Integrated Rehabilitation for Children with Cerebral palsy: A Prospective Observational Study

  • Objectives: This study aimed to analyse: 1) the clinical effectiveness and safety of traditional Korean Medicine (TKM) and frequency of TKM therapies used and their relationship with conventional treatments. Methods: This prospective observational study enrolled children with cerebral palsy (CP) aged 6-78 months (n=126). The children who used herbal medication for >30 days or acupuncture treatment >12 sessions within 6 months were defined as the integrated rehabilitation (IR) group; the remaining participants were included in the conventional rehabilitation (CR) group. Results: Changes in the Gross Motor Function Measure-66 (GMFM-66) were greater in the IR group (6.4±6.1) than in the CR group (4.6±5.8). The reduction in the number of other health problems was greater in the IR group than in the CR group. The GMFM-66 improvement was greatest in gross motor function classification system level 1. There was no between-group difference in the frequency of rehabilitation therapy (10.9±6.6 and 12.0±9.9 in the IR and CR groups, respectively). Conclusion: TKM may offer additional benefits in terms of the GMFM-66 score and other health problems. However, there is a need for further randomized controlled trials involving a restricted CP type and a controlled treatment type and intensity to confirm these findings.

  • Parkinson's disease (PD) is a chronic degenerative disorder of the central nervous system. It has no cure, but current treatments can relieve symptoms and maintain quality of life. As PD progresses, controlling its symptoms becomes difficult. Here, we present the treatment of two patients with idiopathic PD using traditional Korean medicine (TKM) and temporomandibular joint balancing therapy (TBT). We measured the progress of the two patients using the unified PD rating scale (UPDRS) and the Hoehn and Yahr (HY) scale. Combined treatment of TKM and TBT decreased the total UPDRS score from 52 to 26 after 21 days in case 1, while it decreased the total UPDRS score from 91 to 65 after 20 days in case 2. In both patients, the HY scale score was maintained at 3, and no adverse events were observed. Thus, the combined treatment of TKM and TBT can produce a treatment response in PD patients.

Case Report for a Refractory Levator Ani Syndrome Treated with Traditional Korean Medication

  • Objectives: To inform a clinical case of a refractory levator ani syndrome, that was improved by treatments of traditional Korean medicine (TKM). Methods: A 55-years old female had complained severe anorectal pain which didn't respond to Western medicine therapeutics during 5 months including 45-day hospitalization. Whereas the symptom was rapidly resolved by the treatment in a Korean medicine hospital, and the clinical outcome was monitored. Result: There was no abnormality explaining the anorectal pain from blood tests, gastrointestinal endoscopy, sonogram and computed tomography for abdomen and pelvis. The patient was diagnosed with a levator ani syndrome. Based on the typical feature of tenderness of lower abdomen as well as beating sign around belly, an herbal drug, Shihogayonggolmoryo-tang (柴胡加龍骨牡蠣湯) was prescribed. In addition, the warm acupuncture at BL 31 to BL 33 and moxibustion on lower abdomen were given to the patient. The anorectal pain was radically reduced from treatment 7-day, and it almost disappeared within treatment 25-day. Conclusion: This case report would provide information for the potential of TKM therapies focused on the refractory levator ani syndrome which no satisfactory therapy exist.

  • Objective: The purpose of this study is to report the therapeutic possibility of a primary peritoneal carcinoma (PPC) patient with Traditional Korean Medicine (TKM). Methods: A 37 years old female patient diagnosed with PPC suffered from operation site pain, neck pain, nausea, headache and fatigue after undergoing operation and chemotherapy (Paclitaxel/Carboplain). The patient was treated with acupuncture, pharmacoacupuncture, moxibution and herbal medicine. Laboratory study and National Cancer Institute Common Terminology Criteria for Adverse Events (NCI-CTCAE), version 5.0 were used to evaluate the safety and adverse events of treatments. The performance status was measured by Eastern Cooperative Oncology Group (ECOG) scale. Results: After treatments, cancer did not show any progression. Related symptoms such as postoperative pain, nausea, fatigue and quality of life have improved. Conclusion: This case study implies that TKM may have therapeutic possibility in treating PPC patient.

  • The prevalence rate of plant poisoning is growing up gradually. However, it could be difficult to get proper treatment without detail medical history taking due to the diversity of poisonous plants and their various symptoms. Scopolia Japonica, which grows naturally in Korea, is reported to cause severe toxicity which could be lethal. A few therapeutic measures have been reported in Traditional Korean Medicine (TKM), but it has not been reported yet that those measures have been successfully proven in the clinical practice in modern times. This is a case report of toxicity caused by scopolia japonica ingestion. A 65years-old male patient with cognitive impairment, behavioral disorder, gait difficulty, dysarthria, dysphagia and abnormal pupil reflex recovered from those symptoms during taking Gamigamdu-tang and getting acupuncture. This study shows the clinical aspects of Scopolia Japonica toxicity and the possibility of TKM treatments as one of plant poisoning treatments.

위암 수술 후 장마비에 대한 한의학적 치료의 효과 : 체계적 문헌 고찰 및 메타분석 (The Effectiveness of Traditional Korean Medicine for Treating Postoperative Ileus in Gastric Cancer Patients: A Systematic Review and Meta-Analysis)

  • 목적: 본 연구는 위암 수술 후 장마비의 한의학적 치료 효과를 평가하기 위해 수행하였다. 방법: 2007년 1월 1일부터 2022년 5월 11일까지 출판된 연구들을 6개의 데이터베이스를 통해 수집하였다. 수술 후 한의학적 치료를 받은 위암 환자를 장마비 관련 지표들을 통해 관찰한 연구들을 선정하였다. 결과: 한약, 침, 뜸, 전기혈위자극, 이혈요법, 족삼리혈의 약물 주입을 수술 후 장마비 치료법으로 사용한 27편의 연구를 선정하였다. 한약, 한약과 침 병용요법, 침, 뜸, 전기혈위자극, 이혈요법, 족삼리혈의 네오스티그민 주입 치료군에서 수술 후 첫 가스 배출까지의 시간이 감소하였고 (p<0.00001), 수술 후 첫 배변까지의 시간이 한약. 한약과 침 병용요법, 침, 뜸, 전기혈위자극, 이혈요법, 족삼리혈의 네오스티그민 주입 치료군에서 유의미하게 감소하였다 (p<0.00001). 심각한 이상반응은 나타나지 않았다. 결론: 한의학적 치료는 위절제술 후 위암 환자에게 수술 후 장마비의 예방과 치료법으로 활용될 수 있으며 수술 후 장마비 치료의 임상적 효과를 명확히 하기 위해 후속 연구가 필요하다.

  • Purpose: This study was to report the effect of Traditional Korean Medicine to Rheumatoid arthritis after parturition. Methods: A woman who complained of arthralgia, edema, sweating and chilling after parturition was enrolled in this study. We treated her by oriental treatments (herb medicine, acupuncture and moxibustion) to improve her symptoms. We checked her by symptoms and serologic test. Results: After treatments, the clinical symptoms and serologic signs improved. Conclusions: This case study shows that TKM treatment is effective for rheumatoid arthritis after parturition.

  • Purpose: To report the effect of Traditional Korean medical treatments on 5 patients with cervical intraepithelial neoplasia with high-risk human papilloma virus (HPV). Methods: The patients were diagnosed with cervical intraepithelial neoplasia with High-Risk human papilloma virus. The patients were treated by Traditional Korean Medicine such as herb medication and fumigation therapy. Results: After 3~6 months treatments, cervical intraepithelial neoplasia grades 1-2 lesions regressed and high-risk HPV infections were not detected. Conclusions: The case report shows that Korean medical treatment can be an effective option for treating lower grade of cervical intraepithelial neoplasia with high-risk human papilloma virus.

  • Objectives : This study was aimed to review the trend of clinical studies in Journal of Oriental Neuropsychiatry since 2001 to 2010. Methods : We collected 260 clinical studies and analyzed them for publish year, sorts of disease or symptoms, pattern identification diagnosis, and classification of clinical studies. Among them, we selected 46 clinical experimental studies and these studies were further analyzed for publish year, research institution, sort of disease or symptoms, sorts of treatments, number of subject, and pattern identification diagnosis. Results : The number of clinical studies in Journal of Oriental Neuropsychiatry have increased since the year of 2001. There were 62 kinds of diseases and symptoms and Hwa-Byung had the highest number of studies followed by depression, headache, insomnia and so on. The clinical experimental studies, which took up 17% of the clinical studies, have increased since 2001 and was varied with year. There were 13 reserch institutions for clinical experimental studies and they studied about stress, dementia, Hwa-Byung, and so on. Most of studies researched treatment about acupuncture, herb medicine, and meditation treatment for persons under 144. There are only 22% of studies using pattern identification diagnosis. Conclusions : For the development of oriental medicine for psychiatric disease, we need more qualifying clinical studies like RCT. We hope more researchers for psychiatry disease of oriental medicine will be interested in the publication of clinical studies and this will serve to produce advance of oriental medicine as evidence based medecine.

일차성 불면증의 추나 치료에 대한 문헌 연구 보고 (Chuna Manual Therapy for Primary Insomnia; A Review of Clinical Study)

  • Objectives To evaluate the evidence supporting the effectiveness of Chuna manual therapy for primary insomnia. Methods The researcher conducted search across the 3 electronic databases (Pubmed, CAJ and Oasis) to find all of randomized controlled clinical trials(RCTs) that used Chuna manual therapy as a treatment for primary insomnia. Results Four RCTs met inclusion criteria. The meta-analysis showed positive results for the use of Chuna manual therapy in terms of the PSQI when compared to medication treatments alone. Positive results were also obtained, in terms of the PSQI, when comparing Chuna manual therapy combined with acupuncture therapy to acupuncture therapy alone, but was not statistically significant. Conclusions The review found encouraging but limited evidence of Chuna manual therapy for primary insomnia. We recommend clinical trials which compare the effectiveness of Chuna manual therapy with usual care to obtain stronger evidence without the demerits of trial design.
