function, emotional function, and functional independence in the institutionalized aged according to functional group activity program (self help Tai Chi exercise plus functional task). Methods: Study subjects were 20 institutionalized aged from June to October in 2010. The subjects received functional group activity program two times a week for 15 weeks. Physical function (grip strength, coordination, lower extremity strength, balance, gait, trunk flexibility), perceptual and cognitive function, emotional function(depression, social skill), and functional independence were measured before and after the program. Results: The subjects showed significantly increased physical function (coordination, lower extremity strength, gait, trunk flexibility), perceptual and cognitive function, emotional function (depression, social skill), and functional independence. The functional group activity program may be an effective strategy for institutionalized elders to enhance their functions. Conclusion: The functional group activity program may be effective on elderly institutions which have limitation in human, material, environmental resources.

Effect of Dual Task Training in Visual Control and Unstable Base on the Gait of Stroke Patient

  • This study examines changes in walking ability among patients with stroke after applying dual-task training under the condition of visual control and unstable supporting ground; the purpose is to provide reference data for selecting intervention methods that enhance the walking ability of patients with stroke. Among the patients with stroke who received rehabilitation treatment(at Rehabilitation Hospital B in Gyeonggi, South Korea from May 2014 to July 2014), 29 patients were selected as research subjects; all of them understood the purpose and contents of this research and agreed to participate in the experiment. The research subjects were divided into a visual control and unstable supporting ground dual-task(VUDT) group(10 patients), a visual control dual-task(VDT) group(10 patients), and an unstable supporting ground dual-task(UDT) group(9 patients); all of the subjects received 30-minute trainings, three times a week for a total of four weeks. A Timed-Up-and-Go(TUG) test was performed to investigate the change of walking function among the subjects, and a 10m walking test was conducted to measure their walking speed. According to the study results, all three groups showed significant differences after dual-task training; the dual-task training group under the condition of visual control and unstable supporting ground showed the most prominent change. This study confirmed that dual-task training using visual control and unstable supporting ground has a positive impact on the walking ability of patients with stroke. Through the study results, we found that implementing dual-task training under the condition of visual control and unstable supporting ground can more effectively improve the walking ability of patients with stroke, rather than performing visual control dual-task training or unstable supporting ground dual-task training only.
