The One Case of Infertle Women Taking Herbal Medicine before In Vitro Fertilization to Birth (체외수정 전부터 출산까지 한약을 병용한 난임 1례)

  • Purpose: Many infertile women are receiving in vitro fertilization-embryo transfer (IVF-ET). But side effects occur after IVF-ET and pregnancy rate is still low. So this study is to report the effect of herbal medicine on a woman receiving IVF-ET and herbal medicine's low risk on a pregnant woman. Methods: The patient was underwent artificial insemination five times and IVF-ET three times. In this study, the patient steadily took a herbal medicine before IVF-ET to birth. Results: The side effects of IVF-ET and the miscarriage symptoms ware decreased. And by taking a herbal medicine during IVF-ET, the patient was pregnant and gave birth when she was 33 weeks pregnant by cesarian section. Conclusions: This case study shows that herbal medicine is effective for reducing side effects of IVF-ET and increasing pregnancy rate. And in this case the patient gave birth to healthy babies, althogh steadily taking herbal medicine during pregnancy.

Research about Changes in Symptoms After Taking Sasang Medicine according to Sasang Constitution (약물 복용 후 체질에 따른 소증 변화에 대한 연구)

  • Objectives The purpose of this study was to survey changes in symptoms caused by Sasang medicines. Methods We recruited 45 subjects who were classified 15 typical Soeumin, Soyangin, Taeumin and did a survey about symptoms. We split Soeumin into 3 groups and prescribed medicines Palmulgunja-tang, Yanggyuksanhwa-tang, Taeumjowi-tang. We did same process in Soyangin and Taeumin. After taking medicines for 4 weeks, we did a survey about physical changes. Results There was no consistent changes in symptoms. But medicines which were not suitable to their Sasang constitution induced known reactions. And medicines which were suitable to their Sasang constitution induced reactions. Conclusions Changes in symptoms appeared after taking medicine, but they can be induced by medicine.

A Systematic Review and Meta Analysis of Randomized Controlled Trials of the Clinical Effect on Taking a Herbal Medicine on Postpartum Lactation (한약 복용이 산후 유즙분비에 미치는 효과에 대한 체계적 문헌고찰)

  • Objectives: The purpose of this study is to assess the effect of taking herbal medicine for the postpartum lactation. Methods: We searched 9 databases for recent three years, that contained four english, two chinese, one japanese and two korean database from September 17, 2016 to December 31, 2020. Randomized controlled trials (RCTs) were eligible. Measurement of outcome included total curative effective rate, volume of lactation, volume of milk supplement, and serum prolactin concentration. The risk of bias was assessed by two independent authors using the Cochrane risk of bias tool. Results: Total 725 of studies was screened, 11 RCTs were finally selected. Number of participants per study ranged from 60 to 257. The treatment group of taking a herbal medicine is effective for improving total curative effective rate, volume of lactation, volume of milk supplement, and serum prolactin concentration compared with control group. Because most of the studies had considerable heterogeneity in terms of type of intervention and comparison for outcome measurement, meta analysis for quantitative analysis was impossible. Conclusions: This studies showed that taking a herbal medicine is effective on increasing volume of lactation and speeding up the start of the first breastfeeding. However, included studies suffered from incomplete reporting, high or unclear risk of bias and substantial heterogeneity between studies. In the Future, further high-quality RCTs are needed to prove effectiveness of herbal medicine for breastfeeding and reduce the risk of bias.

Effect of Lifestyle Modification Program on Blood Pressure Improvement in Patients with Hypertension (고혈압 환자에서 생활습관 개선 프로그램의 혈압개선효과)

  • Objectives: The purpose of this study was to know different effect with uncontrolled hypertension patients after providing health promotion program which consisted with medicine, exercise, nutrition. Methods: The subjects of this study was comprised by uncontrolled hypertension patients in spite of medication and didn't care the pressure by medication. The health promotion program was progressed by group exercise three times a week, nutrition education once a week and medical consultation once a month for 12 weeks. Subjects were measured for body composition(weight, fat mass, % body fat and body mass index), hemo-dynamics(systolic blood pressure(SBP), diastolic blood pressure(DBP), and resting heart rate), and physical fitness (cardiopulmonary endurance, muscular strength, muscular endurance, balance, and flexibility). Results: Groups showed significant improvement in every measure except resting heart rate. SBP is decreased both taking drug group about 18.4mmHg and without taking drug group about 19.4mmHg.(p<0.001) DBP is decreased both taking drug group about 8.7mmHg and without taking drug group about 9.0 mmHg.(p<0.001) Conclusion: There are no statistical significant differences of SBP and DBP decreasing effects by medication, Since effects of decreasing pressure are not different by medication, I think the health promotion program is effective to uncontrolled hypertension patients to decrease pressure.

The Study on Three-portion and Nine-position Pulse Taking Diagnosis (삼부구후진단(三部九候診斷)에 대한 고찰(考察);"내경"과 "난경"을 중심으로)

  • This present study tried to observe an each feature that the difference of Neijing(內經) and Nanjing(難經) descriptions about the three-portion and nine-position pulse taking(三部九候脈). It was interpreted and contradistinguished that discrimination of position, diagnosis object, and the perceptions of Earth of the center among the Five Phase of each literature were described in Neijing and Nanjing for the three-portion and nine-position(三部九候). In Neijing, the three-portion and nine-position method divided three parts the whole body. Then the each three parts again divided with Heaven, Earth and Man(天地人). About the corresponding parts of pulse diagnosis, there mentioned for the head and the Zang-Fu organs(臟腑), but not mentioned for the hand and foot. In addition, Earth is assigned to the Earth(土), an each Earth accounted for the source of life. In Nanjing, three-portion divided Chon, Gwan, and Cheok(寸關尺) and each spots separated three stage of pulse taking. For the pulse taking spots and diagnosis, there mentioned the hand and foot instead of the unclear mention of Zang-Fu organs. Then Gwan spot and middle stage of pulse taking were assigned to the Earth, respectively. It was emphasized stomach Qi(胃氣) that the region of Earth, Gwan spot and middle stage among the pulse taking spots each literature were described in Neijing, Nanjing.

Status of Hypertensive Patients' Drug-taking in Health Center and Its related Factors (보건소 방문 고혈압환자의 약물복용 실태와 관련요인)

  • The purpose of this study is to show that, before registration of chronic degenerative diseases and enforcing management system in health centers, the management of hypertension could achieved comprehensively and continuously by knowing the realities of drug-taking and its related factors those who are being cared for in health centers. For one year, a questionnaire about drug-taking realities of hypertensive patient was distributed at 8 health centers in Taegu from March 24. 1997 to April 24. 1997. The questionnaires were then collected and analysed. The results are as follows: Of 691 patients, 77.4% of patient were taking medicine regularly every day. The reasons why patients weren't taking medicine every day was as follows: the blood pressure became normal(34%, the highest of the group), patients forgot(28.2%), it was difficult to buy the medicine(15.4%), there was no effect even though the patient took medicine continuously. The experience of medical treatment outside of health centers was 28.9%. The types and percentages of alternative medicine are herbs (50.5%), health food(24.5%), folk remedy(13.7%). Among them, 44.6% of patients didn't know if the medicine was effective. Medical treatment places excluding health centers was pharmacies(63.5%-the highest rate). In simple analysis, the older one was the more regularly one took the medicine, but there was no statistical relations. Of health activities, patients eating low-salt diet showed that they took the medicine regularly. Nonsmoking patients and those who experienced drug side-effect and those who doubted doctor's prescriptions and instructions showed that they took alternative, excluding modem medicine. Anti-hypertensive drug medication status according to recognition, attitude, experiences of hypertension showed that patients who thought that they should take anti-hypertensive drug during the whole life time took the medicine regularly(82.3%). The reasons for patients turning to alternatives varied. In case of having subjective symptom(34.1%), those who thought they knew the hypertension well(36.6%), they decided to use alternatives. In multiple logistic regression analysis the key statistics were as follows: The older patients, patients who ate low-salt diet, patients who thought that they should take anti-hypertensive drug during the whole life time, all showed that they took medicines regularly. And also patients who experienced drug side-effect, doubted prescriptions and instructions, and patients who had subjective symptoms, patients who thought that he knew the hypertension very well, all showed higher rate of using alternative medicine. In the future, at the health centers, to register and manage hypertension patients effectively, we must educate patients about regular drug-taking, and alternatives without doctor's prescriptions.

Case of the Anti-Type 2 Diabetes Effects on the Prescription of Red Onion (한약과 자색양파 제재로 호전된 제2형 당뇨병 42례 보고)

  • The purpose of this study is to observe clinically the effects of red onion prescription on type 2 diabetes. The subject of this study were middle-aged people. We measured clinically physiological index, body composition, radial arterial pulse wave before taking medicine and after taking medicine for 4 month (the prescription of red onion). The study results were as follows. In physiological Index, diastolic blood pressure(mmHg)(p<0.001), Fasting blood glucose(mg/dL)(p=0.001), and HbA1c(%)(p<0.001) were decreased significantly after taking medicine. In body Composition, waist-hip ratio(p=0.031), body fat percentage(p=0.006), fat mass(kg)(p=0.005), and muscle mass(kg)(p=0.001) were decreased significantly after taking medicine. Blood Circulation Index, ECO(L/min)(p=0.038) and ECI($L/min/m^2$)(p=0.007) were increased significantly after taking medicine, ECR($dyne^*sec^*cm^{-5}$)(p=0.002), and ECRI($dyne^*sec/cm$)(p=0.001) were decreased significantly after taking medicine for 4 month. The prescription of red onion was showed significant change in physiological index, character of body composition, blood circulation index. Based on this result, it is considered that red onion prescription has the positive effects on type 2 diabetes and blood circulation clinically. In the future, diversified studies on the effects of red onion prescription are needed.

Study to Examine the Awareness of the Parents, whose Children are Attending an Elementary School in Gyeonggi-do, on Herbal Medication and Health Functional Food (수도권 지역 초등학생 학부모의 한약 및 건강기능식품의 인식 실태에 대한 연구)

  • Objectives: The purpose of this study is to examine the different preferences between herbal medicine and health functional food, and analyze the reasons for the preferences to figure out the better way of promoting herbal medicine. Methods: 500 questionnaires has been handed out to the parents having elementary school students in Gyeonggi-do province, and 331 questionnaires has been collected and evaluated for this study. Results: Among the 331 responses, 58.4% were female, and 41.6% were male. The percentages of the subjects who consume health supplements and that of herbal medicines were 67.4% and 59.2% respectively. The preference rate for the health supplements was 57.6%, and for herbal medicines was 42.4%. The major reason of taking the health supplements was that they were convenient to consume(54%). The major reason for choosing herbal medicines was that people 'expect them to be more effective'(72.7%). However, the problem with Herbal medicine was their consumptions were inconvenient (39.8%), and their tastes was unfavorable. The most preferable form of herbal medication was pills(50.6%), followed by syrup(22.9%). Pertinent price for herbal medication, the participants were favorable to pay 30,000 within a week. For the question on when to start taking the herbal medication, 18 months after was the popular answers from the participants. Conclusions: Among the 331 subjects, the percentage of consuming the health supplements (67.4%) was higher than that of consuming the Herbal Medicine (59.2%). Also the preference was higher than the Herbal Medicine. The reason why the participant preferred health supplements was that they were 'easy to take', The reason for subjects' great preference in herbal medicine was based on their conception that herbal medicines were highly effective. When taking herbal medicine, participants addressed inconvenience in consuming, and unfavorable taste as the biggest problems. Also they thought that pills would be most convenient form of herbal medicine to consume. For the proper price, they were willing to pay less than 30000won, lower than a market price, per a week for the herbal medicine. Most parents thought that proper age of taking herbal medicine is ' 18 month after the birth'. Taking these into account, different perspectives on herbal medicine is necessary at the very moment.

Severe Airway Obstruction due to Massive Retropharyngeal Hematoma in a Warfarin-Taking Patient with a Normal International Normalized Ratio

  • Warfarin is used as part of the treatment of various diseases, and laboratory monitoring of its effects is required. Airway hematoma secondary to warfarin is rare, but can be fatal because of potential airway obstruction. Rapid definitive airway establishment is crucial if airway obstruction is suspected. This complication is more likely to occur in those with elevated coagulation laboratory values. However, we experienced a patient in whom a massive retropharyngeal hematoma caused airway obstruction after a non-severe motor vehicle collision. The patient had been taking warfarin, and had coagulation parameter values within the normal ranges. A major fracture or hemorrhage was not anticipated. Upon examination, a massive retropharyngeal hematoma was noted. Orotracheal intubation failed due to an airway obstruction. Emergency tracheostomy and an operation for hematoma removal were performed. Physicians must always consider the possibility of airway hematoma in warfarin-taking patients with normal coagulation values regardless of the severity of mechanism of injury.

The clinical effect of Cheongpa-jeon on the group of patients who were treated under conservative remedies for LBP(Low Back Pain) (요통(腰痛)의 보존적(保存的) 치료법(治療法)을 시행한 환자군(患者群)에 대한 청파전의 임상적(臨床的) 효과(效果))

  • Low back pain(LBP) is known as one of the diseases that produce severe pains with 80% of the whole population having experienced it and reported that, even at this moment, 20-30% of the world population is suffering from it. The writer has performed 'Conservative remedies (non-surgical procedures taken)' from $1^{st}$ of January 2003 to $31^{st}$ of December the same year for 76 cases who have been hospitalized in Jaseng Hospital and adopted as having been diagnosed either HIVD L4-5 or HIVD L5-S1 or as having both. The clinical analysis was made through statistical researches which include : the changing degree in SLR test of the Cheongpa-Jeon(CP) taking patient group and the non-taking group, the change of VAS(Visual analogue scale) against pains, and the days of hospital treatment. The result demonstrated that the CP taking group showed higher changes in SLR test, bigger reduction in VAS changes and the days for hospitalization shorter than the group that didn't take CP.
