Comparison of Efficacy of Propofol When Used with or without Remifentanil during Conscious Sedation with a Target-Controlled Infuser for Impacted Teeth Extraction

  • Background: Clinical use of propofol along with remifentanil for intravenous sedation is increasing in these days, but there are not enough researches to evaluate proper target concentration when these drugs are infused by using target controlled infusion (TCI) pump in dental treatment cases. In this study, we compared efficacy of TCI conscious sedation and target concentration of propofol when it used with or without remifentanil during conscious sedation with the help of a TCI for the surgical extraction of impacted teeth. Methods: After IRB approval, all the charts of patients who had undergone surgical extraction of impacted teeth under propofol TCI sedation for 6 months were selected and reviewed for this study. After reviewal of charts, we could divide patients in two groups. In one group (group 1), only propofol was selected for sedation and initial effect site concentration of propofol was $1{\mu}g/ml$ (n = 33), and in another group (group 2), both propofol and remifentanil was infused and initial effect site concentration of each drug was $0.6{\mu}g/ml$ and 1 ng/ml respectively (n = 25). For each group, average propofol target concentration was measured. In addition, we compared heart rate, respiratory rate, and systolic and diastolic blood pressure as well as oxygen saturation. Besides, BIS, sedation scores (OAAS/S), and subjective satisfaction scores were compared. Results: Between group 1 and 2, there were no significant differences in demographics (age, weight and height), and total sedation time. However, total infused dose and the effect site target concentration of propofol was $163.8{\pm}74.5mg$ and $1.13{\pm}0.21{\mu}g/ml$ in group 1, and $104.3{\pm}46.5mg$ and $0.72{\pm}0.26{\mu}g/ml$ in the group 2 with $1.02{\pm}0.21ng/l$ of the effect site target concentration of remifentanil, respectively. During sedation, there were no differences between overall vital sign, BIS and OAAS/S in 2 groups (P > 0.05). However, we figured out patients in group 2 had decreased pain sensation during sedation. Conclusions: Co-administration of propofol along with remifentanil via a TCI for the surgical extraction of impacted teeth may be safe and effective compared to propofol only administration.

A Feasibility Study of the IMRT Optimization with Pseudo-Biologic Objective Function (유사생물학적 대상 함수를 이용한 IMRT 최적화 알고리즘 가능성에 관한 연구)

  • The pseudo-biologic objective function has been designed for the IMRT optimization. The RTP Tool Box (RTB) was used for this study. The pseudo-biologic function is similar to the biological objective function in mathematical shape, but uses physical parameters. The concepts of the TCI (Target Coverage Index) and the OSI (Organ Score Index) have been introduced for the target and the normal organs, respectively. The pseudo-biologic objective function s has been defined using these TCI and OSI's. The OSI's from the pseudo-biological function showed better results than from the physical functions, while TCI's showed similar tendency. These results revealed the feasibility of the pseudo-biologic function as an IMRT objective function.

The Seasonal Pattern Assessment Questionnaire in Relation to the Temperament and Character Inventory of Personality in Korean High School Students (SPAQ에 의한 계절성과 TCI에 의해 평가된 기질과 성격과의 관련성 - 한국 고등학생을 대상으로 -)

  • Objectives : Recently, many authors investigated the relation of temperament and character to seasonality and the genetic factors in seasonality and personality. In this study, we investigated the relation between personality structure obtained from the psychobiological Temperament and Character Inventory (TCI) and self-reported seasonal variations in mood and behavior measured by the Seasonal Pattern Assessment Questionnaire (SPAQ). Methods : 600 high-school students who live in Seoul of Korea, were participated in this study. Subjects were surveyed with the Korean translation of the SPAQ of Rosenthal, an instrument for investigating mood and behavioral changes with respect to the seasons of the you. Korean version of the TCI of Cloninger was also used for assessment of personality. Results : Female students were significantly higher scores on harm avoidance, reward dependence, and cooperativeness than male students who had significant higher scores on persistence and self·transcendence than the formers. GSS were showed positive correlation with harm avoidance and self-transcendence, and negative correlation with cooperativeness and self-directedness. By employing logistic regressions, the regression coefficients of SAD were significantly negative in SD, and positive in ST. Also, the regression coefficients of S-SAD were significantly negative in CO, and positive in ST. Those with GSS at least 11 were significantly higher mean ST values compared to the remaining individuals, and those with worst in winter were significantly lower mean CO values compared to the remaining individuals. Those who considered the seasonality as a problem at least to a moderate degree were associated with high HA and ST. Conclusion : We add the evidence that the seasonality is related to the temperament and character. Although there have been many reports about the seasonality in relation to the temperament and character in foreign countries, there has been very limited report from Korea before. We hope that this result would be a helpful in the clinical field in Korea.

  • These days global warming is the most important problem and the most important factor is high emission of carbon dioxide. The 23% of carbon dioxide emission for building construction must be reduced. Thermal conductivity is the most basic factor that can decrease the energy consumption especially insulation. Therefore, an accurate and continuous thermal conductivity measurement can be a way to save energy. In this paper, there are methods about how to investigate thermal conductivity measurements and comparing two methods which are the Heat Flow Meter 436 and TCi.

Effects of Temperament and Character on the Specialty Interests of Medical Students (기질 및 성격 특성이 의과대학 학생의 관심 전공 선택에 미치는 영향)

  • Objectives In the previous studies, it has been suggested that temperament and character may affect specialty choice in medical students. The aim of this study is to investigate the effects of temperament and character on specialty interests in medical students. Methods A total 132 third-year medical students initially participated in this study. Among these participants, 128 students had filled out both demographic and Temperament and Character Inventory (TCI) questionnaires. Socio-demographic data, specialty interests and Cloninger's TCI scores were obtained. The specialty interests were categorized into surgical, medical and other departments. Results Among 128 students, 88 (68.7%), 29 (22.7%) and 22 (8.6%) students chose medical, surgical and other departments, respectively. Students choosing surgery departments showed lower Harm Avoidance scores (F = 4.39, df = 1, p = 0.04) and higher Self-Directedness scores (F = 4.30, df = 1, p = 0.04) than those who chose medical departments. There was no significant differences in Novelty Seeking, Reward dependence, Persistence, Cooperativeness, Self-Transcendence scores between groups. Conclusions To the best of our knowledge, this is the first study that has demonstrated the relationship between TCI scores and specialty interests of medical students in Korea. The current study suggests that temperament and character influence specialty interests in medical students. These findings could have implications for medical education research and career counseling.

An Association Study of the Brain-Derived Neurotrophic Factor Genes Polymorphisms and Personality Traits (Brain-Derived Neurotrophic Factor(BNDF) Val66Met 유전자 다형성과 성격 특성에 대한 연합연구)

  • Background:Brain-derived neurotrophic factor(BDNF) genes are thought to be important factors in some personality traits. The goal of this study was to determine the role of these genes in personality traits. Method:The participants included 170 healthy adults with no history of psychiatric disorders and other physical illnesses for the last 6 months. All participants were tested by the Temperament and Character Inventory (TCI). BDNF Val64Met gene polymorphisms were analyzed with PCR(Polymerase Chain Reaction). Differences on TCI dimensions and sub-scales among groups were examined with ANOVA. Result:There was a significant correlation between BDNF Val64Met and Persistence(PS)(p=0.036) in female subjects, but none with the other TCI dimensions. A post-hoc comparison revealed significant a difference between Val/Val and Met/Met (p=0.031). Conclusion:Our study suggests that the BDNF Val64Met gene polymorphism is associated with persistence in Korean female subjects, but the small number of subjects limits generalization of our results. Further studies with a larger number of homogenous subjects are needed to confirm whether the BDNF gene is related to personality traits.

An Association Study of the Dopamine D2 Receptor Taq1A Polymorphism and Temperament in Korean Pathological Gamblers (한국인 병적 도박 환자에서 기질특성과 도파민 D2 수용체 Taq1A 다형성의 연합 연구)

  • Objective It has been suggested that the dopamine D2 receptor gene (DRD2) is associated with pathological gambling (PG). We investigated the association of the DRD2 Taq1A polymorphism and the temperament in PG using Cloninger's temperament and characteristic inventory (TCI). Methods 104 PG patients and 114 control subjects were recruited. Tests for DRD2 Taq1A polymorphism were conducted in both PG patients and controls. PG patients were requested to complete the TCI. Results There were no significant differences in frequencies of the genotype (${\chi}^2$ = 0.77, p = 0.681), allele (${\chi}^2$ = 0.52, p = 0.469), and allele (A1) carrier (${\chi}^2$ = 0.15, p = 0.695) between the PG patients and the control group. When we compared the TCI profile in the PG patients according to genotypes, there were significant differences in harm-avoidance (HA, p = 0.033), and self-directedness (SD, p = 0.012) among genotypes. These difference were more evident between A1 allele carriers and non-carriers (HA, p = 0.009 and SD, p = 0.004). Conclusion Present results suggest Taq1A polymorphism may not play an important role in the susceptibility to pathological gambling in our population. However, Taq1A polymorphism might be associated with some temperament in Korean PG patients.

A Study on Patterning and Grading by the Impact of Traffic Culture Index (교통문화지수 영향요인에 의한 유형화와 영향정도에 관한 연구)

  • This study suggests strategies to prevent traffic accidents by utilizing impact factors per each cluster and the typical patterns of 81 cities based on the statistical analysis of the data concerning the TCI which was developed from the partnership of the Traffic Safety Authority and the Green Traffic Movement Corporation in 2002 and 2003. The Principal Component Analysis and Cluster Analysis on impact factors and TCI result in 4 components and 4 clusters. Also as the results of Stepwise Multiple Regression Analysis examining the relationship between impact factors and TCI, R2 values of these models show high to all clusters. According to the results, we suggest strategies to prevent traffic accidents per cluster concretely and it is necessary to analyze how effective the invested facilities are in reducing traffic accidents in the future.

Propofol Target Controlled Infusion (TCI) Sedation for Dental Treatment in the Exaggerated Gag Reflex Patient (심한 구역반사를 가진 성인환자에서 프로포폴 목표농도 주입법으로 시행한 깊은 진정)

  • The gag reflex is a physiological reaction, but, an exaggerated gag reflex can be a severe limitation not only to treat dental caries but also to do oral exam. Procedures such as surface anesthesia of the palate and pharyngeral area, sedation, or general anesthesia can be options as behavioral management. But, there are no golden rule for the sever gag reflex patients. We present a case report of propofol intravenous sedation using TCI pump for simple dental treatment. A 44-year-old man, who had past history of general anesthesia for dental treatment because of severe gag reflex, was scheduled intravenous sedation for simple dental treatment. After 8 hour fasting he entered the clinic for persons with disabilities. We explained about intravenous deep sedation and got informed consent. First, we kept intravenous catheter (22G) in the arm and started monitoring ECG, non-invasive blood pressure, pulse oximetry and end-tidal $CO_2$ through nasal cannula. We started propofol infusion with TCI pump at the target concentration of 3 mcg/ml. The patient became sedated, but he showed involuntary movement during dental treatment, so we increased the target concentration to 4 mcg/ml. We finished the dental treatment without complications during 30 min. And after 40 min recovery room stay he was discharged without any complications.
