Optimal Echo phase of FLASH sequence for Brain Enhancement scan of mouse at 9.4T MRI system (9.4T MRI FLASH Sequence에서 마우스의 뇌 조영증강 검사를 위한 적정 Echo phase)

  • The objective of study was to investigate the optimal echo phase for mouse brain enhancement scan using fast low angle shot (FLASH) sequence of 9.4T magnetic resonance imaging (MRI). For quantification based on this method, an MR phantom experiment and clinical research were done. The phantom experiment was conducted by fabricating three phantoms with different molar concentration of gadolinium to create changes in echo phase of 9.4T FLASH sequence used in mouse brain scans. In the phantom experiment, SSI was 25~27 [arbitrary units, a.u.] in each of 33 phases from $6{\pi}$ to $28{\pi}$, while RSP was 30~100 mmol. MPSI was 47~52 [a.u], while MPP, where MPSI is seen, was 0.8~9 mmol. EPMS was 80.8~108.0%, while ASIMP was formed between 21.1 and 31.8 [a.u]. In the clinical research, Finally, the occurrence rate of artifact that expressed -1 nd +1. The present study was able to quantify the degree of enhancement at FLASH sequence of 9.4T MRI, as well as identify the optimal echo phase during mouse brain enhancement scan.

Radiological Diagnosis for Posttraumatic Olfactory Dysfunction (외상 후 후각이상에 대한 방사선학적 진단)

  • Objective : To evaluate objectively the sites of injury in patients with posttraumatic olfactory deficits and to suggest the diagnostic procedure for evaluation of posttraumatic anosmia. Methods : Ten patients with posttraumatic olfactory dysfunction were examined by means of olfactory testing, sinoscopy, contrast filled paranasal sinus computed tomography(contrast filled PNS CT) and magnetic resonance imaging(MRI). Five normal persons without olfactory dysfunction were also evauluated. The aerodynamic patency of olfactory cleft was examined by contrast filled PNS CT. The olfactory system(oflactory bulbs, olfactory tracts, inferior frontal region, hippocampi, or temporal lobes) was investigated in detail with MRI. The difference in the size of the olfactory bulb between normal volunteers and anosmic patients was evaluated by Student's t test. Results : Contrast filled dynamic CT scan was useful method for the evaluation of dynamic patency of the olfactory cleft. Paranasal CT scan of the all anosmic patients showed dynamic reflux of contrast media in olfactory cleft on valsalva maneuver. For the largest cross-sectional area and great height, the difference in olfactory bulb size between normal volunteers and patients was statistically significant(p<0.001) in MRI study. Conclusion : Posttraumatic anosmia was completely evaluated by olfactory testing, sinoscopy, and contrast filled CT scan for differentiation between conductive type and neurogenic type. Neurogenic anosmia was confirmed by perfect localization with MRI study.

Occlusal Analysis of the Patients with Temporomandibular Disorders Using T-Scan II System (T-Scan II 시스템을 이용한 측두하악장애 환자의 교합 분석)

  • Correlation between occlusal contact pattern and TMD have been hypothesized and partially investigated but results are controversial and not conclusive. The purposes of this study were to compare right-to-left difference of occlusal contact pattern, through contact points, contact force and occlusal balance, in the patients with unilateral TMD and also to evaluate its change related with TMD treatment. 36 patients with unilateral TMD from Department of Oral Medicine in Dankook University Dental Hospital were selected in this study (M:F=7:29, mean age of $29.2{\pm}14.8$ years). A computerized T-Scan II system (Tekscan, INC., USA) was employed for occlusal analysis and the simultaneity and occlusal balance through the number of tooth contact and magnitude of contact force were determined before and after TMD treatment. The number of contact points and contact force was more on the unaffected side than the affected side before treatment (p=0.056 and p=0.060, respectively) while significant difference between both sides was not found after treatment. The number of contact points and contact force on the affected sides significantly increased after treatment (p=0.038 and p=0.052), but the unaffected sides exhibited no significant difference between before and after treatment. In addition, sides difference in relative contact force decreased from about 27% to about 12% after TMD treatment (p=0.001). According to the results of this study, it is likely that unilateral TMD impairs right-to-left occlusal balance and that conservative TMD treatment alleviates the imbalance, subsequently leading to more symmetrical occlusal condition with increased contact points and force.

Changes of bite force and occlusal contacts after stabilization splint therapy (교합안정장치 사용후 교합력 및 교합접촉의 변화양상에 관한 연구)

  • This study was done to identify the changes of bite force and occlusal contact between before and after stabilization splint therapy. 16 female patients, accepted for orthodontic treatment at the Department of Orthodontics, College of Dentistry, Chosun University, were selected as the sample of this investigation. For comparisons, the samples of 16 were divided into just before stabilization splint, 1 month after stabilization splint, 2 months after stabilization splint, and 3 months after stabilization splint and used the T-scan system to identify the bite force and occlusal contact changes for each group. Statistical analysis of the data was carried out ANOVA tests, and Turkey tests using $SPSS/PC^+$. The results were as follows : 1. Bite force change from just before treatment to 1 month after stabilization splint therapy was statistical significantly decreased (P<0.05). 2. Bite force changes from 1 to 3 months after stabilization splint therapy showed no statistical significance (p>0.05). 3. The changes of anterior occlusal contacts showed no statistical significance regardless of the wearing periods of stabilization splint (P>0.05). 4. The changes of posterior occlusal contacts from just before treatment to 1 month after stabilization splint therapy was statistical significantly decreased (P<0.05). 5. The changes of posterior occlusal contacts Outing 1 to 3 months after stabilization splint therapy showed no statistical significance (P>0.05). 6. Posterior teeth rather than anterior teeth were more influenced by the changes of the number of occlusal contacts. To sum up above results, we may respect to capturing and stabilizing centric relation Position just 1 month after stabilization splint therapy.

A correlation between bruxism and eccentric occlusal interferences (이갈이와 편심위 교합간섭의 상관관계 분석)

  • Purpose: The purpose of this study was to analyze correlation between bruxism and occlusal contacts on balancing side. Materials and Methods: The purpose of this study is to compare the difference of group function and balancing side occlusal contacts according to bruxism and sex, A total of 100 adults that aged 26-37 years (39 bruxers and 61 non-bruxers) were examined. The lateral excursion and balanced occlusal contacts were analyzed to determine the correlation with bruxism. The occlusal contacts were recorded by T-Scan system and articulating paper. Results: The group function was the highest in 61.5% of bruxers and 47.5% of non-bruxers. In comparison between males and females, group function was 58.9% in males and 37.0% in females. Occlusal contacts on non-working side occurred in 48 out of 100 patients. There were 51.2% of the cases in the bruxers and 46.0% in the non-bruxers. Statistically, there was no correlation between the bruxism and occlusal contacts on non-working side. There was no correlation between sex and balancing occlusal contacts. Conclusion: There was no significant correlation between bruxism and occlusal contacts on non-working side. The group function was the highest in bruxers when lateral excursion was occurred.

Usefulness of Prone Position on PET-CT in Breast Cancer (유방암 PET-CT 검사에서 Prone(복와위)자세의 유용성 평가)

  • Purpose: In FDG-PET/CT of breast cancer, a sensitivity was 80~96% and a specificity was 75~95% commonly. It was valuable to identify a cancer in early stage been difficult in Mammography. Most of the PET/CT scans have been examined on supine position, so, the image of breast has been acquired by reconstructed whole body scan image. However, using prone position with a compensator, a shape of breast was reassembly shown to be real by gravity. Therefore, the purpose of this study was to evaluate diagnostic value of prone position in FDG PET-CT of breast cancer. Materials and Methods: 30 female patients with doubtful or positive breast cancer were examined. The PET-CT whole body scan was acquired at 60 minutes after $^{18}F$-FDG injection on Supine position. Then, regional breast spot scan was progressed on prone position using a compensator. Each image was evaluated by physicians blinded to patient's data, and statistical analysis did through SUVs measured in PET-CT images. Results: In 27 of 30 patients, prone position was shown accurate discrimination and diagnostic value, but in another 3 patients had a lesion 1cm below, PET-CT couldn't detect it, unlike MRI. Consequently, prone position distinguished a lesion better than Supine position, because of low degree of metamorphosis by gravity. The SUVs analysis of each position was significant (p value=0.004). Conclusion: In PET-CT of breast cancer, prone position could detect micrometastasis as well as primary lesion, better than supine position. Therefore, this study proposes that any technical change considered morphological feature like prone position can offer adequate and useful diagnostic information, together with complementary quantitative analysis.

AN EXPERIMENTAL STUDY ON OCCLUSAL AND VERTICAL CHANGES AFTER DENTURE PROCESSING (의치온성시 발생하는 교합 및 수직고경 변화에 관한 실험적 연구)

  • The purpose of this study was to compare the amount of the vertical pin opening of $33^{\circ}$ resin and $0^{\circ}$ resin teeth during processing and to determine whether the changes in tooth contacts by using articulation ribbon and T-Scan system follow any identifiable pattern, and to determine if there is any correlation between the changes in tooth contacts and the amount of the vertical pin opening after processing. Through statistical analyses on the data from this study, the following conclusions were obtained. 1. The amounts of the vertical pin opening were 0.78m, 0.87mm for $0^{\circ}$ teeth and $33^{\circ}$ teeth, respectively. But there was no significant difference between them. 2. The total number of contact points recorded by articulating ribbon after processing was decreased to 442.5% of that before processing. 3. The mean values for the number of contact points per tooth recorded by articulating ribbon after processing were 4.3 for the second molar, 3.8 for the first molar, 1.3 for the second premolar, 0.8 for the first premolar. The reduction of contact points per tooth became greater moving in an anterior direction. 4. The correlation between the reduction of contact points and the amount of the pin opening after processing was not significant. 5. The mean values for the numbers of contact points per tooth recorded by T-Scan system after processing were 2.2, 2.5, 1.0, and 0.5 at the second molar, the first molar, the second premolar, and the first premolar, respectively. 6. The correlation between the number of contact points by using articulating ribbon and TScan system after processing was not significant.

Feasibility Study of Applying the Acrylic Assistant Equipment (ACR) to Reduce Patient's Discomfort in Lower Abdomen MRI Scan (하복부 MRI 검사 시 환자의 불편함을 줄이기 위한 아크릴 보조 장치 사용의 타당성 조사)

  • In lower abdominal MRI scan, patients have been tested by physically contacting with the body array coil. In this study, we have designed the acrylic assistant equipment (ACR) which allows the contactless scan of the patient to the coil and evaluated the feasibility by comparing the acquired images with ACR to those obtained without ACR. We tested 10 cases (F: 5, m: 5) by using the Ingenia $3.0T^{TM}$ MR system and dStreamTM torso coil (Philips Healthcare, Netherlands). We implemented T1 AX TSE and eTHRIVE (GRE) techniques. The scanned images were quantitatively and qualitatively assessed. In qualitatively, the TSE shows 4.44 and 4.56 mean values with and without the ACR and 4.34 and 4.28 at the GRE, respectively. In quantitatively, the TSE shows 12.15 CNR, 17.95 SNR and 12.71 CNR, 18.96 SNR with and without the ACR. And GRE shows 17.72 CNR, 22.59 SNR and 18.26 CNR, 24.47 SNR with and without the ACR, respectively. We have designed and implemented the acrylic assistant equipment to lower abdominal patients. Our data indicate that it is possible to obtain similar image qualities to current lower abdominal MRI scan without the physical contact to the patient.

The Role of High Dose Rate (HDR) Intracavitary Radiation Therapy for the Management of Nasopharyngeal Carcinoma (비인강암 환자의 고선량 강내 방사선 치료의 효과)

  • From September 1989 to June 1992,22 patients with nasopharyngeal carcinoma were treated in Asan Medical Center with an external beam of 60 Gy followed by a boost dose of 15 Gy HDR brachytherapy. There were 5 females and 17 males with median age of 44 years (range: 20-69 years). All patients were histologically confirmed and staged by physical examination, CT scan and/or MRI. By the AJCC TNM staging system, there were 2 patients with stge II (T2NO), 4 with stage III (T3NO, T1-3N1), and 16 with stage IV (T4 or N2-3). Four patients received chemotherapy with 5-FU and cisplatin prior to radiotherapy. All patients were followed up periodically by a telescopic examination and radiologic imaging study of CT scan or MRI with a median follow-up time of 13 months (range: 3-34 months). Twenty one patients showed a complete response ore month after completing therapy and one patient showed a complete response after three months. At the time of this analysis, seventeen patients remain alive without evidence of disease, but four patients developed distant metastasis and one patient died a month after treatment. The local control rate was $100{\%}$ in a median follow-up time of 13 months. The two year overall and disease free survival rates by the Kaplan-Meier method were $94{\%}$ and $67{\%}$, respectively. Serious radiation sequelae have not been observed yet. Although longer follow-up is needed, this retrospective analysis suggests that HDR brachytherap. given as a boost therapy for nasoharyngeal carcinoma may improve the local control. To reduce the incidence of distant metastasis, we need to develop a more effective systemic chemotherapy.

Usefulness of CTAC Shift Revision Method of Artifact by Diaphragm in PET/CT (PET/CT 검사에서 횡격막에 의한 인공물의 CTAC Shift 보정방법의 유용성)

  • Purpose: Currently, decrement revision using LDCT is used in PET/CT. But cold artifacts are often found in decrement revision image by mismatch between LDCT image and Emission image near diaphragm due to patient's respiration. This research studied reduction of cold artifact by patient's respiration using CTAC Shift among revision methods. Materials and Methods: From March to September in 2012, 30 patients who had cold artifacts by respiration were targeted using PET/CT Discovery 600 (GE Healthcare, MI, USA) equipment. Patients with cold artifacts were additionally scan in diaphragm area, and the image shown cold artifacts at whole body test were revised using CTAC Shift. Cold artifacts including image, additional scan image and CTAC Shift revision image were evaluated as 1~5 points with naked eye by one nuclear medicine expert, 4 radiotechnologists with over 5 year experience. Also, standard uptake value of 3 images was compared using paired t-test. Results: Additional scan image and CTAC Shift revision image received relatively higher score in naked eye evaluation than cold artifacts including image. The additional scan image and CTAC Shift revision image had high correlation as the results of ANOVA test of standard uptake value and did not show significant difference. Conclusion: When cold artifacts are appeared by patient's respiration at PET/CT, it causes not only patient inconvenience but troubles in test schedule due to extra radiation exposure and time consumption by additional scan. But if CTAC Shift revision image can be acquired with out additional scan, it is considered to be helped in exact diagnosis without unnecessary extra radiation exposure and additional scan.
