  • Objectives : To get to know the effects of placental extract and laser theraphy for Temporomandibular Joint Disorder, we have tried a injection of placental extract and laser theraphy into 聽宮(SI19), 聽會(GB02) for patient with Temporomandibular Joint Disorder. Methods : In this case clinical assessment of temporomandibular joint dysfunction is assessed by the VAS score and the Korean Version of Research Diagnostic Criteria for Temporomandibular Disorders(RDC/TMD). Results : The VAS score was significantly decreased after Placental extract herbal acupuncture and laser theraphy Conclusion : Placental extract herbal acupuncture and laser theraphy effectively resolved pain for Temporomandibular Disorders.

이명과 악관절장애간의 상호관계 문헌연구 (The Literature Study on the Relationship between Tinnitus and Temporomandibular disorder)

  • Objective : We investigated relationship between tinnitus and temporomandibular disorder(TMD) and tried to find approach possibility in oriental medicine. Method : We searched Pubmed, Scopus, The Cocharane library and additional sources for published and unpublished trials. Search word was "Tinnitus and TMD" Result : We investigated 18 studies. We divided these studies into three parts, epidemiological studies, mechanical studies and treatment studies. In many epidemiological studies. there was significant relationship between tinnitus and TMD. The mechanism was not clear but in recently several studies proposed neurological approach. TMD treatment showed significant improvement in tinnitus accompanied by TMD. Conclusion : In tinnitus accompanied by TMD, TMD treatment can be available to resolve or reduce tinnitus. Especially in oriental medicine acupuncture can be applied to treat this.

  • Objective: The aim of this study is to investigate whether patients with temporomandibular disorder have more anxiety than normal people. Methods: Patients group(n=50) with TMD and normal group(n=30) without TMD and other disease completed STAl(State-Trait Anxiety Inventory), and there was comparison of mean scores between patients and normal groups. Results: The mean of total STAl scores of patients group was $91.32{\pm}20.32$, whereas that of normal group was $81.27{\pm}12.97(p<0.01)$. The mean of state-anxiety scores of patients group was $45.68{\pm}11.30$, whereas that of normal group was $40.70{\pm}7.28(p<0.05)$. The mean of trait-anxiety scores of patients group was $45.64{\pm}10.02$, whereas that of normal group was $40.57{\pm}7.22(p<0.05)$. There was a tendency that patients with TMD had more anxiety. So, doctors had better give attention to patients' anxiety when they treat the patients with TMD.

  • Objectives : To study the cervical lordosis and roentgenographic analysis of cervical disc space in neck pain with or without temporomandibular disorder(TMD). Methods : Radiographic measures of cervical lordosis, cervical disc space narrowing were collected, statiscally analyzed. Results : TMD group showed a significant increase in cervical lordotic angle as compared with non-TMD group, whereas no significant change in cervical disc degeneration. Conclusions : The findings from this study suggest that the curvature of the cervical spine is related to the subject's TMD.

  • Objectives : This study aimed to investigate relationship between the psychological factors, the prevalence, and perception of temporomandibular disorder in Korean adults. Methods : A total of 7203 data were derived from Korean national health and nutritional examination survey. All data were analyzed using R-COMMANDER(12.2 SDI) program. Results : The prevalence of temporomandibular disorder in Korean adults was 3.1% and 10.5% of adults with temporomandibular disorder (TMD) had more than one TMD related symptom. Women had higher prevalence rate of TMD related symptom than men. Prevalence rate in TMD was low in the younger age, the higher education level, higher income, and the professionals. Psychological factors including cognition of stress and depression was shown to be closely related with suicidal ideation and TMD prevalence. Conclusions : To prevent TMD, it is necessary to provide the systematic oral health education and to develop the combined psychological counseling with treatment program.

Change of Balance Ability in Subjects with Pain-Related Temporomandibular Disorders

  • Purpose: Temporomandibular disorder (TMD) is a condition defined as pain and dysfunction of temporomandibular joints and masticatory muscles. Abnormal interconnections between temporomandibular muscles and cervical spine structures can cause the changes of postural alignment and balance ability. The aim of this study was to investigate changes in static balance ability in subjects with painrelated TMD. Methods: This study conducted on 25 subjects with TMD and 25 control subjects with no TMD. Pressure pain thresholds (PPTs) of the masseter and temporalis muscles were measured using a pressure algometer. Static balance ability was assessed during one leg standing using an Inertial Measurement Unit (IMU) sensor. During balance task, the IMU sensors measured motion and transfer movement data for center of mass (COM) motion, ankle sway and hip sway. Results: PPTs of masseter and temporalis muscles were significantly lower in the TMD group than in the control group (p<0.05). One leg standing, hip sway, and COM sway results were significantly greater in the TMD group (p<0.05), but ankle sways were not different between group. Conclusion: We suggest pain-related TMD is positively related to reduced PPTs of masticatory muscles and to static balance ability. These results should be considered together with global body posture when evaluating or treating pain-related TMD.

A clinical evaluation of botulinum toxin-A injections in the temporomandibular disorder treatment

  • Background: This study clinically evaluated the effect of botulinum toxin type A (BTX-A) in the temporomandibular disorder (TMD) treatment using Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD). Methods: A total of 21 TMD patients were recruited to be treated with BTX-A injections on the bilateral masseter and temporalis muscles and were followed up by an oral and maxillofacial surgeon highly experienced in the TMD treatment. For each patient, diagnostic data gathering were conducted according to the RDC/TMD. Characteristic pain intensity, disability points, chronic pain grade, depression index, and grade of nonspecific physical symptoms were evaluated. Wilcoxon signed-rank test was applied for statistical analysis. Results: The results showed that more than half of the participants (85.7 %) had parafunctional oral habits such as bruxism or clenching. In comparison between pre- and post-treatment results, graded pain score, characteristic pain intensity, disability points, chronic pain grade, and grade of nonspecific physical symptoms showed statistically significant differences after the BTX-A injection therapy (p < 0.05). Most patients experienced collective decrease in clinical manifestations of TMD including pain relief and improved masticatory functions after the treatment. Conclusions: Within the limitation of our study, BTX-A injections in masticatory musculatures of TMD patients could be considered as a useful option for controlling complex TMD and helping its associated symptoms.

The Effects of Academic Stress and Self-Control on Temporomandibular Disorder of Adolescents

  • Purpose: The aim of this study was to investigate the effects of academic stress and self-control ability on temporomandibular disorder (TMD) of adolescents. Methods: Participants were 1,112 teenagers who were 1st grade students of middle and high school. After checking their academic stress and self-control ability through a self-administrated questionnaire, the students were examined their TMD by two trained dentists. We drew a comparison the level of their academic stress and self-control ability between the students with and without TMD. We divided them to four groups according to the level of academic stress and self-control. And we checked prevalence of TMD in each level. Results: The group of students with TMD was higher academic stress score level than the students without TMD (p<0.05). And they had lower self-control score level than the students without TMD (p<0.05). Even if they were under the same academic stress, the group of students with higher self-control score was less TMD prevalence than the students with lower score. Conclusions: Academic stress makes adolescents to increase TMD and high ability of self-control makes them to decrease it. And these tendencies are seemed to have a great influence on young male student.

일부 보건계열 학생들의 국가시험 스트레스와 측두하악장애의 관계 (Relationship between national exam stress and temporomandibular disorder(TMD) in allied health students)

  • Objective: The aim of the study was to investigate degree of national exam's stress, symptom of temporomanibular disorder(TMD) in allied health students and relationship between of them. Methods: A self-reported questionnaire was completed by 305 students of dental hygiene and physical therapy in Chungbuk and Gyeongnam in December 2015. Except incomplete questionnaire, 260 data were analyzed using SPSS 12.0 program. The study instruments consisted of subjective symptom of temporomandibular disorder, bad habit of mouth and degree of national exam's stress. Results: National exam stress was higher in female and subjective unhealthy students(p<0.05). High group and usual group on national exams's stress showed high subjective symptom(p<0.05). National exam's stress were related to subjective symptom of TMD and bad habit of mouth(p<0.05, p<0.01). Subjective symptom of TMD were related to bad habits of mouth(p<0.01). Conclusion: The degree of stress, TMD and bad habits of mouth on national exam test was high. therefore, it is important to manage the national exam's stress and to develop program in order to reduce the national exam's stress in allied health students.

  • Temporomandibular disorder(TMD) is described as a cluster of disorders characterized by pain in the preauricular area and/or the muscles of mastication; limitations or deviations in mandibular range of motion; and noises in the TMJ during mandibular function. The most common symptom in TMD patients is pain that is aggravated by chewing or other jaw function. These symptoms are appeared when the stimuli loaded in TMJ are over the physiologic tolerance. The primary goal in treatment of TMD is to alleviate pain and lor mandibular dysfunction. TMD treatment can be divided into 2 categories: reversible and irreversible methods. Reversible methods include medication, thermal therapy, habit modification, physical therapy, appliance therpy and arthrocentesis and lavage and irreversible methods include arthroscopic lysis, surgery, occlusal adjustment et al. It is widely accepted that reversible methods are ther first choice of treatments. However if reversible ones are not effective, irreversible methods are considered.
