The Clinical Analysis of Primary Malignant Hepatic Tumor in Childhood (소아의 원발성 악성 간종양에 대한 임상적 고찰)

  • The purpose of this study is to evaluate children who underwent hepatic resection for primary malignant hepatic tumor in the period from January 1994 to December 2001. A total of 8 patients, seven with hepatoblastoma (HB) and one with hepatocellular carcinoma (HCC), were studied. One HCC was resectable at the initial diagnosis, but five cases of unresectable HB received two cycles of transarterial chemoembolization (TACE) before operation. One patient with an unresectable HB with bone marrow metastasis was operated after one cycle of TACE and one cycle of systemic chemotherapy based on CCG-823F protocol. Another unresectable HB patient received systemic chemotherapy instead of TACE before operation. Postoperative chemotherapy was administered to all of the patients after complete surgical resection on CCG-823F protocol. All 6 patients who underwent TACE and neoadjuvant chemotherapy showed marked reduction in tumor volume and a clear outline of the lesion. Major complication was not noticed. Mean alpha-fetoprotein (${\alpha}$-FP) level at diagnosis, after neoadjuvant chemotherapy and after postoperative chemotherapy was 9,818 (42-35,350), 664, and 10.1 ng/mL, respectively. Half life of the ${\alpha}$-FP after complete resection was 5.1 days (3.0-8.7 days). Median follow up period was 57.1 months (10-97 months) and all the patients are alive with NED. In conclusion, preoperative chemotherapy, especially TACE, is effective, safe, and useful to treat initially unresectable hepatoblastoma, and serial level of the serum ${\alpha}$-FP is a useful tumor marker for diagnosis and monitoring therapeutic responses.

Risk Factors Associated with Complications of Carbon Monoxide Poisoning (일산화탄소 중독 환자에서 합병증 발생에 연관된 인자에 대한 분석)

  • Purpose: Carbon Monoxide Poisoning is an important medical emergency in Korea, but the factors leading to its serious manifestation are not well studied. Methods: We conducted a 5-year retrospective study of 98 carbon monoxide poisoned patients who visited the emergency departments of the Medical Center between December 2004 and March 2009. We categorized the patients into those exhibiting only local symptoms (group 1) and those showing systemic symptoms and complications (group 2). We compared the general characteristics as well as the clinical and laboratory findings between both groups. Results: The male to female ratio was 1.18. The most common systemic symptom was a mental change (43.9%), while the most common complication was rhabdomyolysis (31.6%). Poisoned area closed private room in group 2 were 23 (41.8%) cases and burning region in group 2 were 16(29.1%) cases (p=0.956). Individuals who were accidentally poisoned comprised of 43 (78.2%) cases while those that attempted suicidal poisoning comprised 12 (21.8%) cases (p=0.016). The most common symptom at arrival was mental change 33 (60.0%) cases in group 2. The mean time exposed to carbon monoxide was 43$\pm$3.97 hours in group 1 and 55$\pm$10.11 in group 2 (p=0.012). The patient's age, context of poisoning, symptom at arrival, and time exposed to the poison were found to be significant risk factors for complications by logistic regression analysis. Conclusion: Carbon monoxide poisoning is an emergency medical condition and the risk factors involved in the development of serious complications must be evaluated.

  • Bacillus velezensis YC7010 is an endophytic bacterium isolated from the rice rhizosphere in Jinju, Republic of Korea, with properties conductive to growth promotion, antibiosis and induced systemic resistance to significant, soil-borne rice fungal and bacterial pathogens. The genome of B. velezensis YC7010 comprises a 3,975,683 bp circular chromosome which consists of 3,790 protein-coding genes (86tRNA and 27rRNA genes). Based on genomic analysis, we identified genes involved in colonization and establishment inside the plant, biosynthesis of antibiotic compounds such as surfactin, plipapastatin, bacillibactin, and bacillaene, as well as the production of the phytohormones and volatile compounds which serve to promote the plants growth and development.

Analysis of Oral Pathogenic Microorganisms by Elderly's Systemic and Oral Health Status of the Elderly Over 65 Years

  • Korean society is experiencing rapid growth due to industrialization and urbanization, and the aging of population is emerging. Degradation of the masticatory muscle due to aging and reduction of the number of mastications causes dry mouth syndrome with decreased saliva in the elderly. Changes in the amount and composition of saliva cause various oral diseases, especially, periodontal disease is closely related to the microorganisms that form the plaque. In the elderly education, stimulation saliva was collected at the first visit and the last visit to observe changes in microorganisms in the oral cavity. For the collection of the probes, the paraffin wax was chewed for 5 minutes. The samples were immediately refrigerated in a 50 cc plastic tube and then stored at $-20^{\circ}C$ until the next use. The subjects of this study were the elderly people aged 65 years or older, and the study was conducted on 16 persons, 3 males and 13 females. The distribution of saliva microorganisms according to rolling brushing method showed significant difference in all three groups (Aa & Red complex, Orange complex and Green complex) before and after the education. In the group who answered that they had dentures, it was confirmed that the amount of microorganisms before and after education was significantly reduced in all three groups. There was a significant decrease in the amount of microorganisms in the Aa & Red complex and Orange complex group in the case of hypertension related to systemic disease. In conclusion, the higher the interest of oral health and general health of the subjects, the more effective the education.

Clinical Features and Hearing Outcomes of Sudden Sensorineural Hearing Loss in Diabetic Patients

  • Background and Objectives: This study aimed to evaluate the clinical features and the clinical factors associated with prognosis of sudden sensorineural hearing loss (SSNHL) in diabetic patients. Subjects and Methods: Forty-nine diabetic with unilateral SSNHL were retrospectively included. All patients received systemic high dose steroid therapy within one month after onset and had more than one month of follow-up audiogram. The basic characteristics of the patients, initial and follow-up audiograms, laboratory data, and methods of steroid treatment were collected. Results: Compared to reference values in healthy subjects, 79%, 55%, and 45% of the patients had higher values of mean neutrophil-lymphocyte ratio (NLR), platelet-lymphocyte ratio (PLR), and lymphocyte-monocyte ratio (LMR), respectively. Older patients had significantly less degree of hearing loss, but they also had significantly worse hearing thresholds in the unaffected ear. After steroid treatment, less than half patients (47%) showed hearing recovery. Simultaneous intratympanic dexamethasone (ITD) injections with systemic steroid did not confer an additional hearing gain or an earlier recovery rate in diabetic patients with SSNHL. In the multivariate analysis, initial hearing thresholds of affected ear and timing of steroid treatment were significantly associated with hearing prognosis in diabetic patients with SSNHL. Conclusions: Diabetic patients with SSNHL tended to have increased NLR, LMR, and PLR, which are reported to be associated with microvascular angiopathy. Simultaneous ITD injections to improve hearing recovery in diabetic patients with SSNHL seems unnecessary.

  • The oral cavity is a window into the health of the whole body and a gateway for many harmful bacteria. It is a very important part of our body. The biggest advantage of genetic testing is that it can systematically prevent and manage diseases by examining bacteria in the oral cavity and predicting systemic diseases that may occur in our body through big data AI algorithm analysis. Therefore, in this paper, the researcher's family conducts genetic testing directly to derive the results. In this study, in November 2022, 4 family members of the researcher listened to a prior explanation from 1 dentist and 1 dental hygienist at J Dental Clinic, a preventive dental clinic located in Seoul, and after filling out the consent form, oral examination and genetic testing were performed. Genetic testing was performed with Dr.*** for adults and He***** products for middle and elementary school students. Genetic testing, which is currently being conducted in Korea, has the advantage that subjects can access it relatively easily without drawing blood, but it also has limitations such as time and cost. Nevertheless, I think it is a part to be highly evaluated in that systemic diseases can be predicted through oral microorganisms.

Biological and Molecular Characterization of a Korean Isolate of Orthotospovirus chrysanthinecrocaulis (Formerly Chrysanthemum Stem Necrosis Virus) Isolated from Chrysanthemum morifolium

  • Biological and molecular characterization of a Korean isolate of Orthotospovirus chrysanthinecrocaulis (formerly known as chrysanthemum stem necrosis virus, CSNV) isolated from Chrysanthemum morifolium was determined using host range and sequence analysis in this study. Twenty-three species of indicator plants inoculated mechanically CSNV-Kr was investigated for determination of host range. CSNV-Kr induced various local and systemic symptoms in the inoculated plant species. CSNV-Kr could not infect three plant species and induced symptomless in systemic leaves in Nicotiana tabacum cultivars, though the plant samples reacted positively with the antiserum to CSNV by double-antibody sandwich-enzyme-linked immunosorbent assay. The complete genome sequence of CSNV-Kr was determined. The L RNA of CSNV-Kr consists of 8,959 nucleotides (nt) and encodes a putative RNA-dependent RNA polymerase. The M RNA of CSNV-Kr consists of 4,835 nt and encodes the movement protein (NSm) and the glycoprotein precursor (Gn/Gc protein). The S RNA of CNSV-Kr consists of 2,836 nt and encodes NSs protein and N protein. The Gn/Gc and N sequence of CSNV-Kr were compared with those of previously published CSNV isolates originating from different countries at nucleotide and amino acid levels. The Gn/GC sequence of CSNV-Kr shared 98.8-99.5% identity with CSNV isolated from other countries and the N sequence of CSNV-Kr shared 98.8-99.6% identity. No particular region of variability could be found in either grouping of viruses. All of the CSNV isolates did not show any relationship according to geographical origins and isolation hosts, suggesting no distinct segregation of the CSNV isolates.

Systemic Review of Acupuncture Treatment for Female Chronic Constipation (여성 만성 변비의 침 치료 효과에 대한 체계적 문헌 고찰)

  • Objectives: The purpose of this systemic review is to overview and evaluate the efficacy of acupuncture in female patients with chronic constipation. Methods: We conducted a comprehensive search of the literature to identify randomized controlled trials that investigated the effects of acupuncture on female patients with chronic constipation. Key words searched included "Female Constipation", "Women Constipation", "Acupuncture" in database KISS, DBPIA, RISS, Cochrane Central, Scopus, Pubmed, CNKI. Results: Nine randomized controlled trials met our inclusion criteria and were included in this systematic review. Our analysis found that acupuncture was associated with significant improvements in spontaneous bowel movements, complete spontaneous bowel movements, stool consistency, and quality of life in female patients with chronic constipation. Acupuncture was generally well-tolerated, with few adverse effects reported. Conclusions: Our findings suggest that acupuncture treatment would be effective way to treat chronic constipation of female patients.

Gene Expression Analysis in Cucumber Leaves Primed by Root Colonization of Pseudomonas chlororaphis O6 upon Challenge-inoculation with Corynespora cassiicola.

  • Colonization of Pseudomonas chlororaphis O6, a nonpathogenic rhizobacterium, on the roots induced systemic resistance in cucumber plants against tai-get leaf spot, a foliar disease caused by Corynespora cassiicola. A cDNA library was constructed using mRNA extracted from the cucumber leaves 12 h after inoculation with C. cassiicola, which roots had been previously treated with O6. To identify the genes involved in the O6-mediated induced systemic resistance (ISR), we employed a subtractive hybridization method using mRNAs extracted from C cassiicola-inoculated cucumber leaves with and without previous O6 treatment on the plant roots. Differential screening of the cDNA library led to the isolation of 5 distinct genesencoding a GTP-binding protein, a putative senescence-associated protein, a galactinol synthase, a hypersensitive-induced reaction protein, and a putative aquaporin. Expressions of these genes are not induced by O6 colonization alone. Before challenge inoculation, no increase in the gene transcriptions could be detected in previously O6-treated and untreated plants but, upon subsequent inoculation with the pathogenic fungus, transcription levels in O6-treated plants rose significantly faster and stronger than in untreated plants. Therefore, the O6-mediated ISR may be associated with an enhanced capacity for the rapid and effective activation of cellular defense responses which becomes apparent only after challenge inoculation on the distal, untreated plant parts, as suggested by Conrath et al. (2002).
